A queimadura corresponde ao quadro resultante da ação direta ou indireta do calor do fogo, de substâncias químicas ou da eletricidade sobre o organismo humano. Alguns estudos apontam as crianças como as maiores vítimas. Procedimentos inerentes ao processo de recuperação são reconhecidos como particularmente dolorosos. A frequência de procedimentos médicos invasivos, situação constante ao paciente vítima de queimadura, favorece a emissão de comportamentos concorrentes pela criança, bem como pode ocasionar a generalização do caráter aversivo do procedimento invasivo para estímulos presentes no ambiente, justificando estudos sobre a eficácia de intervenções neste contexto. A pesquisa foi dividida em dois estudos com objetivos específicos. Estudo 1 (n=5): caracterizar comportamentos de crianças vítimas de queimadura emitidos durante procedimento de curativo sem sedação em enfermaria e comparar a frequência de comportamentos concorrentes e não concorrentes emitidos por estas crianças em duas sessões consecutivas deste procedimento. Estudo 2 (n=2): analisar os efeitos do uso de instrução sobre a frequência de comportamentos concorrentes e não concorrentes em crianças vítimas de queimadura durante procedimento de curativo sem sedação em enfermaria. Participaram sete crianças (4 meninos e 3 meninas; 7 a 12 anos) vítimas de queimadura em área corporal inferior a 70% e seus acompanhantes, selecionados em um Centro de Tratamento de Queimados (CTQ). Como instrumentos, foram utilizados: (a) Protocolo de consulta à equipe de saúde; (b) Lista de Verificação Comportamental para crianças/adolescentes (CBCL 6-18anos); (c) Roteiro de entrevista com a criança; (d) Escala de observação comportamental OSBD (Observation Scale of Behavioral Distress); (e) Escala facial para avaliar a percepção de dor da criança (FACES); e (f) Manual de instrução sobre o procedimento de curativo. No Estudo 1, observou-se maior frequência de comportamentos concorrentes sobre os não concorrentes nas duas sessões de curativo. No Estudo 2, após o uso do Manual de instrução, observou-se redução estatisticamente significante na frequência do comportamento concorrente Choramingar (= 0,0447) e aumento significativo (= 0,0324) na frequência do comportamento não concorrente Auxiliar na execução do procedimento. Houve correlação significativa (p-valor = 0,0066) entre a frequência de comportamentos não concorrentes entre as crianças do Estudo 1 e as que participaram do Estudo 2, sugerindo que a intervenção, por meio de instruções, foi eficaz para aumentar a frequência de comportamentos não concorrentes nas crianças participantes do Estudo 2.

A brinquedoteca hospitalar é um direito legalmente assegurado às crianças por meio da Lei 11.104/05, entretanto ainda não concretizado integralmente no país. O presente estudo objetivou descrever e analisar as condições de serviços e espaços disponibilizados pelas brinquedotecas hospitalares em Belém do Pará. Fizeram parte da pesquisa quatro hospitais que possuem este serviço. A pesquisa envolveu 10 técnicos e 39 crianças e seus acompanhantes. Para os técnicos foram utilizados um roteiro de Entrevista; a Escala Autoavaliativa de Índices de Qualidade (EAIQ), sendo do tipo Likert, composta por 27 itens fechados e três abertos. Para as crianças e seus responsáveis foram aplicados roteiros de entrevista. Foram realizados também observações e registro fotográfico. A coleta de dados foi iniciada após a assinatura do Termo de Consentimento Livre e Esclarecido, as entrevistas e a aplicação da EAIQ foram feitas individualmente. Os dados oriundos das entrevistas foram agrupados em categorias a partir dos tópicos presentes nos roteiros e os da escala segundo a literatura, ambos analisados qualitativamente. Dentre os principais resultados encontrados estão: a- Todos os hospitais do estudo eram público; b- no tocante a conceituação do espaço, as resposta dos técnicos e acompanhantes esteve em consonância com a literatura e a legislação vigente; c- existem poucos registros acerca da implantação e funcionamento dos espaços; d- as equipes se diferiram tanto em relação ao número de membros quanto à formação, sendo que em metade delas falta uma rotina sistemática de reuniões, contudo foram avaliadas positivamente pela clientela, e- verificou-se que três delas funcionam no mínimo cinco dias por semana e que todas oferecem atividades livres, dirigidas; f- no tocante ao acervo lúdico, este se diferenciou em relação à quantidade, mas as instituições dispunham de brinquedos de faz-de-conta, blocos de montar e jogos de tabuleiro, g- verificou-se que as crianças apresentaram pouca restrição às brincadeiras que gostariam de realizar no hospital e relataram que o local preferido dentro desse contexto é aquele em que podem brincar. O estudo permitiu traçar um perfil destas brinquedotecas, verificar os aspectos que favorecem a concretização dos objetivos desses espaços e reflexões sobre possibilidades de melhorias.

The Brazilian population is ageing rapidly, and chronic diseases have increased. Due to deficient health care services, the diagnosis and treatment of such diseases occur in the tertiary level, which increases costs and reduces the possibilities of early diagnoses. In view of the elderly population‟s increase and of the great demand at hospitalization units, it is important to learn about the difficulties and facilities faced by nursing teams when giving care to hospitalized individuals. In order to reach the objectives, a cross-sectional, prospective, descriptive, analytical and qualitative study was performed with basis on Bardin‟s Content Analysis. Among the difficulties were patients‟ limitations, dependence level, behaviors and habits, interference from companions, feeling of abandonment, perception of needs, dealing with suffering and lack of time for adequate care. As to facilities, acceptance of the disease, adherence to treatment, collaboration, trust in the team, and older patients‟ politeness were reported, which shows that passiveness is an important indicator in caring for the elderly. Older individuals suffer the outcomes of certain physical, psychological and mental deficits. When facing disease conditions, they require special care, including hospitalization and greater attention. The nursing team provides daily care and follows patients‟ development; however, its members are still not knowledgeable enough about the ageing process. This contributes to increase prejudice and erroneous stereotypes about older persons. Therefore, not understanding such process compromises the full care to be provided to older patients. This leads team members to accelerate the care provision process in order to meet the daily work demand, thus compromising older patients‟ autonomy and making them more dependent on the team, whereas the process should follow the opposite path

The aim was to reflect what action to prevent accidents used by caregivers / participants in the home. This is a prospecive cross-sectional descriptive study conducted at the Pediatric Inpatient Unit, Hospital of the Medical School of Botucatu – Unesp, during the months from May to August 2010.A questionnaire was used in the form of check list (Annex I) on measures to prevent accidents. The participant chose two alternatives taking into consideration the order of priority in the prevention of accidents. This study shows that for the age group from 29 days to 2 years, the companions chose these preventive measures for falls, burns, poisoning and drowning, “do not put near the windows mobile” (27.8%), “no access the match, lighter and appliances (22.2%), leave toxic substances out of reach and sight of children (41.7%) and empty buckets, tubs and pools after use (27.8% ). Regarding the age group 2-6 years, the measures for these types of accidents were not put near the windows mobile (26.5%), do not have access to matches, lighters and household appliances (20.9% ), leave toxic products out of reach and sight of children (27.9%) and maintain tanks, tanks and wells or struck with some protection (20.2%). For ages 6 to 10 years, the measures chosen to more such accidents were dry liquid spilled on the ground (23.5%), do not have access to matches, lighters and household appliances (22.5% ), leave toxic products out of reach and sight of children (31%) and keep tanks, tanks and wells or struck with some protection (23.5%). One fact that attracted our attention was that the preventive measures most frequently used by caregivers, regardless of age, show results that differ in few alternatives. It is essential to the understanding of the escorts / participants on the importance of injury prevention. Even today is predominantly defined as accident, injury must be addressed through measures

The humanization of childbirth implies the understanding of this event as an important experience, and requires the redefinition of human relationships from the review of the assistance project, the understanding of pregnant women condition and human rights. To evaluate child birth assistance using a score that was developed by Botucatu Cuesta Regional Management and Jurumirim Valley Collegiates. This is an epidemiological, descriptive study that is inserted in the field of evaluation of services, programs or health projects. The data has been obtained by direct observation of deliveries, emphasizing the humane care. Results: Approximately one third of the women gave birth in a tertiary hospital (34.1%) and it was their first babies (33%).24.8% of the women received prenatal care in high-risk services. 67.1% of the births were normal, and 84.7% had no companions in the delivery room. In 47.1% of the cases the delivery was performed by obstetrician who used anesthesia in 44.7% and episiotomy in 48.2% of the deliveries. More than half of the newborns were attended by the pediatrician in the delivery room and had a delivery graph completed. Although the present study shows that 67.1% of the births were normal, caesarean rate can be considered excessive, as the WHO points out that c-sections above 15% are unlikely to be justifiable. It is important to emphasize that the Ministry of Health has to have a commitment with all women to promote safe motherhood, even in cases when the pregnancy involves a risk for both the mother and the fetus. It is noteworthy that the created score allowed us to assess variables related to the humanization of childbirth and only average and quite similar situations among the three services were evidenced. We hope that with this study, managers and professionals that work in this area can be subsidized in order to offer effective humane assistance and quality service in the delivery

The 131I (radioactive iodine) is one of the most used radionuclides in nuclear medicine for diagnosis and treatment. The present study evaluates the dosimetric aspects related to radioiodine therapy after thyroidectomy in patients with thyroid cancer. The samples were studied with 50 patients undergoing treatment, by assessing the exposures of workers (nursing assistants, staff hygiene, medical and physical), the general public (accompanying and family members) and on the environment. To evaluate the workers, was made a survey of the environmental conditions of the room radioiodine and routines adopted by them during the treatment period. Estimating the dose equivalent per month for each employee, we observed that the maximum levels obtained for nursing assistants, the team of hygiene, medical and physicians were considered low in relation to the extent permitted by law. In order to assess the public, some situations have been suggested for the calculation of equivalent doses in which it was possible to verify the fundamental importance of isolating the patient in the 2 days first. Regarding the environment, the radioactive waste generated by patient had volume of 1.0 m3 and activity estimated at 0.91 mCi, taking a decay time for eliminating them about 75 days to reach the allowable value of 2 μCi / kg system of collecting garbage. Therefore, all radioactive waste removed from the patient's room should be sent to the warehouse for temporary storage of radioactive waste, located away from normal work areas

The aim of the present study was to analyze the impact suffered by 40 people when discovering that one of their family members was diagnosed with cancer, to analyze their coping strategies and expectations towards the patient’s illness and future. The study made use of the Coping Strategies Inventory by Folkman and Lazarus. Results show that most of the patients’ caregivers are their married children, with an average age of 45.7 (dp=12.67), and gainfully employed. These family members reported the negative impact of the diagnosis, with the predominant feelings of sadness and fear of loss, even though they had a positive perspective about the future, expecting the patient’s recovery. As to the functional coping strategies, the most used were the resolution of problems, followed by social support; the least used was positive revaluation. As to the dysfunctional strategies, the most used ones were escape and avoidance and the least used was taking the responsibility for the illness. Conclusions are that despite suffering with the negative impact of the disease, the family members are optimistic about the patient’s future and seek to use strategies that solve the problem effectively, without blaming themselves for the patient’s illness.

Objective. To asses fairy tales reading as a humanization strategy of hospitalized children care. Methodology. Qualitative study in which data was collected from: fairy tales reading: individual interview, questionnaire, drawings and stories created by the children. Results. During reading sessions, improvement in the children’s reactions was seen: They were more focused, participative, enthusiastic and happy; those accompanying the children liked and accepted this intervention. Data analysis collected from Interviews, questionnaires, narratives and drawings, showed results in favor of the tales reading practice. Conclusion. The humanization strategy of care supported by fairy tales reading, had a positive impact in the children’s admission to the hospital.

Objective. To estimate the prevalence of bacteria isolated in samples from the hands of school-age children at a hospitalization unit. Methodology. In 2009, strains were cultured from the hands of 90 school-age children at the pediatric hospitalization unit of Hospital Estadual Bauru (São Paulo, Brazil). After culture of the samples, the isolated bacteria were identified. Results. In 98% of the samples taken from the children, bacteria were isolated. Coagulase-negative Staphilococcus was isolated in 64% of the samples, followed by Staphilococcus aureus (5%) and Pseudomonas aeruginosa (1%). Conclusion. In most of the samples from the children’s hands, bacteria were isolated. Therefore, educative actions about hygiene habits in- and outside the hospital environment should be reinforced, aimed at children and their companions.

The aim was to describe the positive and negative aspects in a family communication at an emergency care unit. This is a descriptive research with a qualitative approach. The sample consisted of 20 family members / caregivers of patients admitted to the emergency room of a university hospital. Data collection was carried out through semi-structured interviews applied during the period of May-June 2011. Data analysis was performed using the methodological framework of the Collective Subject Discourse which resulted in four themes: Lack of information compromising the family/health professional ties; The ideal model of information provided by the professional regarding the condition of the hospitalized relative; Assessment of information provided about hospital routines; and The role of the relative/companion. Therefore, for the patient and his family, communication is presented as a tool on the care given with the function of transmitting safety, respect and guidance. However when it is incomplete, it becomes a potential form of stress and dissatisfaction.

Esta pesquisa teve como objetivo analisar os efeitos de instrução e de treino parental sobre comportamentos observados em cuidadores e em crianças com diagnóstico de câncer durante procedimento de punção venosa em ambulatório. Participaram nove cuidadores em três condições (Rotina, Manual e Treino). Fez-se análise de características familiares, estilo parental, efeitos de um manual de instruções e de treino parental, com sessões de observação direta do comportamento durante punção venosa. Os resultados apontam efeito positivo do manual para mudança de comportamento em curto prazo. Após treino parental, observou-se aumento nas taxas de monitoria positiva do cuidador, com relatos de generalização desses comportamentos para outros contextos. Discute-se a importância do estilo parental como fator de proteção à criança com câncer.
