Background A high level of red blood cell distribution width (RDW) is a novel prognostic marker that may reflect an underlying inflammatory state. It has recently shown that when increased, it is related to cardiovascular disease, mortality, and metabolic syndrome (MetS) in the general population. Objectives To analyse the potential relation between high levels of RDW and cardiovascular risk (CVR) and MetS in HIVpatients. Patients and methods Observational, cross-sectional study of a series of HIVoutpatients attended in our Hospital. Demographic, anthropometric, clinical, and fasting lab data were recorded in all cases. CVR at 10 years was evaluated by Framingham equation, and MetS diagnosed according to the National Cholesterol Education Program criteria. Statistic program: SPSS 17.0. Results 666 patients were included, 79.3% were men, and mean age was 44.7 years. Mean CD4 count was 506 cells/ mm3 , 87.5% of the patients were on antiretroviral therapy, and 85.3% had undetectable HIV viral load. Mean RDW was 13.07% (range: 7.7-33.6%; 75th percentile 14,1%), with a prevalence of MetS of 15.7, 9.3, 18.8 and 16.6% first through fourth RDW quartile, and of patients with CVR >20% of 8.4, 4.0, 4.4 and 6.4%, respectively (p>0,05). The highest quartile of RDW (>14.1%) was associated with AIDS (OR 1.6, 95%CI 1.0-2.4; p 0.02), detectable HIV viral load (OR 1.5, 95%CI 1.01-2.4; p 0.04), and hypertension (OR 2.3, 95%CI 1.4-4.0; p 0.001). Conclusions In HIV-infected outpatients, higher RDW is related with detectable HIV viral load and with AIDS. Although it was associated with a traditional CVR factor as hypertension, we found no relation with MetS nor with higher CVR.

ABSTRACT: BACKGROUND: The prevalence of obesity has increased in societies of all socio-cultural backgrounds. To date, guidelines set forward to prevent obesity have universally emphasized optimal levels of physical activity. However there are few empirical data to support the assertion that low levels of energy expenditure in activity is a causal factor in the current obesity epidemic are very limited. METHODS: The Modeling the Epidemiologic Transition Study (METS) is a cohort study designed to assess the association between physical activity levels and relative weight, weight gain and diabetes and cardiovascular disease risk in five population-based samples at different stages of economic development. Twenty-five hundred young adults, ages 25-45, were enrolled in the study; 500 from sites in Ghana, South Africa, Seychelles, Jamaica and the United States. At baseline, physical activity levels were assessed using accelerometry and a questionnaire in all participants and by doubly labeled water in a subsample of 75 per site. We assessed dietary intake using two separate 24-h recalls, body composition using bioelectrical impedance analysis, and health history, social and economic indicators by questionnaire. Blood pressure was measured and blood samples collected for measurement of lipids, glucose, insulin and adipokines. Full examination including physical activity using accelerometry, anthropometric data and fasting glucose will take place at 12 and 24 months. The distribution of the main variables and the associations between physical activity, independent of energy intake, glucose metabolism and anthropometric measures will be assessed using cross-section and longitudinal analysis within and between sites. DISCUSSION: METS will provide insight on the relative contribution of physical activity and diet to excess weight, age-related weight gain and incident glucose impairment in five populations' samples of young adults at different stages of economic development. These data should be useful for the development of empirically-based public health policy aimed at the prevention of obesity and associated chronic diseases.

Opinnäytetyöni tarkoituksena on selvittää, miten luontoleikkejä on mahdollista soveltaa lasten toimintaterapiaan. Käsitän työssä ihmisen ja ympäristön kuuluvan kiinteästi toisiinsa. Tämän vuoksi toimintaa, tässä yhteydessä luontoleikkiä, ei voi ymmärtää ellei ota yksilön ja hänen ympäristönsä vuorovaikutuksellisia tekijöitä huomioon. Tämän vuoksi selvitän opinnäytetyössäni myös luontoympäristön sekä luonnonmateriaalien sovellutusmahdollisuuksia toimintaterapiassa. Opinnäytetyön teoriaa on ohjannut toimintaterapiaan sovellettu ekologinen viitekehys yhdessä ekoterapeuttisten ajatusmallien sekä ympäristöön liittyvien tutkimustulosten kanssa. Toiminnallinen opinnäytetyöni sisältää erillisen, ihmisiltä keräämäni luontoleikkivihkon, jota voi käyttää soveltaen toimintaterapiaan. Vihkosta löytyy yli 50 erilaista luontoleikkiä, jotka ovat olleet vastaajille jollakin tapaa merkityksellisiä. Vihkosta löytyy luonnon eri elementtejä sisältäviä leikkejä: maa-, vesi-, eläin-, metsä-, puu- ja kasvistoleikkejä. Näiden elementtien käyttömahdollisuudet ja ominaisuudet tulevat esille opinnäytetyön eri vaiheissa. Yhteys työelämään näkyy toimintaterapeutti yksityisyrittäjä Riika Östbergin soveltaessa leikkivihkon leikkejä kahden lapsen toimintaterapiaprosessiin. Toimintaterapeutin tekemät jäsennellyt havainnot ja niistä tekemäni johtopäätökset, tukevat työhön kokoamaani teoriaa. Tuloksien perusteella luontoleikkejä, luontoympäristöä ja luonnonmateriaaleja on mahdollista käyttää ennakkosuunnitelmien avulla lasten toimintaterapiassa. Toimintaterapeutti voi ohjata myös lapsen tukiverkostoa huomaamaan, kuinka luontoleikkitoiminta on terveyttä ja hyvinvointia edistävä, ekologinen sekä taloudellinen vaihtoehto lapsen toiminnan ja toimintakyvyn edistämiseksi. Luontoympäristössä lapsen on mahdollista harjoitella luovasti samoja taitoja kuin muutenkin toimintaterapiassa. Toivon, että opinnäytetyö saa lukijat miettimään omaa paikkaansa omassa ympäristössään. Tarkoituksenani on kannustaa ihmisiä käyttämään luontoa lapsen kasvun ja kehityksen tukena. Luontoteema on jo pelkän ajankohtaisuutensa ja monipuolisuutensa vuoksi hyvä tutkimuksen aihe. Tätä opinnäytetyötä voisi lähteä luontevasti jatkamaan haastattelemalla lapsia, perehtyen heidän omakohtaisiin luontoleikkikokemuksiinsa.

Opinnäytetyöni on monimuototyö, jonka teososa on luonto-ohjelma/dokumenttielokuva The Nature of Rebirth, jonka toteutin yhteistyössä neljän muun opiskelijan kanssa. Toimin toisena käsikirjoittajana, ohjaajana ja tuottajana. Luonto-ohjelman keskeisenä ideana on kertoa luonnosta tavalla, joka korostaa ihmisen ja luonnon suhdetta, vuorovaikutusta sekä yhteenkietoutuneisuutta. Luonto-ohjelman aiheena on Euroopan vanhin metsä Bialowieza Puolassa. Kirjallisessa osiossa tarkastelen ja reflektoin luonto-ohjelman hermeneuttista sisällönluomista moniosaajan (käsikirjoittaja-ohjaaja-tuottajan) näkökulmasta. Hermeneuttisen sisällönluomisen käsite kuvastaa sitä prosessia, jonka kautta mielestäni lopullinen luonto-ohjelma luotiin. Se on työskentelytapa, joka perustuu kaaoksen ja järjestyksen väliseen jännitteeseen ja vuorovaikutukseen. Jännitteen ja vuorovaikutuksen kautta intuitiivisesta ideasta luodaan vaihe vaiheelta lopullista sisältöä. Hermeneuttinen sisällönluominen sopi tuotantoomme, jossa haasteena oli hakea muotoa ja tapaa kertoa Euroopan vanhimmasta metsästä ihmisten kautta. Hermeneuttinen sisällönluominen soveltuu sellaisille tuotannoille, joissa tekijällä on samanaikaisesti limittäin erilaisia pidemmän aikavälin tuotantoja. Rajaan tarkasteluni niihin tuotannon vaiheisiin, joissa hermeneuttinen sisällönluomisen prosessi oli suuressa roolissa joko omassa mielessäni tai työryhmän välisissä dialogeissa ja yhteistyössä. Nämä olivat ikään kuin solmukohtia, jotka piti ratkaista, jotta sisällönluominen olisi voinut taas jatkua. Luonto-ohjelman The Nature of Rebirth tuotannossa solmukohdat painottuivat erityisesti ennakkotuotantovaiheessa käsikirjoituksen luomiseen ja jälkituotantovaiheessa leikkausvaiheeseen. Solmukohtien lisäksi käsittelen luonto-ohjelmamme sisältöä laajemmassa yhteiskunnallisessa ja ohjelmavirran kontekstissa. Lähdekirjallisuutena olen käyttänyt sekä luonto-ohjelmiin että dokumenttielokuviin liittyviä teoksia ja artikkeleita. Lisäksi olen soveltanut monitieteisesti muiden tieteenalojen teorioita ja tutkimuksia, esimerkiksi hermeneuttisen sisällönluomisen viitekehyksessä. Osa tiedoista perustuu luentoihin Stadiassa tai keskusteluihin alan asiantuntijoiden kanssa. Lisäksi kirjallisessa osuudessa tuon esiin omia huomioitani sekä luonto-ohjelman tekijänä että luonto-ohjelmien katsojana.

Abstract The object of game theory lies in the analysis of situations where different social actors have conflicting requirements and where their individual decisions will all influence the global outcome. In this framework, several games have been invented to capture the essence of various dilemmas encountered in many common important socio-economic situations. Even though these games often succeed in helping us understand human or animal behavior in interactive settings, some experiments have shown that people tend to cooperate with each other in situations for which classical game theory strongly recommends them to do the exact opposite. Several mechanisms have been invoked to try to explain the emergence of this unexpected cooperative attitude. Among them, repeated interaction, reputation, and belonging to a recognizable group have often been mentioned. However, the work of Nowak and May (1992) showed that the simple fact of arranging the players according to a spatial structure and only allowing them to interact with their immediate neighbors is sufficient to sustain a certain amount of cooperation even when the game is played anonymously and without repetition. Nowak and May's study and much of the following work was based on regular structures such as two-dimensional grids. Axelrod et al. (2002) showed that by randomizing the choice of neighbors, i.e. by actually giving up a strictly local geographical structure, cooperation can still emerge, provided that the interaction patterns remain stable in time. This is a first step towards a social network structure. However, following pioneering work by sociologists in the sixties such as that of Milgram (1967), in the last few years it has become apparent that many social and biological interaction networks, and even some technological networks, have particular, and partly unexpected, properties that set them apart from regular or random graphs. Among other things, they usually display broad degree distributions, and show small-world topological structure. Roughly speaking, a small-world graph is a network where any individual is relatively close, in terms of social ties, to any other individual, a property also found in random graphs but not in regular lattices. However, in contrast with random graphs, small-world networks also have a certain amount of local structure, as measured, for instance, by a quantity called the clustering coefficient. In the same vein, many real conflicting situations in economy and sociology are not well described neither by a fixed geographical position of the individuals in a regular lattice, nor by a random graph. Furthermore, it is a known fact that network structure can highly influence dynamical phenomena such as the way diseases spread across a population and ideas or information get transmitted. Therefore, in the last decade, research attention has naturally shifted from random and regular graphs towards better models of social interaction structures. The primary goal of this work is to discover whether or not the underlying graph structure of real social networks could give explanations as to why one finds higher levels of cooperation in populations of human beings or animals than what is prescribed by classical game theory. To meet this objective, I start by thoroughly studying a real scientific coauthorship network and showing how it differs from biological or technological networks using divers statistical measurements. Furthermore, I extract and describe its community structure taking into account the intensity of a collaboration. Finally, I investigate the temporal evolution of the network, from its inception to its state at the time of the study in 2006, suggesting also an effective view of it as opposed to a historical one. Thereafter, I combine evolutionary game theory with several network models along with the studied coauthorship network in order to highlight which specific network properties foster cooperation and shed some light on the various mechanisms responsible for the maintenance of this same cooperation. I point out the fact that, to resist defection, cooperators take advantage, whenever possible, of the degree-heterogeneity of social networks and their underlying community structure. Finally, I show that cooperation level and stability depend not only on the game played, but also on the evolutionary dynamic rules used and the individual payoff calculations. Synopsis Le but de la théorie des jeux réside dans l'analyse de situations dans lesquelles différents acteurs sociaux, avec des objectifs souvent conflictuels, doivent individuellement prendre des décisions qui influenceront toutes le résultat global. Dans ce cadre, plusieurs jeux ont été inventés afin de saisir l'essence de divers dilemmes rencontrés dans d'importantes situations socio-économiques. Bien que ces jeux nous permettent souvent de comprendre le comportement d'êtres humains ou d'animaux en interactions, des expériences ont montré que les individus ont parfois tendance à coopérer dans des situations pour lesquelles la théorie classique des jeux prescrit de faire le contraire. Plusieurs mécanismes ont été invoqués pour tenter d'expliquer l'émergence de ce comportement coopératif inattendu. Parmi ceux-ci, la répétition des interactions, la réputation ou encore l'appartenance à des groupes reconnaissables ont souvent été mentionnés. Toutefois, les travaux de Nowak et May (1992) ont montré que le simple fait de disposer les joueurs selon une structure spatiale en leur permettant d'interagir uniquement avec leurs voisins directs est suffisant pour maintenir un certain niveau de coopération même si le jeu est joué de manière anonyme et sans répétitions. L'étude de Nowak et May, ainsi qu'un nombre substantiel de travaux qui ont suivi, étaient basés sur des structures régulières telles que des grilles à deux dimensions. Axelrod et al. (2002) ont montré qu'en randomisant le choix des voisins, i.e. en abandonnant une localisation géographique stricte, la coopération peut malgré tout émerger, pour autant que les schémas d'interactions restent stables au cours du temps. Ceci est un premier pas en direction d'une structure de réseau social. Toutefois, suite aux travaux précurseurs de sociologues des années soixante, tels que ceux de Milgram (1967), il est devenu clair ces dernières années qu'une grande partie des réseaux d'interactions sociaux et biologiques, et même quelques réseaux technologiques, possèdent des propriétés particulières, et partiellement inattendues, qui les distinguent de graphes réguliers ou aléatoires. Entre autres, ils affichent en général une distribution du degré relativement large ainsi qu'une structure de "petit-monde". Grossièrement parlant, un graphe "petit-monde" est un réseau où tout individu se trouve relativement près de tout autre individu en termes de distance sociale, une propriété également présente dans les graphes aléatoires mais absente des grilles régulières. Par contre, les réseaux "petit-monde" ont, contrairement aux graphes aléatoires, une certaine structure de localité, mesurée par exemple par une quantité appelée le "coefficient de clustering". Dans le même esprit, plusieurs situations réelles de conflit en économie et sociologie ne sont pas bien décrites ni par des positions géographiquement fixes des individus en grilles régulières, ni par des graphes aléatoires. De plus, il est bien connu que la structure même d'un réseau peut passablement influencer des phénomènes dynamiques tels que la manière qu'a une maladie de se répandre à travers une population, ou encore la façon dont des idées ou une information s'y propagent. Ainsi, durant cette dernière décennie, l'attention de la recherche s'est tout naturellement déplacée des graphes aléatoires et réguliers vers de meilleurs modèles de structure d'interactions sociales. L'objectif principal de ce travail est de découvrir si la structure sous-jacente de graphe de vrais réseaux sociaux peut fournir des explications quant aux raisons pour lesquelles on trouve, chez certains groupes d'êtres humains ou d'animaux, des niveaux de coopération supérieurs à ce qui est prescrit par la théorie classique des jeux. Dans l'optique d'atteindre ce but, je commence par étudier un véritable réseau de collaborations scientifiques et, en utilisant diverses mesures statistiques, je mets en évidence la manière dont il diffère de réseaux biologiques ou technologiques. De plus, j'extrais et je décris sa structure de communautés en tenant compte de l'intensité d'une collaboration. Finalement, j'examine l'évolution temporelle du réseau depuis son origine jusqu'à son état en 2006, date à laquelle l'étude a été effectuée, en suggérant également une vue effective du réseau par opposition à une vue historique. Par la suite, je combine la théorie évolutionnaire des jeux avec des réseaux comprenant plusieurs modèles et le réseau de collaboration susmentionné, afin de déterminer les propriétés structurelles utiles à la promotion de la coopération et les mécanismes responsables du maintien de celle-ci. Je mets en évidence le fait que, pour ne pas succomber à la défection, les coopérateurs exploitent dans la mesure du possible l'hétérogénéité des réseaux sociaux en termes de degré ainsi que la structure de communautés sous-jacente de ces mêmes réseaux. Finalement, je montre que le niveau de coopération et sa stabilité dépendent non seulement du jeu joué, mais aussi des règles de la dynamique évolutionnaire utilisées et du calcul du bénéfice d'un individu.

Introduction et but de l'étude. - Le Nutrition Day 2010 réalisédans le service de dialyse aiguë de notre CHU a montré un taux dedénutrition de 64 % chez le patient dialysé hospitalisé avec un déficitnutritionnel moyen le jour de dialyse de 1 000 kcal et 45 g deprotéines. Les horaires de repas dans l'unité d'hospitalisation et dedialyse au centre de dialyse se chevauchent. Le but de cette étude estd'évaluer l'impact de l'organisation institutionnelle sur l'apportprotéino-énergétique du patient hémodialysé hospitalisé.Matériel et Méthodes. - Étude exploratoire et transversale. Laconsommation alimentaire et les obstacles potentiels à l'alimentationd'origine logistique ont été relevés durant deux jours consécutifs,un jour de dialyse (JD) et un jour sans dialyse (JSD). Les motifsde non consommation ou de consommation partielle des repas etcollations ont été relevés auprès des patients immédiatement aprèsles repas principaux, au moyen d'entretiens semi-dirigés. Lesingesta ont été comparés aux besoins protéino-énergétiques pour lespatients hémodialysés (ESPEN, 2006). Une évaluation nutritionnellea été réalisée chez tous les patients.Résultats. - Vingt-six patients (85 % d'hommes) ont été inclus,âgés de 65,7 ± 10,6 ans (moy ± ET). Le BMI moyen est de 24,9± 5,9 kg/m2, le score de Charlson de 7,2 ± 2,7 et 54 % ont un NRS-2002 ≥ 3. Plus de deux tiers des patients (68 %) ont perdu du poidset 64 % sont dénutris. Au total, 147 repas et 56 collations ont été étudiés,dont 74 repas et 32 collations pour le JD. Le JD, 56 % des collationsne sont pas consommées contre 21 % le JSD. La couverturemoyenne des besoins énergétiques et protéiques le JD est respectivementde 48 % et 57 %, sans différence avec le JSD. Les motifs denon consommation les plus fréquemment cités sont, par ordredécroissant : satiété précoce, inappétence, dégoût des mets proposés,peurs et représentations alimentaires et mises à jeun pour procéduresmédicales. Ils sont identiques les JD et les JSD. Lesobstacles à l'alimentation inhérents au patient sont 2,6 fois plus fréquentsque ceux liés à l'organisation hospitalière. Une douleur(légère à modérée), une dyspnée et une xérostomie affectent respectivement100 %, 54 % et 48 % des patients, qui ne considèrent pasces symptômes comme une cause de non consommation alimentaire.Conclusion. - Les apports protéino-énergétiques sont insuffisantset près de deux tiers des sujets sont dénutris. L'organisationhospitalière n'est pas identifiée par les patients comme un obstacleexpliquant le déficit énergétique et protéique quotidien. Les obstaclesà l'alimentation orale sont principalement inhérents à l'état desanté des patients. L'instauration d'une alimentation spécifique auxpatients dialysés hospitalisés (« humide », fractionnée, enrichie enprotéines et en énergie) de même que la prescription plus systématiqued'un support nutritionnel devraient contribuer à la couverturede leurs besoins nutritionnels.

There is no registered treatment (ttr) for pts with mCRPC who have progressive disease during or shortly after docetaxel (doc). EGFR overexpression increases in prostate cancer during the course of the disease. We investigated efficacy and safety of the combination of the monoclonal EGFR antibody cetuximab (cet) and doc in pts with mCRPC who are doc-refractory. Methods: Pts with mCRPC progressing during or < 90 days after at least 12 weeks of doc were included. Ttr consisted of the same doc regimen as prior to progression (35mg/m2 d1,8,15 q4w or 75mg/m2 q3w) in combination with cet (400mg/m2 d1, then 250mg/m2 weekly). Primary endpoint was progression free survival (PFS) at 12 weeks defined as absence of PSA progression or progression of metastases (mets). Secondary endpoints included toxicity, PFS at 24 weeks, PSA response, response of measureable disease and overall survival. 35 pts were needed in a Simon's two stage optimal design with a power of 90% and a significance level of 5% in order to test PFS rate at 12 weeks of £10% vs ?30%. Results: 35 evaluable pts were enrolled at 15 Swiss centers between 7/08 and 9/09. Median follow up was 14.8 months. Confirmed PFS at 12 weeks was 34% (95%CI 19-52%), PFS at 24 weeks was 20% and overall survival was 12.0 months (95%CI 7.1 -15.6). 20% (7/35) had a confirmed decline in PSA ? 50% and 31% (11/35) had a confirmed PSA decline ? 30%. Of pts with measurable disease (n=24) PR, SD and PD at week 12 was 4%, 54% and 25%, respectively (17% not evaluable). 3/9 (33%) pts with PDduring last doc ttr before inclusion reached the primary endpoint compared to 7/18 (39%) with PR or SD to last doc. 54% of evaluable pts experienced grade 3 and 6% grade 4 toxicity. Discussion: The result of the primary endpoint was promising in this first trial to test cet in combination with doc in pts with docetaxel-refractory mCRPC. Because this goal was achieved in such a highly pretreated pts population it appears that inhibition of the EGFR pathway may play a more important and persistent role in the treatment of prostate cancer than perceived so far. AIM OF THE STUDY: We assessed the relation between metabolic syndrome (MetS) and its components and colorectal cancer. METHODS: We analysed data from a multicentre case-control study conducted in Italy and Switzerland, including 1378 cases of colon cancer, 878 cases of rectal cancer and 4661 controls. All cases were incident and histologically confirmed. Controls were subjects admitted to the same hospitals as cases with acute non-malignant conditions. MetS was defined according to the International Diabetes Federation criteria. Odds ratios (ORs) and the corresponding 95% confidence intervals (CIs) were estimated by multiple logistic regression models, including terms for major identified confounding factors for colorectal cancer. RESULTS: With reference to each component of the MetS, the ORs of colorectal cancer in men were 1.27 (95% CI, 0.95-1.69) for diabetes, 1.24 (95% CI, 1.03-1.48) for hypertension, 1.14 (95% CI, 0.93-1.40) for hypercholesterolaemia and 1.26 (95% CI, 1.08-1.48) for overweight at age 30. The corresponding ORs in women were 1.20 (95% CI, 0.82-1.75), 0.87 (95% CI, 0.71-1.06), 0.83 (95% CI, 0.66-1.03) and 1.06 (95% CI, 0.86-1.30). Colorectal cancer risk was increased in men (OR=1.86; 95% CI, 1.21-2.86), but not in women (OR=1.13; 95% CI, 0.66-1.93), with MetS. The ORs were 2.09 (95% CI, 1.38-3.18) in men and 1.15 (95% CI, 0.68-1.94) in women with > or =3 components of the MetS, as compared to no component. Results were similar for colon and rectal cancers. CONCLUSION: This study supports a direct association between MetS and both colon and rectal cancers in men, but not in women.

ObjectiveCandidate genes for non-alcoholic fatty liver disease (NAFLD) identified by a bioinformatics approach were examined for variant associations to quantitative traits of NAFLD-related phenotypes.Research Design and MethodsBy integrating public database text mining, trans-organism protein-protein interaction transferal, and information on liver protein expression a protein-protein interaction network was constructed and from this a smaller isolated interactome was identified. Five genes from this interactome were selected for genetic analysis. Twenty-one tag single-nucleotide polymorphisms (SNPs) which captured all common variation in these genes were genotyped in 10,196 Danes, and analyzed for association with NAFLD-related quantitative traits, type 2 diabetes (T2D), central obesity, and WHO-defined metabolic syndrome (MetS).Results273 genes were included in the protein-protein interaction analysis and EHHADH, ECHS1, HADHA, HADHB, and ACADL were selected for further examination. A total of 10 nominal statistical significant associations (P<0.05) to quantitative metabolic traits were identified. Also, the case-control study showed associations between variation in the five genes and T2D, central obesity, and MetS, respectively. Bonferroni adjustments for multiple testing negated all associations.ConclusionsUsing a bioinformatics approach we identified five candidate genes for NAFLD. However, we failed to provide evidence of associations with major effects between SNPs in these five genes and NAFLD-related quantitative traits, T2D, central obesity, and MetS.

AIMS: We examined, in a country of the African region, i) the prevalence of the metabolic syndrome (MetS) according to three definitions (ATP, WHO and IDF); ii) the distribution of the MetS criteria; iii) the level of agreement between these three definitions and iv) we also examined these issues upon exclusion of people with diabetes. METHODS: We conducted an examination survey on a sample representative of the general population aged 25-64 years in the Seychelles (Indian Ocean, African region), attended by 1255 participants (participation rate of 80.3%). RESULTS: The prevalence of MetS increased markedly with age. According to the ATP, WHO and IDF definitions, the prevalence of MetS was, respectively, 24.0%, 25.0%, 25.1% in men and 32.2%, 24.6%, 35.4% in women. Approximately 80% of participants with diabetes also had MetS and the prevalence of MetS was approximately 7% lower upon exclusion of diabetic individuals. High blood pressure and adiposity were the criteria found most frequently among MetS holders irrespective of the MetS definitions. Among people with MetS based on any of the three definitions, 78% met both ATP and IDF criteria, 67% both WHO and IDF criteria, 54% both WHO and ATP criteria and only 37% met all three definitions. CONCLUSION: We identified a high prevalence of MetS in this population in epidemiological transition. The prevalence of MetS decreased by approximately 32% upon exclusion of persons with diabetes. Because of limited agreement between the MetS definitions, the fairly similar proportions of MetS based on any of the three MetS definitions classified, to a substantial extent, different subjects as having MetS.

BACKGROUND: Metals are known endocrine disruptors and have been linked to cardiometabolic diseases via multiple potential mechanisms, yet few human studies have both the exposure variability and biologically-relevant phenotype data available. We sought to examine the distribution of metals exposure and potential associations with cardiometabolic risk factors in the "Modeling the Epidemiologic Transition Study" (METS), a prospective cohort study designed to assess energy balance and change in body weight, diabetes and cardiovascular disease risk in five countries at different stages of social and economic development. METHODS: Young adults (25-45 years) of African descent were enrolled (N = 500 from each site) in: Ghana, South Africa, Seychelles, Jamaica and the U.S.A. We randomly selected 150 blood samples (N = 30 from each site) to determine concentrations of selected metals (arsenic, cadmium, lead, mercury) in a subset of participants at baseline and to examine associations with cardiometabolic risk factors. RESULTS: Median (interquartile range) metal concentrations (μg/L) were: arsenic 8.5 (7.7); cadmium 0.01 (0.8); lead 16.6 (16.1); and mercury 1.5 (5.0). There were significant differences in metals concentrations by: site location, paid employment status, education, marital status, smoking, alcohol use, and fish intake. After adjusting for these covariates plus age and sex, arsenic (OR 4.1, 95% C.I. 1.2, 14.6) and lead (OR 4.0, 95% C.I. 1.6, 9.6) above the median values were significantly associated with elevated fasting glucose. These associations increased when models were further adjusted for percent body fat: arsenic (OR 5.6, 95% C.I. 1.5, 21.2) and lead (OR 5.0, 95% C.I. 2.0, 12.7). Cadmium and mercury were also related with increased odds of elevated fasting glucose, but the associations were not statistically significant. Arsenic was significantly associated with increased odds of low HDL cholesterol both with (OR 8.0, 95% C.I. 1.8, 35.0) and without (OR 5.9, 95% C.I. 1.5, 23.1) adjustment for percent body fat. CONCLUSIONS: While not consistent for all cardiometabolic disease markers, these results are suggestive of potentially important associations between metals exposure and cardiometabolic risk. Future studies will examine these associations in the larger cohort over time.

Tämän diplomityön kirjallisessa osassa on tarkasteltu sulfaattiselluloosatehtaan ympäristöpäästöjen vähentämiseen liittyvää ongelmallisuutta sekä luomaan pääpiirteittäinen kuvaus sulfaattiselluloosatehtaan kemikaalikierrosta lähinnä rikki- ja natriumyhdisteiden kiertoja huomioiden. Pääasiassa kemikaalisäästöjen ja ympäristösyiden vuoksi tehtävää kemikaalikierron sulkemista seuraa rikki- ja natriumtaseisiin sekä kemikaalikierron vierasaineisiin liittyviä ongelmia, kuten prosessilaitteistojen likaantumista, korroosiota ja hajukaasuyhdisteiden muodostumista. Diplomityön mallinnusosassa on luotu Metsä-Botnian Rauman sellutehtaan kemikaalikierron malli pohjautuen tehtaan virtauskaavioihin sekä muuhun tehtaalta saatuun prosessi-informaatioon. BALAS-simulointiohjelmiston avulla luotuja mallikaavioita on käytetty hyväksi tehtaan kokonais- rikki- ja natriumtaseiden laskennassa. Varsinainen simulointi on jätetty projektin aikataulullisten syiden vuoksi jatkoprojektin piiriin.

Tämä teksti on työselostus indeksin ekologinen jalanjälki metsäosion laskennan menetelmiä selvittävästä tutkimuksesta. Tutkimusraportti alkaa selostuksella, jossa kerrotaanaiheen taustaa ja yhteys, missä liikutaan: indikaattorien hyödyntäminen, politiikka kestävän kehityksen ympärillä sekä hieman kestävän kehityksen kokonaisuudesta. Tämä on johdanto varsinaiseen tutkimuksen sisältöön. Tekstissä raportoidaan ekologisen jalanjäljen vuoden 2001 laskentataulukon mukainen metsäosion laskentamenetelmä sekä myös hiilidioksidin sitoutumisen jalanjäljen ja biokapasiteetin laskentamenetelmät. Tutkimuksen tuloksia ovat laskennasta löytyneet epäkohdat ja niiden kehittäminen. Tekstissä koetetaan myös selventää ekologisen jalanjäljen kokonaisuutta ja sitä miten sen tuloksia tulisi lukea.
