Healthcare-associated infections (HAIs) are known to increase the risk for patient morbidity and mortality in different healthcare settings and thereby to cause additional costs. HAIs typically affect patients with severe underlying conditions. HAIs are prevalent also among pediatric patients, but the distribution of the types of infection and the causative agents differ from those detected in adults. The aim of this study was to obtain information on pediatric HAIs in Finland through an assessment of the surveillance of bloodstream infections (BSIs), through two outbreak investigations in a neonatal intensive care unit (NICU), and through a study of postoperative HAIs after open-heart surgery. The studies were carried out at the Hospital for Children and Adolescents of Helsinki University Central Hospital. Epidemiological features of pediatric BSIs were assessed. For the outbreak investigations, case definitions were set and data collected from microbiological and clinical records. The antimicrobial susceptibilities of the Serratia marcescens and the Candida parapsilosis isolates were determined and they were genotyped. Patient charts were reviewed for the case-control and cohort studies during the outbreak investigations, as well as for the patients who acquired surgical site infections (SSIs) after having undergone open-heart surgery. Also a prospective postdischarge study was conducted to detect postoperative HAIs in these patients. During 1999-2006, the overall annual BSI rate was 1.6/1,000 patient days (range by year, 1.2–2.1). High rates (average, 4.9 and 3.2 BSIs/1,000 patient days) were detected in hematology and neonatology units. Coagulase-negative staphylococci were the most common pathogens both hospital-wide and in each patient group. The overall mortality was 5%. The genotyping of the 15 S. marcescens isolates revealed three independent clusters. All of the 26 C. parapsilosis isolates studied proved to be indistinguishable. The NICU was overcrowded during the S. marcescens clusters. A negative correlation between C. parapsilosis BSIs and fluconazole use in the NICU was detected, and the isolates derived from a single initially susceptible strain became less susceptible to fluconazole over time. Eighty postoperative HAIs, including all severe infections, were detected during hospitalization after open-heart surgery; 34% of those HAIs were SSIs and 25% were BSIs. The postdischarge study found 65 infections that were likely to be associated with hospitalization. The majority (89%) of them were viral respiratory or gastrointestinal infections, and these often led to rehospitalizations. The annual hospital-wide BSI rates were stable, and the significant variation detected in some units could not be seen in overall rates. Further studies with data adequately adjusted for risk factors are needed to assess BSI rates in the patient groups with the highest rates (hematology, neonatology). The outbreak investigations showed that horizontal transmission was common in the NICU. Overcrowding and lapses in hand hygiene probably contributed to the spreading of the pathogens. Following long-term use of fluconazole in the NICU, resistance to fluconazole developed in C. parapsilosis. Almost one-fourth of the patients who underwent open-heart surgery acquired at least one HAI. All severe HAIs were detected during hospitalization. The postdischarge study found numerous viral infections, which often caused rehospitalization.

El presente trabajo se realizó en los labortarios de la Escuela Nacional de Agricultura y Ganadería, durante los meses comprendidos entre Junio y Noviembre de 1966. Se examinaron nueve fincas en el Departamento de Managua con un total de 926 vacas en producción lactea. Para la determinacion de mastitis, se uso la prueba de Kloz Gerber y para el aislamiento de staphilococcus patogeno los medios de Hanitol Salt, Agar Sangre, Caldo nutritivo y para la prueba de coagulase el plasma sanguineo. Para la prueba de sensibilidad se usaron los antibioticos siguientes: Oxitetraciclina, CLortetraciclina, Penicilina, Dihidroestreptomicina, Puradantina, Cloranfenicol. De 926 vacas examinadas, 49 (5.29%) presentaron mastitis producidas por staphilococcus patógeno. De las vacas que fueron disgnosticadas con mastitis, el 42.25% fue originada por staphilococcus patogeno. La raza que presento mayor susceptibilidad al staphilococcus patogeno fue la Pardo Suizo y la de menos susceptibilidad, la vaca criolla. La penicila, fue el antibiotico que mostro mayor promedio de zona de inhibicion al staphilococcus patogeno. Los staphilicoccus patogenos aislados de leches de vacas mamiticas, no presentaron en este trabajo, resistencia absoluta a los antibioticos usados.

Os Staphylococcus coagulase-negativos (SCN) são encontrados na pele e mucosas de seres humanos e outros animais, já que algumas espécies são parte constituinte da microbiota normal destes mesmos sítios, e podem constituir um reservatório para SCN. A espécie Staphylococcus epidermidis, é reconhecida como grande oportunista e agente de graves infecções nosocomiais e comunitárias, além de associado com infecções em pacientes submetidos a implantes com dispositivos médicos, e a espécie Staphyloccus haemolyticus é a segunda espécie mais isolada de hemoculturas humanas, sendo uma das espécies que apresenta elevada resistência aos antimicrobianos. O presente estudo teve como objetivo principal investigar a presença de SCN em fômites (estetoscópios, termômetros e esfigmomanômetros) no ambiente hospitalar, identificar as espécies S. haemolyticus e S. epidermidis e correlacionar seus perfis de resistência aos antimicrobianos com a capacidade de produção de biofilme. A técnica de multiplex-mPCR foi empregada na determinação das espécies e a fenotipagem foi realizada pelos testes fenotípicos convencionais. Os perfis de resistência aos antimicrobianos foram verificados através do teste de disco-difusão, determinação da CIM (oxacilina e vancomicina), determinação da CBM e presença do gene mecA. A capacidade de produção de biofilme foi investigada pelos testes do Ágar Vermelho do Congo e ensaios de aderência em superfícies abióticas (poliestireno e vidro) na presença e ausência de oxacilina e vancomicina, além da PCR para o gene icaAD. Os resultados demonstraram que pelos testes bioquímicos convencionais, a espécie mais encontrada foi S. epidermidis (43,5%). Após a confirmação pela técnica de PCR, 29 amostras (82%) foram identificadas como S. epidermidis, e 6 amostras (18%) foram identificadas como S. haemolyticus. Todas as amostras foram multirresistentes, oxacilina resistentes e vancomicina sensíveis, sendo que apenas 5 amostras S. epidermidis (17,2%) foram tolerantes a oxacilina. A presença do gene mecA foi detectada em 71,4% das amostras. Apesar da maioria das amostras ter apresentado capacidade de produzir slime e/ou biofilme não foi observada total correlação com a presença do gene icaAD enfatizando a natureza multifatorial da produção de biofilme. As amostras aderiram melhor ao esfigmomanômetro, e também, neste fômites, foi encontrado a maior porcentagem de amostras positivas para a produção de slime. Para aderência ao vidro e aderência ao poliestireno não foi encontrada correlação com os fômites. Foram isoladas amostras S. epidermidis de todos os sítios hospitalares estudados e S. haemolyticus só não foi encontrado em Enfermaria de Clínica Médica. Em relação aos fômites, S. epidermidis foi encontrado em todos os fômites estudados, e S. haemolyticus, apenas foi encontrado em esfigmomanômetro e em outros fômites. Os fômites estão servindo como fontes de transmissão e disseminação de micro-organismos, sendo necessário maiores estudos a respeito.

A resistência aos antimicrobianos pela produção de β-lactamases em enterobactérias é um problema adicional neste cenário. Staphylococcus spp é considerado um patógeno humano freqüentemente associado a infecções adquiridas no ambiente hospitalar. O objetivo desse trabalho foi estudar a resistência aos antimicrobianos em cepas de Enterobactérias e Staphylococcus spp. isoladas de equipamentos utilizados no preparo de dietas destinadas à pacientes internados em um hospital universitário no município do Rio de Janeiro, além de verificar a presença de genes codificadores de enterotoxinas nas cepas de Staphylococcus spp. As enterobactérias e os Staphylococcus spp. foram isolados e identificados por metodologia convencional. Para o teste de susceptibilidade aos antimicrobianos foram utilizados discos contendo antimicrobianos clinicamente relevantes. Foi determinada a concentração inibitória mínima (CIM) pela técnica de microdiluição em caldo para os antimicrobianos clinicamente relevantes. A pesquisa de genes que codificam β-lactamases em enterobactérias foi realizada pela técnica de reação em cadeia da polimerase (PCR) e sequenciamento para os genes blaTEM, blaSHV, blaCTX-M, blaOXA-1 e blaCMY-2. Para Staphylococcus spp. foram pesquisados, através da PCR e sequenciamento, os genes de resistência a meticilina, gentamicina e eritromicina e os genes codificadores de enterotoxinas (sea-see, seg-sej, sen-ser e seu). Foram isoladas 97 cepas de enterobactérias, 40 de liquidificador e 57 de batedeira e 40 cepas de Staphylococcus spp., 20 de cada equipamento. Dentre as enterobactérias, o gênero Enterobacter foi isolado com maior frequencia (n=37, 38%). Foram ainda isoladas seis cepas de Salmonella spp. Das enterobactérias, 80% (n=79) foram resistentes a pelo menos um antimicrobiano e 38% (n=37) a três ou mais. A expressão fenotípica presuntiva de b-lactamases do tipo AmpC foi detectada em 32 cepas. O gene blaCMY-2 não foi encontrado. Doze cepas apresentaram halos de inibição com screaning positivo para a pesquisa de ESBL. Foi detectada a presença do gene blaSHV em K. pneumoniae subsp. pneumoniae. O sequenciamento desse gene permitiu identificá-lo como sendo blaSHV-36. Dentre os Staphylococcus spp., oito foram identificados como coagulase-positivas (SCP) e 32 como coagulase-negativas (SCN). As oito cepas de SCP foram identificadas como S. aureus subsp. aureus. Dentre os SCN, as espécies identificadas com maior frequência foram: S. caprae (n=7, 17,5%), S. simulans (n=5, 12,5%) e S. epidermidis (n=4, 10%). Destas, 83% (n=33) foram resistentes a pelo menos um antimicrobiano e 30% (n=12) foram resistentes a mais do que três. Duas cepas (5%) de S. epidermidis apresentaram perfil de resistência a até seis antimicrobianos e genes de resistência a meticilina, a eritromicina e a gentamicina. Todas as sete cepas que foram resistentes no TSA e na CIM a gentamicina, apresentaram o gene aac(2)/aph(6). O gene ermB foi observado ainda em uma S hominis subsp hominis que apresentou resistência na CIM e no TSA. Foi detectada a presença de pelo menos um gene codificador de enteroxotxina em 83% (n=33) das cepas. O gene seg foi detectado em 11 cepas, o gene sei em 17 cepas e o gene sen em 31 cepas. Os resultados encontrados implicam as dietas preparadas com esses equipamentos como veículo de disseminação de enteropatógenos e Staphylococcus spp. com marcadores de resistência e genes codificadores de enterotoxinas relevantes no ambiente hospitalar.

A general survey carried out on several brands of frozen prawn products has shown that along with the standard plate count (SPC), the numbers of pathogenic organisms like Escherichia coli, enterococci and coagulase positive staphylococci have also to be taken into consideration for the evaluation of the quality of these products. No correlation could be established between the total plate count and the number of E. coli, enterococci or staphylococci. Enumeration of enterococci, however, is advocated as a better index of faecal contamination of the products than E. coli.

The total viable bacterial populations in the oysters and the sea water from the edible oyster farm at Tuticorin were in the range of 10 super(3) to 10 super(4) per ml and 1 super(2) to 10 super(3) per ml respectively. The maximum most probable number of faecal coliform recorded during the one year period of study of both the oysters and seawater were 33 per 100 ml. Pathogenic bacteria (Salmonella sp., Vibrio cholerae, coagulase positive staphylococci and faecal streptococci were absent in oysters and farm water. Study of 197 (98 taken from oyster liquid and 99 from oyster farm water) randomly isolated cultures indicated that gram negative asporogenus rod-like bacteria of the Vibrio, Flavobacterium, Achromobacter and Pseudomonas groups were the dominant flora of the oyster liquid as well as seawater.

Assessment of quality of fish and shrimps landed at the fish harbour, Cochin, was made over the period January, 1980 to December, 1982. A total of 201 samples were analysed. Nearly 75% of the samples scored between 6-10 in a 10 point hedonic scale and the percentage unacceptability based on sensory assessment was 5. 5%. In 10.1% of the samples, total volatile nitrogen was >30 mg% and in 8.3% of the samples trimethylamine- nitrogen was >10 mg%. Both, the gr Torrymeter and Intellectron Fish Tester VI readings marked significant correlation with sensory scores and chemical indices; but failed to bear any significant correlation with bacterial counts. Of the 5-1 total samples 66.7% had total plate count (TPC) ≥10 super(5) g super(-1) and 8.5% were considered unacceptable based on TPC >5x10 super(5) g super(-1); 63.2% of the samples were free from Escherichia coli; 26.4% had >20 E. coli g super(-1) and 20.4% of the samples contained faecal streptococci >10 super(3) g super(-1). Seven percent of the samples showed coagulase positive staphylococci >100 g super(-1). Salmonella could not be detected in any of the samples examined. Crushed ice samples and the platform of the harbour had high bacterial loads.

Quality of 181 samples of ready-to-eat fish products comprising fried fish, fish curry and fish/prawn pickles collected from Cochin and Calicut were studied. Salmonella was absent in all the samples. V. cholerae was tested in the samples collected at Cochin and was absent in all the cases. Coliforms, E. coli, faecal streptococci and coagulase-positive staphylococci were present in some of the samples studied. The study indicated the necessity to improve the sanitary and hygienic conditions of the hotels engaged in the preparation of these products. The study further indicated that fried fish and fish curry shall not be served after 6 hours of their preparation. Added care is to be exercised in the selection of shrimps and fish for the preparation of pickles.

Sixty six samples of frozen fishery products consisting of frozen crab meat, frozen prawns, frozen cuttlefish and frozen mussel collected from nine cold stores situated in and around Cochin city were examined for coagulase positive staphylococci. Forty eight samples (72.72%) revealed the presence of coagulase positive staphylococci, which were then tested for enterotoxigenity, employing cellophane-over-agar method for toxin production and optimum-sensitivity-plate (OSP) method for toxin detection. Seventy seven out of one hundred and two (75.49%) coagulase positive staphylococcal isolates from the above products produced enterotoxins A, B, C, D and E either singly or in combinations. Enterotoxin A and D were detected more often than others. The occurrence of enterotoxigenic staphylococci in frozen fishery product is reported for the first time in this country.

Staphylococcus epidermidis, the most frequently isolated coagulase-negative staphylococcus, is the leading cause of infection related to implanted medical devices (IMDs). This is directly related to its capability to establish multilayered, highly structured biofilms on artificial surfaces. At present, conventional systemic therapies using standard antimicrobial agents represent the main strategy to treat and prevent medical device-associated infections. However, device-related infections are notoriously difficult to treat and bacteria within biofilm communities on the surface of IMDs frequently outlive treatment, and removal of the medical device is often required for successful therapy. Importantly, major advances in this research area have been made, leading to a greater understanding of the complexities of biofilm formation of S. epidermidis and resulting in significant developments in the treatment and prevention of infections related to this member of the coagulase-negative group of staphylococci. This review will examine the pathogenesis of the clinically significant S. epidermidis and provide an overview of the conventional and emerging antibiofilm approaches in the management of medical device-associated infections related to this important nosocomial pathogen.

The aim of the present study was to describe the practice of central venous catheter (CVC) removal and outcomes of catheter-related bloodstream infection (CR-BSI) in adult haematology patients. Patients were identified retrospectively according to diagnosis coding of inpatient episodes and evaluated when, on examination of medical records, there had been evidence of sepsis with strong clinical suspicion that the source was the CVC. Demographic and bacteriological data, as well as therapeutic measures and clinical outcomes, were recorded. One hundred and three patient episodes were evaluated. The most frequent type of CVC was the Hickman catheter and the most frequently isolated pathogen was coagulase-negative staphylococci. Twenty-five percent of episodes were managed with catheter removal. Treatment failure, defined as recurrence of infection within 90 days or mortality attributed to sepsis within 30 days, occurred significantly more frequently in the group managed without catheter removal (52.5% versus 4%, P

Propionibacterium acnes and coagulase-negative staphylococci (CoNS) are opportunistic pathogens implicated in prosthetic joint and fracture fixation device-related infections. The purpose of this study was to determine whether P. acnes and the CoNS species Staphylococcus lugdunensis, isolated from an "aseptically failed" prosthetic hip joint and a united intramedullary nail-fixed tibial fracture, respectively, could cause osteomyelitis in an established implant-related osteomyelitis model in rabbits in the absence of wear debris from the implant material. The histological features of P. acnes infection in the in vivo rabbit model were consistent with localized pyogenic osteomyelitis, and a biofilm was present on all explanted intramedullary (IM) nails. The animals displayed no outward signs of infection, such as swelling, lameness, weight loss, or elevated white blood cell count. In contrast, infection with S. lugdunensis resulted in histological features consistent with both pyogenic osteomyelitis and septic arthritis, and all S. lugdunensis-infected animals displayed weight loss and an elevated white blood cell count despite biofilm detection in only two out of six rabbits. The differences in the histological and bacteriological profiles of the two species in this rabbit model of infection are reflective of their different clinical presentations: low-grade infection in the case of P. acnes and acute infection for S. lugdunensis. These results are especially important in light of the growing recognition of chronic P. acnes biofilm infections in prosthetic joint failure and nonunion of fracture fixations, which may be currently reported as "aseptic" failure. Two ionic liquids, 1-ethylpyridinium docusate (IL1) and tri-n-butyl(2-hydroxyethyl)phosphonium docusate (IL2), were designed and synthesized with the explicit intention of imparting a combination of plasticization and antimicrobial efficacy when incorporated into medical grade poly(vinyl chloride)s (PVCs). The glass transition (T-g) of PVC can be reduced by >20 degrees C on addition of 15 wt% IL2. Both IL1 and IL2 leached to varying extents from the base PVC resins rendering the surface of the PVCs hydrophilic. The antimicrobial activity of both ILs is related to the presence and concentration of both cationic and anionic component of the ILs leached from the PVC and inversely proportional to the extent of PVC gelation. Blends of the PVCs with IL1 displayed antibacterial activity against almost all Gram-positive bacteria tested, including coagulase-negative Staphylococci (CoNS) and methicillin-resistant Staphylococcus aureus (MRSA), but not with IL2 at low concentration in contrast to our previous study when high concentrations of IL2 were used. The more hydrophilic IL1 when added to PVC retards biofilm formation.

Staphylococcus aureus are Gram-positive bacteria who integrate the human microbiota. Nevertheless, these bacteria can be pathogenic to the humans. Due to the increasing occurrence of antibiotic-resistant S. aureus new approaches to control this pathogen are necessary. The antimicrobial photodynamic inactivation process (PDI) is based in the combined use of a light source, an oxidizing agent like oxygen and an intermediary agent (a photosensitizer). These three components interact to form cytotoxic reactive oxygen species that irreversibly damage vital constituents of the microbial cells and ultimately lead to cell death. In fact, PDI is being shown to be a promising alternative to the antibiotic approach in the inactivation of pathogenic microorganisms. However, information on effects of photosensitization on particular virulence factors is strikingly scarce. The objective of this work was to evaluate the effect of PDI on virulence factors of S. aureus. For this, as photosensitizer the 5,10,15,20-tetrakis(1-methylpyridinium-4-yl)porphyrin tetra-iodide (Tetra-Py+-Me) and six strains of S. aureus (one reference strain, one strain with 1 enterotoxin, two strains with 3 enterotoxins and two strains resistant to methicillin, MRSA – one with 5 enterotoxins and the other without enterotoxins) were used. The effect of photosensitization on catalase activity, beta hemolysis, lipases, thermonuclease, enterotoxins, coagulase production and resistance to methicillin was assessed. The results indicate that the expression of some virulence factors in the cells subjected to this therapy is affected. Additionally the susceptibility of the strains to PDI did not decrease upon successive treatments.
