Obstructive sleep apnea (OSA) is a secondary cause of hypertension and independently associated with target-organ damage in hypertensive patients. However, OSA remains largely underdiagnosed and undertreated. The aim of the present study was to evaluate the characteristics and clinical predictors of OSA in a consecutive series of patients followed up in a hypertension unit. A total of 99 patients (age 46 +/- 11 years, body mass index 28.8 kg/m(2), range 25.1 to 32.9) underwent polysomnography. The clinical parameters included age, gender, obesity, daytime sleepiness, snoring, Berlin Questionnaire, resistant hypertension, and metabolic syndrome. Of the 99 patients, 55 (56%) had OSA (apnea-hypopnea index >5 events/hour). Patients with OSA were older and more obese, had greater levels of blood pressure, and presented with more diabetes, dyslipidemia, resistant hypenension, and metabolic syndrome than the patients without OSA. Of the patients with OSA, 51% had no excessive daytime sleepiness. The Berlin Questionnaire and patient age revealed a high sensitivity (0.93 and 0.91, respectively) but low specificity (0.59 and 0.48, respectively), and obesity and resistant hypertension revealed a low sensitivity (0.58 and 0.44, respectively) but high specificity (0.75 and 0.91, respectively) for OSA. Metabolic syndrome was associated with high sensitivity and specificity for OSA (0.86 and 0.85, respectively). Multiple regression analysis showed that age of 40 to 70 years (odds ratio 1.09, 95% confidence interval 1.03 to 1.16), a high risk of OSA on the Berlin Questionnaire (odds ratio 8.36, 95% confidence interval 1.67 to 41.85), and metabolic syndrome (odds ratio 19.04, 95% confidence interval 5.25 to 69.03) were independent variables associated with OSA. In conclusion, more important than the typical clinical features that characterize OSA, including snoring and excessive daytime sleepiness, the presence of the metabolic syndrome is as an important marker of OSA among patients with hypertension. Decisions for intensive care unit (ICU) admissions in patients with advanced cancer are complex, and the knowledge of survival rates and prognostic factors are essential to these decisions. Ours objectives were to describe the short- and long-term survival of patients with metastatic solid cancer admitted to an ICU due to emergencies and to study the prognostic factors presented at ICU admission that could be associated with hospital mortality. We retrospectively analysed the charts of all patients with metastatic solid cancer admitted over a 1-year period. This gave a study sample of 83 patients. The ICU, hospital, 1-year and 2-year survival rates were 55.4%, 28.9%, 12.0% and 2.4% respectively. Thrombocytopenia (odds ratio 26.2; P = 0.006) and simplified acute physiology score (SAPS II) (odds ratio 1.09; P = 0.026) were independent factors associated with higher hospital mortality. In conclusion, the survival rates of patients with metastatic solid cancer admitted to the ICU due to emergencies were low, but of the same magnitude as other groups of cancer patients admitted to the ICU. The SAPS II score and thrombocytopenia on admission were associated with higher hospital mortality. The characteristics of the metastatic disease, such as number of organs with metastasis and central nervous system metastasis were not associated with the hospital mortality.

Individual randomized clinical trials (RCTs) with cholinesterase inhibitors (ChEIs) aiming to delay the progression from mild cognitive impairment (MCI) to Alzheimer`s disease (AD) have not found significant benefit of their use for this purpose. The objective of this study is to meta-analyze the RCTs conducted with ChEIs in order to assess whether pooled analysis could show the benefit of these drugs in delaying the progression from MCI to AD. We searched for references of published and unpublished studies on electronic databases (Medline, Embase, Web of Science, and Clinical Trial Database Registry, particularly the Clinicaltrials.gov-http://www.clinicaltrials.gov). We retrieved 173 references, which yielded three references for data extraction. A total of 3.574 subjects from four RCTs were included in the meta-analysis. Among 1,784 subjects allocated in the ChEI-treatment group, 275 (15.4%) progressed to AD/dementia, as opposed to 366 (20.4%) out of 1,790 subjects in the placebo group. The relative risk (RR) for progression to AD/dementia in the ChEI-treated group was 0.75 [CI(95%) 0.66-0.87], z = -3.89, P < 0.001. The patients on the ChEI group had a significantly higher all-cause dropout risk than the patients on the placebo group (RR = 1.36 CI(95%) [1.24-1.49]; z = 6.59, P < 0.001). The RR for serious adverse events (SAE) in the ChEI-treated group showed no significantly statistical difference from the placebo group (RR = 0.95 [CI(95%) 0.83-1.09], z = -0.72, P = 0.47). The subjects in the ChEI-treated group had a marginally, non-significant, higher risk of death due to any cause than those in the placebo-treated group (RR = 1.04, CI(95%) 0.63-1.70, z = 0.16, P = 0.86). The long-term use of ChEIs in subjects with MCI may attenuate the risk of progression to AD/dementia. This finding may have a significant impact on public health and pharmaco-economic policies.

Background. The pericardial biopsy has opened a new perspective for the etiologic diagnosis of pericardial effusions, because adequate pericardial visualization via the use of a video camera can provide more accurate results. We assessed the usefulness of videopericardioscopy for the diagnosis and treatment of pericardial effusion of indeterminate origin. Methods. We conducted a retrospective study of clinical data from patients who underwent videopericardioscopy examination for pericardial effusion without an established diagnosis. The video-assisted pericardioscopy procedure was performed through a small incision in the xiphoid area. Results. From January 1998 to January 2007, 101 consecutive patients underwent videopericardioscopy evaluation for pericardial effusion. Ten patients were excluded because of lack of data. Fifty men and 41 women were included ( mean age, 50 years; range, 14-76 years). All of the patients had moderate or significant pericardial effusion as demonstrated by echocardiography or computed tomography. The following diagnoses for the pericardial effusions were established: nonspecific inflammation, 50 cases ( 54.94%); neoplastic disorders, 22 cases ( 24.17%); tuberculous, 11 cases ( 12.08%); bacterial inflammatory process, 3 cases ( 3.29%); chylopericardial, 2 cases ( 2.19%); fungal infection, 2 cases ( 2.19%); and viral infection, 1 case ( 1.09%). Pericardioscopy evaluation provided the definitive diagnosis via the pericardial biopsy in 36.26% of the cases and via the results of fluid analyses in 13.18% of the cases; the use of both methods established the definitive diagnosis in 45.05% of the cases in this group of patients. The overall morbidity rate was 4.3%, and the most common complication was arrhythmia due to intraoperative manipulation, which ceased with the removal of the instruments from the pericardial cavity. We had 1 death, by cardiac tamponade, in the perioperative period. Conclusion. Videopericardioscopy is a safe and efficient method for obtaining a better diagnosis of and satisfactory therapeutic results for pericardial effusions of indeterminate cause, and such results are obtained via an improved exploration of the pericardial cavity.

Background Disease management programs (DMPs) are developed to address the high morbi-mortality and costs of congestive heart failure (CHF). Most studies have focused on intensive programs in academic centers. Washington County Hospital (WCH) in Hagerstown, MD, the primary reference to a semirural county, established a CHF DMP in 2001 with standardized documentation of screening and participation. Linkage to electronic records and state vital statistics enabled examination of the CHF population including individuals participating and those ineligible for the program. Methods All WCH inpatients with CHF International Classification of Diseases, Ninth Revision code in any position of the hospital list discharged alive. Results Of 4,545 consecutive CHF admissions, only 10% enrolled and of those only 52.2% made a call. Enrollment in the program was related to: age (OR 0.64 per decade older, 95% CI 0.58-0.70), CHF as the main reason for admission (OR 3.58, 95% CI 2.4-4.8), previous admission for CHF (OR 1.14, 95% CI 1.09-1.2), and shorter hospital stay (OR 0.94 per day longer, 95% CI 0.87-0.99). Among DMP participants mortality rates were lowest in the first month (80/1000 person-years) and increased subsequently. The opposite mortality trend occurred in nonenrolled groups with mortality in the first month of 814 per 1000 person-years in refusers and even higher in ineligible (1569/1000 person-years). This difference remained significant after adjustment. Re-admission rates were lower among participants who called consistently (adjusted incidence rate ratio 0.62, 95% CI 0.52-0.77). Conclusion Only a small and highly select group participated in a low-intensity DMP for CHF in a community-based hospital. Design of DMPs should incorporate these strong selective factors to maximize program impact. (Am Heart J 2009; 15 8:459-66.)

Among the possible mechanisms explaining the worsening of asthma due to gastroesophageal reflux disease (GERD) is the increase in bronchial hyperresponsiveness. The effects of GERD on bronchial hyperresponsiveness in patients with bronchial asthma have yet to be studied in significant detail. The aim of this study was to determine the effects of esophageal acid perfusion on bronchial responsiveness to bradykinin in patients with both asthma and GERD. In 20 patients with asthma and GERD disease, esophageal pH was monitored with a pH meter and bronchial responsiveness was evaluated by aerosol inhalation of bradykinin during esophageal acid perfusion and, 24 h earlier or later the patients were submitted to another bronchial provocation test without acid infusion. No significant changes were observed in FEV1, FEF25-75%, FVC, or PEF during acid perfusion. The response to the bronchial provocation test did not differ between the control day and the day of acid infusion (p = 0.61). The concentration provoking a 20% fall in FEV1 (geometric mean +/- geometric SD) was 1.09 +/- 5.84 on the day of acid infusion and 0.98 +/- 5.52 on the control day. There is no evidence that acid infusion changes bronchial responsiveness to bradykinin. These findings strongly question the significance of acid infusion as a model to study the pathogenesis of GERD-induced asthma.

OBJECTIVE-This study sought to investigate an association of HbA1c (A1C) with incident heart failure among individuals without diabetes and compare it to fasting glucose. RESEARCH DESIGN AND METHODS-We studied 11,057 participants of the Atherosclerosis Risk in Communities (ARIC) Study without heart failure or diabetes at baseline and estimated hazard ratios of incident heart failure by categories of A1C (<5.0, 5.0-5.4 [reference], 5 5-59, and 6.0-6.4%) and fasting glucose (<90, 90-99 [reference], 100-109, and 110-125 mg/dl) using Cox proportional hazards models. RESULTS-A total of 841 cases of incident heart failure hospitalization or deaths (International Classification of Disease, 9th/10th Revision, 428/150) occurred during a median follow-up of 14.1 years (incidence rate 5.7 per 1,000 person-years). After the adjustment for covariates including fasting glucose, the hazard ratio of incident heart failure was higher in individuals with A1C 6.0-6.4% (1.40 [95% CI, 1 09-1.79]) and 5.5-6.0% (1.16 [0.98-1 37]) as compared with the reference group. Similar results were observed when adjusting for insulin level or limiting to heart failure cases without preceding coronary events or developed diabetes during follow-up. In contrast, elevated fasting glucose was not associated with heart failure after adjustment for covariates and A1C. Similar findings were observed when the top quartile (A1C, 5.7-6.4%, and fasting glucose, 108-125 mg/dl) was compared with the lowest quartile (<5 2% and <95 mg/dl, respectively). CONCLUSIONS-Elevated A1C (>= 5.5-6 0%) was associated with incident heart failure in a middle-aged population without diabetes, suggesting that chronic hyperglycemia prior to the development of diabetes contributes to development of heart failure. Purpose: This study evaluated the effect of different microwave polymerization cycles on the color changes of a microwave-processed denture base resin after accelerated aging and immersion in beverages. Materials and Methods: Specimens of light pink acrylic resin were divided into three groups according to polymerization cycle: (A) 500 W for 3 minutes, (B) 90 W for 13 minutes + 500 W for 90 seconds, and (C) 320 W for 3 minutes + 0 W for 4 minutes + 720 W for 3 minutes. Control groups were a heat-processed acrylic resin (T) and a chemically activated denture repair resin (Q). Eight specimens per group were aged in an artificial aging chamber and evaluated at 20, 192, and 384 hours. Another series of 40 specimens per group were immersed in water, coffee, tea, cola, or red wine and evaluated at 1, 12, and 36 days. Color was measured by a spectrophotometer before and after aging or immersion. Color changes (Delta E) were analyzed by ANOVA/Bonferroni t-test (alpha = 0.05). Results: Mean Delta E (+/- SD) after 384 hours of accelerated aging were (A) 2.51 +/- 0.50; (B) 3.16 +/- 1.09; (C) 2.89 +/- 1.06; (T) 2.64 +/- 0.34; and (Q) 9.03 +/- 0.40. Group Q had a significantly higher Delta E than the other groups. Color changes of immersed specimens were significantly influenced by solutions and time, but the five groups showed similar values. Mean Delta E at 36 days were (water) 1.4 +/- 0.8; (coffee) 1.3 +/- 0.6; (tea) 1.7 +/- 0.5; (cola) 1.4 +/- 0.7; and (red wine) 10.2 +/- 2.7. Results were similar among the five test groups. Conclusions: Color changes of the microwave-polymerized denture base resin tested were not affected by different polymerization cycles after accelerated aging or immersion in beverages. These changes were similar to the conventional heat-polymerized acrylic resin test, but lower than the repair resin after accelerated aging.

Introdução: Os ensaios de liberação do interferon- γ (ELIG) surgiram como uma alternativa para o diagnóstico de infecção latente pelo Mycobacterium tuberculosis (ILTB). Neste estudo, nós comparamos o desempenho de um dos ELIG, teste Quantiferon TB Gold in tube – QFT, com a prova tuberculínica (PT) em dois pontos de corte (≥ 5 mm e ≥ 10 mm), em profissionais de saúde da atenção básica à saúde (ABS). Métodos: Estudo transversal realizado em profissionais de saúde da ABS de quatro capitais nacionais com alta incidência de TB. O resultado do teste QFT foi comparado com o resultado da PT nos pontos de corte ≥ 5mm e ≥ 10 mm. Resultados: Foram incluídos 632 profissionais de saúde. Ao considerar o ponto de corte ≥ 10 mm para a PT, a concordância entre QFT e a PT foi de 69% (k = 0,31) e para o ponto de corte ≥ 5 mm, a concordância entre os testes foi de 57% (k = 0,22). Devido a baixa concordância entre a PT e o QFT, nós avaliamos os possíveis fatores associados com a discordância entre eles. Ao comparar o grupo PT- / QFT- com o grupo PT+ / QFT-, no ponto de corte ≥ 5 mm, a idade entre 41-45 [OR = 2,70, IC 95%: 1,32-5,51] e 46-64 [OR = 2,04, IC 95%: 1,05-3,93], presença de cicatriz vacinal do BCG [OR = 2,72, IC 95%: 1,40-5,25] e trabalhar apenas na ABS [OR = 2,30, IC 95 %: 1,09-4,86] apresentaram associação estatística significativa. Para o ponto de corte ≥ 10 mm, a presença de cicatriz vacinal do BCG [OR = 2,26, IC 95%: 1,03-4,91], ter tido contato domiciliar com paciente portador de tuberculose ativa [OR = 1,72, IC 95%: 1,01-2,92] e ter feito a PT anteriormente [OR = 1,66, IC 95%: 1,05-2,62] revelaram associação estatística significativa. Curiosamente, a discordância observada no grupo PT- / QFT + não apresentou associação estatistica com nenhuma das variáveis consideradas, independentemente do ponto de corte da PT. Conclusões: Apesar de termos identificado que a vacina BCG contribuiu para a discordância entre os testes, as recomendações brasileiras para o início do tratamento da ILTB não devem ser alteradas devido as limitações do QFT.

O ataque dos ácaros Calacarus heveae Feres e Tenuipalpus heveae Baker em seringueira pode causar intenso desfolhamento precoce das plantas. É provável que a queda de folhas antes do período de senescência normal resulte em diminuição da capacidade fotossintética e como conseqüência, da produção. Este trabalho teve por objetivo avaliar o efeito do desfolhamento provocado por ácaros sobre a produção de látex da seringueira. O experimento foi desenvolvido no município de Reginópolis, SP, com o clone PB 235, no período de setembro de 2002 a agosto de 2003, com dois tratamentos: área tratada com defensivos agrícolas para evitar o desfolhamento e área sem pulverização. As plantas foram submetidas ao sistema de sangria 1/2 S d/5 6d/7. 10m/y. ET 3.3% 4/y e a produção foi pesada mensalmente. As amostragens dos ácaros foram realizadas com intervalo de 7 a 10 dias. O desfolhamento foi avaliado pela medição, com auxílio de um luxímetro, da intensidade de luz sob a copa das plantas. Houve diferença significativa na ocorrência dos ácaros. Para C. heveae, a média geral, de todas as avaliações, foi de 0,34 ácaros/cm² na área tratada e de 0,93 ácaros/cm², na área sem tratamento. Para T. heveae esses valores foram de 0,06 e 1,09 ácaros/cm², respectivamente. Como consequência, houve diferença significativa com relação ao desfolhamento e à produção nos meses de maio, junho, julho e agosto.

OBJETIVO: Descrever e analisar as características associadas ao risco de ocorrência de distúrbios psiquiátricos menores para a categoria de motoristas e cobradores de ônibus urbanos na cidade de São Paulo (Brasil), em 1990. METODOLOGIA: Estudou-se a prevalência dos distúrbios psiquiátricos menores (DPM) em uma amostra de 925 motoristas e cobradores, controlando e identificando as variáveis de confundimento existentes, através da análise de regressão logística, utilizando-se um modelo progressivo, passo a passo. RESULTADOS: Observou-se uma prevalência de DPM de 20,3% no conjunto dos dois grupos de trabalhadores, sendo significativamente maior entre os cobradores (28%) do que entre os motoristas (13%) (p < 0,0001). A análise de regressão logística mostrou o trânsito intenso (or = 1,99; com intervalo de confiança de 95% = 1,39-2,84) e a condição ocupacional de cobrador (or = 1,84; ic95% = 1,09-3,10) como os fatores de maior risco para os DPM. Além desses, mostraram-se fatores de risco, o déficit de sono (dormir menos que 6 horas diárias), o absenteísmo, utilização de banco sem mecanismos de regulagem, migração da região Nordeste e alterações na escala de trabalho. CONCLUSÕES: O risco para a categoria ocupacional cobrador sugere a importância deste trabalho específico como determinante do sofrimento psíquico, levando a considerar-se a relação com os passageiros como um possível fator de risco, pois esta é nuclear no trabalho do cobrador. Recomenda-se a realização de novos estudos que busquem entender melhor a relação com os passageiros, a especificidade do trabalho do cobrador e do motorista e seus problemas de saúde, de modo a subsidiar-se mudanças na organização do trabalho do sistema.

OBJETIVO: Estudar a associação entre consumo de cafeína na gestação e ocorrência de baixo peso ao nascer, prematuridade e restrição do crescimento intra-uterino. MÉTODOS: A investigação se desenvolveu por estudo caso-controle. Foram selecionados 354 recém-nascidos vivos de partos únicos com peso menor que 2.500 g (casos) e 354 com 3.000 g ou mais (controles). A ingesta de cafeína foi calculada considerando-se o consumo diário de café, refrigerante e chá. Os resultados foram ajustados por análise de regressão logística múltipla para as variáveis de confundimento: idade materna, escolaridade, renda, situação conjugal, cor, paridade, fumo, filhos anteriores de baixo peso, peso prévio à gestação, trabalho, intervalo gestacional, consultas durante o pré-natal e hipertensão arterial. RESULTADOS: Os resultados mostraram os seguintes "odds ratio", ajustados entre o consumo diário de cafeína <300 mg/dia e >300mg/dia, e o baixo peso ao nascer, respectivamente: 0,72 (IC95%, 0,45-1,25) e 0,47 (IC95%, 0,24-0,92); prematuridade: 0,59 (IC95%, 0,32-1,09) e 0,32 (IC95%, 0,15-0,72); e retardo do crescimento intra-uterino: 1,16 (IC95%, 0,45-3,01) e 0,64 (IC95%, 0,20-1,98). CONCLUSÃO: Na amostra estudada, a ingesta de cafeína não foi identificada como fator de risco para prejuízo do crescimento intra-uterino ou para a duração da gestação.

OBJETIVO: Determinar os fatores de risco para o nascimento de crianças pequenas para a idade gestacional. MÉTODOS: Nos meses de outubro a dezembro de 1996, cinco maternidades de Pelotas, RS, foram visitadas diariamente para entrevistar puérperas. O peso ao nascer foi obtido do registro dos berçários. A idade gestacional foi calculada a partir da data da última menstruação. Considerou-se como pequena para a idade gestacional (PIG) aquelas crianças cujo peso ao nascer, de acordo com o sexo e a idade gestacional, estava abaixo do percentil 10 da população de referência de Williams. O teste qui-quadrado foi usado nas análises bivariadas e a regressão logística, não-condicional, na análise multivariada. RESULTADOS: Foram estudadas 1.082 puérperas e a prevalência de nascimento PIG foi de 13,1%. Mesmo após controle para possíveis fatores de confusão, as crianças cuja renda familiar era <1 salário-mínimo apresentaram uma maior chance de serem PIG (OR: 8,81 IC95% 1,12-69,46) do que aquelas com renda ³10 salários-mínimos. Um pré-natal de baixa qualidade também aumentou a chance de PIG (OR: 3,28 IC95% 1,09-9,91). Baixa estatura materna e exposição ao tabaco também estiveram associados com nascimentos PIG. CONCLUSÕES: O baixo nível socioeconômico, tabagismo materno, estatura materna e pré-natal de baixa qualidade foram os principais fatores de risco para PIG.
