Introduction: 5-Fluorouracil (5-FU) is considered to be the backbone of colorectal cancer (CRC) systemic therapy since the great majority of recommended regimens include its administration. A clinical picture consisting of chest pain, sometimes cardiac enzyme elevation, electrocardiogram abnormalities consistent with myocardial ischemia, and normal coronary angiogram associated with 5-FU administration have been infrequently reported. The clinical dilemma is: Which chemotherapy regimen should we use in CRC patients with a previous acute coronary syndrome (ACS) associated with 5-FU? Case Report: We describe the case of a 55-year-old otherwise healthy woman with metastatic colon adenocarcinoma who presented an ACS probably secondary to arterial vasospasm while receiving continuous intravenous 5-FU infusion (mFOLFOX6 regimen). After the ACS, the patient was treated with raltitrexate plus oxaliplatin (TOMOX) and subsequently with irinotecan plus cetuximab with no other cardiac event. Conclusion: The risk of cardiotoxicity associated with 5-FU is low but real. The probable mechanism is arterial vasospasm, as suggested by our case report. Both the use of the TOMOX regimen and irinotecan plus cetuximab seems to be safe regimens to be considered in this clinical scenario. © 2009 Humana Press Inc.

Clinical studies on nasal topical medications require the standardization of nasosinusal normality in order to establish control groups through a specific evaluation of the upper airways. Aim: to standardize the evaluation of candidates for control groups in clinical studies on nasal topical medications. Material and Methods: healthy male volunteers of 18 to 50 years of age, asymptomatic from the nasosinusal standpoint were subjected to a sequential and excluding assessment made up of clinical evaluation, immediate hypersensitivity skin test, saccharin test, flexible nasofibroscopy and nasal cytology. Study design: Crosssectional contemporary cohort. Results: Of the 33 people originally enrolled, 14 (42.4%) were excluded for clinical reasons. Of the 19 remaining, 2 (10.5%) had atopy diagnosed in the skin test and were excluded. 17 were tested with saccharin and presented normal mucociliary clearance. Evaluation by nasal endoscopy showed abnormality in 2 cases (11.8%) and these were excluded. The remaining 15 were submitted to nasal cytology, which proved normal, representing 45.5% of those initially included. Conclusion: The proposed protocol for sequential and excluding evaluation was effective in defining candidates for the establishment of control groups in clinical studies on nasal topical medications. © Revista Brasileira de Otorrinolaringologia. According to the current guidelines recommendations the authors reviewed the treatment of community-acquired pneumonia (CAP) and hospital-acquired pneumonia (HAP). Aim: The purpose of this study was to compare the effectiveness of a high-volume evacuation and a conventional intraoral suction system and aspirating tips for capturing aluminum oxide particles during use of an air-abrasion device. Methods: A phantom head was fixed at the dental chair head with secured a metallic device with 5 horizontal shafts, corresponding to operator's clockrelated working positions, and one vertical shaft to simulate the operator's nasal cavity. Petri plates were fixed to the shafts at distances of 20, 40 and 60 cm from the center of the oral cavity of the phantom head to collect the aluminum oxide particles spread over during air abrasion. The dust was aspirated with two types of suction tips used with both suction systems: a conventional saliva ejector and a saliva ejector customized by the adaptation of a 55-mm-diameter funnel. Results: The amount of particles showed that the greatest abrasive particle deposition occurred at a distance of 20 cm from the center of the oral cavity of the phantom head at 9 o'clock operatory position with the conventional saliva ejector attached to high-volume evacuation system. Conclusions: The greatest deposition of aluminum oxide particles occurred at the shortest distance between the operator and the center of the oral cavity, while using the high-volume evacuation system associated to the conventional suction tip.

It was reviewed the definition, mechanisms and classification of functional constipation. The diagnostic resources available were showed and it was underlined the importance to investigate and exclude the multiple causes of secondary constipation. The therapy was divided in general measures, like diet and specialized, involving laxatives and others alternatives. It was commented the different categories of laxatives, their actions and adverse effects. It was concluded that the best approach to functional defecatory disorder is biofeedback. Otherwise, for defecatory disturbance caused by mechanical obstruction and refractory constipation due to slow transit the therapeutical option is surgery.

The indwelling urethral catheter has an important role for patients with urinary retention, bladder obstruction, neurological damage and other diseases. Urine contains minerals which precipitate in alkaline pH, crystallize and block the urological catheter. The crystallization of the ionic components of urine occurs in the presence of urease, an enzyme produced by Proteus mirabilis. This bacterium adheres to inanimate surfaces and forms biofilms. The aim of this study was to investigate the formation of crystalline biofilm on the luminal surface of siliconized latex catheters by means of scanning electron microscope, after channeling artificial urine infected with Proteus mirabilis. The experiment was performed in vitro using a dynamic flow system. The artificial urine compounds were salts of calcium, magnesium, phosphates, urea and egg albumin, and it was infected with Proteus mirabilis ATCC 25933. The urine flow was stopped after crystallization of the ionic components. Crystallization was observed after alkalinization of urine. Scanning electron microscopy showed the presence of crystals and morphologies typical of bacilli embedded in an amorphous mass on the internal lumen of the catheter. The present study showed that catheter encrustation may limit the use of long-term indwelling catheter. © SBEB - Sociedade Brasileira de Engenharia Biomédica.

Aim: To evaluate the influence of construction bite in the dentoskeletal changes induced by Klammt Appliance. Methods: The sample consisted of 17 children, with Class II malocclusion and initial mean age of 8.5 years. The construction bite was obtained using an Exactobite on edge-toedge anteroposterior relationship with 3 mm interincisal clearance. The height of the acrylic was determined by initial overbite associated to interincisal clearance and measured with digital caliper. The amount of advancement was obtained and measured by initial overjet in the lateral radiography. Pearson's correlation, linear regression and ANOVA were used to determine the relationship between dentoskeletal and construction bite variables. Results: The increase in the height of the acrylic promotes a greater inhibition of the forward displacement of the nasal spine and reduction in the facial growth index. The increase in the mandibular advancement induces more downward displacement of nasal spine and pogonion; a counter-clockwise rotation of palatine plane; an increase in mandibular length, maxillary alveolar height and interincisal angle; a decrease in mandibular alveolar height, the intermaxillary discrepancy and overjet; and palatal tipping of upper incisors. Conclusions: The different dimensions of the construction bite influence the dentoskeletal changes induced by the appliance in Class II treatment.

Background: Previous studies have pointed out that the mere elevation of the maxillary sinus membrane promotes bone formation without the use of augmentation materials. Purpose: This experimental study aimed at evaluating if the two-stage procedure for sinus floor augmentation could benefit from the use of a space-making device in order to increase the bone volume to enable later implant installation with good primary stability. Materials and Methods: Six male tufted capuchin primates (Cebus apella) were subjected to extraction of the three premolars and the first molar on both sides of the maxilla to create an edentulous area. The sinuses were opened using the lateral bone-wall window technique, and the membrane was elevated. One resorbable space-making device was inserted in each maxillary sinus, and the bone window was returned in place. The animals were euthanatized after 6 months, and biopsy blocks containing the whole maxillary sinus and surrounding soft tissues were prepared for ground sections. Results: The histological examination of the specimens showed bone formation in contact with both the schneiderian membrane and the device in most cases even when the device was displaced. The process of bone formation indicates that this technique is potentially useful for two-stage sinus floor augmentation. The lack of stabilization of the device within the sinus demands further improvement of space-makers for predictable bone augmentation. Conclusions: It is concluded that (1) the device used in this study did not trigger any important inflammatory reaction; (2) when the sinus membrane was elevated, bone formation was a constant finding; and (3) an ideal space-making device should be stable and elevate the membrane to ensure a maintained connection between the membrane and the secluded space. © 2009 Wiley Periodicals, Inc.

Ankylosis of the temporomandibular joint in children is one the most difficult and complex conditions managed by oral and maxillofacial surgeons, and often leads to some facial deformity. Distraction osteogenesis of the mandible provides an excellent treatment for mandibular airway obstruction in children who do not respond to conservative measures, and allows for early removal of the tracheostomy. We report the case of a 1-year-old boy with severe micrognathia and temporomandibular ankylosis who was dependent on a tracheostomy; he was treated with piezosurgery and mandibular advancement by distraction osteogenesis. © 2010 The British Association of Oral and Maxillofacial Surgeons. The main purpose of this work is to study coincidences of fiber-preserving self-maps over the circle S 1 for spaces which are fiberbundles over S 1 and the fiber is the Klein bottle K. We classify pairs of self-maps over S 1 which can be deformed fiberwise to a coincidence free pair of maps. © 2012 Pushpa Publishing House.

This study aimed to characterize the true epidemiological role played by the Chinese goose (Anser cygnoides) as a potential source of infection by the Newcastle disease virus (NDV). For this, Specific-Pathogen-Free chicks (SPF) were used and were housed with Chinese geese that had been inoculated with a pathogenic strain (velogenic viscerotropic, strain São João do Meriti) of NDV (DIE50=108.15/0.1 mL) pathogenic to chickens, by the ocular-nasal route. Each group was composed of 6 SPF Leghorn chicks and 3 geese. At 6 days (Group I) and 14 days (Group II) after inoculation of the Chinese geese with NDV, SPF chicks were put into direct contact with each goose group. Cloacal swabs were collected from both species (Chinese geese and SPF chicks) 6, 10 and 20 days after challenge to genome viral excretion by Reverse Transcription Polymerase Chain Reaction (RT-PCR). Chinese geese did not demonstrate any clinical signs of Newcastle disease (ND). They were refractory to the clinical disease with the NDV. However, NDV genome was detected 20 days after challenge. Therefore, NDV carrier status was demonstrated by Chinese geese. Moreover, 100% of SPF chicks housed with the infected Chinese geese had died by 6 (Group I) and 14 days (Group II) after challenge. Thus, the transmission of the pathogenic virus from the Chinese geese to cohabiting SPF chicks was evident within 20 days of the experimental infection. This reveals the epidemiological importance of Chinese geese as a potential transmitter of NDV infection to other commercial birds that could be raised in close proximity.

Studies were made to clarify the role that was played by the lovebirds (Agapornis roseicollis) in the epidemiological plan, under the perspective of its being a potential source of infection of Newcastle Disease Virus (NDV). The study used Specific-Pathogen-Free chicks (SPF) that were housed with lovebirds inoculated with a pathogenic strain (velogenic viscerotropic) of NDV pathogenic to chickens, by the ocular-nasal via. Each group was composed of six SPF chicks and four lovebirds. After five days of the inoculation of the lovebirds with NDV, SPF chicks were put together with each group of lovebirds. Cloacae swabs were collected after 9, 14 and 21 days post-challenge in both species (lovebirds and SPF chicks) for genome viral excretion by Reverse Transcription Polymerase Chain Reaction (RT-PCR). Lovebirds did not demonstrate any clinical signs of NDV. They were refractory to the clinical disease with the NDV. However, NDV genome was detected 9 and 21 days after challenge. This study shows that lovebirds can be carriers NDV. Moreover, 100% of SPF chicks allocated with the infected lovebirds demonstrated clinical signs and lesions suggestive of NDV. In these birds, NDV genome was detected 9, 14 and 21 days after challenge. Thus, the transmission of the pathogenic virus from the lovebirds to SPF chicks that were housed together was evident until 21 days of the experimental infection. This study reveals the importance of lovebirds from the epidemiological point of view as potential source of infection of the NDV to other avian species that could be raised near this species. © Asian Network for Scientific Information, 2012.

Background: Decreased heart rate variability (HRV) is related to higher morbidity and mortality. In this study we evaluated the linear and nonlinear indices of the HRV in stable angina patients submitted to coronary angiography. Methods. We studied 77 unselected patients for elective coronary angiography, which were divided into two groups: coronary artery disease (CAD) and non-CAD groups. For analysis of HRV indices, HRV was recorded beat by beat with the volunteers in the supine position for 40 minutes. We analyzed the linear indices in the time (SDNN [standard deviation of normal to normal], NN50 [total number of adjacent RR intervals with a difference of duration greater than 50ms] and RMSSD [root-mean square of differences]) and frequency domains ultra-low frequency (ULF) ≤ 0,003 Hz, very low frequency (VLF) 0,003 - 0,04 Hz, low frequency (LF) (0.04-0.15 Hz), and high frequency (HF) (0.15-0.40 Hz) as well as the ratio between LF and HF components (LF/HF). In relation to the nonlinear indices we evaluated SD1, SD2, SD1/SD2, approximate entropy (-ApEn), α1, α2, Lyapunov Exponent, Hurst Exponent, autocorrelation and dimension correlation. The definition of the cutoff point of the variables for predictive tests was obtained by the Receiver Operating Characteristic curve (ROC). The area under the ROC curve was calculated by the extended trapezoidal rule, assuming as relevant areas under the curve ≥ 0.650. Results: Coronary arterial disease patients presented reduced values of SDNN, RMSSD, NN50, HF, SD1, SD2 and -ApEn. HF ≤ 66 ms§ssup§2§esup§, RMSSD ≤ 23.9 ms, ApEn ≤-0.296 and NN50 ≤ 16 presented the best discriminatory power for the presence of significant coronary obstruction. Conclusion: We suggest the use of Heart Rate Variability Analysis in linear and nonlinear domains, for prognostic purposes in patients with stable angina pectoris, in view of their overall impairment. © 2012 Pivatelli et al.; licensee BioMed Central Ltd.

Background: Staphylococcus is a clinically important genus because of its capacity to produce enterotoxins and to cause food poisoning. Staphylococci are the most frequent microorganisms of the skin and mucosal microbiota, with an estimated 20 to 40% of individuals carrying these bacteria on their hands or nose. Since nutrition professionals are involved in the handling and preparation of foods and are possible carriers of these bacteria, the objective of this study was to investigate the presence of Staphylococcus on the hands and in the nasal fossae of undergraduate nutrition students and to determine the enterotoxigenic capacity of these microorganisms. Methods and Findings: A total of 201 strains were isolated from the hands and nose of 61 nutrition students. Of these, 180 (89.5%) were identified as coagulasenegative staphylococci and 21 (10.5%) as S. aureus. Thirty-seven (18.4%) Staphylococcus isolates were producers of enterotoxin A. Toxin production was detected in 5 (19%) of the S. aureus isolates and in 31 (17.2%) of the coagulase-negative staphylococci. Conclusions: This study demonstrated a large number of enterotoxin-producing staphylococci on the hands and nose of nutrition students and professionals involved in the handling and preparations of foods. These findings indicate the need for adequate hygiene measures to prevent food poisoning. © iMedPub.
