The importance of thrombosis and anticoagulation in clinical practice is rooted firmly in several fundamental constructs that can be applied both broadly and globally. Awareness and the appropriate use of anticoagulant therapy remain the keys to prevention and treatment. However, to assure maximal efficacy and safety, the clinician must, according to the available evidence, choose the right drug, at the right dose, for the right patient, under the right indication, and for the right duration of time. The first International Symposium of Thrombosis and Anticoagulation in Internal Medicine was a scientific program developed by clinicians for clinicians. The primary objective of the meeting was to educate, motivate and inspire internists, cardiologists and hematologists by convening national and international visionaries, thought-leaders and dedicated clinician-scientists in Sao Paulo, Brazil. EL Presidente de Chile, Excelentísimo Señor Eduardo Frei, en carta dirigida el 6 del presente a los señores Raúl Prebisch, Director General del Instituto Latinoamericano de Planificación Económica y Social, José Antonio Mayobre, Secretario Ejecutivo de la CEPAL, Felipe Herrera, Presidente del Banco Interamericano de Desarrollo, y Carlos Sanz de Santa María, Presidente del Comité de los Nueve, hace un serio llamamiento con el fin de que los economistas mencionados, haciendo uso de su calidad de personas imparciales, sin implicancias de orden político y nacional, presenten propuestas concretas a los gobiernos latinoamericanos, que permitan examinar los problemas económicos sin dificultad y suspicacia, en beneficio del desarrollo económico y social de los pueblos de toda la región.

The Brazilian network for genotyping is composed of 21 laboratories that perform and analyze genotyping tests for all HIV-infected patients within the public system, performing approximately 25,000 tests per year. We assessed the interlaboratory and intralaboratory reproducibility of genotyping systems by creating and implementing a local external quality control evaluation. Plasma samples from HIV-1-infected individuals (with low and intermediate viral loads) or RNA viral constructs with specific mutations were used. This evaluation included analyses of sensitivity and specificity of the tests based on qualitative and quantitative criteria, which scored laboratory performance on a 100-point system. Five evaluations were performed from 2003 to 2008, with 64% of laboratories scoring over 80 points in 2003, 81% doing so in 2005, 56% in 2006, 91% in 2007, and 90% in 2008 (Kruskal-Wallis, p = 0.003). Increased performance was aided by retraining laboratories that had specific deficiencies. The results emphasize the importance of investing in laboratory training and interpretation of DNA sequencing results, especially in developing countries where public (or scarce) resources are used to manage the AIDS epidemic.
