Swiss ambulatory care is characterized by independent, and primarily practice-based, physicians, receiving fee for service reimbursement. This study analyses supply sensitive services using ambulatory care claims data from mandatory health insurance. A first research question was aimed at the hypothesis that physicians with large patient lists decrease their intensity of services and bill less per patient to health insurance, and vice versa: physicians with smaller patient lists compensate for the lack of patients with additional visits and services. A second research question relates to the fact that several cantons are allowing physicians to directly dispense drugs to patients ('self-dispensation') whereas other cantons restrict such direct sales to emergencies only. This second question was based on the assumption that patterns of rescheduling patients for consultations may differ across channels of dispensing prescription drugs and therefore the hypothesis of different consultation costs in this context was investigated.

Objectives. To investigate procedural gender equity by assessing predisposing, enabling and need predictors of gender differences in annual medical expenditures and utilization among hypertensive individuals in the U.S. Also, to estimate and compare lifetime medical expenditures among hypertensive men and women in the U.S. ^ Data source. 2001-2004 the Medical Expenditure Panel Survey (MEPS);1986-2000 National Health Interview Survey (NHIS) and National Health Interview Survey linked to mortality in the National Death Index through 2002 (2002 NHIS-NDI). ^ Study design. We estimated total medical expenditure using four equations regression model, specific medical expenditures using two equations regression model and utilization using negative binomial regression model. Procedural equity was assessed by applying the Aday et al. theoretical framework. Expenditures were estimated in 2004 dollars. We estimated hypertension-attributable medical expenditure and utilization among men and women. ^ To estimate lifetime expenditures from ages 20 to 85+, we estimated medical expenditures with cross-sectional data and survival with prospective data. The four equations regression model were used to estimate average annual medical expenditures defined as sum of inpatient stay, emergency room visits, outpatient visits, office based visits, and prescription drugs expenditures. Life tables were used to estimate the distribution of life time medical expenditures for hypertensive men and women at different age and factors such as disease incidence, medical technology and health care cost were assumed to be fixed. Both total and hypertension attributable expenditures among men and women were estimated. ^ Data collection. We used the 2001-2004 MEPS household component and medical condition files; the NHIS person and condition files from 1986-1996 and 1997-2000 sample adult files were used; and the 1986-2000 NHIS that were linked to mortality in the 2002 NHIS-NDI. ^ Principal findings. Hypertensive men had significantly less utilization for most measures after controlling predisposing, enabling and need factors than hypertensive women. Similarly, hypertensive men had less prescription drug (-9.3%), office based (-7.2%) and total medical (-4.5%) expenditures than hypertensive women. However, men had more hypertension-attributable medical expenditures and utilization than women. ^ Expected total lifetime expenditure for average life table individuals at age 20, was $188,300 for hypertensive men and $254,910 for hypertensive women. But the lifetime expenditure that could be attributed to hypertension was $88,033 for men and $40,960 for women. ^ Conclusion. Hypertensive women had more utilization and expenditure for most measures than hypertensive men, possibly indicating procedural inequity. However, relatively higher hypertension-attributable health care of men shows more utilization of resources to treat hypertension related diseases among men than women. Este trabajo muestra los resultados de una encuesta realizada en 2010 sobre consumo de drogas adictivas en alumnos de 7 Facultades de la UNCUYO y los compara con los resultados obtenidos 12 años antes con una encuesta equivalente. La encuesta fue semiestructurada, con 17 preguntas, autocumplimentada, individual y anónima, con una muestra de 1108 alumnos de ambos sexos. De esa encuesta se utilizaron 7 preguntas específicas sobre drogas adictivas y una pregunta sobre consumo de medicamentos en general, con 46 opciones entre las que figuraban 10 denominaciones comerciales de benzodiacepinas, como drogas adictivas de prescripción. Para las comparaciones estadísticas se utilizó Chi cuadrado. El ranking de drogas utilizadas fue relativamente similar en 1998 y 2010 pero, exceptuando tabaco y “otras drogas" que se mantuvieron estables, el resto aumentó alrededor de 4 veces en el periodo. El 82,8% de los alumnos manifestó consumir alcohol (siempre + a veces); de 1 a 5 vasos semanales el 79%. En todas las Facultades, cerveza y Fernet encabezaron el ranquing de preferencias por bebidas alcohólicas. La prevalencia del consumo de tabaco en el total de la muestra se mantuvo constante en 1998 y 2010 (alrededor del 28%) pero el consumo entre Facultades fue variable y en Artes fue significativamente más elevado (42%). Siguen en el ranking marihuana con 10% de prevalencia, tranquilizantes con alrededor del 6% y cocaína con poco más del 1%. Finalmente “otras drogas" (Hachis, LSD, extasis, anabólicos, anfetaminas, “hongos") representaron solamente un 3% de prevalencia. Artes y Ciencias Políticas mostraron las mayores prevalencias de consumo y Derecho e Ingeniería las menores. Esto indica implementar acciones preventivas y correctivas particulares para cada Facultad.

Los fármacos en el agua han sido considerados en los últimos años un problema medioambiental grave, y se ha incrementado el interés por los efectos que pueden producirse en el medio acuático. Aunado a este problema se encuentra el consumo excesivo de medicamentos no controlados, los cuales pueden ser desechados sin tener el tratamiento adecuado; por lo que se ingresan a los cursos de agua. Estos contaminantes emergentes son compuestos cuyo vertido supone un problema sanitario y ambiental. Se trata de contaminantes solubles en agua por lo que son capaces de estar presentes en todas las etapas del ciclo del agua. Han sido numerosos estudios los que se han realizado en diferentes países, ya que su presencia se ha convertido en un tema emergente en la química del medio ambiente, debido a que en las investigaciones realizadas muestran que no hay una eliminación completa a pesar de los distintos procesos que se aplican en las plantas de tratamiento de aguas residuales. Esta contaminación, incrementa la necesidad de conocer cuál es el efecto toxicológico sobre los organismos acuáticos y, en consecuencia, en las personas. La bacteria Escherichia Coli, es un organismo muy estudiado, debido a que se encuentra en los intestinos de los animales y humanos y por lo consiguiente en las aguas negras. Teniendo en cuenta la crítica situación, se planteó estudiar el efecto sobre la bacteria E. coli de 4 fármacos: Atenolol, Azitromicina, Estradiol e Ibuprofeno, para conocer cual era su comportamiento y el efecto que podían producir la presencia de los fármacos en la eliminación por procesos de oxidación. Así también, los efectos producidos sobre E. Coli, después de estar en contacto con los fármacos 1, 3 y 7 días. Se observó que los fármacos tienen efectos en el aumento o eliminación de los microrganismos dependiendo de los tiempos de exposición y la concentración del fármaco. Así mismo se observó que los microorganismos asimilan mejor las concentraciones menores de fármacos, a tiempos de contacto mayores de 24 horas. Con todos los desinfectantes de estudio se observaron ligeras resistencias de la bacteria ante la presencia de los fármacos. Drugs in water have been considered in recent years a serious environmental problem, and has increased interest in the effects that may occur in the aquatic environment. Added to this problem is the excessive consumption of non-controlled drugs, which can be disposed of without proper treatment, so they enter waterways. These are compounds emerging contaminants being discharged is a health and environmental problem. It is water soluble contaminants and are therefore able to be present in all stages of the water cycle. There have been numerous studies conducted in different countries, since their presence has become an emerging issue in environmental chemistry, because in the research shows that there isn’t a removal despite the different processes used in wastewater treatment plants. This contamination, increases the need to know what is the toxicological effects on aquatic organisms and, consequently, in people. The bacterium Escherichia coli, is a well-studied organism because it is found in the intestines of animals and humans and is therefore in the wastewater. Given the critical situation, was proposed to study the effect on the bacterium E. coli of 4 drugs: Atenolol, Azithromycin, Estradiol and Ibuprofen, to know what his behavior and the effect it could produce the presence of drugs in the removal by oxidation processes. Also, the effects on E. Coli, after being in contact with the drug 1, 3 and 7 days. It was noted that the drugs have effects on the growth or elimination of microorganisms depending on exposure time and the drug concentration. Also it was observed that the microorganisms assimilate lower concentrations of drug better over 24 hours. With all disinfectants study were observed resistances of the bacteria in the presence of the drugs.

The I-Save Rx Program, which imports prescription drugs into the United States from other countries, and was found to be in violation of federal law is also covered in this report.

This study aimed to explore purchases of non-prescription medicines in New Zealand. Researchers were stationed for 5 days in 12 pharmacies throughout New Zealand during June and July of 1999. A brief questionnaire was administered, for each medicine purchased, to all available purchasers aged 16 years and over. At least partial data were collected from 2,597 medicine purchases (approximately 71.2% of medicine sales). Respiratory products comprised 42% of sales. Pharmacists were involved in 19.9% of medicine sales. Pharmacy staff featured in 62.2% of 792 influences on first-time purchases. This study tested a viable method for data collection and yielded valuable pharmaceutical marketing data.

Substance misuse is common in early psychosis, and impacts negatively on outcomes. Little is known about effective interventions for this population. We report a pilot study of brief intervention for substance misuse in early psychosis ( Start Over and Survive: SOS), comparing it with Standard Care(SC). Twenty-five in-patients aged 18 - 35 years with early psychosis and current misuse of non-opioid drugs were allocated randomly to conditions. Substance use and related problems were assessed at baseline, 6 weeks and 3, 6 and 12 months. Final assessments were blind to condition. All 13 SOS participants who proceeded to motivational interviewing reported less substance use at 6 months, compared with 58% (7/12) in SC alone. Effects were well maintained to 12 months. However, more SOS participants lived with a relative or partner, and this also was associated with better outcomes. Engagement remained challenging: 39% (16/41) declined participation and 38% (5/13) in SOS only received rapport building. Further research will increase sample size, and address both engagement and potential confounds.

Understanding the structures and functions of membrane proteins is an active area of research within bioscience. Membrane proteins are key players in essential cellular processes such as the uptake of nutrients, the export of waste products, and the way in which cells communicate with their environment. It is therefore not surprising that membrane proteins are targeted by over half of all prescription drugs. Since most membrane proteins are not abundant in their native membranes, it is necessary to produce them in recombinant host cells to enable further structural and functional studies. Unfortunately, achieving the required yields of functional recombinant membrane proteins is still a bottleneck in contemporary bioscience. This has highlighted the need for defined and rational optimization strategies based upon experimental observation rather than relying on trial and error. We have published a transcriptome and subsequent genetic analysis that has identified genes implicated in high-yielding yeast cells. These results have highlighted a role for alterations to a cell's protein synthetic capacity in the production of high yields of recombinant membrane protein: paradoxically, reduced protein synthesis favors higher yields. These results highlight a potential bottleneck at the protein folding or translocation stage of protein production.

Successful innovation of prescription drugs requires a substantial amount of marketing support. There is, however, much concern about the effects of marketing expenditures on the demand of pharmaceutical products (Manchanda et al., Market Lett 16(3/4):293–308, 2005). For example, excessive marketing could stimulate demand for products in the absence of a fundamental need. It also has been suggested that increased marketing expenditures may reduce the price elasticity of demand and allow firms to charge higher prices (Windmeijer et al., Health Econ 15(1):5–18, 2005). In this paper, we present the outcomes of an empirical study in which we determine the effects of pharmaceutical marketing expenditures using a number of frequently used “standardized” models. We determine which models perform best in terms of predictive validity and adequate descriptions of reality. We demonstrate, among others, that the effects of promotional efforts are brand specific and that most standardized models do not provide adequate descriptions of reality. We find that marketing expenditures have no or moderate effects on demand for pharmaceutical products in The Netherlands.

Background: Information seeking is an important coping mechanism for dealing with chronic illness. Despite a growing number of mental health websites, there is little understanding of how patients with bipolar disorder use the Internet to seek information. Methods: A 39 question, paper-based, anonymous survey, translated into 12 languages, was completed by 1222 patients in 17 countries as a convenience sample between March 2014 and January 2016. All patients had a diagnosis of bipolar disorder from a psychiatrist. Data were analyzed using descriptive statistics and generalized estimating equations to account for correlated data. Results: 976 (81 % of 1212 valid responses) of the patients used the Internet, and of these 750 (77 %) looked for information on bipolar disorder. When looking online for information, 89 % used a computer rather than a smartphone, and 79 % started with a general search engine. The primary reasons for searching were drug side effects (51 %), to learn anonymously (43 %), and for help coping (39 %). About 1/3 rated their search skills as expert, and 2/3 as basic or intermediate. 59 % preferred a website on mental illness and 33 % preferred Wikipedia. Only 20 % read or participated in online support groups. Most patients (62 %) searched a couple times a year. Online information seeking helped about 2/3 to cope (41 % of the entire sample). About 2/3 did not discuss Internet findings with their doctor. Conclusion: Online information seeking helps many patients to cope although alternative information sources remain important. Most patients do not discuss Internet findings with their doctor, and concern remains about the quality of online information especially related to prescription drugs. Patients may not rate search skills accurately, and may not understand limitations of online privacy. More patient education about online information searching is needed and physicians should recommend a few high quality websites.

This research assesses the impact of user charges in the context of consumer choice to ascertain how user charges in healthcare impact on patient behaviour in Ireland. Quantitative data is collected from a subset of the population in walk-in Urgent Care Clinics and General Practitioner surgeries to assess their responses to user charges and whether user charges are a viable source of part-funding healthcare in Ireland. Examining the economic theories of Becker (1965) and Grossman (1972), the research has assessed the impact of user charges on patient choice in terms of affordability and accessibility in healthcare. The research examined a number of private, public and part-publicly funded healthcare services in Ireland for which varying levels of user charges exist depending on patients’ healthcare cover. Firstly, the study identifies the factors affecting patient choice of privately funded walk-in Urgent Care Clinics in Ireland given user charges. Secondly, the study assesses patient response to user charges for a mainly public or part-publicly provided service; prescription drugs. Finally, the study examines patients’ attitudes towards the potential application of user charges for both public and private healthcare services when patient choice is part of a time-money trade-off, convenience choice or preference choice. These services are valued in the context of user charges becoming more prevalent in healthcare systems over time. The results indicate that the impact of user charges on healthcare services vary according to socio-economic status. The study shows that user charges can disproportionately affect lower income groups and consequently lead to affordability and accessibility issues. However, when valuing the potential application of user charges for three healthcare services (MRI scans, blood tests and a branded over a generic prescription drug), this research indicates that lower income individuals are willing to pay for healthcare services, albeit at a lower user charge than higher income earners. Consequently, this study suggests that user charges may be a feasible source of part-financing Irish healthcare, once the user charge is determined from the patients’ perspective, taking into account their ability to pay.

The importance of considering the differences between the male and female sex in clinical decision-making is crucial. However, it has been acknowledged in recent decades that clinical trials have not always adequately enrolled women or analyzed sex-specific differences in the data. As these deficiencies have hindered the progress of understanding women’s response to medications, agencies in the United States have worked towards the inclusion of women in clinical trials and appropriate analysis of sex-specific data from clinical trials. This review outlines the history and progress of women’s inclusion in clinical trials for prescription drugs and presents considerations for researchers, clinicians, and academicians on this issue.

The aim of my Ph. D. thesis is to generalize a method for targeted anti-cancer drug delivery. Hydrophilic polymer-drug conjugates involve complicated synthesis; drug-encapsulated polymeric nanoparticles limit the loading capability of payloads. This thesis introduces the concept of nanoconjugates to overcome difficulties in synthesis and formulation. Drugs with hydroxyl group are able to initiate polyester synthesis in a regio- and chemo- selective way, with the mediation of ligand-tunable Zinc catalyst. Herein, three anti-cancer drugs are presented to demonstrate the high efficiency and selectivity in the method (Chapter 2-4). The obtained particles are stable in salt solution, releasing drugs over weeks in controlled manner. With the conjugation of aptamer, particles are capable to target prostate cancer cells in vitro. These results open the gateway to evaluate the in vivo efficacy of nanoconjugates for target cancer therapy (Chapter 5). Mechanism study of the polymerization leads to the discovery of chemosite selective synthesis of prodrugs with acrylate functional groups. Functional copolymer-drug conjugates will expand the scope of nanoconjugates (Chapter 6). Liposome-aptamer targeting drug delivery vehicle is well studied to achieve reversible cell-specific delivery of non-hydoxyl drugs e.g. cisplatin (Chapter 7). New monomers and polymerization mechanisms are explored for polyester in order to synthesize nanoconjugates with variety on properties (Chapter 8). Initial efforts to apply this type of prodrugs will be focused on the preparation of hydrogels for stem cell research (Chapter 9).
