O estudo teve como objetivo identificar fatores de risco para diabetes tipo 2 (DM 2) em uma população de adolescentes de escolas particulares de Fortaleza, Brasil. Foram avaliados 794 alunos, de 12 a 17 anos, em doze escolas, nos meses de maio, junho, agosto e setembro de 2007. Aplicou-se um formulário abordando aspectos sociodemográficos, IMC, pressão arterial, glicemia capilar e sedentarismo. Aproximadamente 24% dos participantes tinham o IMC elevado, 65% eram sedentários e 51% tinham antecedentes familiares de DM 2. Naqueles com maior renda, 73,5% tinham antecedentes familiares de DM 2 (p=0,04). Por volta de 39% dos adolescentes apresentavam pelo menos dois fatores de risco para DM 2. A maior parte dos fatores de risco para DM 2, identificados neste estudo, são modificáveis, portanto passíveis de intervenções preventivas no contexto escolar.

O estudo visa avaliar as ações educativas na promoção do autogerenciamento dos cuidados em diabetes mellitus. Vinte e sete indivíduos com diabetes tipo 2, atendidos no programa educativo do hospital-escola de Belo Horizonte, Minas Gerais, foram acompanhados durante quatro meses no ano de 2008. As ações educativas nos grupos consistiam de três encontros mensais, nos quais eram desenvolvidas dinâmicas lúdicas e interativas, e o atendimento individual realizado por meio da educação dialógica. A avaliação foi feita mediante questionário específico: autogerenciamento dos cuidados e exames clínicos no tempo inicial e após quatro meses da intervenção. Os indivíduos tinham idade média de 60,9 ±8,4 anos; o tempo médio da doença de 8,7±6,7 anos, tinham Ensino Fundamental incompleto e renda familiar de um a três salários mínimos 55, 6% (27). Os resultados foram satisfatórios no teste de autogerenciamento dos cuidados. Observou-se redução nos níveis de HbA1c e que as ações educativas favoreceram o autocuidado e o autocontrole da doença.

La Diabetes Mellitus (DM) es una enfermedad metabólica, crónica, no transmisible y de etiología multifactorial, causada por defectos en la secreción y acción de la insulina(1), constituye el 90-95 % de todos los casos de Diabetes(2). Es una enfermedad crónica que afecta a un gran número de personas, representando un problema personal y de salud pública de enormes proporciones. En Colombia está dentro de las 10 primeras causas de consulta y mortalidad, y la prevalencia en Bogotá en personas de 30-64 años es de 7.3% (3.7-10.9) en hombres y de 8.7 (5.2-12.3) en mujeres. Se estima que la mitad de los casos de Diabetes permanecen sin ser especificados. Muchos de estos pacientes presentan complicaciones crónicas en el momento del diagnóstico por lo que el inicio de la enfermedad antecede a su detección en varios años.

Syfte: Syftet med denna studie var att undersöka hur patienter upplevde patient-undervisningen samt hur sjuksköterskans roll såg ut i patientundervisningen. Metod: Uppsatsen har genomförts som en systematisk litteraturstudie. Artiklar har sökts i databaserna ELIN samt Cinahl. Artiklar publicerade mellan 1999-2009 och som var tillgängliga i fulltext inkluderades i resultatdelen som omfattade 14 kvalitativa studier samt en litteraturstudie. Resultat: I resultatet framgick att patienterna upplevde att gruppundervisning var ett bra alternativ till enskild undervisning då de kunde ta del av andras erfarenheter och på det sättet få mer kunskap om sjukdomen. Dessutom upplevde de att det var givande att träffa personer med samma sjukdom. I studien framgick att enskild undervisning var önskvärt från både patienter och sjuksköterskor då det kunde upplevas lättare att ta upp de frågor de hade där. Sjuksköterskan kunde vara ett stort stöd för patienten. Förutom att förmedla sina kunskaper och undervisa patienten var lyssna, uppmuntra och stötta något som sjuksköterskorna ansåg vara viktigt i mötet med patienten. Något som togs upp av patienter och sjuksköterskorna var att de ansåg att det vore bra om patienterna under besöken träffade samma personer då det skulle underlätta uppföljningarna.

Denna litteraturstudies syfte var att identifiera och evidensgradera de omvårdnadsåtgärder med vilka sjuksköterskan kan motivera patienter med diabetes typ-2 till egenvård. De 27 utvalda artiklarna, som svarade mot uppsatsens syfte, söktes från databaserna Cinahl, Elin@dalarna, Medline och PubMed. För att kunna bedöma utsedda kvantitativa och kvalitativa artiklars vetenskapliga kvalitet granskades artiklarna utifrån modifierade granskningsmallar efter förlagor av Forsberg och Wengström (2008). Kvalitetsbedömningen gav vid handen att av 27 artiklar, var 3 av hög vetenskaplig kvalitet och 24 artiklar var av medelgod kvalitet men ingen artikel bedömdes vara av låg kvalitet. På basen av artikelkvaliteten evidensgranskades de funna omvårdnadsåtgärderna i enlighet med Bahtsevanis (2008) rekommendationer. I litteraturstudien identifierades fyra huvudområden i omvårdnadsåtgärder inom diabetesvården: Livsstilsförändringar, Utbildning, Egenvård samt Personalens betydelse. Omvårdnadsåtgärderna Livsstilsförändringar, Utbildning och Egenvård vilade på måttligt vetenskapligt underlag och omvårdnadsåtgärden Personalens betydelse vilade på stark vetenskaplig grund. Resultaten diskuterades utgående från omvårdnadsteoretiker Orems humanistiska modell, som inbegriper egenvårdsbrist, som är en beskrivning av och förklaring till varför en person behöver omvårdnad, vilken sker när egenvårdskapaciteten är mindre än personens egenvårdskrav. Modellen bygger på antaganden om och generella kännetecken på till exempel omvårdnadssituationer och inkluderar begreppen omvårdnadskapacitet, hjälpmetoder och olika omvårdnadssystem.

Syfte: Syftet med litteraturstudien var att söka evidens för metoder som kan stödja patienter med typ-2 diabetes till livsstilsförändringar. Metod: En litteraturstudie, som innehåller 16 kvantitativa och kvalitativa vetenskapliga artiklar, som var publicerade mellan 2000-2010. Databasen som användes var Cinahl och sökmotorn Elin@Dalarna. Sökorden som använts var: diabetes type 2, type 2 diabetes, lifestyle changes, patient education, empowerment, health behaviour och self-management. Resultat: I resultaten kom uppsatsförfattarna fram till olika kategorier som visar olika åtgärder till livsstilsförändringar. Studierna baserades på olika åtgärder såsom; datorintervention, utbildningar, telefonsamtal, motiverande samtal, transteoretiska modellen och sjuksköterskespecialister. Empowerment var enligt uppsatsförfattarna det övergripande temat som de flesta studier baserade åtgärderna på.

Portadores de diabetes parecem ter mais queixas de olho seco do que o resto da população. Acredita-se que isto possa estar associado a uma forma de neuropatia diabética expressa por uma redução na sensibilidade corneana desses pacientes. Nossos principais objetivos neste estudo foram avaliar a influência da diabetes melito tipo 2 na sensibilidade corneana central e verificar se há uma associação entre a sensibilidade corneana central e a síndrome do olho seco em indivíduos com a doença. Assim, 62 portadores de diabetes tipo 2 foram submetidos a um exame oftalmológico de rotina, a uma ceratoestesiometria e a testes específicos para avaliar olho seco e polineuropatia distal simétrica. Num outro grupo, 20 voluntários saudáveis tiveram seus olhos avaliados da mesma forma, exceto pela não realização dos testes específicos para disfunção lacrimal. Entre os indivíduos diabéticos avaliados, foram observados 53.2% com hipoestesia corneana, 54.2% com retinopatia diabética, 45.9% com polineuropatia distal simétrica e 51.6% com a síndrome do olho seco. Entre os principais achados, observamos associações significativas envolvendo: diabetes tipo 2 e hipoestesia corneana central, síndrome do olho seco e hipoestesia corneana central, produção lacrimal reflexa (avaliada pelo teste de Schirmer II) e sensibilidade corneana central e retinopatia diabética proliferativa e sensibilidade corneana central. Uma possível associação foi encontrada envolvendo síndrome do olho seco retinopatia diabética proliferativa. Os autores discutem os resultados obtidos e os mecanismos envolvidos.

A 19-year-old female with type 1 diabetes for four years, and a 73-year-old female with type 2 diabetes for twenty years developed sudden-onset nephrotic syndrome. Examination by light microscopy, immunofluorescence, and electron microscopy (in one case) identified minimal change disease (MCD) in both cases. There was a potential causative drug (meloxicam) for the 73-year-old patient. Both patients were treated with prednisone and responded with complete remission. The patient with type 1 diabetes showed complete remission without relapse, and the patient with type 2 diabetes had two relapses; complete remission was sustained after associated treatment with cyclophosphamide and prednisone. Both patients had two years of follow-up evaluation after remission. We discuss the outcomes of both patients and emphasize the role of kidney biopsy in diabetic patients with an atypical proteinuric clinical course, because patients with MCD clearly respond to corticotherapy alone or in conjunction with other immunosuppressive agents. OBJECTIVES The aim of this study was to describe the process to obtain Food and Drug Administration (FDA) approval for the expanded indication for treatment with the Resolute zotarolimus-eluting stent (R-ZES) (Medtronic, Inc., Santa Rosa, California) in patients with coronary artery disease and diabetes. BACKGROUND The R-ZES is the first drug-eluting stent specifically indicated in the United States for percutaneous coronary intervention in patients with diabetes. METHODS We pooled patient-level data for 5,130 patients from the RESOLUTE Global Clinical Program. A performance goal prospectively determined in conjunction with the FDA was established as a rate of target vessel failure at 12 months of 14.5%. In addition to the FDA pre-specified cohort of less complex patients with diabetes (n = 878), we evaluated outcomes of the R-ZES in all 1,535 patients with diabetes compared with all 3,595 patients without diabetes at 2 years. RESULTS The 12-month rate of target vessel failure in the pre-specified diabetic cohort was 7.8% (upper 95% confidence interval: 9.51%), significantly lower than the performance goal of 14.5% (p < 0.001). After 2 years, the cumulative incidence of target lesion failure in patients with noninsulin-treated diabetes was comparable to that of patients without diabetes (8.0% vs. 7.1%). The higher risk insulin-treated population demonstrated a significantly higher target lesion failure rate (13.7%). In the whole population, including complex patients, rates of stent thrombosis were not significantly different between patients with and without diabetes (1.2% vs. 0.8%). CONCLUSIONS The R-ZES is safe and effective in patients with diabetes. Long-term clinical data of patients with noninsulin-treated diabetes are equivalent to patients without diabetes. Patients with insulin-treated diabetes remain a higher risk subset. Background Low diet quality and depression symptoms are independently associated with poor glycemic control in subjects with type 2 diabetes (T2D); however, the relationship between them is unclear. The aim of this study was to determine the association between diet quality and symptoms of depression among Cuban-Americans with and without T2D living in South Florida. Methods Subjects (n = 356) were recruited from randomly selected mailing list. Diet quality was determined using the Healthy Eating Index-2005 (HEI-05) score. Symptoms of depression were assessed using the Beck Depression Inventory (BDI). Both linear and logistic regression analyses were run to determine whether or not these two variables were related. Symptoms of depression was the dependent variable and independent variables included HEI-05, gender, age, marital status, BMI, education level, A1C, employment status, depression medication, duration of diabetes, and diabetes status. Analysis of covariance was used to test for interactions among variables. Results An interaction between diabetes status, gender and HEI-05 was found (P = 0.011). Among males with a HEI-05 score ≤ 55.6, those with T2D had a higher mean BDI score than those without T2D (11.6 vs. 6.6 respectively, P = 0.028). Among males and females with a HEI-05 score ≤ 55.6, females without T2D had a higher mean BDI score compared to males without T2D (11.0 vs. 6.6 respectively, P = 0.012) Conclusions Differences in symptoms of depression according to diabetes status and gender are found in Cuban-Americans with low diet quality.

Ethnicities within Black populations have not been distinguished in most nutrition studies. We sought to examine dietary differences between African Americans (AA) and Haitian Americans (HA) with and without type 2 diabetes using the Healthy Eating Index, 2005 (HEI-05), and the Alternate Healthy Eating Index (AHEI). The design was cross-sectional (225 AA, 246 HA) and recruitment was by community outreach. The eating indices were calculated from data collected with the Harvard food-frequency questionnaire. African Americans had lower HEI-05 scores (−8.67, 13.1); , than HA. Haitian American females and AA males had higher AHEI than AA females and HA males, respectively, () adjusting for age and education. Participants with diabetes had higher adherence to the HEI-05 (1.78, 6.01), , and lower adherence to the AHEI (16.3, −3.19), , , than participants without diabetes. The findings underscore the importance of disaggregating ethnicities and disease state when assessing diet.

Background: Blacks have a higher incidence of diabetes and its related complications. Self-rated health (SRH) and perceived stress indicators are associated with chronic diseases. The aim of this study was to examine the associations between SRH, perceived stress and diabetes status among two Black ethnicities. Materials and Methods: The cross-sectional study included 258 Haitian Americans and 249 African Americans with (n = 240) and without type 2 diabetes (n = 267) (N = 507). Recruitment was performed by community outreach. Results: Haitian-Americans were less likely to report ‘fair to poor’ health as compared to African Americans [OR=0.58 (95% CI: 0.35, 0.95), P = 0.032]; yet, Haitian Americans had greater perceived stress than African Americans (P = 0.002). Having diabetes was associated with ‘fair to poor’ SRH [OR=3.14 (95% CI: 2.09, 4.72),P < 0.001] but not perceived stress (P = 0.072). Haitian-Americans (P = 0.023), females (P = 0.003) and those participants having ‘poor or fair’ SRH (P < 0.001) were positively associated with perceived stress (Nagelkerke R2=0.151). Conclusion: Perceived stress associated with ‘poor or fair’ SRH suggests that screening for perceived stress should be considered part of routine medical care; albeit, further studies are required to confirm our results. The findings support the need for treatment plans that are patient-centered and culturally relevant and that address psychosocial issues.
