We report a case of patient in whom a gastric remnant cancer developed about five years after a gastric bypass for morbid obesity. We review the literature on gastric cancer after gastroplasty. Access of gastric remnant after gastroplasty (Fobi-Capella) prevents evaluation and treatment of its disorders.

Access to the gastric remnant and duodenum is lost after Roux-en-Y gastric bypasses for morbid obesity. Laparoscopic transgastric endoscopic retrograde cholangiopancreatography has recently been described to manage biliary problems in such cases. We describe the first brazilian case of management of choledocholithiasis after a Roux-en-Y gastric bypass using this approach.

The gastric bypass is a good option in the therapy of morbid obesity. Nevertheless, it must be considered the rare condition as occurred in a patient with previous abdominal surgery with Situs Inversus Totalis. A 24 year-old male patient with body mass index of 40 Kg/ m², multiple dietary failures, and arterial hypertension as co-morbidities, with a anterior paramedial right incision due to a previous appendicectomy (8 years ago).With a indication for bariatric surgery, was performed Roux-en-Y gastric bypass by laparoscopic procedure, with previous planning of Situs Inversus Totalis.

OBJECTIVE: to evaluate the importance of treatment of deformities caused by massive localized lymphedema (MLL) in the severely obese. METHODS: in a period of seven years, nine patients with morbid obesity and a mean age of 33 years underwent surgical resection of massive localized lymphedema with primary synthesis. This is a retrospective study on the surgical technique, complication rates and improved quality of life. RESULTS: all patients reported significant improvement after surgery, with greater range of motion, ambulation with ease and more effective hygiene. Histological analysis demonstrated the existence of a chronic inflammatory process marked by lymphomonocitary infiltrate and severe tissue edema. We observed foci of necrosis, formation of microabscesses, points of suppuration and local fibrosis organization, and pachydermia. The lymphatic vessels and some blood capillaries were increased, depicting a framework of linfangiectasias. CONCLUSION: surgical treatment of MLL proved to be important for improving patients' quality of life, functionally rehabilitating them and optimizing multidisciplinary follow-up of morbid obesity, with satisfactory surgical results and acceptable complication rates, demonstrating the importance of treatment and awareness about the disease.

OBJECTIVE: To assess psychiatric symptoms, substance use, quality of life and eating behavior of patients undergoing bariatric surgery before and after the procedure.METHODS: We conducted a prospective longitudinal study of 32 women undergoing bariatric surgery. To obtain data, the patients answered specific, self-administered questionnaires.RESULTS: We observed a reduction in depressive and anxious symptoms and also in bulimic behavior, as well as an improved quality of life in the physical, psychological and environmental domains. There was also a decrease in use of antidepressants and appetite suppressants, but the surgery was not a cessation factor in smoking and / or alcoholism.CONCLUSION: a decrease in psychiatric symptoms was observed after bariatric surgery, as well as the reduction in the use of psychoactive substances. In addition, there was an improvement in quality of life after surgical treatment of obesity.

Our aim was to determine if anatomical abnormalities of the upper airway (UA) and facial skeleton of class III severely obese patients are related to the presence and severity of obstructive sleep apnea syndrome (OSAS). Forty-five patients (69% females, mean age 46.5 ± 10.8 years) with a body mass index (BMI) over 40 kg/m² underwent UA and facial skeletal examinations as well as polysomnography. Mean BMI was 49 ± 7 kg/m² and mean neck circumference was 43.4 ± 5.1 cm. Polysomnographic findings showed that 22% had a normal apnea-hypopnea index (AHI) and 78% had an AHI over 5. The presence of OSAS was associated with younger age (P = 0.02), larger neck circumference (P = 0.004), presence of a voluminous lateral wall (P = 0.0002), posteriorized soft palate (P = 0.0053), thick soft palate (P = 0.0014), long uvula (P = 0.04), thick uvula (P = 0.0052), and inferior turbinate hypertrophy (P = 0.04). A larger neck circumference (P = 0.02), presence of a voluminous lateral wall (P = 0.04), posteriorized soft palate (P = 0.03), and thick soft palate (P = 0.04) were all associated with OSAS severity. The prevalence of OSAS in this group was high. A larger neck circumference and soft tissue abnormalities of the UA were markers for both the presence and severity of OSAS. Conversely, no abnormalities in the facial skeleton were associated with OSAS in patients with morbid obesity.

Obesity is a multifactorial disorder often associated with many important diseases such as diabetes, hypertension and other metabolic syndrome conditions. Argyrophil cells represent almost the total population of endocrine cells of the human gastric mucosa and some reports have described changes of specific types of these cells in patients with obesity and metabolic syndrome. The present study was designed to evaluate the global population of argyrophil cells of the gastric mucosa of morbidly obese and dyspeptic non-obese patients. Gastric biopsies of antropyloric and oxyntic mucosa were obtained from 50 morbidly obese patients (BMI >40) and 50 non-obese patients (17 dyspeptic overweight and 33 lean individuals) and processed for histology and Grimelius staining for argyrophil cell demonstration. Argyrophil cell density in the oxyntic mucosa of morbidly obese patients was higher in female (238.68 ± 83.71 cells/mm2) than in male patients (179.31 ± 85.96 cells/mm2) and also higher in female (214.20 ± 50.38 cells/mm2) than in male (141.90 ± 61.22 cells/mm2) morbidly obese patients with metabolic syndrome (P = 0.01 and P = 0.02, respectively). In antropyloric mucosa, the main difference in argyrophil cell density was observed between female morbidly obese patients with (167.00 ± 69.30 cells/mm2) and without (234.00 ± 69.54 cells/mm2) metabolic syndrome (P = 0.001). In conclusion, the present results show that the number of gastric argyrophil cells could be under gender influence in patients with morbid obesity. In addition, gastric argyrophil cells seem to behave differently among female morbidly obese patients with and without metabolic syndrome.

The prevalence of obesity has increased to epidemic status worldwide. Thousands of morbidly obese individuals undergo bariatric surgery for sustained weight loss; however, mid- and long-term outcomes of this surgery are still uncertain. Our objective was to estimate the 10-year mortality rate, and determine risk factors associated with death in young morbidly obese adults who underwent bariatric surgery. All patients who underwent open Roux-in-Y gastric bypass surgery between 2001 and 2010, covered by an insurance company, were analyzed to determine possible associations between risk factors present at the time of surgery and deaths related and unrelated to the surgery. Among the 4344 patients included in the study, 79% were female with a median age of 34.9 years and median body mass index (BMI) of 42 kg/m2. The 30-day and 10-year mortality rates were 0.55 and 3.34%, respectively, and 53.7% of deaths were related to early or late complications following bariatric surgery. Among these, 42.7% of the deaths were due to sepsis and 24.3% to cardiovascular complications. Male gender, age ≥50 years, BMI ≥50 kg/m2, and hypertension significantly increased the hazard for all deaths (P<0.001). Age ≥50 years, BMI ≥50 kg/m2, and surgeon inexperience elevated the hazard of death from causes related to surgery. Male gender and age ≥50 years were the factors associated with increased mortality from death not related to surgery. The overall risk of death after bariatric surgery was quite low, and half of the deaths were related to the surgery. Older patients and superobese patients were at greater risk of surgery-related deaths, as were patients operated on by less experienced surgeons.

Le récepteur mélanocortine de type 4 (MC4R) est un récepteur couplé aux protéines G impliqué dans la régulation de la prise alimentaire et de l’homéostasie énergétique. Quatre-vingt pour cent des mutants du MC4R reliés à l’obésité morbide précoce (OMP) sont retenus à l’intérieur de la cellule. Le système de contrôle de qualité (SCQ) est probablement responsable de cette rétention, par la reconnaissance d’une conformation inadéquate des mutants. Le rétablissement de l’expression à la surface cellulaire et de la fonctionnalité de ces mutants est donc d’intérêt thérapeutique. Dans cette optique, des composés lipophiles spécifiques pour le MC4R ont été sélectionnés sur la base de leur sélectivité. Nous avons démontré qu’ils agissent à titre de chaperone pharmacologique (CP) en rétablissant l’expression à la surface cellulaire et la fonctionnalité des récepteurs mutants S58C et R165W, et qu’ils favorisent leur N-glycosylation complexe (maturation). Le suivi par BRET du site d’action des CP du MC4R suggère une action en aval de l’interaction calnexine-MC4R. De manière générale, une CP peut avoir un effet différent selon le mutant traité en induisant des conformations distinctes du récepteur plus ou moins aptes à se dissocier du SCQ et à activer la voie de signalisation, et un mutant peut répondre différemment selon la CP utilisée par des différences d’affinité pour le ligand, la CP et les effecteurs. Une meilleure compréhension du mode d’action des CP pourrait aider au développement de nouvelles approches thérapeutiques non seulement pour l’OMP, mais aussi pour d’autres maladies conformationnelles causées par le mauvais repliement de protéines.

La obesidad es un problema de salud global siendo la cirugía bariatrica el mejor tratamiento demostrado. El Bypass gástrico (BGYR) es el método más utilizado que combina restricción y malabsorcion; sin embargo los procedimientos restrictivos se han popularizado recientemente. La Gastro-gastroplastia produce restricción gástrica reversible por medio de un pouch gástrico con anastomosis gastrogástrica y propusimos su evaluación Métodos: Estudio retrospectivo no randomizado que evaluó archivos de pacientes con GG y BGYR laparoscópicos entre febrero de 2008 y Abril de 2011 Resultados: 289 pacientes identificados: 180 GG y 109 BGYR de los cuales 138 cumplieron criterios de inclusión, 77 (55.8%) GG y 61 (44,2%) BGYR, 18 (13%) hombres y 120 (87%) mujeres. Para GG la mediana del peso inicial fue 97,15 (± 17,3) kg, IMC inicial de 39,35 (± 3,38) kg/m2 y exceso de peso de 37,1 (±11,9). La mediana de IMC a los 1, 6 y 12 meses fue 34,8 (±3,58) kg/m2, 30,81 (±3,81) kg/m2, 29,58 (±4,25) kg/m2 respectivamente. La mediana de % PEP 1, 6 y 12 meses fue 30,9 (±14,2) %, 61,88 (±18,27) %, 68,4 (±19,64) % respectivamente. Para BGYR la mediana del peso inicial fue 108,1 (± 25,4) kg, IMC inicial 44,4 (± 8,1) y exceso de peso de 48,4 (±15,2) %. La mediana de IMC a los 1, 6 y 12 meses fue 39 (±7,5) kg/m2, 33,31 (±4,9) kg/m2, 30,9 (±4,8) kg/m2 respectivamente. La mediana de % PEP 1, 6 y 12 meses fue 25,9 (±12,9) %, 61,87 (±18,62) %, 71,41 (±21,09) % respectivamente. Seguimiento a un año Conclusiones: La gastro-gastroplastia se plantea como técnica restrictiva, reversible, con resultados óptimos en reducción de peso y alternativa quirúrgica en pacientes con obesidad. Son necesarios estudios a mayor plazo para demostrar mantenimiento de cambios en el tiempo

La prevalencia de la Pérdida de Cabello de Patrón Femenino (FPHL) aumenta con la edad y no ha sido documentada en la población latinoamericana. Este estudio describe la prevalencia y variables asociadas a la FPHL en una consulta Dermatológica. Métodos: Estudio descriptivo en 99 mujeres mayores de 18 años asistentes a consulta dermatológica con diagnóstico de FPHL. Se realizó una prueba piloto en 22 pacientes, y posteriormente se analizaron los datos de las 99 pacientes acorde a la clasificación de Ludwig, edad, comorbilidades y alteraciones del estado del ánimo. Resultados: La prevalencia estimada de FPHL fue de 20.84%. 43,4% de las pacientes estudiadas pertenecen a edades entre 31 a 49 años; el 50,5% fueron diagnosticadas con FPHL grado II. El 62,6% tienen algún antecedente médico de importancia. El 54,4% manifestaron alguna alteración del estado del ánimo. La totalidad de las pacientes con Grado III tuvieron algún antecedente médico de importancia (ORP= 1,14 IC95% (1; 1,26) (p < 0,05)). De las pacientes con Grado II, el 61,1% presentaron alguna alteración secundaria del estado del ánimo (ORP=2,58 IC95% (1,14; 5,84) (p<0,05)). Conclusiones: Debido al aumento de la FPHL en la población colombiana la cual conlleva a múltiples gastos del sistema salud, se hacen necesarios más estudios con el fin de caracterizar la enfermedad. Se recomienda un estudio con un mayor marco muestral con el fin de caracterizar la prevalencia y las variables relacionadas con la FPHL en toda la población Colombiana.

Em Portugal 0,6% dos adultos são obesos mórbidos. Como a cirurgia bariátrica tem vindo a ganhar popularidade no tratamento desta população, o objectivo principal do presente trabalho é analisar a associação entre a perda de peso e os hábitos de actividade física em adultos obesos mórbidos submetidos a cirurgia bariátrica. Como objectivo secundário estudámos a associação entre os hábitos de actividade física actuais e a maximização da perda de peso, bem como, a sua associação à capacidade aeróbia, nesta população. Este trabalho divide-se em duas partes fundamentais, a primeira uma revisão sistemática da literatura que sumarizou os efeitos da actividade física e/ou exercício físico após cirurgia bariátrica como parte de uma abordagem multidisciplinar. Este estudo permitiu constatar impactos benéficos sobre variáveis antropométricas e da composição corporal, níveis de actividade física/exercício físico, aptidão física, aptidão física funcional, qualidade de vida e marcadores bioquímicos após cirurgia bariátrica. A segunda parte, um estudo experimental onde participaram 75 obesos mórbidos submetidos a cirurgia bariátrica com idades compreendidas entre os 18 e os 65 anos. Os sujeitos responderam a um questionário sobre hábitos de actividade física, que analisou os hábitos de actividade física e a sua associação à aptidão cardiorrespiratória e à maximização da perda de peso nesta população. Verificou-se uma associação positiva entre a perda de peso e actividade física. Observou-se, ainda, que os participantes que relataram maior intensidade e mais tempo dispendido em actividade física/exercício físico apresentaram um aumento no consumo máximo de oxigénio no pós-operatório. Propõe-se, portanto, que a actividade física/exercício físico seja uma parte essencial de um programa de acompanhamento após cirurgia bariátrica.

A obesidade acarreta consequências ao nível da diminuição de saúde mas também a nível social, sendo os obesos vítimas de elevada discriminação devido ao seu peso. O objetivo principal deste trabalho é analisar a associação entre a atividade física e a perceção de discriminação em obesos submetidos a cirurgia bariátrica. O objetivo secundário prende-se com o estudo da associação entre a atividade física e a preocupação com forma corporal. O trabalho está dividido em duas partes distintas, sendo a primeiro dedicada à revisão sistemática da literatura que abordou os temas da discriminação, insatisfação corporal e atividade física nos obesos. Desta revisão concluí-se que a discriminação e insatisfação com forma corporal contribuem para a manutenção de comportamentos sedentários, no entanto com a prática de atividade física estas tendem a diminuir. A segunda parte do trabalho consiste num estudo observacional retrospetivo, constituído por uma amostra de 98 sujeitos com obesidade mórbida que tenham realizado uma cirurgia bariátrica, com idades compreendidas entre os 26 e os 73 anos. Com recurso à aplicação de questionários avaliou-se a atividade física e a sua associação com a discriminação e preocupação com forma corporal. Os resultados obtidos indicam que a discriminação não se encontra associada à atividade física nem ao IMC. Verificou-se que a atividade física de intensidade moderada e vigorosa está associada à diminuição da preocupação com forma corporal. A preocupação com forma corporal está também associada ao IMC. A atividade física deve ser integrada no programa multidisciplinar que acompanha a cirurgia.

Background Underweight and severe and morbid obesity are associated with highly elevated risks of adverse health outcomes. We estimated trends in mean body-mass index (BMI), which characterises its population distribution, and in the prevalences of a complete set of BMI categories for adults in all countries. Methods We analysed, with use of a consistent protocol, population-based studies that had measured height and weight in adults aged 18 years and older. We applied a Bayesian hierarchical model to these data to estimate trends from 1975 to 2014 in mean BMI and in the prevalences of BMI categories (<18·5 kg/m2 [underweight], 18·5 kg/m2 to <20 kg/m2, 20 kg/m2 to <25 kg/m2, 25 kg/m2 to <30 kg/m2, 30 kg/m2 to <35 kg/m2, 35 kg/m2 to <40 kg/m2, ≥40 kg/m2 [morbid obesity]), by sex in 200 countries and territories, organised in 21 regions. We calculated the posterior probability of meeting the target of halting by 2025 the rise in obesity at its 2010 levels, if post-2000 trends continue. Findings We used 1698 population-based data sources, with more than 19·2 million adult participants (9·9 million men and 9·3 million women) in 186 of 200 countries for which estimates were made. Global age-standardised mean BMI increased from 21·7 kg/m2 (95% credible interval 21·3–22·1) in 1975 to 24·2 kg/m2 (24·0–24·4) in 2014 in men, and from 22·1 kg/m2 (21·7–22·5) in 1975 to 24·4 kg/m2 (24·2–24·6) in 2014 in women. Regional mean BMIs in 2014 for men ranged from 21·4 kg/m2 in central Africa and south Asia to 29·2 kg/m2 (28·6–29·8) in Polynesia and Micronesia; for women the range was from 21·8 kg/m2 (21·4–22·3) in south Asia to 32·2 kg/m2 (31·5–32·8) in Polynesia and Micronesia. Over these four decades, age-standardised global prevalence of underweight decreased from 13·8% (10·5–17·4) to 8·8% (7·4–10·3) in men and from 14·6% (11·6–17·9) to 9·7% (8·3–11·1) in women. South Asia had the highest prevalence of underweight in 2014, 23·4% (17·8–29·2) in men and 24·0% (18·9–29·3) in women. Age-standardised prevalence of obesity increased from 3·2% (2·4–4·1) in 1975 to 10·8% (9·7–12·0) in 2014 in men, and from 6·4% (5·1–7·8) to 14·9% (13·6–16·1) in women. 2·3% (2·0–2·7) of the world's men and 5·0% (4·4–5·6) of women were severely obese (ie, have BMI ≥35 kg/m2). Globally, prevalence of morbid obesity was 0·64% (0·46–0·86) in men and 1·6% (1·3–1·9) in women. Interpretation If post-2000 trends continue, the probability of meeting the global obesity target is virtually zero. Rather, if these trends continue, by 2025, global obesity prevalence will reach 18% in men and surpass 21% in women; severe obesity will surpass 6% in men and 9% in women. Nonetheless, underweight remains prevalent in the world's poorest regions, especially in south Asia.
