Venous ulcers (VU) is a chronic injury of the lower extremities and because of its high incidence and recurrence implies long and complex treatments, damaging the quality of life (QOL) and self-esteem (SE) of the people. This study aimed to analyze the association between self-esteem with the quality of life of people with venous ulcers treated in primary care. Cross sectional analytical study with a quantitative approach conducted with 44 people met with VU at 13 primary care units 2 and Mixed units in Natal/RN. The study was approved by the Ethics Committee in Research of the Federal University of Rio Grande do Norte (UFRN), CAAE: 07556312.0.0000.5537. Held data collection from February to April 2014 and used three instruments: a structured form covering sociodemographic, health care and clinical variables, the Rosenberg Self-Esteem Scale and the SF-36. The collected data were entered into a database and processed on computerized software for descriptive and inferential analyzes. The results showed a predominance of people with UV females (65,9%), with more than 60 years (59,1%), married or in a stable relationship (52,3%), low education (86,4%) without occupation (68,2%) and less than one minimum wage income (81,8%). Regarding assistance characteristics was observed that most patients performed the dressing with appropriate material (72,7%), professional or trained caregiver (61,4%) did not use compression therapy (81,8%), treating the injury for more than 6 months (77,3%), lack of guidelines for the use of compression therapy, elevation of legs, and regular exercise (77,3%) and consulting the angiologist last year (52,3% ). Regarding clinical features of the lesion was found that most of the recurrent lesions are (77,3%), over one year of current lesion (52,3%) medium to large lesions (54,8%), without signs of infection (61,3%) and pain (79,5%). The mean SE of respondents was 9,3 (± 5,1). The relations between the SE and the sociodemographic variables, health care and clinics showed that individuals without a partner (a) (p = 0,01), who did not wear compression therapy (p = 0,04), with more 6 months of treatment (p = 0,01) and larger lesions (p = 0,01) had a lower SE. The mean domain and the dimensions of the SF-36 were lower emphasizing the functional capacity 36.5 (± 27,6) and the physical aspects of 15.3 (± 30,6). There were significant correlations between AE people with VU and the domains and dimensions of the SF-36: physical functioning (r = -0,432), general health (r = -0,415), vitality (r = -0,573), aspects social (r = -0,517), mental health (r = -0,612) and mental health dimensions (r = -0,612) and physical health (r = -0,473). Based on these results it is concluded by rejecting the null hypothesis and accept the alternative proposed in the study in which it was found that there is a negative correlation between the SE and the QOL of people with venous ulcers

Proposals that aim a redirection of current health attention models in Brazil are indispensible due to the need of health issues and its challenges imposed by society. These issues come forth in Higher Education Institutions in an attempt to devise ways to face these demands. The research aims to analyze the Pedagogical Project (PP) present in the Dentistry Graduation Course in the Federal University of Rio Grande do Norte in Brazil-(UFRN). This is done in coherence the Brazilian National Curricular Guidelines considering main competences established in the PP. This research was approved by the Ethics Committee at UFRN under document number 285/201. The work is of descriptive nature and was realized with 30 students in the Dentistry Graduation Course. Interviews were realized with the use of problem situation approach. The research was also supported by documental studies that dealt with syllabus present in the disciplines taught at UFRN. Data were processed with the use of the ALCESTE 4.9 software. It is possible to acknowledge that some conservative conceptions arise, even though there is use of active methodologies and innovation that aim to promote reflection and articulation for general competence development such as proposed in the Pedagogical Project in Dentistry Graduation Course at UFRN. These conceptions are mainly present in the teaching-learning process where students do not have full participation. Thus it is possible to conclude that even though there are advances and breakthroughs. This is seen with that fact that there was inclusion of multidisciplinary clinical work as well optional courses in the curriculum. It was also seen that there was occasional use of active teaching methodologies in Dentistry at UFRN. But there is still a need for a didactical and methodological resizing. These actions require the need for progressive development of competences and abilities during the formative process according to what was established in the Brazilian National Curricular Guidelines

Background: Cardiovascular disease is the leading cause of death in Brazil, and hypertension is its major risk factor. The benefit of its drug treatment to prevent major cardiovascular events was consistently demonstrated. Angiotensin-receptor blockers (ARB) have been the preferential drugs in the management of hypertension worldwide, despite the absence of any consistent evidence of advantage over older agents, and the concern that they may be associated with lower renal protection and risk for cancer. Diuretics are as efficacious as other agents, are well tolerated, have longer duration of action and low cost, but have been scarcely compared with ARBs. A study comparing diuretic and ARB is therefore warranted.Methods/design: This is a randomized, double-blind, clinical trial, comparing the association of chlorthalidone and amiloride with losartan as first drug option in patients aged 30 to 70 years, with stage I hypertension. The primary outcomes will be variation of blood pressure by time, adverse events and development or worsening of microalbuminuria and of left ventricular hypertrophy in the EKG. The secondary outcomes will be fatal or non-fatal cardiovascular events: myocardial infarction, stroke, heart failure, evidence of new subclinical atherosclerosis and sudden death. The study will last 18 months. The sample size will be of 1200 participants for group in order to confer enough power to test for all primary outcomes. The project was approved by the Ethics committee of each participating institution.Discussion: The putative pleiotropic effects of ARB agents, particularly renal protection, have been disputed, and they have been scarcely compared with diuretics in large clinical trials, despite that they have been at least as efficacious as newer agents in managing hypertension. Even if the null hypothesis is not rejected, the information will be useful for health care policy to treat hypertension in Brazil. Background: Blood pressure (BP) within pre-hypertensive levels confers higher cardiovascular risk and is an intermediate stage for full hypertension, which develops in an annual rate of 7 out of 100 individuals with 40 to 50 years of age. Non-drug interventions to prevent hypertension have had low effectiveness. In individuals with previous cardiovascular disease or diabetes, the use of BP-lowering agents reduces the incidence of major cardiovascular events. In the absence of higher baseline risk, the use of BP agents reduces the incidence of hypertension. The PREVER-prevention trial aims to investigate the efficacy, safety and feasibility of a population-based intervention to prevent the incidence of hypertension and the development of target-organ damage.Methods: This is a randomized, double-blind, placebo-controlled clinical trial, with participants aged 30 to 70 years, with pre-hypertension. The trial arms will be chlorthalidone 12.5 mg plus amiloride 2.5 mg or identical placebo. The primary outcomes will be the incidence of hypertension, adverse events and development or worsening of microalbuminuria and of left ventricular hypertrophy in the EKG. The secondary outcomes will be fatal or non-fatal cardiovascular events: myocardial infarction, stroke, heart failure, evidence of new sub-clinical atherosclerosis, and sudden death. The study will last 18 months. The sample size was calculated on the basis of an incidence of hypertension of 14% in the control group, a size effect of 40%, power of 85% and P alpha of 5%, resulting in 625 participants per group. The project was approved by the Ethics committee of each participating institution.Discussion: The early use of blood pressure-lowering drugs, particularly diuretics, which act on the main mechanism of blood pressure rising with age, may prevent cardiovascular events and the incidence of hypertension in individuals with hypertension. If this intervention shows to be effective and safe in a population-based perspective, it could be the basis for an innovative public health program to prevent hypertension in Brazil.Trial Registration: Clinical Trials NCT00970931. © 2011 Fuchs et al; licensee BioMed Central Ltd.

A Encefalopatia Crônica Não Progressiva da Infância (ECNP) é a sequela neurológica com maior comprometimento motor para a criança, e continua sendo na atualidade a hipóxicoisquemia perinatal a maior causa de lesão cerebral. É conhecida como Paralisia Cerebral, sendo definida por uma sequela de agressão encefálica, caracterizada, principalmente, por um transtorno persistente, mas não invariável do tônus, da postura e do movimento, que aparece na primeira infância. A caracterização da ECNP se faz considerando as condições anatômicas, etiológicas, semiológicas e não evolutiva. Neste estudo adotou-se a classificação baseada em aspectos anatômicos e clínicos, que enfatizam o sintoma motor, enquanto elemento principal do quadro clínico. A neuroimagem tem fundamental importância para o diagnóstico e prognóstico de lesões cerebrais, exercendo a importante função de descartar ou confirmar a presença de lesões em recém-nascidos e nas crianças com alterações no desenvolvimento. A Tomografia Cerebral (TAC) e a Ressonância Magnética do Crânio (RM) vêm desempenhando enorme papel para o estudo dos vários tecidos que constituem o sistema nervoso. Assim este estudo teve o objetivo geral de avaliar os padrões neuropatológicos nas substâncias branca e cinzenta, obtidos por TAC ou RM de Crânio, de pacientes com história clínica de ECNP hipóxico-isquêmica perinatal, correlacionando os dados obtidos por neuroimagem com os padrões motores obtidos por exame clínico-neurológico. Foram obedecidas as normas vigentes para estudo em seres humanos impostas pela Resolução CNS 196/96, submetida ao Comitê de Ética e Pesquisa da Plataforma Brasil sob o Nº 112168. A população foi constituída por pacientes com idade de zero a sete anos, de ambos os sexos, atendidos no Ambulatório de Paralisia Cerebral do Projeto Caminhar do Hospital Universitário Bettina Ferro de Souza (HUBFS), com diagnóstico de ECNP. A amostra do estudo foi composta por 15 crianças com diagnóstico de ECNP por Hipóxia neonatal. Para o diagnóstico radiológico em neuroimagem foram utilizados os dados dos laudos da TAC e da RM de Crânio. A avaliação clínico-neurológica utilizou para a avaliação do movimento o modelo da escala Gross Motor Function Classification System (GMFCS E&R), elaborada por Palisano, que gradua a criança em cinco níveis no qual o Nível I corresponde à normalidade e o Nível V a maior gravidade de limitação. Das 15 crianças avaliadas quanto ao movimento e a relação do Nível de Motricidade pela GMFCS E&R 05 crianças apresentavam nível V, 04 crianças nível IV, 05 crianças nível III e 01 criança nível II. Quanto ao imageamento cerebral 46% realizaram TAC e 54% RM do Crânio. A RM de Crânio apresentou-se neste estudo como a imagem de eleição, pois das 8 crianças que realizaram o exame, 6 apresentavam alterações. Ficou evidente que o exame por imagem de eleição para a criança que apresenta Encefalopatia Crônica não Progressiva é a RM de Crânio, podendo se adotar como protocolo para a conclusão diagnóstica, evitando expor a criança a uma carga elevada de RX como ocorre na TAC, e ainda, evitando gastos desnecessários para a saúde pública.

A modalidade violência psicológica é mais conhecida pela sua “invisibilidade” no âmbito público em razão de, entre outros fatores, ocorrer mais frequentemente na esfera privada, bem como por não deixar marcas físicas. Atualmente, a Lei 11.340/2006, batizada de “Lei Maria da Penha”, depois de sancionada, traduz uma forma de amparo legal e institucionalizado para as mulheres. Empregamos a concepção de gênero segundo Scott (1991), como uma das ferramentas analíticas que permitem identificar nexos entre a construção socioeconômica da violência e as políticas do Estado. Neste panorama, apresentamos como objetivo geral desta pesquisa empírica desvelar algumas (in) visíveis sequelas psíquicas e sociais e de modo específico as repercussões na subjetividade da mulher que vivencia situações de violência psicológica ocorridas em âmbito doméstico e intrafamiliar. As análises foram realizadas na perspectiva Gestáltica, uma abordagem psicológica do contato consciente, cuja intervenção permite o fortalecimento do suporte interno e auto-regulação saudável, de modo a superar situações que obscurecem as funções e fronteiras de contato. Trata-se de uma pesquisa clínico-qualitativa de base fenomenológico-existencial-gestáltica e hermenêutica. Os procedimentos utilizados foram: submissão do projeto ao Comitê de Ética do CCS/UFPA; obtenção da autorização Institucional; identificação e convite a três mulheres para participarem da pesquisa, segundo o perfil de inclusão na amostra: disponibilidade para a pesquisa, faixa etária de 25 a 45 anos, que esteve ou está vivenciando situação de violência psicológica com seu marido/companheiro. Posteriormente, foi assinado o Termo de Consentimento Livre e Esclarecido e realizado as entrevistas semi-dirigidas através de perguntas abertas (gravadas em áudio). As mesmas foram transcritas e analisadas. O local da pesquisa foi o Centro de Referência Maria do Pará. Utilizamos para a análise dos discursos coletados a compreensão de Ricouer (1975) e os conceitos Gestálticos de contato, funções e fronteiras de contato, mecanismos de defesa, self, ajustamento criativo e awareness. O resultado aponta para o desvelamento de vividos permeados de agressões verbais em forma de humilhações, xingamentos, ofensas, ciúmes, desqualificação de sua aparência física, falta de diálogo, isolamento social e emocional, medo, sofrimento, dor, angústia, culpa, vergonha, sentimentos de ódio, raiva, tristeza e impotência diante de tal violência. Concluímos que a “invisibilidade” de tais experiências de violência psicológica gera visíveis interrupções no contato consigo mesma, em suas relações familiares e sociais, bem como, imprime profundas e danosas desestruturações na personalidade e na maneira da mulher expressar sua subjetividade.

Purpose: To investigate the predictors of intolerance to beta-blockers treatment and the 6-month mortality in hospitalized patients with acute coronary syndrome (ACS).Methods: This was a single-center, prospective, and longitudinal study including 370 consecutive ACS patients in Killip class I or II. BBs were prescribed according to international guidelines and withdrawn if intolerance occurred. The study was approved by the institutional ethics committee of our university. Statistics: the clinical parameters evaluated at admission, and the related intolerance to BBs and death at 6 months were analyzed using logistic regression (p<0.05) in PATIENTS.Results: BB intolerance was observed in 84 patients and was associated with no prior use of statins (OR: 2.16, 95%CI: 1.26-3.69, p= 0.005) and Killip class II (OR: 2.5, 95%CI: 1.30-4.75, p=0.004) in the model adjusted for age, sex, blood pressure, and renal function. There was no association with ST-segment alteration or left anterior descending coronary artery plaque. Intolerance to BB was associated with the greatest risk of death (OR: 4.5, 95%CI: 2.15-9.40, p<0.001).Conclusions: After ACS, intolerance to BBs in the first 48 h of admission was associated to non previous use of statin and Killip class II and had a high risk of death within 6 months.
