Objetivo: Evaluar prospectivamente el grado de disfunción sexual en pacientes con incontinencia urinaria de esfuerzo antes y después de la cinta suburetral libre de tensión mediante el cuestionario PISQ-12 validado en español. Materiales y Métodos: Estudio observacional longitudinal de antes y después. Se seleccionaron 60 mujeres sexualmente activas con algún grado de disfunción sexual, entre abril del 2014 hasta marzo del 2015, diagnosticadas con incontinencia urinaria, programadas para colocación de cinta suburetral transobturadora en el Hospital Universitario Mayor Méderi. La mayoría de las pacientes presentaron algún grado de prolapso genital y requirieron corrección quirúrgica asociada a la cinta. Todas las pacientes respondieron el cuestionario PISQ-12 antes y 6 meses después del procedimiento. Resultados: La edad promedio fue 48 ± 4.58 años. El grado de prolapso con mayor frecuencia fue el estadío II del POP-Q 55% (n=33). El 96.7% (n=50) de las pacientes requirieron además de la colocación de la cinta suburetral corrección quirúrgica del prolapso genital. En la evaluación preoperatoria la disfunción sexual se distribuyó así: Severa: 70%, Moderada 18.3% y Leve 11.7%, después de 6 meses postoperatorios se encontró una diferencia estadísticamente significativa del cambio en el grado de disfunción sexual así: Moderada 41.5% y Leve 58.2% donde ninguna paciente quedó clasificada con disfunción severa. Discusión y Conclusión: Las pacientes que presentaron disfunción sexual severa obtuvieron mayor cambio en el grado de disfunción, luego de la colocación de la cinta suburetral.

BACKGROUND/AIMS: Estrogens are important effectors of reproduction and are critical for upregulating female reproductive behavior or lordosis in females. In addition to the importance of transcriptional regulation of genes by 17beta-estradiol-bound estrogen receptors (ER), extranuclear signal transduction cascades such as protein kinase A (PKA) are also important in regulating female sexual receptivity. GPR30 (G-protein coupled receptor 30), also known as GPER1, a putative membrane ER (mER), is a G protein-coupled receptor that binds 17beta-estradiol with an affinity that is similar to that possessed by the classical nuclear ER and activates both PKA and extracellular-regulated kinase signaling pathways. The high expression of GPR30 in the ventromedial hypothalamus, a region important for lordosis behavior as well as kinase cascades activated by this receptor, led us to hypothesize that GPR30 may regulate lordosis behavior in female rodents. METHOD: In this study, we investigated the ability of G-1, a selective agonist of GPR30, to regulate lordosis in the female mouse by administering this agent prior to progesterone in an estradiol-progesterone priming paradigm prior to testing with stud males. RESULTS: As expected, 17beta-estradiol benzoate (EB), but not sesame oil, increased lordosis behavior in female mice. G-1 also increased lordosis behavior in female mice and decreased the number of rejective responses towards male mice, similar to the effect of EB. The selective GPR30 antagonist G-15 blocked these effects. CONCLUSION: This study demonstrates that activation of the mER GPR30 stimulates social behavior in a rodent model in a manner similar to EB.

Gender differences in perceptions of sexual intent and commitment have been the subject of formal and informal inquiry for considerable time. One evolutionary theory, Error Management Theory (EMT), predicts that opposite-sex perceptions of female sexual intent and male commitment intent reflect intrinsic biases that minimize gender-specific evolutionary costs. The results supporting these hypotheses were obtained from subjects regardless of mood. We hypothesized that mood would influence ratings of sexual and commitment intent. Sixty participants (30 males, 30 females) were recruited and exposed to a positive and negative mood condition in counterbalanced groups using video stimuli. Preliminary analyses found an unexpected effect of order of mood induction, necessitating separate analyses of the Positive-Negative (PN) and Negative-Positive (NP) groups. Contrary to the original study, there were no gender effects. Positive moods led to increased ratings of both sexual and commitment intent across genders. Further, negative to positive mood-change was associated with significantly increased ratings. Both males and females attributed significantly higher sexual intent to same-sex rivals than themselves, but only males assessed themselves as having significantly higher commitment intent than same-sex rivals. The EMT model may require adaptation to acknowledge effects of variables such as mood on its predictions of gender-specific biases.

Treatment of sexual problems can prevent much anxiety and the development of depression. This article outlines approaches useful in managing the more common male sexual difficulties encountered in family practice.

Sexual arousal is a complex and dynamic element of women’s sexuality. Accounts vary, however most multidimensional models highlight the cognitive, affective and physiological components of the female sexual arousal response. While literature examining the peripheral physiological changes that occur during female sexual arousal abounds, there has been a dearth of literature pertaining to the cognitive and affective components. Thus, while many authors have included cognitive and emotions as independent components of the female sexual arousal response, there has been little empirical research to validate this approach. The aim of the current thesis was to examine the cognitive and affective components of female sexual arousal in more depth, investigating the nature of the relationship between these components under various experimental conditions. In order to do so, two integrated studies were conducted, each highlighting the effects of either external or internal variables on women’s subjective sexual arousal, absorption, positive affect and negative affect. Study One was designed to evaluate women’s emotional and cognitive processing of two elements of erotic film – foreplay and erotic context. 60 women were asked to report their subjective sexual arousal, absorption, positive affect and negative affect in response to one of four erotic film excerpts. The erotic excerpts varied in accordance with the degree of foreplay (low vs. high) depicted and the context in which the sexual activities took place (novel vs. habitual context). Women in the study responded more favourably to the high foreplay erotic film excerpt, subsequently reporting higher degrees of subjective sexual arousal, absorption and positive affect. Women also responded favourably to the erotic excerpt filmed in a novel context, reporting greater subjective sexual arousal as a result. The environment in which the sexual encounters were filmed failed to have an effect however, on women’s absorption or their positive or negative affect. The results of Study One suggest that stimulus specific variables, such as the degree of foreplay depicted, have a significant influence on female cognitive and emotional processing of erotic film. The results also suggest that a relationship exists between absorption, subjective sexual arousal and positive affect, albeit a correlational one. Specifically, there was evidence of parallel processing during sexual arousal, as participant reported sexual arousal, absorption and positive affect all increased and decreased in unison. Based on the results it was suggested that future research attempt to experimentally manipulate one of these variables, to examine its direct effect on the remaining variable. Thus, Study Two aimed to examine the effects of absorption on women’s cognitive and emotional processing of erotica. Study Two manipulated absorption at two levels (high vs. low), examining the impact of these states on participants’ subsequent absorption, subjective sexual arousal and positive and negative affect. 62 women were asked to read one of two sets of test session instructions. The first, participant-oriented instruction set, instructed participants to immerse themselves in the erotic film excerpt, as if they were active participants in the sexual exchange. The second, spectator-oriented instruction set, directed participants to observe and evaluate the erotic film. These instructions were designed to elicit high and low degrees of absorption, respectively. The utility of this approach when manipulating female absorption, was demonstrated by self reported ratings of absorption, given at the conclusion of the film presentation. Participants were also asked to report their subjective sexual arousal and positive and negative affect at the conclusion of the erotic film presentation. The findings of this study suggest that the adoption of a participant-oriented (high absorption) perspective elicits more favourable responses from participants than a spectator-oriented (low absorption) perspective, with participants in the former experimental group reporting greater degrees of subjective sexual arousal and positive affect. Negative affect was equivalent across experimental conditions, with the participants reporting that they experienced little to no aversive feelings during either of the experimental conditions. The results suggest that the degree to which a women immerses and absorbs herself in a sexual stimulus has a significant impact in her subsequent cognitive and affective processing of that stimulus. More specifically, it appears that women respond more favourably when they are highly absorbed and immersed in a stimulus, reporting greater subjective sexual arousal and positive affect. Overall, the results of Studies One and Two highlight the dynamic and complex nature of female sexual arousal. It appears that women have definite cognitive and affective responses to sexual stimuli. The magnitude of these responses may be mediated by a number of factors however, including the intrinsic qualities of the stimulus and the degree to which the woman attends to the stimulus. Both these variables act to either enhance or inhibit the sexual arousal response. There results have important implications for current sexuality literature. While women’s cognitions and emotions in response to erotic film were generally highly correlated, in some instances they differed, warranting their inclusion as separate elements in models of female sexual arousal. Furthermore, it might be suggested that the inclusion of an additional variable – absorption – into current models of female sexual arousal would prove beneficial, aiding researchers to better understand and predict the arousal process. As such, recommendations are made for a revised model of female sexual arousal. In terms of future directions, the results of the present thesis have implications for the treatment of sexual dysfunctions, suggesting that clinicians need to understand the internal and external variables that might contribute to the aetiology and maintenance of their presenting problems.

Introduction. There are limited outcome data on the etiology and efficacy of psychological interventions for male and female sexual dysfunction as well as the role of innovative combined treatment paradigms.
Aim. This study aimed to highlight the salient psychological and interpersonal issues contributing to sexual health and dysfunction, to offer an etiological model for understanding the evolution and maintenance of sexual symptoms, and to offer recommendations for clinical management and research.
Methods. This study reviewed the current literature on the psychological and interpersonal issues contributing to male and female sexual dysfunction.
Main Outcome Measure. This study provides expert opinion based on a comprehensive review of the medical and psychological literature, widespread internal committee discussion, public presentation, and debate.
Results. Medical and psychological therapies for sexual dysfunctions should address the intricate biopsychosocial influences of the patient, the partner, and the couple. The biopsychosocial model provides an integrated paradigm for understanding and treating sexual dysfunction.
Conclusions. There is need for collaboration between healthcare practitioners from different disciplines in the evaluation, treatment, and education issues surrounding sexual dysfunction. In many cases, neither psychotherapy alone nor medical intervention alone is sufficient for the lasting resolution of sexual problems. The assessment of male, female, and couples’ sexual dysfunction should ideally include inquiry about predisposing, precipitating, maintaining, and contextual factors. Research is needed to identify efficacious combined and/or integrated treatments for sexual dysfunction.

Locating potential mates is critical to mating. We studied males’ association with females and mate-searching patterns in the guppy, Poecilia reticulata, a promiscuous live-bearer. In the field, we examined whether male guppies respond differently to a shoal of conspecific fish based on the members of the shoal. We found that more males were attracted to shoals that contained receptive females than to shoals of nonreceptive females or males. We also conducted laboratory experiments to investigate how males use olfactory cues of nonreceptive and receptive females to search for and associate with females. We gave males the option to associate with nonreceptive females when olfactory cues of receptive or nonreceptive females were present and absent, and when olfactory cues were presented alone. Males associated with females most strongly when both cues were presented simultaneously, but when cues were presented separately males’ association with females differed with respect to the olfactory cues that were added. Males associated with females equally with visual and olfactory cues presented separately when the odour cues were from receptive females. However, when the odour cues were from nonreceptive females, males associated with females less with olfactory than visual cues. Searching activity increased when males had access only to olfactory cues. Taken together these results suggest that olfactory cues influence males’ association with females and searching behaviour, and these changes in behaviour are likely to maximize a male’s opportunity to encounter receptive females.

The use of manualized treatment programs offers a useful research framework for assessing psychotherapeutic interventions for female sexual dysfunctions (FSDs), but it does not address all issues related to methodological rigor and replication, and raises new research issues in need of discussion. Aims. The goals of this manuscript are to review the literature on treatment trials utilizing manualized psychotherapy treatments for FSD and to explore the benefits and research issues associated with the flexible use of treatment manuals. Methods. The method used was the review of the relevant literature. Results. While the use of manualized treatments for FSDs can address certain methodological issues inherent in psychotherapy research, flexibility in manual administration is necessary in order to allow tailoring for individual needs that can be beneficial to both the participant and the research. The flexible use of manuals, as opposed to strict manual adherence, may also be more relevant for clinical utility. Conclusions. In order to administer manualized treatments for FSDs with appropriate flexibility, while also maximizing internal validity and replicability, the authors recommend that predetermined decision rules be utilized to guide individual tailoring, that potential gaps in the manual be identified and addressed, and that differing levels of motivation and readiness for treatment be taken into consideration in the treatment protocol. This research investigated the effect of erotic stimulus content on women’s subjective sexual arousal. It was concluded that subtle changes in specific stimulus content did not affect women’s arousal. The research also found that women’s expectations predicted how aroused they would become. Absorption was also associated with women’s subjective arousal.

In this essay we consider the construction of cultural identity, motherhood and the family in ABCD, a film of the Indian diaspora that had its world premiere at the 2001 London Film Festival. This film reads family, apparently within familiar narrative structures such as the U.S.-immigrant story, as portrayed in films like Goodbye, Columbus and My Big Fat Greek Wedding, and the "leaving home" story, as classically portrayed in Pride and Prejudice, where a young person needs to escape from her clueless family. The irritating presence of the mother in the film, and the quickness with which her two children appear to make life-determining decisions following her death, seem to invite discussions of plot and character organized around ideas of individual development, self-improvement and understanding. This is the territory of the desire plot, an account of family history captured for the twentieth century by Freudian-Lacanian readings which position sexual desire within the unconscious history of familial fantasies, understood as vertical and Oedipal. In this territory, mothers and old ladies become, as Mary Jacobus memorably phrased it, little more than "the waste products" of a system in which marriageable women are objects of exchange between men (142) and a mother's death would be expected to grease the wheels of narrative. Identity and narrative are inextricably linked here: a certain understanding of narrative as developmental and teleological paves the way for an understanding of identity as either/or. There are problems, however, in trying to read ABCD as a bildüngsroman structured by what Susan Freidman calls "the temporal plots of the family romance, its repetitions and discontents" (137), rendering the "Indian" characteristics of the plot unreadable, and the apparently self-defeating nature of the characters' choices and behavior, rather pointless. A central [End Page 16] difficulty is that the film both responds to and resists readings based on the Oedipal model of the bildüngsroman with its focus on linear development through time.

Objectives: This study examined the prevalence of male sexual dysfunction (MSD) and how different criteria alter prevalence rates. Methods: Five hundred seventy-three Australian men completed an online survey. Results: Of these men, 42.2% were sexually functional and 57.8% reported 1 or more sexual dysfunctions. No matter which criteria were used, the most prevalent MSD was erectile dysfunction, followed by premature ejaculation, hypoactive sexual desire disorder, and delayed ejaculation. Frequencies varied according to different definitions and measures used to diagnose MSD. More than half of the men with MSD experienced only 1 sexual dysfunction. Conclusions: These results demonstrate the high prevalence of MSD and the relatively low level of comorbidity of MSDs. © 2014 Taylor & Francis Group, LLC.

Este estudo teve por objetivo avaliar os fatores associados à disfunção sexual em mulheres de meia idade. Realizou-se um estudo descritivo transversal, que compreendeu 370 mulheres, entre 40 a 65 anos, atendidas nas Unidades Básicas de Saúde de cada distrito sanitário (Norte, Sul, Leste e Oeste) da cidade de Natal, no estado do Rio Grande do Norte, Brasil. Para avaliar a função sexual utilizou-se o Female Sexual Function Index (FSFI). A sintomatologia climatérica foi avaliada por meio do Menopause Rating Scale (MRS). O Índice de Blatt-Kupperman (IMBK) foi utilizado para avaliação quantitativa global da ocorrência de sintomas/queixas. A atividade física foi avaliada pelo questionário International Physical Activity Questionnaire - IPAQ (versão curta). A avaliação da qualidade de vida geral se deu pelo WHOQOL-Bref. A análise estatística foi realizada utilizando o programa estatístico MINITAB version16. Além de análises descritivas das variáveis categorizadas, utilizou-se o teste qui-quadrado de Person com o intuito de verificar possíveis associações entre as variáveis sociodemográficas, comportamentais, clínicas, níveis de atividade física, sintomatologia climatérica, qualidade de vida e a função sexual das mulheres estudadas. Desenvolveu-se a regressão logística para verificar a influência dessas variáveis sobre a disfunção sexual. Considerou-se o nível de significância de 5% para todos os testes. Os resultados mostraram que a média de idade das mulheres estudadas foi de 49,8 (±8,1) anos. Do total dessas mulheres, 67% apresentaram disfunção sexual. Observou-se que 54,5% delas se encontravam na pré-menopausa. Avaliando a influência das variáveis sobre a função sexual; faixa etária (56-65) (p<0,001), estado civil (divorciada/separada) (p < 0,001), escolaridade (baixa) (p=0,017), menopausa (p < 0,001), histerectomia (p = 0,016), nível de atividade física (sedentária) (p=0,002), sintomas do climatério (forte) (p<0,001) e qualidade de vida (baixa) (p<0,001), estiveram associados à disfunção sexual em mulheres de meia idade. Concluiu-se neste estudo que fatores sociodemográficos, clínicos, comportamentais, níveis de atividade física, sintomatologia climatérica e qualidade de vida influenciam significativamente a função sexual na mulher de meia idade

A síndrome da insuficiência androgênica na mulher (SIA) desperta, mesmo nos dias atuais, muitas discussões e encerra muitas controvérsias. Sabe-se, no entanto, que os níveis plasmáticos de testosterona declinam progressivamente ao longo do período reprodutivo. Conceitua-se a SIA como o conjunto de sintomas clínicos, a presença de biodisponibilidade diminuída de testosterona e os níveis normais de estrogênios. Entre os principais sintomas, citam-se o comprometimento do bem-estar, o humor disfórico, a fadiga sem causa aparente, o comprometimento do desejo sexual, o emagrecimento e a instabilidade vasomotora em mulheres pós-menopáusicas sob terapêutica estrogênica. Esses sintomas, no entanto, são potencialmente atribuíveis a diferentes etiologias e dificultam o correto diagnóstico na maioria dos casos, ainda que ele seja lembrado com freqüência em pacientes que se submetem à ooforectomia bilateral. O diagnóstico da SIA parece ser essencialmente clínico, não havendo a necessidade das dosagens laboratoriais para a sua comprovação. Não se deve indicar a terapêutica androgênica (TA) em pacientes que não estejam adequadamente estrogenizadas. Considera-se a testosterona o hormônio ideal para a TA. As pacientes com sintomas sugestivos de SIA, excluídas outras causas identificáveis, especialmente se pós-menopáusicas, são candidatas à TA. Não existem dados de segurança sobre a TA em usuárias em longo prazo. A via transdérmica - através de adesivos, cremes e gel - parece ser preferível à oral.

The aim of the present study was to investigate the effects of hydrocortisone during the prenatal period and its later repercussions on the fertility and sexual behavior of male rats. Pregnant rats were treated (s.c.) with hydrocortisone acetate, at 1.5 mg/day on the 17th, 18th, and 19th days of gestation. Decreased body weight and no alteration in anogenital distance were observed in male offspring. Adulthood, presented reductions of body weight, plasma testosterone levels, and seminal-vesicle wet weight without secretion as well as no alteration in the wet weights of the testes, epididymis, and seminal vesicle with secretion in the treated group. Males exposed to hydrocortisone during the prenatal period were able to mate with normal females, which became pregnant but exhibited an increased number of post-implantation losses. In spite of this, these treated males exhibited decreased male sexual behavior and the appearance of female sexual behavior after these male rats were castrated and pretreated with exogenous estrogen. These results indicate that exposure to hydrocortisone in the later stages of pregnancy may have a long-term effect on the fertility and sexual behavior of mate rats, suggesting an incomplete masculinization and defeminization of the central nervous system. In order to investigate the participation of estrogen during the period of brain sexual differentiation, male rats were treated with clomiphene citrate in the neonatal phase. Fertility and sexual behavior were assessed during adult life. Sexual maturation, body weight, and wet weight of the testes were unchanged. Although the adult male rats treated with clomiphene in the neonatal phase presented a significant reduction in the frequency of mounts, 90% of these rats were able to mate with normal females, which became pregnant. However, these females exhibited a significantly increased number of pre- and post-implantation losses. When these adult male rats were castrated and received estrogen, 60% presented female sexual behavior (receptive behavior and acceptance of mount). Thus, treatment of pups with clomiphene immediately after birth has a long-term effect on the reproductive physiology and sexual behavior of male rats. (C) 2002 Elsevier B.V. All rights reserved.
