Background: As the human body ages, the arteries gradually lose their elasticity and become stiffer. Although inevitable, this process is influenced by hereditary and environmental factors. Interestingly, many classic cardiovascular risk factors affect the arterial stiffness. During the last decade, accelerated arterial stiffening has been recognized as an important cardiovascular risk factor associated with increased mortality as well as with several chronic disorders. Objectives: This thesis examines the role of arterial stiffness in relation to variations in a physiological feature in healthy individuals. In addition, the effect on arterial stiffness of an acute transitory disease and the effect of a chronic disease are studied. Furthermore, the thesis analyzes the prognostic value of a marker of arterial stiffness in individuals with chronic disease. Finally, a potential method of reducing arterial stiffness is evaluated. Material and study design: The first study examines pulse wave reflection and pulse wave velocity in relation to muscle fibre distribution in healthy middle-aged men. In the second study, pulse wave reflection in women with current or previous preeclampsia is compared to a healthy control group. The effect of aging on the different blood pressure indices in patients with type 1 diabetes is examined in the third study, whereas the fourth paper studies the relation between these blood pressure indices and mortality in type 2 diabetes. The fifth study evaluates how intake of a fermented milk product containing bioactive peptides affects pulse wave reflection in individuals with mild hypertension. Results and conclusions: Muscle fibre type distribution is not an independent determinant of arterial stiffness in middle-aged males. Pulse wave reflection is increased in pregnant women with preeclampsia, but not in previously preeclamptic non-pregnant women. Patients with type 1 diabetes have a higher and more rapidly increasing pulse pressure, which suggests accelerated arterial stiffening. In elderly type 2 diabetic patients, very high and very low levels of pulse pressure are associated with higher mortality. Intake of milk-derived bioactive peptides reduces pulse wave reflection in hypertensive males but not in hypertensive females.

Background. Patients with type 1 diabetes are at markedly increased risk of vascular complications. In this respect it is noteworthy that hyperglycaemia that is shown to cause endothelial dysfunction, has clearly been shown to be a risk factor for diabetic microvascular disease. However, the role of hyperglycaemia as a predictor of macrovascular disease is not as clear as for microvascular disease, although type 1 diabetes itself increases the risk of cardiovascular disease substantially. Furthermore, it is not known whether it is the short-term or the long-term hyperglycaemia that confers possible risk. In addition, the role of glucose variability as a predictor of complications is to a large extent unexplored. Interestingly, although hyperglycaemia increases the risk of pre-eclampsia in women with type 1 diabetes, it is unclear whether pre-eclampsia, a condition characterized by endothelial dysfunction, is also a risk factor for microvascular complication, diabetic nephropathy. Aims. This doctoral thesis investigated the role of acute hyperglycaemia and glucose variability on arterial stiffness and cardiac ventricular repolarisation in male patients with type 1 diabetes as well as in healthy male volunteers. The thesis also explored whether acute hyperglycaemia leads to an inflammatory response, endothelial dysfunction and oxidative stress. Finally, the role of pre-eclampsia, as a predictor of diabetic nephropathy in type 1 diabetes was examined. Subjects and methods. In order to study glucose variability and the daily glycaemic control, 22 male patients with type 1 diabetes, without any diabetic complications, were monitored for 72-h with a continuous glucose monitoring system. At the end of the 72-h glucose monitoring period a 2-h hyperglycaemic clamp was performed both in the patients with type 1 diabetes and in the 13 healthy age-matched male volunteers. Blood pressure, arterial stiffness and QT time were measured to detect vascular changes during acute hyperglycaemia. Blood samples were drawn at baseline (normoglycaemia) and during acute hyperglycaemia. In another patient sample, women with type 1 diabetes were followed during their pregnancy and restudied eleven years later to elucidate the role of pre-eclampsia and pregnancy-induced hypertension as potential risk factors for diabetic nephropathy. Results and conclusions. Acute hyperglycaemia increased arterial stiffness as well as caused a disturbance in the myocardial ventricular repolarisation, emphasizing the importance of a strict daily glycaemic control in male patients with type 1 diabetes. An inflammatory response was also observed during acute hyperglycaemia. Furthermore, a high mean daily blood glucose but not glucose variability per se is associated with arterial stiffness. While glucose variability in turn correlated with central blood pressure, the results suggest that the glucose metabolism is closely linked to the haemodynamic changes in male patients with uncomplicated type 1 diabetes. Notably, the results are not directly applicable to females. Finally, a history of a pre-eclamptic pregnancy, but not pregnancy-induced hypertension was associated with increased risk of diabetic nephropathy.

Nos últimos anos, a população de idosos vem crescendo em todo o mundo, e a pesquisa de marcadores de risco cardiovascular mais precoces é de fundamental importância. Um desses marcadores é a doença arterial periférica (DAP), cuja prevalência aumenta com a idade. A DAP pode ser avaliada de forma simples e não invasiva através do índice tornozelo-braquial (ITB) que, por sua vez, pode ser obtido por cálculos diferentes. Até o momento, apenas o método tradicional foi utilizado no cálculo do ITB na população idosa. O objetivo do presente estudo foi avaliar, em idosos hipertensos que se mostrassem independentes para as atividades diárias, os principais fatores relacionados à redução do ITB, considerando-se duas formas distintas de calcular o índice. Os pacientes (n=65) foram submetidos à avaliação clínica, geriátrica e laboratorial, e divididos nos grupos com ITB normal (> 0,9) e ITB reduzido (≤ 0,9). Inicialmente o ITB foi calculado a partir da divisão da maior pressão sistólica dos membros inferiores pela maior pressão das artérias braquiais. Em seguida, o cálculo do ITB foi realizado por um método alternativo, a partir da utilização da menor ao invés da maior média de pressão sistólica nos membros inferiores. A média de idade foi de 74 anos, sendo 76% do sexo feminino. A prevalência de ITB reduzido foi de 18% pelo método convencional e de 32% pelo método alternativo. Na avaliação pelo método convencional, o grupo com ITB baixo apresentou maior prevalência de doenças cardiovasculares (58 vs 9%, p<0,001), diabetes (83 vs 13%, p<0,01), síndrome metabólica (75 vs 41%, p<0,05), e valores significativamente maiores de pressão arterial sistólica (1699 vs 1523 mmHg, p<0,05) e pressão de pulso (877 vs 672 mmHg, p<0,01). A redução do ITB pelo método alternativo mostrou associação com as mesmas variáveis, mas adicionalmente com maior freqüência de tabagistas (29 vs 20%, p<0,05) e maiores níveis de LDL-colesterol (15413 vs 1245 mg/dl, p<0,05). Além disso, o método alternativo foi capaz de detectar pacientes sem alto risco pelo escore de Framingham, mas obesos e com síndrome metabólica. Esses dados apontariam para um valor adicional desta forma de estimar o risco ao escore de Framingham na estratificação de risco cardiovascular, sugerindo sua incorporação na rotina de avaliação de pacientes idosos

A aterosclerose e suas complicações são a principal causa de morbidade e mortalidade no mundo ocidental. O aumento da espessura da camada médio-intimal da carótida está associado com risco para doenças cardiovasculares, pois representa um marcador de aterosclerose subclínica, podendo ser detectada precocemente em indivíduos assintomáticos. O objetivo desse estudo foi identificar variáveis clínicas e nutricionais associadas com a aterosclerose subclínica em mulheres hipertensas. Estudo transversal envolvendo uma amostra de conveniência composta por 116 mulheres hipertensas entre 40 e 65 anos. Dados clínicos, como pressão arterial (PA) sistólica e diastólica, história de tabagismo, atividade física, uso de medicamentos foram coletados; foi feita a análise do perfil lipídico, glicemia e proteína C reativa (PCR); a avaliação dietética obtida pelo Recordatório de 24 horas e pelo Registro de três dias. A espessura médio-intimal (EMI) de carótidas foi realizada pelo aparelho de ultrassonografia. As pacientes foram divididas em dois grupos, de acordo com os valores da espessura médio-intimal de carótidas: EMI 0,9mm ou EMI > 0,9mm. Houve diferença significativa entre os grupos em relação à idade (50,846,62 vs 53,547,13; p=0,044), PA sistólica (134,5216,54 vs 142,9821,47; p=0,020), pressão de pulso (PP) (49,3611,03 vs 60,15 17,77; p<0,001), HDL (48,988,54 vs 44,057,45; p=0,004) e PCR (2,311,21 vs 3,051,34; p=0,016). Não houve diferença significativa em relação aos parâmetros antropométricos, exceto em relação à reactância (65,199,69 vs 61,447,88; p=0,036), avaliada pela bioimpedância elétrica (BIA). Quanto ao padrão de consumo alimentar, somente o consumo de gordura monoinsaturada foi diferente entre os grupos, sendo o maior consumo no grupo com menor valor de EMI (7,882,09 vs 7,022,06; p=0,031). Não houve diferença em relação à frequência de tabagismo e atividade física. Quando foi feita a análise de correlação da amostra, foi encontrada uma correlação entre a EMI de carótidas e idade (r=0,25; p=0,0067), PAS (r=0,19; p=0,0086); PP (r=0,30; p=0,0009), LDL (r=0,19; p=0,0434), assim como com gordura monoinsaturada (r= -0,25; p=0,0087), PCR (r=0,31; p=0,007) e HDL (r=-0,33; p=0,0004), porém apenas as variáveis HDL, PCRus e pressão de pulso mostraram ser preditoras independentes da EMI de carótida após feita uma análise de regressão linear multivariada. A proteína C reativa, HDL colesterol e pressão de pulso são importantes preditores independentes de aterosclerose subclínica.

O propofol é uma droga hipnótico-sedativa, amplamente utilizada em anestesiologia, devido à sua elevada eficácia hipnótica e ao seu despertar rápido e praticamente isento de efeitos residuais. A depressão cardiovascular, que é o efeito adverso mais importante e indesejável do propofol, parece estar intimamente relacionada à dose administrada, à velocidade de injeção do medicamento, à idade e ao estado físico dos pacientes, assim como, às condições de volemia e de reserva cardiovascular dos mesmos, podendo ser intensificada, ainda, pela associação de uma droga opioide ao propofol. A hipotensão arterial induzida pelo propofol parece possuir uma origem multifatorial, pois já foi evidenciada a participação de diversos sistemas nos efeitos produzidos pelo propofol, inclusive, a participação da via opioide no efeito antinociceptivo da droga. Portanto, o objetivo desse estudo foi investigar a participação dos receptores opioides no efeito hipotensor arterial do propofol. A pesquisa foi do tipo randomizada, transversal, aberta e comparativa. Foram estudados 40 pacientes estado físico ASA 1, submetidos a anestesia geral para cirurgias eletivas. Os desfechos avaliados neste estudo foram as pressões arteriais sistólica (PAS), diastólica (PAD) e média (PAM), a pressão venosa central (PVC) e a frequência cardíaca (FC). Portanto, após a monitorização contínua dos pacientes com cardioscopia com análise de ST, pressão arterial invasiva, pressão venosa central, oxicapnometria, análise eletroencefalográfica bispectral (BIS) e gasimetrias arteriais e venosas, estes foram divididos aleatoriamente através de programa de distribuição aleatória gerada por computador, de acordo com os 5 grupos existentes (n = 8 para todos os grupos). Foram 3 grupos de pacientes pré-medicados intravenosamente (iv) com naloxona 1 μg/Kg (PN1) ou 3 μg/Kg (PN3), ou com salina para controle (CP), 2 minutos antes da administração iv de propofol (2,5 mg/Kg). Outros 2 grupos controle da naloxona foram pré-medicados com naloxona 1 μg/Kg (CN1) ou 3 μg/Kg (CN3), mas não receberam propofol. Os resultados demonstraram que há mais de um mecanismo envolvido no efeito hipotensor arterial do propofol, que é composto por uma rápida redução inicial da PAM opioide-independente e uma lenta redução final da PAM, que foi dependente, ao menos parcialmente, de ação opioide. O propofol, na dose de 2,5 mg/Kg, reduziu a PAS, PAD e PAM de forma significativa e independente do efeito hipnótico do anestésico, dos valores iniciais de PVC e de modificações na FC dos pacientes, apesar de ter sido evidenciada a inibição do baroreflexo induzida pelo anestésico. A hipotensão induzida pelo propofol pôde ser parcialmente reduzida, de forma significativa e dose-dependente, pela prévia administração iv de naloxona de 3 μg/Kg, menor dose efetiva. Este efeito da naloxona ocorreu, principalmente, através de um aumento significativo da PAD, sem importar em modificações significativas da PAS, FC, PVC, BIS ou do ECG. Foi concluído que a hipotensão arterial induzida pelo propofol possui uma origem multifatorial, sendo produzida parcialmente por um mecanismo que é opioide-dependente e sensível ao pré-tratamento dos pacientes com a naloxona. Nosso estudo não recebeu nenhum financiamento externo.

Objective: Endothelial function may be impaired in critical illness. We hypothesized that impaired endothelium-dependent vasodilatation is a predictor of mortality in critically ill patients.
Design: Prospective observational cohort study.
Setting: Seventeen-bed adult intensive care unit in a tertiary referral university teaching hospital. Patients: Patients were recruited within 24 hrs of admission to the intensive care unit.
Interventions: The SphygmoCor Mx system was used to derive the aortic augmentation index from radial artery pulse pressure waveforms. Endothelium-dependent vasodilatation was calculated as the change in augmentation index in response to an endothelium-dependent vasodilator (salbutamol).
Measurements and Main Results: Demographics, severity of illness scores, and physiological parameters were collected. Statistically significant predictors of mortality identified using single regressor analysis were entered into a multiple logistic regression model. Receiver operator characteristic curves were generated. Ninety-four patients completed the study. There were 80 survivors and 14 nonsurvivors. The Simplified Acute Physiology Score II, the Sequential Organ Failure Assessment score, leukocyte count, and endothelium-dependent vasodilatation conferred an increased risk of mortality. In logistic regression analysis, endothelium-dependent vasodilatation was the only predictor of mortality with an adjusted odds ratio of 26.1 (95% confidence interval [CI], 4.3-159.5). An endothelium-dependent vasodilatation value of 0.5% or less predicted intensive care unit mortality with a sensitivity of 79% (CI, 59-88%) and specificity of 98% (CI, 94-99%).
Conclusions: In vivo bedside assessment of endothelium-dependent vasodilatation is an independent predictor of mortality in the critically ill. We have shown it to be superior to other validated severity of illness scores with high sensitivity and specificity.

Efectuou-se um estudo prospectivo e observacional em 1200 doentes, maiores de 18 anos, com hipertensão essencial e sem eventos cardiovasculares (CV) prévbs que foram referenciados entre 1991 e 1998 para realização de MAPA no Serviço de Cardiologia do Hospital Infante D. Pedro, Aveiro (HIP), na Unidade de Hipertensão e Risco CV do Hospital Pedro Hispano (HPH), Matosinhos e na Clínica Cross/Moss do Porto. O objectivo principal deste estudo foi avaliar, nesta população, seguida durante 8- 12 anos, o valor preditivo de morbi-mortalidade CV da pressurometria ambulatória de 24 horas (MAPA), nomeadamente de diversos índices e valores dela extraída, tais como: (i) valores médios da pressão arterial (PA) de 24-h, diurna e nocturna; (ii) queda tensional noctuma, (iii) diferentes padrões de queda tensional nocturna em particular do padrão Non Dipper; (iv) PA diferencial de 24h, diurna e nodurna; (v) frequência cardíaca de 24 horas (vi) Ambulatory Arterial Stifness. Foram incluídos registos de MAPA de 1200 doentes, de um total de 2644, dos quais 53,8% eram mulheres e 10,2% eram diabéticos. Ao longo de um follow up médio de 8,2+3,0 anos, verificaram-se 62 óbitos e 152 eventos CV fatais e não fatais compreendendo 79 AVC, 51 eventos coronários e 22 classificados de outros eventos cardiovasculares. O estudo permitiu concluir que: 1- A MAPA foi superior a PA casual como preditor de eventos CV globais e de acidentes cerebrovasculares (AVC). 2 - De entre os valores da MAPA, o valor preditivo dos valores sistólicos (de 24 h, diurnos e nocturnos) foi superior ao dos respectivos valores diastólicos e o dos valores nocturnos foi superior ao dos valores diurnos. 3 - 0s eventos CV totais e os eventos coronários foram mais frequentes nos homens do que nas mulheres, sem diferença relativamente ao AVC. O valor preditivo independente da MAPA (sobretudo da PAS e da PP24h) foi superior nas mulheres vs homens relativamente aos eventos globais e AVC. 4- 0s padrões non-dipper e reverted-dipper de queda tensional nocturna associaram-se a pior prognóstico cardiovascular, relativamente ao padrão dipper. Contudo, dentro do padrão non-dipper, somente os doentes com queda tensional nocturna entre 0-4,9% -non-dippers 1 (mas não os doentes com queda tensional nocturna entre 5,O-9,9% -non-dippers 2) apresentaram risco CV superior ao do padrão dipper e semelhante ao do padrão reverted-dipper. Este achado poderá implicar a reclassificação do padrão non-dipper e do risco CV a ele associado. 5- O índice de rigidez arterial derivado da MAPA (AASI) correlaciona-se com outros índices (ex. a velocidade da onda de pulso) e contribui para estratificar o risco CV (eventos CV e AVC). O valor preditivo CV do AASI não é globalmente superior ao da PP 24h, embora em alguns casos o AASI possa acrescentar informação prognostica adicional a PP.

Purpose: In extreme situations, such as hyperacute rejection of heart transplant or major bleeding per-operating complications, an urgent heart explantation might be the only means of survival. The aim of this experimental study was to improve the surgical technique and the hemodynamics of an Extracorporeal Membrane Oxygenation (ECMO) support through a peripheral vascular access in an acardia model. Methods: An ECMO support was established in 7 bovine experiments (59±6.1 kg) by the transjugular insertion to the caval axis of a self-expanded cannula, with return through a carotid artery. After baseline measurements of pump flow and arterial and central venous pressure, ventricular fibrillation was induced (B), the great arteries were clamped, the heart was excised and right and left atria remnants, containing the pulmonary veins, were sutured together leaving an atrial septal defect (ASD) over the cannula in the caval axis. Measurements were taken with the pulmonary artery (PA) clamped (C) and anastomosed with the caval axis (D). Regular arterial and central venous blood gases tests were performed. The ANOVA test for repeated measures was used to test the null hypothesis and a Bonferroni t method for assessing the significance in the between groups pairwise comparison of mean pump flow. Results: Initial pump flow (A) was 4.3±0.6 L/min dropping to 2.8±0.7 L/min (P B-A= 0.003) 10 minutes after induction of ventricular fibrillation (B). After cardiectomy, with the pulmonary artery clamped (C) it augmented not significantly to 3.5±0.8 L/min (P C-B= 0.33, P C-A= 0.029). Finally, PA anastomosis to the caval axis was followed by an almost to baseline pump flow augmentation (4.1±0.7 L/min, P D-B= 0.009, P D-C= 0.006, P D-A= 0.597), permitting a full ECMO support in acardia by a peripheral vascular access. Conclusions: ECMO support in acardia is feasible, providing new opportunities in situations where heart must urgently be explanted, as in hyperacute rejection of heart transplant. Adequate drainage of pulmonary circulation is pivotal in order to avoid pulmonary congestion and loss of volume from the normal right to left shunt of bronchial vessels. Furthermore, the PA anastomosis to the caval axis not only improves pump flow but it also permits an ECMO support by a peripheral vascular access and the closure of the chest.

This study examined factors contributing to the differences in left ventricular mass as measured by Doppler echocardiography in children. Fourteen boys (10.3 ± 0.3 years of age) and 1 1 girls (10.5 ± 0.4 years of age) participated in the study. Height and weight were measured, and relative body fat was determined from the measurement of skinfold thickness according to Slaughter et al. (1988). Lean Body Mass was then calculated by subtracting the fat mass from the total body mass. Sexual maturation was self-assessed using the stages of sexual maturation by Tanner (1962). Both pubic hair development and genital (penis or breast for boys and girls respectively) development were used to determine sexual maturation. Carotid Pulse pressure was assessed by applanation tomometry in the left carotid artery. Cardiac mass was measured by Doppler Echocardiography. Images of cardiac structures were taken using B-Mode and were then translated to M- Mode. The dimensions at the end diastole were obtained at the onset of the QRS complex of the electrocardiogram in a plane through a standard position. Measurements included: (a) the diameter of the left ventricle at the end diastole was measured from the septum edge to the endocardium mean border, (b) the posterior wall was measured as the distance from to anterior wall to the epicardium surface, and (c) the interventricular septum was quantified as the distance from the surface of the left ventricle border to the right ventricle septum surface. Systolic time measurements were taken at the peak of the T-wave of the electrocardiogram. Each measurement was taken three to five times before averaging. Average values were used to calculate cardiac mass using the following equation (Deveraux et al. 1986). Weekly physical activity metabolic equivalent was calculated using a standardize activity questionnaire (Godin and Shepard, 1985) and peakV02 was measured on a cycloergometer. There were no significant differences in cardiovascular mesurements between boys and girls. Left ventricular mass was correlated (p<0.05) with size, maturation, peakV02 and physical activity metabolic equivalent. In boys, lean body mass alone explained 36% of the variance in left ventricular mass while weight was the single strongest predictor of left ventricular mass (R =0.80) in girls. Lean body mass, genital developemnt and physical activity metabolic equivalent together explained 46% and 81% in boys and girls, respectively. However, the combination of lean body mass, genital development and peakV02 (ml kgLBM^ min"') explained up to 84% of the variance in left ventricular mass in girls, but added nothing in boys. It is concluded that left ventricular mass was not statistically different between pre-adolescent boys and girls suggesting that hormonal, and therefore, body size changes in adolescence have a main effect on cardiac development and its final outcome. Although body size parameters were the strongest correlates of left ventricular mass in this pre-adolescent group of children, to our knowledge, this is the first study to report that sexual maturation, as well as physical activity and fitness, are also strong associated with left ventricular mass in pre-adolescents, especially young females. Arterial variables, such as systolic blood pressure and carotid pulse pressure, are not strong determinants of left ventricular mass in this pre-adolescent group. In general, these data suggest that although there is no gender differences in the absolute values of left ventricular mass, as children grow, the factors that determine cardiac mass differ between the genders, even in the same pre-adolescent age.

The objective of this study was to examine the association between body composition and arterial stiffuess in peri-pubescent boys and girls. Differences in arterial distensibility were measured in 68 children (45 normal weight, 12 overweight, and 11 obese) between the ages of9 to 12 years. Weight classification was based on age and gender-specific body mass index cut-offs, while pubertal maturation was self-reported using Tanner staging. Distensibility was determined using two-dimensional, B-Mode echo Doppler ultrasound to measure changes at the right common carotid artery (CCA) diameter changes, while carotid pulse pressure (cPP) was measured at the left CCA by applanation tonometry. One-way ANOV A analysis revealed significant differences (p<0.001) in all anthropometric measures between the normal weight and overweight children, as well as the normal weight and obese children. Body stature was only higher in obese children compared to normal weight children (p<0.01). No significant differences were found between groups regarding age or Tanner stage. Common carotid artery distensibility showed a significant difference (p<0.01) between normal weight children (0.008 ± 0.002 mmHg-1 ) compared to obese children (0.005 ± 0.002 mmHg-1 ), with a borderline significant difference between the normal and overweight subjects (p=0.06). Introduction: The prevalence of coronary artery disease (CAD) is ever increasing in western industrialized societies. An individuals overall risk for CAD may be quantified by integrating a number of factors including, but not limited to, cardiorespiratory fitness, body composition, blood lipid profile and blood pressure. It might be expected that interventions aimed at improving any or all of these independent factors might improve an individual 's overall risk. To this end, the influence of standard endurance type exercise on cardiorespiratory fitness, body composition, blood lipids and blood pressure, and by extension the reduction of coronary risk factors, is well documented. On the other hand, interval training (IT) has been shown to provide an extremely powerful stimulus for improving indices of cardiorespiratory function but the influence of this training type on coronary risk factors is unknown. Moreover, the vast majority of studies investigating the effects of IT on fitness have used laboratory type training protocols. As a result of this, the influence of participation in interval-type recreational sports on cardiorespiratory fitness and coronary risk factors is unknown. Aims: The aim of the present study was to evaluate the effectiveness of recreational ball hockey, a sport associated with interval-type activity patterns, on indices of aerobic function and coronary risk factors in sedentary men in the approximate age range of 30 - 60 years. Individual risk factors were compiled into an overall coronary risk factor score using the Framingham Point Scale (FPS). Methods: Twenty-four sedentary males (age range 30 - 60) participated in the study. Subject activity level was assessed apriori using questionnaire responses. All subjects (experimental and control) were assessed to have been inactive and sedentary prior to participation in the study. The experimental group (43 ± 3 years; 90 ± 3 kg) (n = 11) participated in one season of recreational ball hockey (our surrogate for IT). Member of this group played a total of 16 games during an 11 week span. During this time, the control group (43 ± 2 years; 89 ± 2 kg) (n = 11) performed no training and continued with their sedentary lifestyle. Prior to and following the ball hockey season, experimental and control subjects were tested for the following variables: 1) cardiorespiratory fitness (as V02 Max) 2) blood lipid profile 3) body composition 5) waist to hip ratio 6) blood glucose levels and 7) blood pressure. Subject V02 Max was assessed using the Rockport submaximal walking test on an indoor track. To assess body composition we determined body mass ratio (BMI), % body fat, % lean body mass and waist to hip ratio. The blood lipid profile included high density lipoprotein, low density lipoprotein and total cholesterol levels; in addition, the ratio of total cholesterol to high density was calculated. Blood triglycerides were also assessed. All data were analyzed using independent t - tests and all data are expressed as mean ± standard error. Statistical significance was accepted at p :S 0.05. Results: Pre-test values for all variables were similar between the experimental and control group. Moreover, although the intervention used in this study was associated with changes in some variables for subjects in the experimental group, subjects in the control group did not exhibit any changes over the same time period. BODY COMPOSITION: The % body fat of experimental subjects decreased by 4.6 ± 0.5%, from 28.1 ± 2.6 to 26.9 ± 2.5 % while that of the control group was unchanged at 22.7 ± 1.4 and 22.2 ± 1.3 %. However, lean body mass of experimental and control subjects did not change at 64.3 ± 1.3 versus 66.1 ± 1.3 kg and 65.5 ± 0.8 versus 64.7 ± 0.8 kg, respectively. In terms of body mass index and waist to hip ratio, neither the experimental nor the control group showed any significant change. Respective values for the waist to hip ratio and body mass index (pre and post) were as follows: 1 ± 0.1 vs 0.9 ± 0.1 (experimental) and 0.9 ± 0.1 versus 0.9 ± 0.1 (controls) while for BMI they were 29 ± 1.4 versus 29 ± 1.2 (experimental) and 26 ± 0.7 vs. 26 ± 0.7 (controls). CARDIORESPIRATORY FITNESS: In the experimental group, predicted values for absolute V02 Max increased by 10 ± 3% (i.e. 3.3 ± 0.1 to 3.6 ± 0.1 liters min -1 while that of control subjects did not change (3.4 ± 0.2 and 3.4 ± 0.2 liters min-I). In terms of relative values for V02 Max, the experimental group increased by 11 ± 2% (37 ± 1.4 to 41 ± 1.4 ml kg-l min-I) while that of control subjects did not change (41 ± 1.4 and 40 ± 1.4 ml kg-l min-I). BLOOD LIPIDS: Compared to pre-test values, post-test values for HDL were decreased by 14 ± 5 % in the experiment group (from 52.4 ± 4.4 to 45.2 ± 4.3 mg dl-l) while HDL data for the control group was unchanged (49.7 ± 3.6 and 48.3 ± 4.1 mg dl-l, respectively. On the other hand, LDL levels did not change for either the experimental or control group (110.2 ± 10.4 versus 112.3 ± 7.1 mg dl-1 and 106.1 ± 11.3 versus 127 ± 15.1 mg dl-1, respectively). Further, total cholesterol did not change in either the experimental or control group (181.3 ± 8.7 mg dl-1 versus 178.7± 4.9 mg dl-l) and 190.7 ± 12.2 versus 197.1 ± 16.1 mg dl-1, respectively). Similarly, the ratio of TC/HDL did not change for either the experimental or control group (3.8 ± 0.4 versus 4.5 ± 0.5 and 4 ± 0.4 versus 4.2 ± 0.4, respectively). Blood triglyceride levels were also not altered in either the experimental or control group (100.3 ± 19.6 versus 114.8 ± 15.3 mg dl-1 and 140 ± 23.5 versus 137.3 ± 17.9 mg dl-l, respectively). BLOOD GLUCOSE: Fasted blood glucose levels did not change in either the experimental or control group. Pre- and post-values for experimental and control groups were 92.5 ± 4.8 versus 93.3 ± 4.3 mg dl-l and 92.3 ± 11.3 versus 93.2 ± 2.6 mg dl-1 , respectively. BLOOD PRESSURE: No aspect of blood pressure was altered in either the experimental or control group. For example, pre- and post-test systolic blood pressures were 131 ± 2 versus 129 ± 2 mmHg (experimental) and 123 ± 2 and 125 ± 2 mmHg (controls), respectively. Pre- and post-test diastolic blood pressures were 84 ± 2 and 83 ± 2 mmHg (experimental) and 81 ± 1 versus 82 ± 1 mmHg, respectively. Similarly, calculated pulse pressure was not altered in the experimental or control as pre- and post-test values were 47 ± 1 versus 47 ± 2 mmlHg and 42 ± 2 versus 43 ± 2 mmHg, respectively. FRAMINGHAM POINT SCORE: The concerted changes reported above produced an increased risk in the Framingham Point Score for the subjects in the experimental group. For example, the pre- and post-test FPS increased from 1.4 ± 0.9 to 2.7 ± 0.7. On the other hand, pre- and post-test scores for the control group were 1.8 ± 1 versus 1.8 ± 0.9. Conclusions: Our data confirms previous studies showing that interval-type exercise is a useful intervention for increasing aerobic fitness. Moreover, the increase in V02 Max we found in response to limited participation in ball hockey (i.e. 16 games) suggests that recreational sport may help reduce this aspect of coronary risk in previously sedentary individual. On the other hand, our results showing little or no positive change in body composition, blood lipids or blood pressures suggest that one season of recreational sport in not in of itself a powerful enough stimulus to reduce the overall risk of coronary artery disease. In light of this, it is recommended that, in addition to participation in recreational sport, the performance of regular physical activity is used as an adjunct to provide a more powerful overall stimulus for decreasing coronary risk factors. LIMITATIONS: The increase in the FPS we found for the experimental group, indicative of an increased risk for coronary disease, was largely due to the large decrease in HDL we observed after compared to above one season of ball hockey. In light of the fact that cardiorespiratory fitness was increased and % body fat was decreased, as well as the fact that other parameters such as blood pressure showed positive (but non statistically significant) trends, the possibility that the decrease in HDL showed by our data was anomalous should be considered. FUTURE DIRECTIONS: The results of this study suggesting that recreational sport may be a potentially useful intervention in the reduction of CAD require to be corroborated by future studies specifically employing 1) more rigorous assessment of fitness and fitness change and 2) more prolonged or frequent participants.

This thesis investigated the impact of pubertal maturation and sex on cardiovagal baroreflex sensitivity (BRS) and arterial properties of the common carotid artery (CCA), and the relationship between CCA arterial properties and BRS. This thesis also investigated the effect of orthostatic stress on arterial properties of the CCA and carotid sinus (CS), as well as their impact on BRS in response to orthostatic stress. Children and adolescents between the ages of 8-18 years were examined. To assess pubertal maturation participants were organized into five pubertal groups based on the criteria of Tanner. BRS was assessed by transfer function analysis in the low frequency range (0.05 – 0.15Hz). Pulse pressure (PP) was measured at the CCA (PPCCA) and CS (PPCS) using applanation tonometry, and at the finger (PPFinger) using photoplethysmography. Ultrasound sonography and applanation tonometry were used to determine the distensibility coefficient (DC) at the CCA (DCCCA) and CS (DCCS). A moderate posture stimulus was implemented by passively moving participants into a 50° seated-recumbent (SR) position. The results demonstrated a sex-by-maturation interaction on BRS (p= 0.019). BRS decreased from early- to post-puberty in males (p<0.01), and remained unchanged in females. Females demonstrated greater BRS compared to males post-puberty (p<0.05). CCA distensibility was not affected by sex or maturation and was not related to BRS. PPCS was greater than PPCCA (p<0.001), while PPFinger was greater than both PPCCA (p<0.001) and PPCS (p<0.001). In response to SR, the relative change in PPFinger was significantly different than the relative change in PPCCA (p<0.001) and PPCS (p<0.001), while the relative change between PPCCA and PPCS were not different. Finally, in response to SR there was a significant decrease in DCCS (p=0.001), but not DCCCA. The relative change in BRS in response to SR was significantly correlated to the relative change in DCCS (p=0.004), but not DCCCA. The findings demonstrated an important sex-dependent maturation effect on BRS in children and adolescents that was not explained by CCA distensibility. Also, the CS and CCA responded differently to orthostatic stress. The CS was more suitable to evaluate the effect of arterial distensibility on BRS in response to posture change.

La grossesse s’accompagne d’importantes modifications hormonales et hémodynamiques. Parmi celles-ci, le système rénine-angiotensine-aldostérone (SRAA) est activé très tôt durant la grossesse. De plus, cette augmentation du SRAA est accompagnée d’élévations du débit cardiaque et du volume plasmatique ainsi que des baisses paradoxales de la pression artérielle et de la résistance vasculaire périphérique. Ceci suggère que la grossesse induit un remaniement des réponses physiologiques normales au SRAA. Une résistance vasculaire à l’action des vasopresseurs est également observée durant la gestation. Ce phénomène serait causé par la modification de la fonction des canaux calciques et potassiques. De plus, il serait possiblement dû à la participation de la Na+/K+-ATPase, par son influence sur le potentiel membranaire des cellules des muscles lisses vasculaires (VSMC). La présence des récepteurs minéralocorticoïdes (MR) dans les VSMC laisse croire que l’aldostérone peut influencer le tonus vasculaire par des effets génomiques et non-génomiques. Compte tenu des connaissances actuelles, nous avons émis l’hypothèse que l’augmentation des taux sériques d’aldostérone durant la grossesse est responsable des changements hémodynamiques observés et que ces effets sont causés par l’activation des MR. Des rates gestantes ont été traitées avec du canrénoate de potassium (20 mg/kg•jr), un antagoniste des MR, durant la dernière semaine de gestation (sur 3). Sur des anneaux aortiques dénudés de leur endothélium, nous avons mesuré les réponses contractiles à la phényléphrine (PhE) et au KCl en présence d’un bloqueur des canaux calciques dépendants du voltage (VDCC), la nifédipine, et d’activateurs des canaux potassiques à large conductance (BKCa) et ceux dépendants de l’ATP (KATP), respectivement le NS-1619 et la cromakalim. Les réponses à la PhE et au KCl sont réduites à partir du 17e jour de gestation et le traitement au canrénoate augmente ces réponses dans tous les groupes. Les modulateurs de canaux ioniques atténuent les réponses à la PhE et au KCl. Cependant, le canrénoate modifie aussi les effets des modulateurs sur les aortes. Aucun effet ou une baisse des réponses est observable chez les rates non gestantes, tandis qu’une hausse de leur effet inhibiteur est notée chez les rates gestantes. Ces effets du canrénoate font croire que l’aldostérone participe à l’adaptation de la réactivité vasculaire durant la grossesse. Par ailleurs, le potentiel membranaire des VSMC pourrait être affecté dans la gestation. Pour vérifier cette hypothèse, nous avons évalué l’activité de la Na+/K+-ATPase, impliquée dans le contrôle du potentiel membranaire. Nos résultats démontrent que l’activité de la pompe est inhibée à partir du 19e jour de gestation. Cet effet est renversé par le canrénoate. Toutefois, comme le renversement de l’inhibition de la pompe est également présent chez les rates gestantes traitées avec du PST 2238, un antagoniste de l’ouabaïne sur la Na+/K+-ATPase, et que le canrénoate agit également comme agoniste partiel de la pompe, nous croyons que la diminution d’activité associée à la gestation est liée à une inhibition de la Na+/K+-ATPase par des stéroïdes cardiotoniques plutôt qu’à un effet des minéralocorticoïdes. L’augmention d’activité de la pompe liée au canrénoate s’accompagne d’une diminution de l’expression de la sous-unité α1, suggérant que la sous-unité α2 est responsable des variations de contractilité de l’aorte, puisque son expression n’est pas modifiée par le canrénoate. Les effets de la diminution de l’expression de la sous-unité α1, influencée par la signalisation du MR, restent à être déterminés. Néanmoins, nos résultats montrent que les modifications d’activité de la Na+/K+-ATPase influencent l’activité des canaux potassiques et que la pompe pourraient être un des éléments primordiaux dans le contrôle de la réactivité vasculaire durant la grossesse. Comme le canrénoate modifie la réactivité vasculaire, nous voulions déterminer ses impacts sur la pression artérielle. Des rates gestantes ont été traitées avec du canrénoate (20 ou 60 mg/kg•jr) et les paramètres hémodynamiques ont été évalués par radiotélémétrie. Aucune modification de la pression artérielle, du rythme cardiaque et de la pression pulsée ne sont mesurées chez les rates recevant le traitement. Toutefois, des augmentations de l’osmolalité, des taux sériques d’aldostérone et de corticostérone ainsi que de l’activité rénine plasmatique sont observées chez les animaux recevant 60 mg/kg•jr. Le canrénoate bloque donc le rétrocontrôle du SRAA. Par contre, les MR ne sont pas les principaux responsables du contrôle de la pression artérielle durant la grossesse. En conclusion, nous avons démontré que le traitement des rates au canrénoate influence la réactivité vasculaire de l’aorte durant la gestation. Cet effet est causé par la modification de l’activité de certains canaux ioniques (VDCC, BKCa et KATP). De plus, le canrénoate renverse l’inhibition de la Na+/K+-ATPase observée durant la gestation. Finalement, les actions locales de cet antagoniste des MR sur les vaisseaux sanguins ne se répercutent pas sur l’effet systémique global et aucune modification de la pression artérielle n’est observée. D’autres études seront toutefois nécessaires pour déterminer les voies de signalisation par lesquelles l’aldostérone module les réponses des canaux ioniques dans les VSMC.

Introducción. Las soluciones de hemodiálisis (HD) tienen una concentración estándar de sodio (Na) 139 mEq/L la cual no permite perder sodio por difusión llevando incluso a balance positivo de Na perpetuando la sensación de sed, hipertensión, hipertrofia ventricular y elevadas ganancias de peso interdialítica. Objetivo. Determinar cuales son los efectos cardiovasculares con el uso soluciones de HD con concentraciones de Na de 132 mEq/L en pacientes con insuficiencia renal crónica (IRC) de la unidad renal RTS cardioinfantil entre agosto 2008 y enero 2009. Métodos. Se realizo un estudio aleatorizado controlado, con doble enmascaramiento, inicialmente una fase de lavado de las concentraciones de sodio de 2 semanas y luego aleatorización en bloques en dos grupos, realizando la intervención durante 8 semanas. Resultados. Se evaluó 82 pacientes de los cuales ingresaron a la intervención 64, distribuidos al grupo de intervención 31 y al grupo control 33 pacientes. Hubo un aumento de la presión de pulso en el grupo de intervención y disminución de la sensación de sed y ganancia de peso interdialítica. Los hallazgos ecocardiográficos mostraron disminución del diámetro de la vena cava inferior en los dos grupos y disminución del volumen telediastólico del ventrículo izquierdo. Se presentaron más eventos adversos en el grupo de intervención. Discusión. Los resultados difieren de la literatura por tener un mayor número de diabéticos en el grupo control y por el tiempo de intervención. No se pueden sacar conclusiones con respecto al aumento de la presión de pulso ni a la disminución del diámetro de la vena cava inferior en HD. Conclusión. La solución de HD con concentración de Na de 132 mEq/L no disminuye la presión arterial, ni el consumo de medicamentos antihipertensivos, sin embargo disminuye la sensación de sed y la ganancia de peso interdialítica.

Extratropical cyclones and how they may change in a warmer climate have been investigated in detail with a high-resolution version of the ECHAM5 global climate model. A spectral resolution of T213 (63 km) is used for two 32-yr periods at the end of the twentieth and twenty-first centuries and integrated for the Intergovernmental Panel on Climate Change (IPCC) A1B scenario. Extremes of pressure, vorticity, wind, and precipitation associated with the cyclones are investigated and compared with a lower-resolution simulation. Comparison with observations of extreme wind speeds indicates that the model reproduces realistic values. This study also investigates the ability of the model to simulate extratropical cyclones by computing composites of intense storms and contrasting them with the same composites from the 40-yr ECMWF Re-Analysis (ERA-40). Composites of the time evolution of intense cyclones are reproduced with great fidelity; in particular the evolution of central surface pressure is almost exactly replicated, but vorticity, maximum wind speed, and precipitation are higher in the model. Spatial composites also show that the distributions of pressure, winds, and precipitation at different stages of the cyclone life cycle compare well with those from ERA-40, as does the vertical structure. For the twenty-first century, changes in the distribution of storms are very similar to those of previous study. There is a small reduction in the number of cyclones but no significant changes in the extremes of wind and vorticity in both hemispheres. There are larger regional changes in agreement with previous studies. The largest changes are in the total precipitation, where a significant increase is seen. Cumulative precipitation along the tracks of the cyclones increases by some 11% per track, or about twice the increase in global precipitation, while the extreme precipitation is close to the globally averaged increase in column water vapor (some 27%). Regionally, changes in extreme precipitation are even higher because of changes in the storm tracks.
