Esse trabalho compreende dois diferentes estudos de caso: o primeiro foi a respeito de um medicamento para o qual foi desenvolvida uma metodologia para determinar norfloxacino (NOR) por espectrofluorimetria molecular e validação por HPLC. Primeiramente foi desenvolvida uma metodologia por espectrofluorimetria onde foram feitos alguns testes preliminares a fim de estabelecer qual valor de pH iria fornecer a maior intensidade de emissão. Após fixar o pH foi feita a determinação de NOR em padrões aquosos e soluções do medicamento usando calibração univariada. A faixa de concentração trabalhada foi de 0500 μg.L-1. O limite de detecção para o medicamento foi de 6,9 μg.L-1 enquanto que o de quantificação foi de 24,6 μg.L-1. Além dessas, outras figuras de mérito também foram estimadas para desenvolvimento da metodologia e obtiveram resultados muito satisfatórios, como por exemplo, os testes de recuperação no qual a recuperação do analito foi de 99.5 a 103.8%. Para identificação e quantificação do NOR da urina foi necessário diluir a amostra de urina (estudada em dois diferentes níveis de diluição: 500 e 1000 x) e também uso do método da adição de padrão (na mesma faixa de concentração usada para medicamento). Após a aquisição do espectro, todos foram usados para construção do tensor que seria usado no PARAFAC. Foi possível estimar as figuras de mérito como limite de detecção de 11.4 μg.L-1 and 8.4 μg.L-1 (diluição de 500 e 1000 x respectivamente) e limite de quantificação de 34 μg.L-1 e 25.6 μg.L-1 (diluição de 500 x e 1000 x respectivamente). O segundo estudo de caso foi na área alimentícia no qual se usou espectroscopia NIR e FT MIR acopladas a quimiometria para discriminar óleo de soja transgênica e não transgênica. Os espectros dos óleos não mostraram diferença significativa em termos visuais, sendo necessário usar ferramentas quimiométricas capazes de fazer essa distinção. Tanto para espectroscopia NIR quanto FT MIR foi feito o PCA a fim de identificar amostras discrepantes e que influenciariam o modelo de forma negativa. Após efetuar o PCA, foram usadas três diferentes técnicas para discriminar os óleos: SIMCA, SVM-DA e PLS-DA, sendo que para cada técnica foram usados também diferentes pré processamento. No NIR, apenas para um pré processamento se obteve resultados satisfatórios nas três técnicas, enquanto que para FT-MIR ao se usar PLS-DA se obteve 100% de acerto na classificação para todos os pré processamentos

Esta dissertação procura investigar os impasses enfrentados pelos profissionais que trabalham na porta de entrada de um ambulatório público de saúde mental. Inicialmente, buscamos estabelecer as bases teóricas para uma clínica da recepção que alie os pressupostos da lógica da atenção psicossocial à clínica do sujeito, a partir do trabalho desenvolvido neste ambulatório. A escuta do sujeito serve de bússola para o acolhimento do usuário e os direcionamentos posteriores, pautados na ética da psicanálise. Verifica-se a importância da disponibilidade do profissional para acolher o usuário e aguardar um tempo necessário para que o endereçamento de uma demanda possa ocorrer. São abordados fragmentos de casos clínicos que demonstram que a aposta no vínculo transferencial tem se mostrado importante para a decisão sobre o encaminhamento dos casos. Neste sentido, procura-se discutir a importância que tem, para a recepção, a interlocução entre os membros da equipe e a articulação com os diversos dispositivos da rede de saúde. Esta discussão permite uma reflexão sobre as mudanças necessárias na prática que se desenrola no ambulatório de modo a não reproduzir práticas do antigo modelo hospitalocêntrico e medicalizador, mostrando sua importância em uma rede, que é criada a cada vez em torno do usuário

With the wide application of rare earth in agriculture, medicament, especially the application of Gd-DTPA as nuclear magnetic resonance image reagent in clinical practice([1]), the studies on the toxicology in biological body, as well as the study on the use as informative probes instead of divalent calcium ion in biological and biochemical research have attracted intensive concern([2]). Phospholipids bilayers have served as a model of biomembrane in the last two decades. The effects of metal ions on the conformation of polar headgroup of dipalmitoylphosphatidylcholine (DPPC) bilayers have been reported([3]). Sphingomyelin is major component of several biological tissues such as brain and nerve cells and has identical polar headgroup to DPPC. The interaction of metal ions with sphingomyelin bilayer remains nonrevealed. This note presents the results of the study on this aspect.

Endodontic lubricants, irrigating solutions and medicaments help reduce the microbial load within root canals. Primary and secondary cases involve different microbes. Each'solution'or combinations thereof could play a significant role but no detailed guidelines exist on their use. An audit was undertaken to compare current practice in Belfast Dental School to the others across the UK and Republic of Ireland (ROI). This audit highlighted three main differences between Belfast and other dental schools. Many other institutions utilized other irrigants besides sodium hypochlorite (NaOCl), different intracanal medicaments, including calcium hydroxide, and higher concentrations of NaOCl. Having gathered this information, we ask, 'Is there sufficient evidence to change the endodontic regime currently used at Belfast Dental School?'. Using the findings from the literature review (Part 1), we introduce new evidence-based protocols for primary and secondary cases for use in Belfast Dental School. Clinical Relevance: In the absence of detailed clinical guidelines on the use of endodontic lubricants, irrigants and medicaments in primary and secondary cases, it is important to be aware of current practice in UK and ROI dental schools where dentists and specialists are trained.

Endodontic lubricants, irrigants and medicaments help prepare and disinfect root canal systems (RCS) but primary and secondary cases involve different microbes and therefore it is unlikely that one protocol will be effective for both case types. Each individual 'solution' or sequence of 'solutions' could play a significant role in each case type, but there are no detailed published guidelines in existence. To help inform clinical practice it was decided to undertake a literature review followed by a UK and Republic of Ireland wide audit on current endodontic 'solution' usage within dental schools. The literature review was undertaken under the following headings: pre-op oral rinse; file lubricants; root canal irrigants and intracanal medicaments and provides an evidence base for protocol development for both primary and retreatment cases. The audit project and the protocols developed from the findings of both the literature review and audit will be presented in Part 2.

Las infecciones respiratorias altas y bajas son una causa común de morbimortalidad infantil. Se ha propuesto el uso de los lisados bacterianos para prevenir las infecciones recurrentes sin embargo su uso aún se considera controversial. Metodología: Se realizó una revisión sistemática de la literatura. La búsqueda se realizó a través de las bases de datos PUBMED, Embase, Ovid, LiLaCS y Cochrane library plus. Se incluyeron metanálisis publicados en idiomas inglés y español, entre los años 1998 y 2012. Se realizó una evaluación de calidad siguiendo la estrategia Quorum y un análisis cualitativo y cuantitativo de los resultados. Resultados: Se incluyeron 4 revisiones sistemáticas de la literatura con metanálisis. Fue apreciable la disminución de las recurrencias de las infecciones respiratorias relacionadas con el uso de los lisados bacterianos. Los lisados bacterianos disminuyen la necesidad de uso de antibióticos. No se encontró evidencia sobre el uso de los lisados sobre desenlaces como la necesidad de intervenciones adicionales, tiempo de hospitalización, costo relacionado con la atención en salud. No se reportaron eventos adversos de importancia. Conclusión: Los lisados bacterianos son eficaces en disminuir la recurrencia de las infecciones respiratorias en pacientes en edad pediátrica.

P>Aim To investigate the antibacterial effect of Tetraclean, MTAD and five experimental irrigants using both direct exposure test with planktonic cultures and mixed-species in vitro biofilm model. Methodology Tetraclean, MTAD and five experimental solutions that were modifications of existing formulae including MTAD + 0.01% cetrimide (CTR), MTAD + 0.1% CTR, MTAC-1 (Tween 80 replaced by 0.01% CTR in MTAD), MTAC-2 (Tween 80 replaced by 0.1% CTR) and MTAD-D (MTAD without the Tween 80 and no CTR added) were used as disinfectants in the experiments. In the direct exposure test, a suspension of Enterococcus faecalis was mixed with each of the solutions. After 0.5, 1, 3 and 10 min, an inactivator was added and the number of surviving bacteria was calculated. A mixed-species biofilm from subgingival plaque bacteria was grown in brain heart infusion broth in anaerobic conditions on synthetic hydroxyapatite discs. Two-week-old biofilms were exposed to the solutions for 0.5, 1 and 3 min. The samples were observed by confocal laser scanning microscopy after bacterial viability staining. The scans were quantitatively analysed, and the volume of killed cells of all cells was calculated for each medicament. Results Tetraclean and MTAC-2 (0.1% CTR) killed planktonic E. faecalis in < 30 s. Complete killing of bacteria required 1 min by MTAC-1, 3 min by MTAD + 0.1% CTR and 10 min by MTAD, MTAD-D and MTAD + 0.01% CTR. In the biofilm test, there were significant differences in microbial killing between the different solutions and times of exposure (P < 0.005). MTAC-2 showed the best performance, killing 71% of the biofilm bacteria in 3 min, followed by MTAC-1 and Tetraclean. MTAD and the three MTAD modifications demonstrated the lowest antibacterial activity. Conclusion Tetraclean was more effective than MTAD against E. faecalis in planktonic culture and in mixed-species in vitro biofilm. CTR improved the antimicrobial properties of the solutions, whereas Tween 80 seemed to have a neutral or negative impact on their antimicrobial effectiveness.

Introduction: The aim of the present study was to determine the disinfection of preparations carried out by using the Protaper or MTwo system in canals infected with Enterococcus faecalis. Methods: Twenty-eight distobuccal canals of upper molars were used, in which the canals were sterilized after being enlarged to #20 file and then contaminated with an inoculation of a culture of E. faecalis. After the incubation period, bacterial samples were collected and were seeded on plates for analysis of colony-forming units (CFU)/mL. The teeth were divided into 2 groups according to the rotary system used for instrumentation; 2 noninstrumented teeth served as the control group. Then bacterial samples were collected and were seeded on plates for analysis of CFU/mL again. The data obtained were evaluated by the Wilcoxon and Mann-Whitney U tests. Results: Bacterial reduction was 81.94% and 84.29%, respectively, in Pro Taper and Mtwo systems, and there was no statistically significant difference (P > .05). Conclusions: Both systems, Pro Taper and Mtwo, reduced the amount of bacteria in the mechanical disinfection of the root canal system, demonstrating that they are suitable for this purpose. The pharmaceutical industry is a segment that is dominated by transnational companies. This characteristic is visible in Brazil, where there are only three domestic laboratories in the top ten pharmaceutical companies list. From the 1970s onwards, the world pharmaceutical market went through deep changes caused mainly to the increase of regulatory control and patent expiration that led to the advent of generic medicaments. In 1999, it was published the law that allowed the introduction of generic medicaments in Brazil, creating a new market that is currently dominated by domestic laboratories. This dissertation proposes to identify the reasons for a subsidiary of foreign laboratory does not achieve the leadership of this market. The literature is based on international political economy and international business concepts that means, relations between subsidiary and head office, domestic and foreign companies and government. Four propositions are presented and tested through multiple sources of evidences. The empirical research was mainly grounded in interviews with key persons and participant observation that allowed access to information, which would be not available for scientific investigation. The results indicate that the relationships considered in this study affect the subsidiary performance in generic medicament market in regard to its ability to make decisions, price competitiveness and wide portfolio creation. The respondents considered those three aspects as success critical factors. The results can be used for future research, aiming to wide the focus of the study about domestic laboratory dominance in generic medicament markets. Another suggestion is to follow this market evolution regarding to the internationalization process of domestic laboratories.

The frequency of disseminated candidiasis caused by yeast has enhancing in intensive care unit. Despite the availability of new antifungal drugs, C. albicans sepsis mortality causes can be as high as 30-40%. So, it has been needed to looking for a new therapeutic medicament that helps in treatment and prevention of this infection. Previous data that demonstrated that particulated β-glucan stimulates the immune system and experiments of this work were conducted to investigating if β-glucan extracted from Saccharomices cerevisiae, could modified the evolution of mouse model C. albicans systemic infection. Balb/c mice with sepsis and β-1,3 glucan treated or not were analyzed the influence of β-1,3 glucan in survival of the animals, in the fungal burdens in kidney, in the production of urea and TNF even in the histopathology of kidney. The experiments shown that the infected animals a nd glucan treated had great survival (p<0,05), less unit form colony in kidney and normal levels of urea. In the kidney histopathology of not glucan treated animals it has seen more lesions when compared with treated animals. So we conclude that β-1,3 glucan could stimulate the immune system against disseminated C. albicans

Alendronate is a known inhibitor of root resorption and the development of alendronate paste would enhance its utilization as intracanal medication. Therefore, this study aimed to investigate the biocompatibility of experimental alendronate paste in subcutaneous tissue of rats, for utilization in teeth susceptible to root resorption. The study was conducted on 15 male rats, weighing similar to 180-200 grams. The rats' dorsal regions were submitted to one incision on the median region and, laterally to the incision, the subcutaneous tissue was raised and gently dissected for introduction of two tubes, in each rat. The tubes were sealed at one end with gutta-percha and taken as control. The tubes were filled with experimental alendronate paste. The animals were killed at 7, 15 and 45 days after surgery and the specimens were processed in laboratory. The histological sections were stained with hematoxylin-eosin and analyzed by light microscopy. Scores were assigned to the in. ammatory process and statistically compared by the Tukey test (P < 0.05). Alendronate paste promoted severe inflammation process at 7 days, with statistically significant difference compared to the control (P < 0.05%). However, at 15 days, there was a regression of in. ammation and the presence of connective tissue with collagen fibers, fibroblasts and blood vessels was observed. After 45 days, it was observed the presence of well-organized connective tissue, with collagen fibers and fibroblasts, and few in. ammatory cells. No statistical difference was observed between the control and experimental paste at 15 and 45 days. The experimental alendronate paste was considered biocompatible with subcutaneous tissue of rat.

Objective. The aim of this study was to evaluate the influence of apical foramen widening on the healing of chronic periapical lesions in dogs' teeth after root canal filling with Sealer 26 or Endomethasone.Study design. Forty root canals of dogs' teeth were used. After pulp extirpation, the canals were exposed to the oral cavity for 180 days for induction of periapical lesions, and then instrumented up to a size 55 K-file at the apical cemental barrier. In 20 roots, the cemental canal was penetrated and widened up to a size 25 K-file; in the other 20 roots, the cemental canal was preserved (no apical foramen widening). All canals received a calcium hydroxide intracanal dressing for 21 days and were filled with gutta-percha and 1 of the 2 sealers: group 1: Sealer 26/apical foramen widening; group 2: Sealer 26/no apical foramen widening; group 3: Endomethasone/apical foramen widening; group 4: Endomethasone/no apical foramen widening. The animals were killed after 180 days, and serial histologic sections from the roots were prepared for histomorphologic analysis. Scores were assigned according to preestablished histomorphologic parameters and analyzed statistically by Kruskal-Wallis and Mann-Whitney U tests.Results. Regarding new cementum formation, repair of cementum and bone resorption areas, presence of microorganisms, inflammatory cell infiltrate and periodontal ligament conditions, significantly better periapical healing was obtained when foramen widening was done and Sealer 26 was used.Conclusion. Apical foramen widening and calcium hydroxide-containing sealer were more favorable to the healing of chronic periapical lesions. (Oral Surg Oral Med Oral Pathol Oral Radiol Endod 2010;109:932-940)
