A sociedade moderna encontra-se numa evolução progressiva e vertiginosa nomeadamente no que respeita às novas tecnologias. Independentemente da área de conhecimento, é de senso comum que cada vez mais é necessária uma formação sólida, sendo fundamental a preparação e a consolidação das futuras gerações na utilização das novas tecnologias. As plataformas de e-learning são hoje em dia uma realidade mais que afirmada e com aplicação em todos os setores de atividade. Na área da saúde, e mais concretamente no que diz respeito ao tema de Suporte Básico de Vida (SBV), foi-nos possível verificar que a maioria das pessoas revela falta de conhecimentos sobre o tema e, não obstante existirem cursos sobre este tema, a disponibilidade para a sua frequência nem sempre é possível pela distância, falta de tempo e disponibilidade para a sua frequência, bem como pelos custos envolvidos. O presente estudo pretende contribuir para uma análise e investigação sobre o potencial de utilização das novas Tecnologias Multimédia, aplicando-as ao ensino concreto de primeiros socorros e especialmente de Suporte Básico de Vida, num recurso educativo digital com o formato de Objeto de Aprendizagem. Para o efeito, será construído um Objeto de Aprendizagem temático, focalizando o tema de SBV. O repositório MERLOT foi utilizado como meio de distribuição global, no sentido de aferir o potencial e facilidade de distribuição e catalogação com metadados. Foi ainda colocado on-line o “SBVOA” (Objeto de Aprendizagem desenvolvido no presente estudo), numa página criada apenas para o efeito. Esta dissertação apresenta o estudo desenvolvido para investigação do potencial de aprendizagem proporcionado pelo Objeto de Aprendizagem desenvolvido, validando se pode ser uma alternativa interativa de educação em SBV, contribuindo deste modo para a diminuição de eventuais perdas de vida.

INTRODUCTION: In 1956, Africanized honeybees (AHB) migrated from Brazil to other regions of the Western Hemisphere, including South, Central, and North America, except for Canada. Despite being productive, they are highly aggressive and cause fatal accidents. This study aimed to evaluate patients at the Clinical Hospital of Botucatu Medical School (HC-FMB) and to propose treatment guidelines. METHODS: From 2005 to 2006, the clinical and laboratorial aspects of 11 patients (7 male and 4 female) and the anatomopathological aspects of one patient who had died in 2003 were analyzed. RESULTS: The age of the surviving patients varied from 5 to 87 years, with a mean of 42.5 years. The majority of accidents occurred in the afternoon, and the number of stings ranged from 20 to 500. The principal signs and symptoms were pain and local inflammatory signs, nausea, tachycardia, and vomiting. Biochemical findings presented increased levels of creatine phosphokinase, lactate dehydrogenase, and aspartate/alanine aminotransferase. An 11-year-old male patient died upon entering the attic of a two-storey building where he was attacked by a swarm, receiving more than 1,000 stings. He was sent to HC-FMB where he was treated, but he died 24h later. Observed at the autopsy were erythematous-purpuric skin lesions besides necrosis at the sting locations, rhabdomyolysis, focal myocardial necrosis, tubular hydropic degeneration and focal tubular acute necrosis of the kidneys, myoglobinuria, and centrolobular necrosis in the liver. CONCLUSIONS: Accidents caused by multiple AHB stings always constitute a medical emergency. As there is no specific antivenom, we have developed guidelines, including first aid, drugs, and the proper removal of stingers.

Introduction Portuguese man-of-war, Physalia physalis (Linnaeus, 1758), are cnidarians capable of discharging intracellular organelles filled with venom, resulting in severe envenomation in humans. Methods We report the clinical and therapeutic aspects of 331 accidents involving Portuguese man-of-war in an outbreak on the coast of the State of São Paulo, Brazil. Results The clinical manifestations of envenomation were rare and mild and mostly local, systemic reactions; there was a low rate of late complications. Conclusions The consequences of envenomation were of moderate severity, and first aid measures were effective in controlling the pain. Outbreaks of accidents involving Portuguese man-of-war occur periodically in the area without a clear explanation.

BACKGROUND: Hypoglycaemia is a poor prognostic indicator in severe malaria. Intravenous infusions are rarely feasible in rural areas. The efficacy of sublingual sugar (SLS) was assessed in a pilot randomized controlled trial among hypoglycaemic children with severe malaria in Mali. METHODS: Of 151 patients with presumed severe malaria, 23 children with blood glucose concentrations < 60 mg/dl (< 3.3 mmol/l) were assigned randomly to receive either intravenous 10% glucose (IVG; n = 9) or sublingual sugar (SLS; n = 14). In SLS, a teaspoon of sugar, moistened with a few drops of water, was gently placed under the tongue every 20 minutes. The child was put in the recovery position. Blood glucose concentration (BGC) was measured every 5-10 minutes for the first hour. All children were treated for malaria with intramuscular artemether. The primary outcome measure was treatment response, defined as reaching a BGC of >or= 3.3 mmol/l (60 mg/dl) within 40 minutes after admission. Secondary outcome measures were early treatment response at 20 minutes, relapse (early and late), maximal BGC gain (CGmax), and treatment delay. RESULTS: There was no significant difference between the groups in the primary outcome measure. Treatment response occurred in 71% and 67% for SLS and IVG, respectively. Among the responders, relapses occurred in 30% on SLS at 40 minutes and in 17% on IVG at 20 minutes. There was one fatality in each group. Treatment failures in the SLS group were related to children with clenched teeth or swallowing the sugar, whereas in the IVG group, they were due to unavoidable delays in beginning an infusion (median time 17.5 min (range 3-40).Among SLS, the BGC increase was rapid among the nine patients who really kept the sugar sublingually. All but one increased their BGC by 10 minutes with a mean gain of 44 mg/dl (95%CI: 20.5-63.4). CONCLUSION: Sublingual sugar appears to be a child-friendly, well-tolerated and effective promising method of raising blood glucose in severely ill children. More frequent repeated doses are needed to prevent relapse. Children should be monitored for early swallowing which leads to delayed absorption, and in this case another dose of sugar should be given. Sublingual sugar could be proposed as an immediate "first aid" measure while awaiting intravenous glucose. In many cases it may avert the need for intravenous glucose.

OBJECTIVE: The occurrence of the 2003 G8 summit in Evian and the threat of major civil riots or even terrorist attacks in the Swiss neighbourhood forced us to imagine a new system of rescue and medical care in case of numerous victims. Previous occurrences of the G8 in Europe or America have demonstrated the need of flexible and mobile structures, able to respond quickly to crowd movements, unlike the usual static structure of rescue systems designed for major accidents. METHODS: We developed a new concept of Mobile Medical Squadrons (MMS) consisting of several vehicles and medical care and rescue human resources. In our concept, each MMS consisted of 3 emergency doctors, 5 paramedics and 9 first-aid workers. They were designed to handle 15 patients, with a large autonomy in terms of rescue, medical care, evacuation and medical authority. The equipment included medical, resuscitation, simple decontamination, evacuation and communication materials. RESULTS: The MMS were dispatched four times during the G8 summit following civil riots. They took care of 12 injured patients. CONCLUSION: The concept of MMS as a reinforcement of the existing rescue and health care resources appears as a new flexible, a modular and useful concept for the medical management of collective prehospital emergency situations. Its use is suggested instead of the traditional static concept of rescue systems designed for major accidents.

En el nou marc universitari que dibuixa l’adaptació de les titulacions a l’Espai Europeud’Educació Superior, es proposa que el titulat de grau en Psicologia acabi la carrera amb lescompetències bàsiques mínimes d’ajut psicològic que li permetin fer front a situacionsd’emergència inesperades i reaccionar de manera professional sense empitjorar la situació operjudicar l’estat de les víctimes, i amb un mínim de seguretat en allò què ha de fer o, al menys,en allò que no ha de fer. Es considera que hauria de tenir, com a mínim, el mateix nivell deformació que tenen altres col·lectius que habitualment es troben implicats en una emergència(personal sanitari, bombers, voluntaris, etc.) per tal que no es trobi en inferioritat de condicionspel que es refereix a la manera de proporcionar primers auxilis psicològics a víctimes. A classic way of delaying drug resistance is to use an alternative when possible. We tested the malaria treatment Argemone mexicana decoction (AM), a validated self-prepared traditional medicine made with one widely available plant and safe across wide dose variations. In an attempt to reflect the real situation in the home-based management of malaria in a remote Malian village, 301 patients with presumed uncomplicated malaria (median age 5 years) were randomly assigned to receive AM or artesunate-amodiaquine [artemisinin combination therapy (ACT)] as first-line treatment. Both treatments were well tolerated. Over 28 days, second-line treatment was not required for 89% (95% CI 84.1-93.2) of patients on AM, versus 95% (95% CI 88.8-98.3) on ACT. Deterioration to severe malaria was 1.9% in both groups in children aged </=5 years (there were no cases in patients aged >5 years) and 0% had coma/convulsions. AM, now government-approved in Mali, could be tested as a first-line complement to standard modern drugs in high-transmission areas, in order to reduce the drug pressure for development of resistance to ACT, in the management of malaria. In view of the low rate of severe malaria and good tolerability, AM may also constitute a first-aid treatment when access to other antimalarials is delayed.

BACKGROUND: E-learning techniques are spreading at great speed in medicine, raising concerns about the impact of adopting them. Websites especially designed to host courses are becoming more common. There is a lack of evidence that these systems could enhance student knowledge acquisition. GOAL: To evaluate the impact of using dedicated-website tools over cognition of medical students exposed to a first-aid course. METHODS: Prospective study of 184 medical students exposed to a twenty-hour first-aid course. We generated a dedicated-website with several sections (lectures, additional reading material, video and multiple choice exercises). We constructed variables expressing the student's access to each section. The evaluation was composed of fifty multiple-choice tests, based on clinical problems. We used multiple linear regression to adjust for potential confounders. RESULTS: There was no association of website intensity of exposure and the outcome - beta-coeficient 0.27 (95%CI - 0.454 - 1.004). These findings were not altered after adjustment for potential confounders - 0.165 (95%CI -0.628 - 0.960). CONCLUSION: A dedicated website with passive and active capabilities for aiding in person learning had not shown association with a better outcome.

Material para el alumnado de Educación Secundaria Obligatoria en el que se proponen cuatro unidades didácticas interdisciplinares en las que se trabaja un amplio abanico de temas transversales utilizando la lengua inglesa como vehículo de comunicación. La cuatro unidades propuestas son las siguientes: 1. The Same But Different, donde se trata el papel de la mujer en algunas civilizaciones contemporáneas, con un proyecto de trabajo final al que van dirigidas todas las actividades de la unidad; 2. Let's Talk About Sex, donde se tratan la pubertad, el sexo y la contracepción, informando al alumnado, que deberá hacer una elección libre y responsable entre las diferentes alternativas; 3. AIDS First Aid, donde se presenta una aproximación médica y social a la enfermedad; 4. Who Cares?, donde se trata el funcionamiento de un centro de salud británico y local, lo que supone que el alumnado deberá modificar y reelaborar sus esquemas de conocimiento, construyendo su propio aprendizaje. El diseño de las unidades didácticas incluye los objetivos, contenidos de las áreas curriculares implicadas así como los propios de la materia de Lengua Extranjera. Se incluyen multitud de actividades didácticas y unas hojas de autoevaluación diseñadas para que los alumnos tomen conciencia de su evolución, y en las que se evalúan los contenidos, el interés mostrado, el método de trabajo, la actitud hacia el tema, la dinámica de la clase y la labor del profesorado.

Three experiments investigated the effectiveness of presenting procedural information through different media and their combinations. Experiment 1 examined the effectiveness of text, line drawings, text and line drawings, video. and video stills for learning a first aid task. The results showed an advantage of text and line drawings and of the video presentation over the other three conditions for both bandaging performance and answering questions about the task. Experiment 2 showed that the beneficial effect of the combination of text and pictures could not be accounted for simply in terms of a dual coding explanation. Rather, the effectiveness of the media and their combinations was influenced by the extent to which they conveyed action information. Finally, Experiment 3 showed no evidence of a contiguity effect: text and pictures were as effective when presented together on the same screen as when they were presented separately.
