BACKGROUND: Cardiovascular diseases (CVDs), including myocardial infarction, heart failure, peripheral arterial disease and strokes, are highly prevalent conditions and are associated with high morbidity and mortality. Cardiac rehabilitation (CR) is an effective form of secondary prevention for CVD but there is a lack of information regarding which specific behaviour change techniques (BCTs) are included in programmes that are associated with improvements in cardiovascular risk factors. This systematic review will describe the BCTs which are utilised within home-based CR programmes that are effective at reducing a spectrum of CVD risk factors.

METHODS/DESIGN: The review will be reported in line with the preferred reporting items for systematic reviews and meta-analyses (PRISMA) guidance. Randomised and quasi-randomised controlled trials of home-based CR initiated following a vascular event (myocardial infarction, heart failure, peripheral arterial disease and stroke patients) will be included. Articles will be identified through a comprehensive search of MEDLINE, Embase, PsycINFO, Web of Science and Cochrane Database guided by a medical librarian. Two review authors will independently screen articles retrieved from the search for eligibility and extract relevant data, identifying which specific BCTs are included in programmes that are associated with improvements in particular modifiable vascular risk factors.

DISCUSSION: This review will be of value to clinicians and healthcare professionals working with cardiovascular patients by identifying specific BCTs which are used within effective home-based CR. It will also inform the future design and evaluation of complex health service interventions aimed at secondary prevention in CVD.

Introdução: Cada vez mais a Reabilitação Cardíaca (RC) tem um início precoce no tratamento e nas necessidades do paciente no sentido de promover a sua autonomia e responsabilização pela recuperação, através de uma abordagem multidisciplinar. Os programas home-based e a inclusão das tecnologias de informação e comunicação são soluções atrativas para o aumento da participação dos doentes selecionados e inclusão de grupos de doentes atualmente sub-representados. Objetivos: Sistematizar a evidência científica atual sobre a efetividade dos programas de reabilitação cardíaca home-based com controlo á distância através da aplicação de novas tecnologias, comparando-a com a reabilitação centre-based/hospital-based, ao nível da adesão e da atividade física. Métodos: Este trabalho consiste numa revisão sistemática da literatura publicada entre 2007 e 2014, através de uma pesquisa em diferentes bases de dados eletrónicas científicas (Elsevier – Science Direct, PEDro, PubMed, Scielo Portugal e B-on) com as palavras-chave: reabilitação cardíaca, home-based, centre-based, hospital-based, reabilitação exercise-based, telemonitorização, smartphone, internet, atividade física, em todas as combinações possíveis. Os estudos foram analisados independentemente por dois revisores quanto aos critérios de inclusão e qualidade dos estudos. Resultados: Dos 101 estudos identificados, apenas dez foram incluídos. Considerando a escala da PEDro, quatro estudos obtiveram um score 5, quatro, um score de 6, e 2 com um score de 7 em 10. Os estudos foram realizados em adultos com idades compreendidas entre os 18 e os 80 anos. Os programas de intervenção dividiram-se em planeamento de atividade física e em autogestão. Todos os programas de exercício físico conduziram a um aumento da capacidade de exercício e consequente, maior controlo de fatores de risco. Pelos níveis de adesão aos PRC home-based e pelos resultados positivos de diferentes parâmetros em relação a reabilitação centre-based/hospital-based é notável a efetividade da telemonitorização baseada em casa. Conclusão: A telemonitorização domiciliária constitui um elemento fundamental para a solução de numerosos problemas destes doentes, tornando-se em métodos simples e de fácil funcionamento para haver sucesso nas taxas de adesão. Com efeito, a utilização das tecnologias de informação e de comunicação permite uma prestação e gestão eficazes dos cuidados de saúde no domicílio.

This paper presents a theoretical model to analyze the privacy issues around location based mobile business models. We report the results of an exploratory field experiment in Switzerland that assessed the factors driving user payoff in mobile business. We found that (1) the personal data disclosed has a negative effect on user payoff; (2) the amount of personalization available has a direct and positive effect, as well as a moderating effect on user payoff; (3) the amount of control over user's personal data has a direct and positive effect, as well as a moderating effect on user payoff. The results suggest that privacy protection could be the main value proposition in the B2C mobile market. From our theoretical model we derive a set of guidelines to design a privacy-friendly business model pattern for third-party services. We discuss four examples to show the mobile platform can play a key role in the implementation of these new business models.

This thesis aims to uncover the dynamics, causes and outcomes of women's reliance on unregulated home-based child care in Ontario, Canada, and the implications ofthis form of care for women's equality. Drawing on a longitudinal qualitative study, I examine the diverse experience of 14 women using home-based child care and engaged in both paid work/training and care work for children under the age of six, and draw comparisons with users of other forms of child care. I argue that home-based child care involves high levels of instability for continuity of care and is chosen largely as a default position based on economic considerations. It represents a compromise between the demands of social reproduction and paid work/training that entangles mothers in relations of exploitation with care providers. Doing so leaves both mothers and care providers socially and economically vulnerable and relying on social networks to fill in the gaps.

Home-based online business ventures are an increasingly pervasive yet under-researched phenomenon. The experiences and mindset of entrepreneurs setting up and running such enterprises require better understanding. Using data from a qualitative study of 23 online home-based business entrepreneurs, we propose the augmented concept of ‘mental mobility’ to encapsulate how they approach their business activities. Drawing on Howard P. Becker's early theorising of mobility, together with Victor Turner's later notion of liminality, we conceptualise mental mobility as the process through which individuals navigate the liminal spaces between the physical and digital spheres of work and the overlapping home/workplace, enabling them to manipulate and partially reconcile the spatial, temporal and emotional tensions that are present in such work environments. Our research also holds important applications for alternative employment contexts and broader social orderings because of the increasingly pervasive and disruptive influence of technology on experiences of remunerated work.
