While bisphosphonates reduce fracture risk over 3 to 5 years, the optimal duration of treatment is uncertain. In a randomized extension study (E1) of the Health Outcomes and Reduced Incidence with Zoledronic Acid Once Yearly - Pivotal Fracture Trial (HORIZON-PFT), zoledronic acid (ZOL) 5 mg annually for 6 years showed maintenance of bone mineral density (BMD), decrease in morphometric vertebral fractures, and a modest reduction in bone turnover markers (BTMs) compared with discontinuation after 3 years. To investigate the longer-term efficacy and safety of ZOL, a second extension (E2) was conducted to 9 years in which women on ZOL for 6 years in E1 were randomized to either ZOL (Z9) or placebo (Z6P3) for 3 additional years. In this multicenter, randomized, double-blind study, 190 women were randomized to Z9 (n=95) and Z6P3 (n=95). The primary endpoint was change in total hip BMD at year 9 vs. year 6 in Z9 compared with Z6P3. Other secondary endpoints included fractures, BTMs, and safety. From year 6 to 9, the mean change in total hip BMD was -0.54% in Z9 vs. -1.31% in Z6P3 (difference 0.78%; 95% confidence interval [CI]: -0.37%, 1.93%; p=0.183). BTMs showed small, non-significant increases in those who discontinued after 6 years compared with those who continued for 9 years. The number of fractures was low and did not significantly differ by treatment. While generally safe, there was a small increase in cardiac arrhythmias (combined serious and non-serious) in the Z9 group but no significant imbalance in other safety parameters. The results suggest almost all patients who have received six annual ZOL infusions can stop medication for up to 3 years with apparent maintenance of benefits. This article is protected by copyright. Temerin is settlements in the municipality of the same name, and њhose is center. This settlement by its characteristics is a very specific for area of Bačke and Vojvodina. One of its main characteristic is a specific spatial and urban development. Although the site of the present Temerin revealed several archaeological sites that attest to the presence of humans in prehistoric times, today’s settlement, under this name, is mentioned first time in the early 14th century. Over the centuries the Temerin developed and expanded in phases, forming elements of the composition of settlements with a number of specific features. Especially in this regard emphasize shape, structure and physiognomy of the settlement. The main characteristic of the physiognomy of the settlement is a large elongated north to south axis of Temerin, which is the main street, and also the longest street in the whole of Serbia (13 km). Along the major axis, and in some places there are strains in relation to linear the square in the form of parts as a result of subsequent colonization and phase expansion of Temerin. This paper aims to study the expansion and urban development Temerin throughout history.

UNLABELLED The FREEDOM study and its Extension provide long-term information about the effects of denosumab for the treatment of postmenopausal osteoporosis. Treatment for up to 8 years was associated with persistent reduction of bone turnover, continued increases in bone mineral density, low fracture incidence, and a favorable benefit/risk profile. INTRODUCTION This study aims to report the results through year 5 of the FREEDOM Extension study, representing up to 8 years of continued denosumab treatment in postmenopausal women with osteoporosis. METHODS Women who completed the 3-year FREEDOM study were eligible to enter the 7-year open-label FREEDOM Extension in which all participants are scheduled to receive denosumab, since placebo assignment was discontinued for ethical reasons. A total of 4550 women enrolled in the Extension (2343 long-term; 2207 cross-over). In this analysis, women in the long-term and cross-over groups received denosumab for up to 8 and 5 years, respectively. RESULTS Throughout the Extension, sustained reduction of bone turnover markers (BTMs) was observed in both groups. In the long-term group, mean bone mineral density (BMD) continued to increase significantly at each time point measured, for cumulative 8-year gains of 18.4 and 8.3 % at the lumbar spine and total hip, respectively. In the cross-over group, mean BMD increased significantly from the Extension baseline for 5-year cumulative gains of 13.1 and 6.2 % at the lumbar spine and total hip, respectively. The yearly incidence of new vertebral and nonvertebral fractures remained low in both groups. The incidence of adverse and serious adverse events did not increase over time. Through Extension year 5, eight events of osteonecrosis of the jaw and two events of atypical femoral fracture were confirmed. CONCLUSIONS Denosumab treatment for up to 8 years was associated with persistent reductions of BTMs, continued BMD gains, low fracture incidence, and a consistent safety profile.

BACKGROUND Data evaluating the chronological order of appearance of extraintestinal manifestations (EIMs) relative to the time of inflammatory bowel disease (IBD) diagnosis is currently lacking. We aimed to assess the type, frequency, and chronological order of appearance of EIMs in patients with IBD. METHODS Data from the Swiss Inflammatory Bowel Disease Cohort Study were analyzed. RESULTS The data on 1249 patients were analyzed (49.8% female, median age: 40 [interquartile range, 30-51 yr], 735 [58.8%] with Crohn's disease, 483 [38.7%] with ulcerative colitis, and 31 [2.5%] with indeterminate colitis). A total of 366 patients presented with EIMs (29.3%). Of those, 63.4% presented with 1, 26.5% with 2, 4.9% with 3, 2.5% with 4, and 2.7% with 5 EIMs during their lifetime. Patients presented with the following diseases as first EIMs: peripheral arthritis 70.0%, aphthous stomatitis 21.6%, axial arthropathy/ankylosing spondylitis 16.4%, uveitis 13.7%, erythema nodosum 12.6%, primary sclerosing cholangitis 6.6%, pyoderma gangrenosum 4.9%, and psoriasis 2.7%. In 25.8% of cases, patients presented with their first EIM before IBD was diagnosed (median time 5 mo before IBD diagnosis: range, 0-25 mo), and in 74.2% of cases, the first EIM manifested itself after IBD diagnosis (median: 92 mo; range, 29-183 mo). CONCLUSIONS In one quarter of patients with IBD, EIMs appeared before the time of IBD diagnosis. Occurrence of EIMs should prompt physicians to look for potential underlying IBD.

Pits have been observed on many cometary nuclei mapped by spacecraft(1-4). It has been argued that cometary pits are a signature of endogenic activity, rather than impact craters such as those on planetary and asteroid surfaces. Impact experiments(5,6) andmodels(7,8) cannot reproduce the shapes of most of the observed cometary pits, and the predicted collision rates imply that few of the pits are related to impacts(8,9). Alternative mechanisms like explosive activity(10) have been suggested, but the driving process remains unknown. Here we report that pits on comet 67P/Churyumov-Gerasimenko are active, and probably created by a sinkhole process, possibly accompanied by outbursts. We argue that after formation, pits expand slowly in diameter, owing to sublimation-driven retreat of the walls. Therefore, pits characterize how eroded the surface is: a fresh cometary surface will have a ragged structure with many pits, while an evolved surface will look smoother. The size and spatial distribution of pits imply that large heterogeneities exist in the physical, structural or compositional properties of the first few hundred metres below the current nucleus surface.

Este paper presenta un resúmen de propuestas teóricas y estrategias que el autor ha venido desarrollando en los últimos años, sobre todo en "La trama (in)visible de la vida social: comunicación, sentido y realidad" (La Crujía, Bs. As. 2003/06). El objetivo es presentar hipótesis y categorías de análisis tentativas que aporten a la construcción de un Programa de Investigación sociocomunicacional tomando como referencia ideal las proposiciones de Lakatos (PIC). El autor considera que las proposiciones que presenta ayudan a asumir ciertas claves conceptuales para fundamentar, demarcar temáticas y problemáticas centrales del campo, a fin de diseñar un posible "mapa de referencia" analítica para el desarrollo de investigaciones articuladas hacia una sistematización del campo comunicacional (o sociocomunicacional)

Cryploendolithic microbial communities in the Ross Desert (McMurdo Dry Valleys) are characterized on the basis of photosymlictic microorganisms and fungi. Two eukaryotic communities (the lichen-dominated and Hemichloris communities) and three cyanobacterial communities (the red Gloeocapsa, Hormathonema-Gloeocapsa and Chroococcidiopsis communities) are described. Eleven coccoid. ohne pleurocapsoid, and five filamentous cyanobacteria occurring in these communities are characterized and illustrated. The moisture grade of the rock substrate seems to affect pH. Formation of primary iron stain, and the distribution of microbial communities.

Laboratory experiments show that undercooling to about -5°C occurs in colonized Beacon sandstones of the Ross Desert, Antarctica. High-frequency temperature oscillations between 5°C and -5°C or -10°C (which occur in nature on the rock surface) did not damage Hemichloris antarctica. In a cryomicroscope, H. antarctica appeared to be undamaged after slow or rapid cooling to -50°C. l4CO2 incorporation after freezing to -20°C was unaffected in H. antarctica or in Trebouxia sp. but slightly depressed in Stichococcus sp. (isolated from a less extreme Antarctic habitat). These results suggest that the freezing regime in the Antarctic desert is not injurious to endolithic algae. It is likely that the freezing-point depression inside the rock makes available liquid water for metabolic activity at subzero temperatures. Freezing may occur more frequently on the rock surface and contribute to the abiotic nature of the surface.

Incorporation of fiber in cereals may lead to quality issues, thus decreasing consumer acceptance. This is partially due to deterioration of the microstructure, one of the primary quality attributes of cereals. The objective of this study was to better understand the mechanisms by which dietary fibers affect the quality of cereal products during extrusioncooking. The study quantified the effect of amount and type of fiber and whole grain on (i) texture, (ii) structure, and (iii) rehydration properties of extruded cereals. New innovative methods were applied and combined with traditional techniques to characterize both the structure and the rehydration properties. Extruded cereals were produced using a starch-based recipe (whole and wheat flours) and two sources of fibers (oat bran concentrate and wheat bran). The oat and wheat bran levels used in this study were 0, 10, and 20%. The different mixtures were extruded in a pilot twinscrew extruder BC21 (Clextral) and then sugar coated after drying. Mechanical properties of extruded cereals were investigated by compression test. The cellular structure was observed by X-ray tomography. The quality of coating (thickness, homogeneity) was analyzed by optical coherence tomography. The rehydration properties of such cereals in milk were evaluated by magnetic resonance imaging and optical coherence tomography. This work revealed that structure assessment of extruded cereals may lead to a better understanding of the effect of fiber addition on texture and rehydration properties. The application of innovative methods, such as optical coherence tomography and magnetic resonance imaging, was found to be useful to quantify the structural properties.

El diagnóstico de las estructuras internas de los PWR, en particular del barrilete del núcleo y su soporte se pueden realizar por medio del análisis de las señales de los detectores de neutrones extra-nucleares. Se han elaborado varios procedimientos que se han usado en diversas plantas en todo el mundo [1], [2]. El objetivo es la vigilancia de la integridad de la estructura del núcleo y la detección temprana y la cuantificación de signos de fatiga, desgaste, etc en las diferentes estructuras tales como el muelle, la placa de sujeción del barrilete del núcleo, etc. Esta vigilancia se ha venido realizando en las tres unidades PWR 2, 3 y 4 de la central sueca de Ringhals desde 1970. Durante las últimas dos décadas el trabajo se ha llevado a cabo en el contexto de un contrato de colaboración entre la Universidad de Chalmers y Ringhals. Esta actividad de colaboración ha consistido tanto en el desarrollo de nuevos métodos, la mejora de éstos así como su aplicación continuada para diagnóstico, vigilancia, incluyendo un análisis de tendencia a lo largo del tiempo. Este trabajo describe el desarrollo realizado en los últimos años con un énfasis especial en los tres últimos.

Surveillance of core barrel vibrations has been performed in the Swedish Ringhals PWRs for several years. This surveillance is focused mainly on the pendular motion of the core barrel, which is known as the beam mode. The monitoring of the beam mode has suggested that its amplitude increases along the cycle and decreases after refuelling. In the last 5 years several measurements have been taken in order to understand this behaviour. Besides, a non-linear fitting procedure has been implemented in order to better distinguish the different components of vibration. By using this fitting procedure, two modes of vibration have been identified in the frequency range of the beam mode. Several results coming from the trend analysis performed during these years indicate that one of the modes is due to the core barrel motion itself and the other is due to the individual flow induced vibrations of the fuel elements. In this work, the latest results of this monitoring are presented.
