Neste trabalho estudamos as alterações histopatológicas encontradas no sistema nervoso central de dois pacientes com "panencefalite subaguda esclerosante" comparando-as com as modificações estruturais determinadas no sistema nervoso central de sete macacos rhesus nos quais este material foi inoculado. Os animais apresentaram sinais de comprometimento neurológico, traduzido por caquexia e paralisia do trem posterior, após um longo perÃodo de incubação, em torno de 18 meses. Dois animais morreram antes de qualquer manifestação neurológica, de infecção pulmonar intercorrente acidental. Nas passagens sucessivas houve um encurtamento do periódo de incubação para cerca de 40 dias. As alterações histopatológicas encontradas, consistiram, nos casos humanos, em leptomeningite focal, focos de neuronofagia, granulomas corticais e nos núcleos basais, grande perda da população neuronal com ocasional estado esponjoso do córtice cerebral, infiltrados perivasculares, e gliose da substância branca, sem perda de mielina. No material experimental foram observadas estas mesmas modificações, se bem que de caráter muito menos intenso. Tanto no material humano como no experimental a mielina estava praticamente normal. Sugere-se que o quadro anátomo-clÃnico chamado "panencefalite subaguda esclerosante (SSPE) possa ser determinado, não apenas pelo vÃrus do sarampo, mas também por outros vÃrus, especialmente os do grupo papova, já encontrado por outros autores, em casos de "panencefalite subaguda esclerosante".

Microtriatoma pratai sp. n. de HemÃptera, Reduviidae, Triatominae, Bolboderini, é descrita no presente trabalho com base em um exemplar coletado em domicÃlio da cidade do Salvador, Estado da Bahia, Brasil. A espécie se distingue facilmente das outras conhecidas do gênero, principalmente por apresentar asas sem manchas e conexivo com largas manchas claras e escuras alternadas.

The morphological sequence of Trypanosoma rangeli development in the alimentary canal of Rhodnius prolixus, is described, with observation made in dissected guts from 6 hours to 45 days post-infection. No metacyclic-forms are produced in the digestive tract at any time, and transmission by the contaminative route must be considered atypical. Amastigotes appear to be an essential stage in the development of T. rangeli in the gut of R. prolixus. The epidemiological importance of the developmental pattern of T. rangeli in the vector´s gut is discussed, and its usefulness for aging infection is considered.

Trypanosoma cruzi infected Rhodnius robustus and/or Rhodnius pictipes were commonly found, in large numbers, in the Brazilian Amazonian palms Maximiliana regia ("inajá"), Acrocomia sclerocarpa ("mucajá") and Orbignya speciosa ("babaçu"). The common opossum, Didelphis marsupialis, was the animal most frequently associated with triatomine infested palms. R. pictipes, frequently light-attracted into houses from palm trees, was the probable source of an acute case of Chagas' disease in the vicinity of Belém. It is considered that triatomine infested palms are likely to cause some cases of acute Chagas' disease in the States of Amazonas and Rondônia. Possible control methods are suggested.

Apresentamos os resultados de observação sobre o ciclo circadiano de atividade hematofágica dos mosquitos, em Granja Calábria, Jacarepaguá, na planÃcie litorânea do Rio de Janeiro, onde realizamos, em isca humana, ao ar livre, capturas semanais, de 8 à s 10, de 13 à s 15 e 18 à s 20 horas, de agosto de 1981 a julho de 1982, além de três capturas horárias de 24 horas seguidas. A maioria das espécies locais revelou caráter crepuscular vespertino e noturno. Contudo Limatus durhami, Phoniomyia davisi, Wyeomyia leucostigma e Wyeomyia (Dendromyia) sp. foram essencialmente diurnas, enquanto Anopheles albitarsis, Culex chidesteri e Culex quinquefasciatus foram obtidas somente no crepúsculo vespertino e à noite. Embora Anopheles aquasalis, Culex coronator, Culex saltanensis, Culex crybda e Coquillettidia venezuelensis fossem preponderantemente noturnas e Phoniomyia deanei e Phoniomyia theobaldi principalmente diurnas, obtivemô-las algumas vezes, fora do horário preferencial, sendo que Phoniomyia deanei teve nÃtido incremento pré-crepuscular vespertino. Aedes scapularis, Aedes taeniorhynchus e Mansonia titillans, espécies mais ecléticas, picaram durante todo o nictêmero, mas com flagrante acentuação crepuscular vespertina.

T lymphocytes reactive with the product of the Mlsa-allele of the minor lymphocyte stimulating (Mls) locus use a predominant T-cell receptor beta-chain variable gene segment (V beta 6). Such V beta 6-bearing T cells are selectively eliminated in the thymus of Mlsa-bearing mice, consistent with a model in which tolerance to self antigens is achieved by clonal deletion.

BACKGROUND: The WOSI (Western Ontario Shoulder Instability Index) is a self-administered quality of life questionnaire designed to be used as a primary outcome measure in clinical trials on shoulder instability, as well as to measure the effect of an intervention on any particular patient. It is validated and is reliable and sensitive. As it is designed to measure subjective outcome, it is important that translation should be methodologically rigorous, as it is subject to both linguistic and cultural interpretation. OBJECTIVE: To produce a French language version of the WOSI that is culturally adapted to both European and North American French-speaking populations. MATERIALS AND METHODS: A validated protocol was used to create a French language WOSI questionnaire (WOSI-Fr) that would be culturally acceptable for both European and North American French-speaking populations. Reliability and responsiveness analyses were carried out, and the WOSI-Fr was compared to the F-QuickDASH-D/S (Disability of the Arm, Shoulder and Hand-French translation), and Walch-Duplay scores. RESULTS: A French language version of the WOSI (WOSI-Fr) was accepted by a multinational committee. The WOSI-Fr was then validated using a total of 144 native French-speaking subjects from Canada and Switzerland. Comparison of results on two WOSI-Fr questionnaires completed at a mean interval of 16 days showed that the WOSI-Fr had strong reliability, with a Pearson and interclass correlation of r=0.85 (P=0.01) and ICC=0.84 [95% CI=0.78-0.88]. Responsiveness, at a mean 378.9 days after surgical intervention, showed strong correlation with that of the F-QuickDASH-D/S, with r=0.67 (P<0.01). Moreover, a standardized response means analysis to calculate effect size for both the WOSI-Fr and the F-QuickDASH-D/S showed that the WOSI-Fr had a significantly greater ability to detect change (SRM 1.55 versus 0.87 for the WOSI-Fr and F-QuickDASH-D/S respectively, P<0.01). The WOSI-Fr showed fair correlation with the Walch-Duplay. DISCUSSION: A French-language translation of the WOSI questionnaire was created and validated for use in both Canadian and Swiss French-speaking populations. This questionnaire will facilitate outcome assessment in French-speaking settings, collaboration in multinational studies and comparison between studies performed in different countries. Realizamos capturas simultâneas de flebótomos, no Parque Nacional da Serra dos Orgãos, Estado do Rio de Janeiro, utilizando três iscas: homem, gambá e galo. Em 298h capturamos 1.155 fêmeas de seis espécies do gênero Lutzomya. L. ayrozai e L. hirsuta foram as espécies mais numerosas; ambas sugaram somente próximo ao solo, sendo decididamente antropofÃlicas e mais ativas entre 17 e 24h. L. fischeri foi a espécie mais freqüente na copa e a que demonstrou maior ecletismo quanto ao hospedeiro, hora e local; na copa sugou mais o galo, especialmente entre 0 e 5h e, no solo, picou com maior intensidade o homem, principalmente entre 20 e 24h.

Les auteurs décrivent le mâle et la femelle d'une nouvelle espèce de phlébotome Lutzomyia brisolai n.sp., anthropophile, rencontrée dans plusieurs stations du piémont andin. Cette espèce, difficile à classer, présente des caractères l'apparentant à L. oliveirai et L. minasensis mais aussi aux espèces du sous-genre Nyssomyia.

Coelhos com infecções maciças (20.000 cercárias) pelo Schistosoma mansoni desenvolvendo dentro de três a dez meses intensas e peculiares lesões no sistema porta intrahepático; estas consisten en endoflebite poliposa e endoflebite granulomatosa oclusiva, que evoluem para a cicatrização, com hialinização dos polipos endoteliais e com ectasia vascular, ou com trombose, organização e recanalização. Nos perÃodos tardios, estas lesões se acompanham de fibrose periportal, septal e de espessamento da trama reticular intra-parenquimal. Embora podendo ser bem intensas, tais lesões têm um caráter focal, pois se relacionam com grupos de vermes alojados em alguns segmentos da veia porta, não determinam hipertensão porta, nem têm semelhanças com as lesões da esquistossomose humana. Os granulomas periovulares quase não aparecem no fÃgado, mas se formam bem nos intestinos, especialmetne nos dois ou três primeiros meses após a infecção. A patologia da esquistossomose no coelho tem, portanto, aspectos peculiares, os quais merecem ser bem conhecidos, uma vez que este modelo pode se revelar de interesse para estudantes dos imunológicos e imunopatológicos.

OBJECTIVE: To investigate the planning of subgroup analyses in protocols of randomised controlled trials and the agreement with corresponding full journal publications. DESIGN: Cohort of protocols of randomised controlled trial and subsequent full journal publications. SETTING: Six research ethics committees in Switzerland, Germany, and Canada. DATA SOURCES: 894 protocols of randomised controlled trial involving patients approved by participating research ethics committees between 2000 and 2003 and 515 subsequent full journal publications. RESULTS: Of 894 protocols of randomised controlled trials, 252 (28.2%) included one or more planned subgroup analyses. Of those, 17 (6.7%) provided a clear hypothesis for at least one subgroup analysis, 10 (4.0%) anticipated the direction of a subgroup effect, and 87 (34.5%) planned a statistical test for interaction. Industry sponsored trials more often planned subgroup analyses compared with investigator sponsored trials (195/551 (35.4%) v 57/343 (16.6%), P<0.001). Of 515 identified journal publications, 246 (47.8%) reported at least one subgroup analysis. In 81 (32.9%) of the 246 publications reporting subgroup analyses, authors stated that subgroup analyses were prespecified, but this was not supported by 28 (34.6%) corresponding protocols. In 86 publications, authors claimed a subgroup effect, but only 36 (41.9%) corresponding protocols reported a planned subgroup analysis. CONCLUSIONS: Subgroup analyses are insufficiently described in the protocols of randomised controlled trials submitted to research ethics committees, and investigators rarely specify the anticipated direction of subgroup effects. More than one third of statements in publications of randomised controlled trials about subgroup prespecification had no documentation in the corresponding protocols. Definitive judgments regarding credibility of claimed subgroup effects are not possible without access to protocols and analysis plans of randomised controlled trials.

En aquesta jornada de l’arròs, un any més, es donaran a conèixer els resultats més rellevants dels assajos realitzats durant la darrera campanya. Part dels treballs estan consolidats des de fa força temps, com serien els assajos de varietats i el control de malalties; mentre que d’altres resulten més novedosos, com seria el control biològic del chilo o l’aplicació de purins a l’arròs; d’altres ja porten algunes campanyes i serà força interessant fer balanç d’aquests treballs. Per finalitzar es farà una important reflexió sobre la situació actual dels fitosanitaris en el cultiu de l’arròs. És important que els assistents puguin debatre les diferents temà tiques abordades i, per tant, enriquir el contingut d’aquesta jornada. Aquesta jornada tècnica és fruit de l’esforç i la coordinació de tots els tècnics i institucions que es dediquen al conreu de l’arròs. Creiem que el contingut de la jornada els serà força interessant i assolirà les seves expectatives.

Background: Voltage-gated sodium channels (Nav1.x) are important players in chronic pain. A particular interest has grown in Nav1.7, expressed in nociceptors, since mutations in its gene are associated to two inherited pain syndromes or insensitivity to pain. Rufinamide, a drug used to treat refractory epilepsy such as the Lennox-Gastaut syndrome, has been shown to reduce the number of action potentials in cortical neurons without completely blocking Na channels. Aim: The goal of this study was to investigate the effect of rufinamide on Nav1.7 current. Methods and results: Whole-cell patch clamp experiments were performed using HEK293 cells stably expressing Nav1.7. Rufinamide significantly decreased peak sodium current by 28.3, 21.2 and 12.5% at concentrations of 500, 100 and 50μM respectively (precise EC50 could not be calculated since higher rufinamide concentrations could not be achieved in physiological buffer solution). No significant difference on the V1/2 of voltage-dependence of activation was seen; however a shift in the steady-state inactivation curve was observed (-82.6 mV to -88.8 mV and -81.8 to -87.6 mV for 50 and 100 μM rufinamide respectively, p <0.005). Frequency-dependent inhibition of Nav1.7 was also influenced by the drug. One hundred μM rufinamide reduced the peak sodium current (in % of the peak current taken at the first sweep of a train of 50) from 90.8 to 80.8% (5Hz), 88.7 to 71.8% (10 Hz), 69.1 to 49.2% (25 Hz) and 22.3 to 9.8% (50 Hz) (all p <0.05). Onset of fast inactivation was not influenced by the drug since no difference in the time constant of current decay was observed. Conclusion: In the concentration range of plasma level in human treated for epilepsy, 15 μM, rufinamide only minimally blocks Nav1.7. However, it stabilizes the inactivated state and exerts frequencydependent inhibition of Nav1.7. These pharmacological properties may be of use in reducing ectopic discharges as a causal and symptom related contributor of neuropathic pain syndrome.
