These two handwritten letters by Timothy Pickering were written on February 14, 1797 and June 14, 1798 to his brother John Pickering and his father Timothy Pickering, respectively. The letter to his brother, John, discusses mutual friends, classmate Thomas Lee, and John’s recent attendance at a sermon by Dr. Joseph Priestley. The letter from Timothy to his father includes a discussion of Timothy’s expenses and the amount of money needed to pay his debts, a request for new shoes for commencement, the news of Timothy’s invitation to join honor society Phi Beta Kappa, and a few comments on his forensics course at Harvard.

The EU has become a loose kind ofsocial federation, a fact that has not been adequately taken into account due to the peculiarities ofthe Maastricht strategy for monetary integration. Yet, a new approach to the economic theory offederalism is required ifone wants to analyze the most pressing issues ofEU social policy. The social insurance view of redistribution and stabilization provides for such an approach. This view supports laboratory federalism in which it is the role ofthe EU Commission to contain systems competition in order to preserve "stability in diversity." The role ofthe EU level would be to promote horizontal and vertical learning processes and to make sure that stability concerns ofthe EU are taken seriously by member countries' governments. The minimum requirements framework for social policy that the EU Commission has adopted must be taken as a point of departure, even though it is a less than satisfactory approach from this point of view. Laboratory standardization, in contrast, would not set specific minimum requirements but meta-standards that protect systems functions and safeguard against systems failures.

Virulence factors from the ROP2-family have been extensively studied in Toxoplasma gondii, but in the closely related Neospora caninum only NcROP2Fam-1 has been partially characterized to date. NcROP40 is a member of this family and was found to be more abundantly expressed in virulent isolates. Both NcROP2Fam-1 and NcROP40 were evaluated as vaccine candidates and exerted a synergistic effect in terms of protection against vertical transmission in mouse models, which suggests that they may be relevant for parasite pathogenicity. NcROP40 is localized in the rhoptry bulbs of tachyzoites and bradyzoites, but in contrast to NcROP2Fam-1, the protein does not associate with the parasitophorous vacuole membrane due to the lack of arginine-rich amphipathic helix in its sequence. Similarly to NcROP2Fam-1, NcROP40 mRNA levels are highly increased during tachyzoite egress and invasion. However, NcROP40 up-regulation does not appear to be linked to the mechanisms triggering egress. In contrast to NcROP2Fam-1, phosphorylation of NcROP40 was not observed during egress. Besides, NcROP40 secretion into the host cell was not successfully detected by immunofluorescence techniques. These findings indicate that NcROP40 and NcROP2Fam-1 carry out different functions, and highlight the need to elucidate the role of NcROP40 within the lytic cycle and to explain its relative abundance in tachyzoites.

SUMMARY Switzerland is facing an aging population and a growing amount of patients with chronic diseases. It is crucial to display health care processes and pathways, to identify inequalities and obstacles, and to point out possibilities for improvements of the Swiss health care system (e.g. increase efficiency). The introductory part of the thesis presents a brief description of the Swiss health care system, health services research and regional variation as well as an introduction of CVD and its epidemiological key figures, aetiology and treatments. This is followed by the description of the utilized methods and data, and the objectives of this thesis. The subsequent sections present the four articles included in this thesis. The first article focuses on a small area analysis on regional variation of avoidable hospitalisations in Switzerland including density of primary care physicians and specialists, rurality and hospital supply factors as explanatory variables in the analysis. Lower rates of avoidable hospitalisations were found in areas with very high supply of primary care physicians, increased avoidable hospitalisation rates in areas with more specialists and in areas with higher proportion of rural residents. The second article aims to examine whether emergency patients with acute ST-segment elevation myocardial infarction were adequately treated, i.e. according to the treatment guidelines, in Switzerland. Results show that older and female patients were less likely to receive revascularization which suggests that the treatment guidelines may not be uniformly applied in Switzerland. Similar to the first article, also in the third article a small area analysis was performed but this time investigating regional variation in costs at the end of life. Strongest associations of cost was found with cause of death, age and language region of the decedents. The strong spatial variation of costs could only partly be explained by the included covariates. Article four aims to examine the relationship of distance to different hospital types and mortality from AMI or stroke. We found that AMI mortality in the Swiss population 30 and older and stroke mortality in those 65 and above increased with distance to central and university hospitals, while adjusting for sociodemographic and economic characteristics of the population. The presentation of the four articles is followed by a discussion, which summarizes the main findings and the strengths and limitations of the presented articles. The thesis concludes with a discussion about the challenges for policy, practice and future research.

A probabilistic function (integrated source contribution function, ISCF) based on backward air mass trajectory calculation was developed to track sources and atmospheric pathways of polycyclic aromatic hydrocarbons (PAHs) to the Canadian High Arctic station of Alert. In addition to the movement of air masses, the emission intensities at the sources and the major processes of partition, indirect photolysis, and deposition occurring on the way to the Arctic were incorporated into the ISCF. The predicted temporal trend of PAHs at Alert was validated by measured PAH concentrations throughout 2004. The PAH levels in the summer are orders of magnitude lower than those in the winter and spring when long-range atmospheric transport events occur more frequently. PAHs observed at Alert are mostly from East Asia (including Russia Far East), North Europe (including European Russia), and North America. These sources account for 25, 45, and 27% of PAHs atmospheric level at Alert, respectively. Source regions and transport pathways contributing to the PAHs contamination in the Canadian High Arctic vary seasonally. In the winter, Russia and Europe are the major sources. PAHs from these sources travel eastward and turn to the north at approximately 120°E before reaching Alert, in conjunction with the well- known Arctic haze events. In the spring, PAHs from Russia and Europe first migrate to the west and then turn to the north at 60°W toward Alert. The majority of PAHs in the summer are from northern Canada where they are carried to Alert via low- level transport pathways. In the fall, 70% of PAHs arriving at Alert are delivered from North American sources.

In order to characterize the provenance of lithogenic surface sediments from the Eastern Mediterranean Sea (EMS), residual (leached) fraction of 34 surface samples have been analysed for their 143Nd/144Nd and 87Sr/86Sr isotope ratios. The sample locations bracket all important entrances of riverine suspended matter into the EMS as well as all sub-basins of the EMS. The combined analyses of these two isotope ratios provide a precise characterization of the lithogenic fraction of surface sediments and record their dilution towards the central sub-basins. We reconstruct provenance and possible pathways of riverine dispersal and current redistribution, assuming more or less homogenous isotopic signatures and flux rates of the eolian fraction over the EMS. Lithogenic sediments entering the Ionian Sea from the Calabrian Arc and the Adriatic Sea are characterized by high 87Sr/86Sr isotope ratios and low epsilon-Nd(0) values (average 87Sr/86Sr=0.718005 and epsilon-Nd(0)=-11.06, n=5). Aegean Sea terrigenous sediments show an average ratio of 87Sr/86Sr=0.713089 (n=5) and values of epsilon-Nd(0)=-7.89 (n=5). The Aegean isotopic signature is traceable up to the southwest, south, and southeast of Crete. The sediment loads entering the EMS via the Aegean Sea are low and spread out mainly through the Strait of Casos (east of Crete). Surface sediments from the eastern Levantine Basin are marked by the highest epsilon-Nd(0) values (-3.3, n=6) and lowest 87Sr/86Sr isotope ratios (average 0.709541, n=6), reflecting the predominant input of the Nile sediment. The influence of the Nile sediment is traceable up to the NE-trending, eastern flank of the Mediterranean Ridge. The characterization of the modern riverine dispersal and eolian flux, based on isotope data, may serve as a tool to reconstruct climate-coupled variations of lithogenic sediment input into the EMS.

As we celebrate 50 years of the Schonell Special Education Research Centre it is timely to consider changes that have occurred in the provision of residential services for people with an intellectual disability. Before the 1970s adults and children were cared for in large institutions using a medical model of care. In the mid-1970s a new developmental model based on education and training was implemented in response to the principle of normalisation and issues of social justice. The most dramatic changes have occurred in the last ten years with the decision to close large institutions and relocate residents into ordinary homes in the community. This paper describes changes in lifestyle for adults with an intellectual disability as a result of the move from institutional to community residential service provision. The Challinor Centre in Ipswich, Queensland, Australia provides examples of lifestyle changes that have occurred under different models of service provision during this time. Community living is described with research evidence validating the advantages of this type of service provision for residents with an intellectual disability. Outcomes have been documented through the use of group results and a case study of one individual following deinstitutionalisation describes the benefits of this new model of residential accommodation
