Purpose: The effectiveness of synchronous carboplatin, etoposide, and radiation therapy in improving survival was evaluated by comparison of a matched set of historic control subjects with patients treated in a prospective Phase II study that used synchronous chemotherapy and radiation and adjuvant chemotherapy. Patients and Methods: Patients were included in the analysis if they had disease localized to the primary site and nodes, and they were required to have at least one of the following high-risk features: recurrence after initial therapy, involved nodes, primary size greater than 1 cm, or gross residual disease after surgery. All patients who received chemotherapy were treated in a standardized fashion as part of a Phase II study (Trans-Tasman Radiation Oncology Group TROG 96:07) from 1997 to 2001. Radiation was delivered to the primary site and nodes to a dose of 50 Gy in 25 fractions over 5 weeks, and synchronous carboplatin (AUC 4.5) and etoposide, 80 mg/m(2) i.v. on Days 1 to 3, were given in Weeks 1, 4, 7, and 10. The historic group represents a single institution's experience from 1988 to 1996 and was treated with surgery and radiation alone, and patients were included if they fulfilled the eligibility criteria of TROG 96:07. Patients with occult cutaneous disease were not included for the purpose of this analysis. Because of imbalances in the prognostic variables between the two treatment groups, comparisons were made by application of Cox's proportional hazard modeling. Overall survival, disease-specific survival, locoregional control, and distant control were used as endpoints for the study. Results: Of the 102 patients who had high-risk Stage I and II disease, 40 were treated with chemotherapy (TROG 96:07) and 62 were treated without chemotherapy (historic control subjects). When Cox's proportional hazards modeling was applied, the only significant factors for overall survival were recurrent disease, age, and the presence of residual disease. For disease-specific survival, recurrent disease was the only significant factor. Primary site on the lower limb had an adverse effect on locoregional control. For distant control, the only significant factor was residual disease. Conclusions: The multivariate analysis suggests chemotherapy has no effect on survival, but because of the wide confidence limits, a chemotherapy effect cannot be excluded. A study of this size is inadequately powered to detect small improvements in survival, and a larger randomized study remains the only way to truly confirm whether chemotherapy improves the results in high-risk MCC. There is some evidence that dietary factors may modify the risk of squamous cell carcinoma (SCC) of the skin, but the association between food intake and SCC has not been evaluated prospectively. We examined the association between food intake and SCC incidence among 1,056 randomly selected adults living in an Australian sub-tropical community. Measurement-error corrected estimates of intake in 15 food groups were defined from a validated food frequency questionnaire in 1992. Associations with SCC risk were assessed using Poisson and negative binomial regression to the persons affected and tumour counts, respectively, based on incident, histologically confirmed tumours occurring between 1992 and 2002. After multivariable adjustment, none of the food groups was significantly associated with SCC risk. Stratified analysis in participants with a past history of skin cancer showed a decreased risk of SCC tumours for high intakes of green leafy vegetables (RR = 0.45, 95% CI = 0.22-0.91; p for trend = 0.02) and an increased risk for high intake of unmodified dairy products (RR = 2.53, 95% CI: 1.15-5.54; p for trend = 0.03). Food intake was not associated with SCC risk in persons who had no past history of skin cancer. These findings suggest that consumption of green leafy vegetables may help prevent development of subsequent SCCs of the skin among people with previous skin cancer and that consumption of unmodified dairy products, such as whole milk, cheese and yoghurt, may increase SCC risk in susceptible persons.

Half of all cancers in the United States are skin cancers. We have previously shown in a 4.5-year randomized controlled trial in an Australian community that squamous cell carcinomas (SCC) but not basal cell carcinomas (BCC) can be prevented by regular sunscreen application to the head, neck, hands, and forearms. Since cessation of the trial, we have followed participants for a further 8 years to evaluate possible latency of preventive effect on BCCs and SCCs. After prolonged follow-up, BCC tumor rates tended to decrease but not significantly in people formerly randomized to daily sunscreen use compared with those not applying sunscreen daily. By contrast, corresponding SCC tumor rates were significantly decreased by almost 40% during the entire follow-up period (rate ratio, 0.62; 95% confidence interval, 0.38-0.99). Regular application of sunscreen has prolonged preventive effects on SCC but with no clear benefit in reducing BCC.

Aim: HER-2/neu amplification occurs in 15-25% of breast carcinomas. This oncogene, also referred to as c-erbB-2, encodes a transmembrane tyrosine kinase receptor belonging to the epidermal growth factor receptor family. HER-2 over-expression is reported to be associated with a poor prognosis in breast carcinoma patients and in some studies is associated with a poorer response to anti-oestrogen therapy. These patients are less likely to benefit from CMF (cyclophosphamide, methotrexate, fluorouracil)-based chemotherapy compared with anthracycline-based chemotherapy. The aim of this study was to evaluate breast carcinomas to determine hormone receptor status and if there is a difference in breast cancer specific survival for HER-2 positive patients. Methods: A total of 591 breast carcinomas were evaluated using immunohistochemistry (IHC) for oestrogen receptor (ERp), progesterone receptor (PRp) and three different HER2 antibodies (CB11, A0485 and TAB250). Percentage of tumour cells and intensity of staining for ERp were evaluated using a semiquantitative method. Results: Of the 591 tumours, 91 (15.4%) showed 3+ membrane staining for HER-2 with one or more antibodies. Of these 91 tumours, 41 (45.1%) were ERp+/ PRp+, seven (7.7%) were ERp+/PR-, six (6.6%) were ERp-/PRp+ and 37 (40.7%) were ERp-/PR-. Of HER-2 positive tumours, 5.5% showed > 80% 3+ staining for ERp compared with 31.8% of 0-2+ HER-2 tumours; 24.2% of HER-2-positive tumours showed 60% or more cells with 2+ or 3+ staining for ERp. Treatment data were available for 209 patients and no difference was observed in breast cancer specific survival (BCSS) with HER-2 status and tamoxifen. Conclusion: Oestrogen receptor status cannot be used to select tumours for evaluation of HER-2 status, and oestrogen and progesterone receptor positivity does not preclude a positive HER-2 status. There is a higher proportion of ERp negative tumours associated with HER-2 positivity, however, more than 20% of HER-2 positive tumours show moderate or strong staining for ERp. HER-2 positive patients in this study did not show an adverse BCSS with tamoxifen treatment unlike some previous studies.

Com esse trabalho, visamos discutir a tentativa de estabelecer um equilíbrio entre o ser humano e natureza na área rural de Judá, pouco antes do reinado de Josias (640-609 a.C.). Nesse caso, pode-se perguntar: seria o mandamento de Deuteronômio 5,12-15 um discurso ecológico? A partir dos estudos de Frank Crüsemann e Haroldo Reimer se admite que partes das leis veterotestamentárias eram destinadas ao assim chamado grupo povo da terra de Judá, visando à manutenção de seu poder. O grupo teria assumido a liderança em Judá mediante um golpe político e, articulando-se, desde então, numa política de aliança para se conservar no poder, mesmo não o assumindo diretamente. Nesse contexto de política de alianças deve-se procurar a implementação do mandamento de Deuteronômio 5,12-15. Ele teria sido escrito por anciãos, um grupo junto ao qual o povo da terra teria se aliado para que ordenassem sentenças jurídicas para a acomodação social. Nesse caso, inicialmente o portão da cidade, espaço oficial para discussões, reclamações e propostas de intermediações, deve ter sido o lugar de elaboração de sentenças jurídicas sobre a utilização de técnicas na agricultura. Sendo elas posteriormente levadas ao tribunal do templo para passar pelas mãos dos sacerdotes, outro braço da coalizão. O uso dos animais de porte, cujo peso prejudicava as pequenas propriedades de terra de Judá, deve ter sido um motivo de incessantes conflitos entre pequenos e grandes proprietários de terra. Ressaltamos assim que apenas os homens mais abastados de Judá tinham acesso a esses animais. Esta solução, segundo se entende, liga o rodízio de culturas ao descanso do campo pertencente ao povo da terra de Judá. Liga-se o termo sábado com a vida da elite rural judaíta do período do reinado de Josias. Uma saída encontrada pelas elites de Judá, a qual nos leva a ponderar uma situação similar que ocorre na América Latina, diante da globalização. Se o texto Deuteronômio 5,12-15 é uma ponderação das elites hegemônicas de Judá que buscam o equilíbrio entre ser o humano e a natureza (ecologia), o discurso ecológico contemporâneo poderá ter neste texto um importante interlocutor. Esse discurso pode ocultar interesses econômicos, completamente diferentes, pois se trata de uma estratégia dominadora e não libertadora, objetivando-se, sobretudo, a reprodução social. O Brasil e os demais países da América Latina vêm sofrendo, há algum tempo, com essa distância entre a elite e o resto da sociedade. Nossas elites utilizam-se, há tempos, do discurso ecológico para se manterem no poder dessas sociedades. Por exemplo, vemos nos noticiários uma quantidade de programas e manchetes ligadas à destruição da natureza. Isso é interessante porque, após terem eles mesmo destruído a natureza, passam agora a defendê-la; controlando as reservas naturais, garantindo sua produtividade e seu status quo no sistema econômico atual.(AU)

Epithelial changes observed in actinic cheilitis (AC) and squamous cell carcinoma of the lower lip (LLSCC) are mainly caused by chronic exposure to ultraviolet rays (UV) and are studied using different immunohistochemical markers trying to evaluate the process of carcinogenesis. The objective of this study was to comparatively evaluate the expression of Ki-67 proteins and IMP-3 in AC and LLSCC to contribute with additional information on carcinogenesis in lower lip. A total of 33 cases of AC and 33 cases of LLSCC were studied, analyzed the clinical and pathological features and immunostaining of Ki-67 and IMP-3. Immunohistochemical analysis of Ki-67 was made through the determination of the proliferation index (PI) and subsequent classification of the cases according to the scores: 0 (0% positive cells) +1 (≤30%) + 2 (> 30% and ≤60%) and +3 (> 60%). For statistical tests cases were classified as unmarked (score 0), low expression (score +1) and high expression (scores +2 and +3). For the expression of IMP-3, the percentage of immunostained epithelial cells was established, and assigned scores: 0 (corresponding to 0%), +1 (up to 30% of positive cells); +2 (From 30% to 60% of immunostained cells) and +3 (over 60% of positive cells). Statistical tests chi-square test, Mann-Whitney and Wilcoxon were used. The significance level was 5%. Most AC chaos was male (78.8%) with mean age of 50 years and cases of LLSCC also were male (69.89%) with an average of 62 years. The Ki-67 was expressed in all cases of AC and in cases of LLSCC, predominantly in the two injuries the score 2, corresponding to 81.8% of cases in ACs and 54.5% in the CELI. The expression of IMP-3 in ACs occurred in 72.7% of cases, predominantly in 36.3% of LLSCC cases score 1. Already in the IMP-3 was expressed in 60.6% of cases, especially in 27.3% of the score of the cases 3. These results allow us to conclude that the expression of IMP3 and proliferative activity are early events in carcinogenesis independently lower lip state of change.

Squamous cell carcinoma (SCC ) is the most common malignancy of the oral cavity (OSCC), with a high mortality rate. Due to this, the discovery of biomarkers that facilitate the understanding of the biological behavior of the tumor and improve treatment is necessary. Urokinase type plasminogen activator (uPA) and its receptor, uPAR, are responsible for the proteolysis of structures of the basement membrana and extracellular matrix, facilitating tumor invasion. This study aims to assess the immuno expression of these proteins in 46 cases of squamous cell carcinoma of the oral tongue (OTSCC). These results were related to the presence of metastasis, clinical TNM staging, locoregional recurrence, outcome of the lesion and histological grading. Immunostaining of each case was evaluated semiquantitatively, in the front of invasion and center of the tumor, in which scores were assigned: 0 (0% of positive cells), 1 (1-10% of positive cells), 2 (11 -50% positive cells) and 3 (more than 50% positive cells). The expression of uPA was observed in 93.5% (n=43) of the cases in the front of invasion, with predominance of score 2 (n=16; 34.8%) and in 67.9% (n=31) of the cases in the center of the tumor, with predominance of score 1 (n=15; 32.6%). Overall, the immunoexpression of uPA was not associated with clinical parameters. Regarding the malignant histological grading, a higher expression of uPA was observed in cases of high-grade malignancy comp ared to low-grade malignancy (p=0.05). Regarding the morphological parameters, increased expression of uPA was observed in the worst mode of invasion (p=0.03 ). The expression of uPAR was observed in 73.9% of cases in the front of invasion, with a predominance of score 1 (n=21; 45.6 %), and in 47.5% (n=21) of the cases in the center of the tumor, with a predominance of score 0 (n=25; 54.4%). Although no statistical differences were observed in relation to lymph node metastasis, clinical TNM staging, outcome, and histological grading, there was a higher expression of uPAR in cases with locoregional recurrence (p=0.04). Regarding the tumor intra -localization, it was observed an increased expression of uPA and uPAR at the front of invasion in relation to the center of the tumor (p<0.001). Regarding the correlation between uPA and uPAR, there was no statistical sign ificance. Based on these results, it is suggested that uPA and uPAR are involved in the progression of CELO, mainly in the deeper region of the tumor.

DNA repair systems, genes and proteins are essential for genome integrity maintenance, avoiding serious diseases such as cancer. Deregulation in the expression of those proteins has been associated with both the risk of development and evolution of various human cancers, including oral squamous cell carcinoma. The purpose of this study was to analyze the immunoreactivity of the DNA repair proteins XRCC1, THIIF and XPF in oral tongue squamous cell carcinoma (OTSCC) and to investigate its association with clinical and histopathological parameters, outcome and 5-year survival rate. Seventy-four cases of OTSCC were analyzed semi-quantitatively through immunohistochemistry. We observed that DNA repair proteins were highly expressed in parenchymal cells; however, we only observed a significant association between XRCC1 high expression and better clinical staging (p=0,02). Oesophageal cancer is an aggressive tumour which responds poorly to both chemotherapy and radiation therapy and has a poor prognosis. Thus, a greater understanding of the biology of oesophageal cancer is needed in order to identify novel therapeutic targets. Among these targets p38 MAPK isoforms are becoming increasingly important for a variety of cellular functions. The physiological functions of p38α and -β are now well documented in contrast to -γ and -δ which are comparatively under-studied and ill-defined. A major obstacle to deciphering the role(s) of the latter two p38 isoforms is the lack of specific chemical activators and inhibitors. In this study, we analysed p38 MAPK isoform expression in oesophageal cancer cell lines as well as human normal and tumour tissue. We observed specifically differential p38δ expression. The role(s) of p38δ and active (phosphorylated) p38δ (p-p38δ) in oesophageal squamous cell carcinoma (OESCC) was delineated using wild-type p38δ as well as active p-p38δ, generated by fusing p38δ to its upstream activator MKK6b(E) via a decapeptide (Gly-Glu)5 linker. OESCC cell lines which are p38δ-negative (KE-3 and -8) grew more quickly than cell lines (KE-6 and -10) which express endogenous p38δ. Re-introduction of p38δ resulted in a time-dependent decrease in OESCC cell proliferation which was exacerbated with p-p38δ. In addition, we observed that p38δ and p-p38δ negatively regulated OESCC cell migration in vitro. Finally both p38δ and p-p38δ altered OESCC anchorage-independent growth. Our results suggest that p38δ and p-p38δ have a role in the suppression of OESCC. Our research may provide a new potential target for the treatment of oesophageal cancer.

Es necesario reflexionar que al decir "nuevo", no implica pensar "mejor". No hablamos de un sujeto social que necesariamente o por "imperio del devenir histórico" ejerce su ciudadanía. No es así, hablamos del sujeto a descubrir, desmembrado de la red social que le dio origen, atravesado por múltiples variables que conforman una batería opuesta a las variables propias del Desarrollo humano. Nos interrogamos acerca del "nuevo sujeto social de las pobrezas". Y nos interesa observar cómo opera la idea madre de Desarrollo Humano en consonancia con la diversidad cultural, étnica y las crisis de alteridad. Para ello tenemos en cuenta que la pobreza no tiene la misma representación social para todas las culturas y étnias. El abordaje que proponemos se realiza desde un concepto pedagógico dialéctico y dialógico que opera con los procesos alfabetizatorios múltiples, entendidos como aquellos procesos de enseñanza-aprendizaje que en su dimensión social se manifiestan en el plano material y simbólico (discursivo) como estrategias articuladoras para el Desarrollo Humano. Esta reflexión se enmarca en el taller "Las Alfabetizaciones: ¿Estrategias para el desarrollo humano?" realizado durante el transcurso de este año. La mirada y la acción propuesta desde la doble hermenéutica atraviesan tres instancias: 1- La propia del aprendizaje del taller, plasmada en la interacción docente-alumno. 2- La propia del curso en el abordaje específico de las temáticas planteadas en la reflexión metodológica y sociológica. 3- La experiencia pedagógica y, a través de ésta, la propuesta de investigación diseñada por los cursantes

El presente trabajo caracteriza el contexto social en el cual surge la teoría del capital humano según la elaboración desarrollada por los especialistas en economía a lo largo de la década de 1950. Presenta sus rasgos salientes y los compara con las nuevas versiones plasmadas en los últimos quince años. Confronta los conceptos de educación de ambos períodos (1950 y 2000), el entramado de ideas que justifican la realización de inversiones educativas y el papel de la sociedad y el Estado en la configuración de las demandas y en la promoción de la educación. Indaga, además, los efectos que los cultores de ambas versiones le asignan al capital humano en lo relativo a la conformación de un tipo específico de sociedad y de individuo. Haciendo hincapié en la educación superior y, dentro de ella, en la universidad, se analiza su papel en cuanto productora de conocimientos y transmisora de capacidades de empleabilidad. Asimismo, se consideran las similitudes y diferencias entre ambos enfoques en lo concerniente a los cambios acaecidos en el mundo del trabajo en los dos períodos, y se presentan los fundamentos que dan forma a modelos similares de solución para afrontar los problemas de la pobreza y la exclusión social
