Objectives. To evaluate the diagnostic value of intraoral palpation at the lateral pterygoid (LP) area as part of the physical examination to detect myofascial pain, according to modified research diagnostic criteria for temporomandibular disorders. Study design. Fouty-four women composed the myofascial pain group, and 33 symptom-free age-matched were the control group. One examiner calibrated and blinded to group distribution performed 2 intraoral bilateral palpations of the lateral pterygoid. Results. The LP area palpation showed sensitivity and specificity values of 79.55% and 77.27%, respectively, and positive and negative likelihood ratios of 3.50 and 0.26, respectively. Conclusions. Palpation at the LP area did not reach acceptable values of specificity, and care must be taken when judging positive response to this procedure.

Objective. The aim of this study was to identify the behavior of masticatory muscles after fractures of the zygomatico-orbital complex (ZOC) and subsequent surgical treatment, by using analyses of bite force, electromyography (EMG), and mandible mobility during a 6-month period after surgery. Study design. Five patients with fractured ZOCs treated surgically by using an intraoral approach and fixation exclusively in the region of the zygomaticomaxillary buttress were evaluated. The control group included 12 other patients. During postoperative follow-up, bite force, mandible mobility, and EMG analysis of the masticatory muscles were evaluated. Results. There was an increase in bite force with time, but a decline in EMG activity during the same period. In the mandible mobility analysis, only maximum mouth-opening values increased significantly after the surgical treatment. Conclusions. The masticatory musculature, according to bite force and EMG, returned to its normal condition by the second month after surgery, and maximum mouth opening was observed after the first month. Objectives: This triple-blind, 2 x 2 crossover in situ study, was undertaken to verify whether the wear resistance of enamel and root dentine would be affected by bleaching with a 10% carbamide peroxide agent and a placebo agent. Methods: Thirty slabs of each. substrate (2 mm x 3 mm x 2 mm) were selected for each phase, after flattening and polishing procedures and microhardness test. After a 7-day lead-in period, one specimen of each substrate was randomly bonded on the facial surface of each one of 30 subject`s upper second premolars. The volunteers received instructions on how to perform toothbrushing and application of gel in the tray. Fifteen volunteers bleached their maxillary arch with a 10% carbamide peroxide bleaching agent for a 2-week period, while the remainders used a placebo agent. After a 1-week washout period, a new set of enamel and root dentine slabs were bonded to the premolars and volunteers were crossed over to the alternate agent for 14 days. The resistance of enamel and root dentine to wear following bleaching, toothbrushing and intraoral exposure was measured with a profilometer, using reference areas. Results: For enamel, ANOVA did not demonstrate significant difference between wear provided by placebo and bleaching agent (p = 0.3713), but higher wear depth was observed for bleached root dentine (p = 0.0346). Conclusions: While overnight bleaching caused no alteration in wear resistance of enamel, root dentine showed increased tissue loss. (C) 2008 Elsevier Ltd. All rights reserved.

Background: Several studies link the seamless fit of implant-supported prosthesis with the accuracy of the dental impression technique obtained during acquisition. In addition, factors such as implant angulation and coping shape contribute to implant misfit. Purpose: The aim of this study was to identify the most accurate impression technique and factors affecting the impression accuracy. Material and Methods: A systematic review of peer-reviewed literature was conducted analyzing articles published between 2009 and 2013. The following search terms were used: implant impression, impression accuracy, and implant misfit.A total of 417 articles were identified; 32 were selected for review. Results: All 32 selected studies refer to in vitro studies. Fourteen articles compare open and closed impression technique, 8 advocate the open technique, and 6 report similar results. Other 14 articles evaluate splinted and non-splinted techniques; all advocating the splinted technique. Polyether material usage was reported in nine; six studies tested vinyl polysiloxane and one study used irreversible hydrocolloid. Eight studies evaluated different copings designs. Intraoral optical devices were compared in four studies. Conclusions: The most accurate results were achieved with two configurations: (1) the optical intraoral system with powder and (2) the open technique with splinted squared transfer copings, using polyether as impression material.

ABSTRACT OBJECTIVE To identify the factors associated with severity of malocclusion in a population of adolescents. METHODS In this cross-sectional population-based study, the sample size (n = 761) was calculated considering a prevalence of malocclusion of 50.0%, with a 95% confidence level and a 5.0% precision level. The study adopted correction for the effect of delineation (deff = 2), and a 20.0% increase to offset losses and refusals. Multistage probability cluster sampling was adopted. Trained and calibrated professionals performed the intraoral examinations and interviews in households. The dependent variable (severity of malocclusion) was assessed using the Dental Aesthetic Index (DAI). The independent variables were grouped into five blocks: demographic characteristics, socioeconomic condition, use of dental services, health-related behavior and oral health subjective conditions. The ordinal logistic regression model was used to identify the factors associated with severity of malocclusion. RESULTS We interviewed and examined 736 adolescents (91.5% response rate), 69.9% of whom showed no abnormalities or slight malocclusion. Defined malocclusion was observed in 17.8% of the adolescents, being severe or very severe in 12.6%, with pressing or essential need of orthodontic treatment. The probabilities of greater severity of malocclusion were higher among adolescents who self-reported as black, indigenous, pardo or yellow, with lower per capita income, having harmful oral habits, negative perception of their appearance and perception of social relationship affected by oral health. CONCLUSIONS Severe or very severe malocclusion was more prevalent among socially disadvantaged adolescents, with reported harmful habits and perception of compromised esthetics and social relationships. Given that malocclusion can interfere with the self-esteem of adolescents, it is essential to improve public policy for the inclusion of orthodontic treatment among health care provided to this segment of the population, particularly among those of lower socioeconomic status.

Introduction: Rotenone is a botanical pesticide derived from extracts of Derris roots, which is traditionally used as piscicide, but also as an industrial insecticide for home gardens. Its mechanism of action is potent inhibition of mitochondrial respiratory chain by uncoupling oxidative phosphorylation by blocking electron transport at complex-I. Despite its classification as mild to moderately toxic to humans (estimated LD50, 300-500 mg/kg), there is a striking variety of acute toxicity of rotenone depending on the formulation (solvents). Human fatalities with rotenone-containing insecticides have been rarely reported, and a rapid deterioration within a few hours of the ingestion has been described previously in one case. Case report: A 49-year-old Tamil man with a history of asthma, ingested 250 mL of an insecticide containing 1.24% of rotenone (3.125 g, 52.1-62.5 mg/kg) in a suicide attempt at home. The product was not labeled as toxic. One hour later, he vomited repeatedly and emergency services were alerted. He was found unconscious with irregular respiration and was intubated. On arrival at the emergency department, he was comatose (GCS 3) with fixed and dilated pupils, and absent corneal reflexes. Physical examination revealed hemodynamic instability with hypotension (55/30 mmHg) and bradycardia (52 bpm). Significant laboratory findings were lactic acidosis (pH 6.97, lactate 17 mmol/L) and hypokalemia (2 mmol/L). Cranial computed tomography (CT) showed early cerebral edema. A single dose of activated charcoal was given. Intravenous hydration, ephedrine, repeated boli of dobutamine, and a perfusor with 90 micrograms/h norepinephine stabilized blood pressure temporarily. Atropine had a minimal effect on heart rate (58 bpm). Intravenous lipid emulsion was considered (log Pow 4.1), but there was a rapid deterioration with refractory hypotension and acute circulatory failure. The patient died 5h after ingestion of the insecticide. No autopsy was performed. Quantitative analysis of serum performed by high-resolution/accurate mass-mass spectrometry and liquid chromatography (LC-HR/AM-MS): 560 ng/mL rotenone. Other substances were excluded by gas chromatography-mass spectrometry (GC-MS) and liquid chromatography-mass spectrometry (LC-MS/MS). Conclusion: The clinical course was characterized by early severe symptoms and a rapidly fatal evolution, compatible with inhibition of mitochondrial energy supply. Although rotenone is classified as mild to moderately toxic, physicians must be aware that suicidal ingestion of emulsified concentrates may be rapidly fatal. (n=3): stridor, cyanosis, cough (one each). Local swelling after chewing or swallowing soap developed at the earliest after 20 minutes and persisted beyond 24 hours in some cases. Treatment with antihistamines and/or steroids relieved the symptoms in 9 cases. Conclusion: Bar soap ingestion by seniors carries a risk of severe local reactions. Half the patients developed symptoms, predominantly swellings of tongue and/or lips (38%). Cognitive impairment, particularly in the cases of dementia (37%), may increase the risk of unintentional ingestion. Chewing and intraoral retention of soap leads to prolonged contact with the mucosal membranes. Age-associated physiological changes of oral mucosa probably promote the irritant effects of the surfactants. Medical treatment with antihistamines and corticosteroids usually leads to rapid decline of symptoms. Without treatment, there may be a risk of airway obstruction.

Oral mucosa is a frequent site of primary herpes simplex virus type 1 (HSV-1) infection, whereas intraoral recurrent disease is very rare. Instead, reactivation from latency predominantly results in asymptomatic HSV shedding to saliva or recurrent labial herpes (RLH) with highly individual frequency. The current study aimed to elucidate the role of human oral innate and acquired immune mechanisms in modulation of HSV infection in orolabial region. Saliva was found to neutralize HSV-1, and to protect cells from infection independently of salivary antibodies. Neutralization capacity was higher in saliva from asymptomatic HSV-seropositive individuals compared to subjects with history of RLH or seronegative controls. Neutralization was at least partially associated with salivary lactoferrin content. Further, lactoferrin and peroxidase-generated hypothiocyanite were found to either neutralize HSV-1 or interfere with HSV-1 replication, whereas lysozyme displayed no anti-HSV-1 activity. Lactoferrin was also shown to modulate HSV-1 infection by inhibiting keratinocyte proliferation. RLH susceptibility was further found to be associated with Th2 biased cytokine responses against HSV, and a higher level of anti- HSV-IgG with Th2 polarization, indicating lack of efficiency of humoral response in the control of HSV disease. In a three-dimensional cell culture, keratinocytes were found to support both lytic and nonproductive infection, suggesting HSV persistence in epithelial cells, and further emphasizing the importance of peripheral immune control of HSV. These results suggest that certain innate salivary antimicrobial compounds and Th1 type cellular responses are critically important in protecting the host against HSV disease, implying possible applications in drug, vaccine and gene therapy design.

For oral rehabilitation with implant-supported prostheses, there are required procedures to create the bone volume needed for installation of the implants. Thus, bone grafts from intraoral or extraoral donor sites represent a very favorable opportunity. This study aimed to review the literature on the subject, seeking to discuss parameters for the indications, advantages and complications of techniques for autogenous bone grafts.

Objectif: L'objectif primaire de cette étude était d'évaluer la validité, la fiabilité et la reproductibilité des mesures dentaires obtenues sur les modèles digitaux iTero (Align Technology, San Jose, Californie) et Unitek TMP Digital (3M, Monrovia, Californie) en comparaison avec celles obtenues sur les modèles de plâtre (gold standard). L'objectif secondaire était de comparer les deux différents matériaux à empreinte (l'alginate et le polyvinylsiloxane-PVS) afin de déterminer si le choix du matériau affectait la précision des mesures. Méthodes: Le premier volet de l'étude impliquait les modèles digitaux Unitek et iTero, obtenus à partir de 25 paires de modèles de plâtre choisis de façon randomisée et provenant de la pratique privée d'un des co-auteurs. Des empreintes d'alginate et de PVS ont été prises sur les modèles de plâtre et numérisées par le scanner Unitek. Les modèles ont ensuite été numérisés avec le scanner iTero. Le deuxième volet de l'étude cherchait à comparer les modèles digitaux iTero (numérisation intra-orale) avec les modèles de plâtre (empreintes d'alginate et de PVS) obtenus à partir de 25 patients de la clinique d'orthodontie de l'Université de Montréal ayant besoin d'un traitement orthodontique. Dans les deux volets de l'étude, deux auteurs ont pris les mesures suivantes sur les différents modèles: largeur mésio-distale de chaque dent de la première molaire à l'autre première molaire, le périmètre d'arcade, les distances intermolaire et intercanine, le surplomb vertical et le surplomb horizontal. Les ratios et excès Bolton 6 et 12, l'espace requis et les différentiels d'espace au maxillaire et à la mandibule, ont été calculés. Résultats: La fiabilité (ICC) entre les modèles digitaux (Unitek et iTero) et les modèles de plâtre était bonne à excellente pour toutes les mesures [ICC=0,762–0,998], et la fiabilité entre les deux matériaux à empreinte était excellente [ICC=0,947–0,996]. Dans les deux volets de l'étude, les mesures faites sur les modèles iTero étaient généralement plus grandes que celles faites sur les modèles de plâtre. Les plus grandes différences moyennes pour la comparaison iTero-plâtre étaient trouvées au niveau de l'espace disponible au maxillaire et à la mandibule (systématiquement plus grande pour cette variable), soit 2,24 mm et 2,02 mm respectivement dans le premier volet, et 1,17 mm et 1,39 mm respectivement dans le deuxième volet. Les différences étaient considérées cliniquement non significatives pour toutes les variables. La reproductibilité intra-examinateur était bonne à excellente pour les modèles de plâtre et les modèles digitaux, à l'exception du différentiel d'espace à la mandibule pour les modèles Unitek [ICC=0,690-0,692]. La reproductibilité inter-examinateur était bonne à excellente pour les modèles de plâtre et les modèles digitaux dans les deux volets de l'étude, mais acceptable à modérée pour les modèles Unitek au niveau des analyses Bolton 6 et 12, et des différentiels d'espace au maxillaire et à la mandibule [ICC=0,362-0,548]. Conclusions: La précision et la fiabilité des mesures dentaires faites sur les modèles digitaux Unitek et iTero étaient cliniquement acceptables et reproductibles en comparaison avec les celles faites sur les modèles de plâtre. Le choix de matériel à empreinte entre l'alginate et le PVS n'affectait pas la précision des mesures. Cette étude semble démontrer que les modèles digitaux Unitek et iTero, utilisés avec leur logiciel respectif, sont une alternative fiable et reproductible aux modèles de plâtre pour le diagnostic et l’analyse des modèles orthodontiques.

A gengivo-estomatite crónica felina é uma inflamação complexa crónica, com severidade e intensidade variáveis. Apesar de não estar definida a sua etiopatogenia, parece haver uma relação entre a inflamação e a ocorrência de lesões de reabsorção dentária, enquanto causa ou enquanto consequência da doença. O tratamento para as duas doenças é inespecífico, mas baseia-se na extração dentária, contornada ou não com tratamentos médicos. Este estudo teve como objetivo determinar a ocorrência de lesões de reabsorção dentária em gatos com gengivo-estomatite crónica e avaliar a existência de uma possível associação entre um padrão de estomatite crónica e a presença de lesões de reabsorção dentária. O objetivo secundário consistiu na determinação da percentagem de sucesso e o grau de satisfação dos proprietários, após a intervenção cirúrgica. Foram incluídos no estudo 27 gatos. Os critérios de inclusão consistiram no diagnóstico de genvivo-estomatite crónica, realização de um exame radiográfico intraoral completo de todos os dentes, seguido de tratamento cirúrgico, com extrações dentárias e, finalmente, a resposta, por parte dos proprietários, a um questionário. A ocorrência de lesões de reabsorção dentária neste estudo foi de 66,67%. Não foi possível estabelecer nenhuma associação entre a gengivo-estomatite crónica felina e o desenvolvimento de lesões de reabsorção dentária. Os padrões ulcerativos, proliferativos e o de estomatite caudal na gengivo-estomatite crónica felina mostraram risco acrescido para lesões de reabsorção dentária, mas sem significado estatístico. 70,37% dos animais atingiu a cura clínica e 29,63% obteve melhoria global, num período médio de 2 meses. O grau de satisfação dos proprietários obteve uma média de 4,52 valores, numa escala de 1 a 5. Apesar da prevalência elevada de lesões de reabsorção dentária, não foi possível identificar a gengivo-estomatite crónica felina, enquanto fator de risco para a sua ocorrência. À semelhança de estudos anteriores, a gengivo-estomatite crónica felina responde a tratamento cirúrgico com extrações dentárias.

Distalization of maxillary molars is indicated for correction of Class II dental malocclusion and for space gain in cases of space deficiency. The ideal treatment with an intraoral fixed appliance for molar distalization should fulfill the following requirements: patient compliance; acceptable esthetics; comfort; minimum anterior anchor loss (as evidenced by inclination of incisors); bodily movement of the molars to avoid undesirable effects and unstable outcomes; and minimum time required during sessions for placement and activations. The purpose of this paper was to present an alternative treatment for space recovery in the area of the maxillary right second premolar when there has been significant mesial movement of the permanent maxillary right first molar. We used a modified appliance that allows unilateral molar distalization in cases of unilateral tooth/arch size discrepancy using the opposite side as anchor, thus reducing the mesialization of the anterior teeth. This study sought to evaluate the microhardness of root dentine adjacent to glass-ionomer and composite resin restorations after erosive challenge. A crossover study was performed in two phases of 4 consecutive days each. One hundred twelve bovine root dentine slabs were obtained, and standardized box-shaped cavities were prepared at center of each specimen. The prepared cavities were randomly restored with glass-ionomer cement or composite resin. The slabs were randomly assigned among 14 volunteers, which wore intraoral palatal device containing four restored root dentin slabs. Starting on the second day, half of the palatal acrylic devices were immersed extraorally in a lemonade-like carbonated soft drink for 90 s, four times daily for 3 days. Alter 3-day wash-out, dentine slabs restored with the alternative material were placed into palatal appliance and the volunteers started the second phase of this study. After erosive challenges. microhardness measurements were performed. Regardless of the restorative material employed, eroded specimens demonstrated lower microhardness value (p < 0.0001). At eroded condition examined in this study, dentine restored with glass-ionomer cement showed higher microhardness values (p < 0.0001). It may be concluded that the glass-ionomer cement decreases the progression of root dentine erosion at restoration margin. Objective: To evaluate the presence of enamel alterations in deciduous maxillary central incisors of infants with unilateral cleft lip and alveolar ridge, with or without cleft palate, and to compare the occurrence and location of these alterations between the central incisor adjacent to the cleft and the contralateral incisor. Design: Intraoral clinical examination was performed after tooth cleaning and drying by a single examiner with the aid of a dental mirror, dental probe, and artificial light, with the child positioned on a dental chair. The defects were recorded in a standardized manner according to the criteria of the Modified Developmental Defects of Enamel Index. Setting: Hospital for Rehabilitation of Craniofacial Anomalies (HRAC) at Bauru, Sao Paulo, Brazil. Patients: One hundred one infants were evaluated. All were white, of both genders, aged 12 to 36 months and had at least two thirds of the crowns of maxillary incisors erupted. Results: Demarcated opacity was the most common defect at both cleft and noncleft sides, followed by diffuse opacity. The occurrence of hypoplasia at the cleft side was 11.8%. Most defects affected less than one third of the crown. Conclusion: The occurrence of enamel defects in deciduous maxillary central incisors of patients with unilateral cleft lip was 42.6%, mainly affecting the cleft side as to both number and severity.

O presente estudo teve por objetivo avaliar se a aplicação tópica de flúor fosfato acidulado (FFA) em alta concentração tem efeito adicional no controle de lesões de esmalte, comparado ao uso de dentifrício fluoretado (baixa concentração). A freqüência de FFA como auxiliar no tratamento de lesões de cárie e a deposição de flúor no esmalte após diferentes aplicações de flúor am alta e baixa concentração também foram avaliadas. Para tanto, 5 indivíduos utilizaram, por 42 dias, próteses parciais removíveis inferiores contendo blocos de esmalte bovino desmineralizados. Os espécimes de esmalte forma divididos em 5 grupos: (1) escovação 3 vezes ao dia com dentifrício fluoretado (DF) (1100 ppm F), (2) DF+1 aplicação tópica FFA (12300 ppm F), (3) DF+2 FFA, (4) DF+3 FFA, e 5) DF+4 FFA. O intervalo entre as aplicações foi de uma semana. Cinco blocos hígidos e 5 blocos desmineralizados foram utilizados como controle e não foram submetidos ao período intraoral. As alterações clínicas foram registradas com relação à textura, coloração e brilho superficiais. Análises de microdureza superficial (MS) e em cortes longitudinais (MCL), de rugosidade superficial (RS) e de conteúdo de flúor depositado no esmalte foram realizadas. Modificações clínicas semelhantes de coloração foram observadas em todos os grupos após formação da lesão in vitro, apesar da ausência de mudanças na textura e brilho superficiais. Após escovação e tratamento com flúor, todos os blocos desmineralizados (esbranquiçados), independentemente do tratamento, tornaram-se mais amarelados. Não foram detectadas mudanças na textura e brilho superficiais. Os valores de MS e de conteúdo de flúor aumentaram (p<0,05) em relação aos blocos demineralizados somente a partir de 2 FFA. Os valores de MCL não mostraram diferenças entre os blocos tratados e os desmineralizados em qualquer distância da superfície do esmalte. Os grupos DF+3 FFA e DF+ 4 FFA foram os únicos capazes de aumentar os valores de MS em relação aos blocos desmineralizados. Estes tratamentos levaram a um aumento significativo de flúor solúvel e insolúvel comparado aos espécimes hígidos e desmineralizados. Ainda que todas as lesões tenham sido controladas clinicamente e não mostrem diferenças de microdureza, parece que aquelas tratadas com maior número de FFA produziram um maior reservatório de flúor disponível para inibir novos processos de desmineralização.

Introduction: Mouth cancer is classified as having one of the ten highest cancer incidences in the world. In Brazil, the incidence and mortality rates of oral cancer are among the highest in the world. Intraoral cancer (tongue, gum, floor of the mouth, and other non-specified parts of the mouth), the accumulated survival rate after five years is less than 50%. Objectives: Estimate the accumulated survival probability after five years and adjust the Cox regression model for mouth and oropharyngeal cancers, according to age range, sex, morphology, and location, for the city of Natal. Describe the mortality and incidence coefficients of oral and oropharyngeal cancer and their tendencies in the city of Natal, between 1980 and 2001 and between 1997 and 2001, respectively. Methods: Survival data of patients registered between 1997 and 2001 was obtained from the Population-based Cancer Record of Natal. Differences between the survival curves were tested using the log-rank test. The Cox proportional risk model was used to estimate risk ratios. The simple linear regression model was used for tendency analyses of the mortality and incidence coefficients. Results: The probability after five years was 22.9%. The patients with undifferentiated malignant neoplasia were 4.7 times more at risk of dying than those with epidermoid carcinoma, whereas the patients with oropharyngeal cancer had 2.0 times more at risk of dying than those with mouth cancer. The mouth cancer mortality and incidence coefficients for Natal were 4.3 and 2.9 per 100 000 inhabitants, respectively. The oropharyngeal cancer mortality and incidence coefficients were, respectively, 1.1 and 0.7 per 100 000 87 inhabitants. Conclusions: A low survival rate after five years was identified. Patients with oropharyngeal cancer had a greater risk of dying, independent of the factors considered in this study. Also independent of other factors, undifferentiated malignant neoplasia posed a greater risk of death. The magnitudes of the incidence coefficients found are not considered elevated, whereas the magnitudes of the mortality coefficients are high
