The aim of this study is to analyze the impact of food shortage on growth performance, by means of energetic reserves (proteins, glycogen and lipids) mobilization and hepatopancreas cells analysis in C. quadricarinatus juveniles maintained in groups, as well as the effect on culture water quality. Two experiments were performed, each of them with two feeding regimes during 45 days. The Control feeding regime, in which crayfish were fed daily (once a day) throughout the experimental period (DF), and the Cyclic feeding regime, in which juveniles were fed for 2 or 4 days (once a day) followed by 2 or 4 days of food deprivation (2F/2D and 4F/4D, respectively) in repeated cycles. Cyclic feeding influenced growth, biochemical composition from hepatopancreas and muscle, and water quality. Juveniles cyclically fed were unable to maintain a normal growth trajectory during 45 days. Apparent feed conversion ratio, apparent protein efficiency ratio, hepatosomatic index and relative pleon mass were similar in cyclic and daily fed animals and no structural damage was found in the hepatopancreas of juveniles subjected to cyclic feeding. The novelty of this study was the significant accumulation of proteins in pleonal muscle in both cyclic feeding regimes (approx. 18%) suggesting that the storage of this constitutive material during food shortage may be an adaptation for a compensatory growth when food becomes abundant again. The cyclic feeding regimes had a positive effect on water quality decreasing inorganic nitrogen concentration. This was due to the reduction in the amount of animal excretes and feces in the group that received approx. 50% less feed. Additionally, water pH was higher in cyclic feeding tanks, as a result of lower organic matter decomposition and consequent release of CO2. Accordingly, total ammonia in the water was significantly lower for the cyclic feeding regimes compared to their respective controls. This study suggests that the protocol of cyclic feeding could be applied at least 45 days in 1 g juveniles maintained in group conditions, without affecting the energetic reserves and hepatopancreas structure, emphasizing the high tolerance of this species to food restriction.

The principle aim of this study was to investigate biological predictors of response and resistance to multiple myeloma treatment. Two hypothesis had been proposed as responsible of responsiveness: SNPs in DNA repair and Folate pathway, and P-gp dependent efflux. As a first objective, panel of SNPs in DNA repair and Folate pathway genes, were analyzed. It was a retrospective study in a group of 454, previously untreated, MM patients enrolled in a randomized phase III open-label study. Results show that some SNPs in Folate pathway are correlated with response to MM treatment. MTR genotype was associated with favorable response in the overall population of MM patients. However, this relation, disappear after adjustment for treatment response. When poor responder includes very good partial response, partial response and stable/progressive disease MTFHR rs1801131 genotype was associated with poor response to therapy. This relation - unlike in MTR – was still significant after adjustment for treatment response. Identification of this genetic variant in MM patients could be used as an independent prognostic factor for therapeutic outcome in the clinical practice. In the second objective, basic disposition characteristics of bortezomib was investigated. We demonstrated that bortezomib is a P-gp substrate in a bi-directional transport study. We obtain apparent permeability rate values that together with solubility values can have a crucial implication in better understanding of bortezomib pharmacokinetics with respect to the importance of membrane transporters. Subsequently, in view of the importance of P-gp for bortezomib responsiveness a panel of SNPs in ABCB1 gene - coding for P-gp - were analyzed. In particular we analyzed five SNPs, none of them however correlated with treatment responsiveness. However, we found a significant association between ABCB1 variants and cytogenetic abnormalities. In particular, deletion of chromosome 17 and t(4;14) translocation were present in patients harboring rs60023214 and rs2038502 variants respectively.

P-Glykoprotein (P-gp) ist ein ATP-verbrauchender Transporter, der in Organschranken exprimiert wird, um Fremdstoffe auszuschleusen, darunter auch Psychopharmaka. Im Rahmen dieser Arbeit wurde im Tiermodell der Maus untersucht, welche pharmakokinetischen und pharmakodynamischen Konsequenzen sich bei Verabreichung von Risperidon als P-gp Modellsubstrat ergeben, wenn die Expression von P-gp induziert wird. Als potenzielle Induktoren wurden Dexamethason, Rifampicin, Quercetin, 5-Pregnen-3ß-ol-20-on-16α-Carbonitril (PCN) und Acitretin geprüft. Es konnte gezeigt werden, dass alle Substanzen die Verteilung von Risperidon und seinem aktiven Metaboliten 9-Hydroxyrisperidon beeinflussten. Während sich für Quercetin und Acitretin leichte P-gp inhibitorische Eigenschaften ergaben, die an Hand von erhöhten Konzentrationen von Risperidon und 9-Hydroxyrisperidon gezeigt werden konnten, führten die bekannten P-gp Induktoren Rifampicin, Dexamethason und PCN zu verringerten Konzentrationen im Vergleich zur Kontrollgruppe. Durch Western Blot Untersuchungen wurde bestätigt, dass die Induktoren die P-gp Expression im Hirngewebe tendenziell steigerten. Dies sprach dafür, dass bei Verabreichung einer Komedikation, die P-gp induziert, mit einer veränderten Verteilung von P-gp Substraten zu rechnen ist. Darüber hinaus konnte nachgewiesen werden, dass durch eine Hemmung bzw. Induktion von P-gp nicht nur die Pharmakokinetik, sondern auch die Pharmakodynamik von Risperidon und 9-Hydroxyrisperidon verändert wird. Dies wurde durch verhaltenspharmakologische Untersuchungen gezeigt. Durch Risperidon induzierte motorische Effekte auf dem RotaRod waren nach Induktion von P-gp abgeschwächt. Dies zeigte sich auch für Haloperidol, welches kein Substrat ist. Da P-gp abhängige Effekte in diesem Fall keine bedeutende Rolle spielen, ist davon auszugehen, dass neben der Induktion von P-gp an der Blut-Hirn Schranke auch andere Mechanismen wie z.B. eine Induktion von Enzymen der CYP-Familie an den beobachteten Effekten beteiligt sind. Bei Untersuchungen von kognitiven Leistungen in der Barnes Maze konnte gezeigt werden, dass Haloperidol im Gegensatz zu Risperidon das Lernverhalten negativ beeinflussen kann. Eine P-gp Induktion schien jedoch keinen deutlichen Einfluss auf das Lernverhalten unter Antipsychotika-Gabe zu haben und sprach vielmehr für substanzabhängige Effekte der einzelnen Antipsychotika bzw. P-gp Modulatoren. Zusatzuntersuchungen zur Hirngängigkeit von Acitretin, einem synthetischen Retinoid, welches derzeit als potenzielles Antidementivum geprüft wird, konnten belegen, dass es die Blut-Hirn Schranke überwindet. Bereits 1h nach Injektion war Acitretin in hoher Konzentration im Gehirn nachweisbar. Durch die Analyse zur Verteilung von Acitretin in Hirngewebe und Serum von P-gp Wildtyp und P-gp doppel knockout Mäusen konnte belegt werden, dass Acitretin nicht P-gp abhängig transportiert wird. Die Daten insgesamt betrachtet, lassen den Schluss zu, dass durch Verabreichung von Medikamenten, die P-gp Modulatoren sind, bei Antipsychotika mit pharmakokinetischen Interaktionen zu rechnen ist, welche die Wirksamkeit der Medikamente einschränken können.

The original cefepime product was withdrawn from the Swiss market in January 2007 and replaced by a generic 10 months later. The goals of the study were to assess the impact of this cefepime shortage on the use and costs of alternative broad-spectrum antibiotics, on antibiotic policy, and on resistance of Pseudomonas aeruginosa toward carbapenems, ceftazidime, and piperacillin-tazobactam. A generalized regression-based interrupted time series model assessed how much the shortage changed the monthly use and costs of cefepime and of selected alternative broad-spectrum antibiotics (ceftazidime, imipenem-cilastatin, meropenem, piperacillin-tazobactam) in 15 Swiss acute care hospitals from January 2005 to December 2008. Resistance of P. aeruginosa was compared before and after the cefepime shortage. There was a statistically significant increase in the consumption of piperacillin-tazobactam in hospitals with definitive interruption of cefepime supply and of meropenem in hospitals with transient interruption of cefepime supply. Consumption of each alternative antibiotic tended to increase during the cefepime shortage and to decrease when the cefepime generic was released. These shifts were associated with significantly higher overall costs. There was no significant change in hospitals with uninterrupted cefepime supply. The alternative antibiotics for which an increase in consumption showed the strongest association with a progression of resistance were the carbapenems. The use of alternative antibiotics after cefepime withdrawal was associated with a significant increase in piperacillin-tazobactam and meropenem use and in overall costs and with a decrease in susceptibility of P. aeruginosa in hospitals. This warrants caution with regard to shortages and withdrawals of antibiotics.

Glycoprotein (GP) VI, the primary collagen receptor on platelets, has been shown to have variable expression, possibly as a consequence of immune modulation. The present study was designed to determine the mechanism by which GP VI clearance occurs. We found that direct activation of GP VI both by a GP VI-specific antibody and by GP VI ligands (collagen and convulxin) reduced binding of biotinylated convulxin to the stimulated platelets. Analysis of immunoblots of platelets and supernatants showed that the stimulated platelets contained less GP VI, while the soluble fraction contained a 57-kDa cleavage product. Stimulation of platelets with PAR-1 agonists (TRAP peptide and thrombin) also caused GP VI cleavage, although the amount of GP VI loss was less than that observed with direct GP VI ligands. The metalloproteinase (MMP) inhibitors GM6001 and TAPI prevented both the clearance of GP VI from the platelet surface and the appearance of the soluble cleavage product. Induction of GP VI cleavage caused specific down-regulation of collagen-induced platelet aggregation, providing a mechanism for the modulation of platelet responsiveness to this important platelet agonist.

Bernard-Soulier syndrome (BSS) is an extremely rare hereditary bleeding disorder, caused by mutations occurring in the Glycoprotein (GP) Ibalpha, GPIbbeta and GP9 genes that encode for the corresponding subunits of platelet GPIb-V-IX adhesion receptor complex. BSS has been reported in many populations, mostly behaving in an autosomal-recessive manner.While the great majority of BSS mutations are unique to a single individual or family, the GP9 1828A>G Asn45Ser mutation, which we have identified in an undocumented Australian Caucasian, has already been reported in multiple unrelated Caucasian families from various Northern and Central European countries. Haplotype analysis of 19 BSS patients from 15 unrelated Northern European families (including 2 compound heterozygote siblings from a British family previously published, and 17 1828A>G Asn45Ser homozygotes), showed that 14 of these BSS patients from 11 of the 1828A>G Asn45Ser homozygote families share a common haplotype at the chromosomal region 3' to the GP9 gene. Hence, the results suggest that the GP9 1828A>GAsn45Ser mutation in these families is ancient, and its frequent emergence in the European population is the result of a founder effect rather than recurrent mutational events. Association of the 1828A>G Asn45Ser mutation with variant haplotypes in 4 other Northern European BSS families raised the possibility of a second founder event, or rare recombinations in these families. Additional members from these 'atypical' lineages would need to be screened to resolve this question.

BACKGROUND: Low back pain (LBP) is by far the most prevalent and costly musculoskeletal problem in our society today. Following the recommendations of the Multinational Musculoskeletal Inception Cohort Study (MMICS) Statement, our study aims to define outcome assessment tools for patients with acute LBP and the time point at which chronic LBP becomes manifest and to identify patient characteristics which increase the risk of chronicity. METHODS: Patients with acute LBP will be recruited from clinics of general practitioners (GPs) in New Zealand (NZ) and Switzerland (CH). They will be assessed by postal survey at baseline and at 3, 6, 12 weeks and 6 months follow-up. Primary outcome will be disability as measured by the Oswestry Disability Index (ODI); key secondary endpoints will be general health as measured by the acute SF-12 and pain as measured on the Visual Analogue Scale (VAS). A subgroup analysis of different assessment instruments and baseline characteristics will be performed using multiple linear regression models. This study aims to examine: 1. Which biomedical, psychological, social, and occupational outcome assessment tools are identifiers for the transition from acute to chronic LBP and at which time point this transition becomes manifest. 2. Which psychosocial and occupational baseline characteristics like work status and period of work absenteeism influence the course from acute to chronic LBP. 3. Differences in outcome assessment tools and baseline characteristics of patients in NZ compared with CH. DISCUSSION: This study will develop a screening tool for patients with acute LBP to be used in GP clinics to access the risk of developing chronic LBP. In addition, biomedical, psychological, social, and occupational patient characteristics which influence the course from acute to chronic LBP will be identified. Furthermore, an appropriate time point for follow-ups will be given to detect this transition. The generalizability of our findings will be enhanced by the international perspective of this study. This thesis analyzes the domestic shortage in the Chinese natural gas market. Both the domestic supply and demand of natural gas are growing fast in China. However, the supply cannot catch up with the demand. Under the present pricing mechanism, the Chinese natural gas market cannot get the equilibrium by itself. Expensive imports are inadequate to fill the increasing gap between the domestic demand and supply. Therefore, the shortage problem occurs. Since the energy gap can result in the arrested development of economics, the shortage problem need to be solved. This thesis gives three suggestions to solve the problem: the use of Unconventional Gas, Natural Gas Storage and Pricing Reform.
