INTRODUCTION: Extraoseus osteosarcoma is a rare tumor confined in soft tissues but with histological features similar to bone osteosarcoma. CLINICAL CASE: We report the case of a young male affected extraosseous osteosarcoma. A detailed analysis of the clinical symptoms and evolution from the beginning of the process to the death of the patient, three years later, is performed. DISCUSSION: The different types of treatment and prognostic factors involving this disease are discussed CONCLUSIONS: Radical surgery followed by chemotherapy is the preferred form of treatment in extraosseus osteosarcoma.The latest advances in chemotherapy have improved the prognosis in this type of tumor

O presente estudo foi elaborado no âmbito da disciplina de Seminários Avançados em Enfermagem I e Investigação Científica. Desenvolveu-se uma investigação de cariz qualitativa, cujo objectivo geral foi o de analisar se as intervenções de enfermagem promovem a qualidade de vida do utente amputado o membro inferior no serviço da Cirurgia do Hospital Baptista de Sousa, recorrendo-se para o efeito a entrevistas semiestruturadas e a observação participante a sete enfermeiros do serviço. O conceito amputação veio sofrendo transformações desde a antiguidade, onde a prática do procedimento era feita de forma rudimentar, baseada apenas numa cirurgia sem nenhum cuidado. Surge então o interesse em tentar compreender e identificar o significado da qualidade de vida nas pessoas sujeitas a uma amputação e verificar a interferência dos cuidados sobre a qualidade de vida das mesmas durante o período de internamento na perspectiva do enfermeiro. Várias são as causas que podem levar a amputação do membro inferior e uma das principais é a Diabetes Mellitus, sendo que esta era considerada uma doença dos idosos, porque atingia mais a camada idosa. No entanto o que se verifica é que hoje esta doença tem vindo a atingir cada vez mais os jovens devido aos comportamentos de risco, como o tabagismo, o uso abusivo do álcool, o sedentarismo, entre outros. Com a presente investigação chegou-se a conclusão que de facto os enfermeiros do serviço de Cirurgia detêm conhecimentos técnicos e científicos para cuidar do utente amputado, conhecem os preceitos ligados a temática qualidade de vida, mas no entanto nos cuidados diários não se evidencia uma qualidade máxima na sua prestação. Os cuidados são básicos de forma geral, não havendo uma especificação quando o cuidar é para uma pessoa amputada o membro inferior. Salienta-se também que o espaço do serviço revela-se insuficiente para garantir uma maximização dessa qualidade de vida durante o internamento, bem como os recursos materiais e humanos. Todavia graças ao sistema, designada enfermagem de proximidade, os enfermeiros têm tarefas específicas, acabando por ficar mais tempo junto dos utentes, o que facilita na melhoria futura dessa qualidade de vida dos utentes internados.

PURPOSE: We describe the results of a preliminary prospective study using different recently developed temporary and retrievable inferior vena cava (IVC) filters. METHODS: Fifty temporary IVC filters (Gunther, Gunther Tulip, Antheor) were inserted in 47 patients when the required period of protection against pulmonary embolism (PE) was estimated to be less than 2 weeks. The indications were documented deep vein thrombosis (DVT) and temporary contraindications for anticoagulation, a high risk for PE, and PE despite DVT prophylaxis. RESULTS: Filters were removed 1-12 days after placement and nine (18%) had captured thrombi. Complications were one PE during and after removal of a filter, two minor filter migrations, and one IVC thrombosis. CONCLUSION: Temporary filters are effective in trapping clots and protecting against PE, and the complication rate does not exceed that of permanent filters. They are an alternative when protection from PE is required temporarily, and should be considered in patients with a normal life expectancy.

Las características geoquímicas (elementos mayores y trazas) de las rocas analizadas son similares a las del arco volcánico de Ke rmadec en Pa c í fico SW. Por último, los bajos contenidos en REE, el patrón de REE con morfología plana, así como los bajos contenidos en elementos incompatibles (K, Rb, Zr, Th) son similares a los de las series tipo IAT presentes en el arco volcánico del Caribe. Estos nuevos datos sobre el volcanismo del Paleógeno de la Sierra Maestra sugieren que los modelos de placas tectónicas que han sido propuestos para explicar el origen del arco volcánico de Sierra Maestra deben ser revisados.

PURPOSE: Characterization of persistent diffuse subretinal fluid using optical coherence tomography (OCT) after successful encircling buckle surgery for inferior macula-off retinal detachment in young patients. METHODS: Institutional retrospective review of six young patients (mean age 31 +/- 6 years; five female, one male) with spontaneous inferior rhegmatogenous macula-off retinal detachment. All patients were treated with encircling buckle surgery and five out of six underwent additional external drainage of subretinal fluid. Mean follow-up was 37 +/- 25 months (range 17-75 months) and included complete ophthalmic and OCT examination. RESULTS: At 6 months, 100% of patients showed persistence of subretinal fluid on OCT. Four patients had diffuse fluid accumulation, whereas two patients showed a 'bleb-like' accumulation of fluid. This fluid was present independent of whether or not patients had been treated with external fluid drainage. Subretinal fluid only started to disappear on OCT between 6 and more than 12 months after surgery. CONCLUSION: Young patients with inferior macula-off retinal detachments and a marginally liquefied vitreous may show persisting postoperative subclinical fluid under the macula for longer periods of time than described previously.

El estudio de nuevos insectos cretácicos hallados en los yacimientos de calizas litográficas españoles ha dado como resultado el reconocimiento de tres nuevas formas de odonatos (Insecta, üdonata). Se han encontrado larvas de Zygoptera del género-tipo Samarura, ya reconocidas en otros yacimientos mundiales, y larvas de anisópteros Libelluloidea en los yacimientos mesozoicos españoles. Se describe Hoyaeshna cretacica n. gen y n. sp. (Aeshnidae Gomphaeschninae). Esta nueva especie presenta una vena psScP que nace en el nodo y recorre parcialmente el campo postnodal supliendo el recorrido que en algunos grupos de odonatos realiza la ScP, por ejemplo, en los Zygoptera Sieblosidae y en los Anisoptera Aeschnidiidae.

Un muestreo intensivo del clasico yacimiento de mamíferos fósles de Santpedor ha dado como resultado la presencia de Theridomys aff. aquatilis, Gliravus aff. priscus, Eucricetodon atavus y Pseudoltinomys gaillardi. Esta asociacin permite atribuir 1os niveles del yacimiento al Oligoceno inferior mas bajo (nivel de Hoogbutsel)

The subdivisions of human inferior colliculus are currently based on Golgi and Nissl-stained preparations. We have investigated the distribution of calcium-binding protein immunoreactivity in the human inferior colliculus and found complementary or mutually exclusive localisations of parvalbumin versus calbindin D-28k and calretinin staining. The central nucleus of the inferior colliculus but not the surrounding regions contained parvalbumin-positive neuronal somata and fibres. Calbindin-positive neurons and fibres were concentrated in the dorsal aspect of the central nucleus and in structures surrounding it: the dorsal cortex, the lateral lemniscus, the ventrolateral nucleus, and the intercollicular region. In the dorsal cortex, labelling of calbindin and calretinin revealed four distinct layers.Thus, calcium-binding protein reactivity reveals in the human inferior colliculus distinct neuronal populations that are anatomically segregated. The different calcium-binding protein-defined subdivisions may belong to parallel auditory pathways that were previously demonstrated in non-human primates, and they may constitute a first indication of parallel processing in human subcortical auditory structures.

Background: Excessive mediastinal shift into the vacated thoracic cavity after pneumonectomy can result in dyspnea without hypoxemia by compression of the tracheobronchial tree, a phenomenon called postpneumonectomy syndrome. More rarely hypoxemia in upright position (platypnea-orthodeoxia syndrome, POS) after pneumonectomy can result from re-opening of an atrial right-to-left shunt through a patent foramen ovale (PFO) due to mediastinal distorsion. Review of literature also shows a unique report of pulmonary veins stenosis resulting in POS without intracardiac shunt after pneumonectomy. Methods: We report the case of a 32-year-old woman who presented POS 6 months after right pneumonectomy for destroyed lung post tuberculosis. Results: The patient described severe dyspnea disappearing when lying. SpO2 decreased from 94% when lying to 60% sitting. Transthoracic echocardiography (TTE) suspected a possible PFO. We first tried to highlight clinical repercussions of PFO by noninvasive exams. Hyperoxia shunt quantification was not tolerated because of increased dyspnea in sitting position. Contrast bubbles TTE was difficult because of the important mediastinal shift but identified only rare left heart bubbles with/without Valsalva both in lying and sitting position, excluding a significant right-to-left shunt. A lung perfusion scintigraphy (injection while sitting) confirmed the absence of systemic isotope uptake. Computed tomographic pulmonary angiography (angio-CT) revealed a stretched but not stenosed left main bronchus, while the shift of the heart into the right cavity was major. Pulmonary angiography did not show embolism but revealed compression of the inferior vena cava (IVC) with impaired venous return to the right heart, as well as compression of the left pulmonary veins. There was no arteriovenous shunt. Cardiac MRI showed torsion of IVC at the level of the diaphragm, and strong atrial contraction contributing to a passive filling of the RV, while the right ventricle was normal. Right catheterism showed major hemodynamic disturbances with negative diastolic pressure in right heart cavities (atrium -12 mm Hg ventricle pressure -7 mm Hg). SaO2 measured in the pulmonary artery decreased from 58% when lying to 45% sitting. Conclusion: We described here an exceedingly rare and complex mechanism explaining POS after right pneumonectomy. Mediastinal repositioning with a silicone breast implant of appropriate size has been scheduled.

La publicación en 1954 y 1958 de los Estudios Críticos 1 y JI dedicados al poblado de la primera Edad del Hierro del Alto de la Cruz (Cortes de Navarra) supuso, sin ningún género de dudas, el arranque de la sistematización de la secuencia cultural y poblacional del área del valle medio del Ebro en el período correspondiente al Bronce Final y las fases más antiguas de la Edad del Hierro.
