The Mississippi Valley-type (MVT) Pb-Zn ore district at Mezica is hosted by Middle to Upper Triassic platform carbonate rocks in the Northern Karavanke/Drau Range geotectonic units of the Eastern Alps, northeastern Slovenia. The mineralization at Mezica covers an area of 64 km(2) with more than 350 orebodies and numerous galena and sphalerite occurrences, which formed epigenetically, both conformable and discordant to bedding. While knowledge on the style of mineralization has grown considerably, the origin of discordant mineralization is still debated. Sulfur stable isotope analyses of 149 sulfide samples from the different types of orebodies provide new insights on the genesis of these mineralizations and their relationship. Over the whole mining district, sphalerite and galena have delta(34)S values in the range of -24.7 to -1.5% VCDT (-13.5 +/- 5.0%) and -24.7 to -1.4% (-10.7 +/- 5.9%), respectively. These values are in the range of the main MVT deposits of the Drau Range. All sulfide delta(34)S values are negative within a broad range, with delta(34)S(pyrite) < delta(34)S(sphalerite) < delta(34)S(galena) for both conformable and discordant orebodies, indicating isotopically heterogeneous H(2)S in the ore-forming fluids and precipitation of the sulfides at thermodynamic disequilibrium. This clearly supports that the main sulfide sulfur originates from bacterially mediated reduction (BSR) of Middle to Upper Triassic seawater sulfate or evaporite sulfate. Thermochemical sulfate reduction (TSR) by organic compounds contributed a minor amount of (34)S-enriched H(2)S to the ore fluid. The variations of delta(34)S values of galena and coarse-grained sphalerite at orefield scale are generally larger than the differences observed in single hand specimens. The progressively more negative delta(34)S values with time along the different sphalerite generations are consistent with mixing of different H(2)S sources, with a decreasing contribution of H(2)S from regional TSR, and an increase from a local H(2)S reservoir produced by BSR (i.e., sedimentary biogenic pyrite, organo-sulfur compounds). Galena in discordant ore (-11.9 to -1.7%; -7.0 +/- 2.7%, n=12) tends to be depleted in (34)S compared with conformable ore (-24.7 to -2.8%, -11.7 +/- 6.2%, n=39). A similar trend is observed from fine-crystalline sphalerite I to coarse open-space filling sphalerite II. Some variation of the sulfide delta(34)S values is attributed to the inherent variability of bacterial sulfate reduction, including metabolic recycling in a locally partially closed system and contribution of H(2)S from hydrolysis of biogenic pyrite and thermal cracking of organo-sulfur compounds. The results suggest that the conformable orebodies originated by mixing of hydrothermal saline metal-rich fluid with H(2)S-rich pore waters during late burial diagenesis, while the discordant orebodies formed by mobilization of the earlier conformable mineralization.

Citalopram, a new bicyclic antidepressant, is the most selective serotonin reuptake inhibitor. In a number of double-blind controlled studies, citalopram was compared to placebo and to known tricyclic antidepressants. These studies have shown their efficacy and good safety. The inefficacy of a psychotropic treatment in at least 20% of depressives has led a number of authors to propose original drug combinations and associations, like antidepressant/lithium (Li), antidepressant/sleep deprivation (agrypnia), antidepressant/ECT, or antidepressant/LT3. The aim of this investigation is to evaluate the clinical effectiveness and safety of a combined citalopram/lithium treatment in therapy-resistant patients, taking account of serotonergic functions, as tested by the fenfluramine/prolactin test, and of drug pharmacokinetics and pharmacogenetics of metabolism. DESIGN OF THE STUDY: A washout period of 3 days before initiating the treatment is included. After an open treatment phase of 28 days (D) with citalopram (20 mg D1-D3; 40 mg D4-D14; 40 or 60 mg D15-D28; concomitant medication allowed: chloral, chlorazepate), the nonresponding patients [less than 50% improvement in the total score on the 21 item-Hamilton Depression Rating Scale (HDRS)] are selected and treated with or without Li (randomized in double-blind conditions: citalopram/Li or citalopram/placebo) during the treatment (D29-D35). Thereafter, all patients included in the double-blind phase subsequently receive an open treatment with citalopram/Li for 7 days (D36-D42). The hypothesis of a relationship between serotoninergic functions in patients using the fenfluramine/prolactin test (D1) and the clinical response to citalopram (and Li) is assessed. Moreover, it is evaluated whether the pharmacogenetic status of the patients, as determined by the mephenytoin/dextromethorphan test (D0-D28), is related to the metabolism of fenfluramine and citalopram, and also to the clinical response. CLINICAL ASSESSMENT: Patients with a diagnosis of major depressive disorders according to DSM III are submitted to a clinical assessment of D1, D7, D14, D28, D35, D42: HDRS, CGI (clinical global impression), VAS (visual analog scales for self-rating of depression), HDRS (Hamilton depression rating scale, 21 items), UKU (side effects scale), and to clinical laboratory examens, as well as ECG, control of weight, pulse, blood pressure at D1, D28, D35. Fenfluramine/prolactin test: A butterfly needle is inserted in a forearm vein at 7 h 45 and is kept patent with liquemine. Samples for plasma prolactin, and d- and l-fenfluramine determinations are drawn at 8 h 15 (base line). Patients are given 60 mg fenfluramine (as a racemate) at 8 h 30. Kinetic points are determined at 9 h 30, 10 h 30, 11 h 30, 12 h 30, 13 h 30. Plasma levels of d- and l-fenfluramine are determined by gas chromatography and prolactin by IRNA. Mephenytoin/dextromethorphan test: Patients empty their bladders before the test; they are then given 25 mg dextropethorphan and 100 mg mephenytoin (as a racemate) at 8 h 00. They collect all urines during the following 8 hours. The metabolic ratio is determined by gas chromatography (metabolic ratio dextromethorphan/dextrorphan greater than 0.3 = PM (poor metabolizer); mephenytoin/4-OH-mephenytoin greater than 5.6, or mephenytoin S/R greater than 0.8 = PM). Citalopram plasma levels: Plasma levels of citalopram, desmethylcitalopram and didesmethylcitalopram are determined by gas chromatography--mass spectrometry. RESULTS OF THE PILOT STUDY. The investigation has been preceded by a pilot study including 14 patients, using the abovementioned protocol, except that all nonresponders were medicated with citalopram/Li on D28 to D42. The mean total score (n = 14) on the 21 item Hamilton scale was significantly reduced after the treatment, ie from 26.93 +/- 5.80 on D1 to 8.57 +/- 6.90 on D35 (p less than 0.001). Velocity-density tests conducted in the laboratory involved small 4-inch diameter by 4.58-inch-long compacted soil cylinders made up of 3 differing soil types and for varying degrees of density and moisture content, the latter being varied well beyond optimum moisture values. Seventeen specimens were tested, 9 with velocity determinations made along two elements of the cylinder, 180 degrees apart, and 8 along three elements, 120 degrees apart. Seismic energy was developed by blows of a small tack hammer on a 5/8-inch diameter steel ball placed at the center of the top of the cylinder, with the detector placed successively at four points spaced 1/2-inch apart on the side of the specimen involving wave travel paths varying from 3.36 inches to 4.66 inches in length. Time intervals were measured using a model 217 micro-seismic timer in both laboratory and field measurements. Forty blows of the hammer were required for each velocity determination, which amounted to 80 blows on 9 laboratory specimens and 120 blows on the remaining 8 cylinders. Thirty-five field tests were made over the three selected soil types, all fine-grained, using a 2-foot seismic line with hammer-impact points at 6-inch intervals. The small tack hammer and 5/8-inch steel ball was, again, used to develop seismic wave energy. Generally, the densities obtained from the velocity measurements were lower than those measured in the conventional field testing. Conclusions were reached that: (1) the method does not appear to be usable for measurement of density of essentially fine-grained soils when the moisture content greatly exceeds the optimum for compaction, and (2) due to a gradual reduction in velocity upon aging, apparently because of gradual absorption of pore water into the expandable interlayer region of the clay, the seismic test should be conducted immediately after soil compaction to obtain a meaningful velocity value.

This study aimed to compare O2 consumption (VO2) determination by the gas-exchange (VO2GE) and Fick (VO2F) methods in cardiac surgical patients. A total of 10 mechanically ventilated postoperative patients were studied prospectively. Thermodilution was performed using three randomly applied techniques: room temperature saline injected at end expiration, room temperature saline randomly injected in the respiratory cycle, and iced saline injected at end expiration. The influence of the number of thermodilution determinations was assessed by comparing results from 2 and 10 injections. The variability of VO2F was greater than that of VO2GE. There was no bias between VO2GE and VO2F values using injectate at room temperature. Accuracy and precision were not improved by increasing the number of cardiac output determinations from 2 to 10. A significant bias was observed using ice-cold injectate, VO2F being 18.0 +/- 15.4 ml/min/m2 lower than VO2GE (p = 0.001). Published results when comparing VO2F and VO2GE are discrepant. However, a significant bias was found in all studies using cold injectate, with lower VO2F values. We conclude that iced injectate should not be used to assess VO2 in critically ill patients.

BACKGROUND & AIMS: The prognostic value of the different causes of renal failure in cirrhosis is not well established. This study investigated the predictive value of the cause of renal failure in cirrhosis. METHODS: Five hundred sixty-two consecutive patients with cirrhosis and renal failure (as defined by serum creatinine 1.5 mg/dL on 2 successive determinations within 48 hours) hospitalized over a 6-year period in a single institution were included in a prospective study. The cause of renal failure was classified into 4 groups: renal failure associated with bacterial infections, renal failure associated with volume depletion, hepatorenal syndrome (HRS), and parenchymal nephropathy. The primary end point was survival at 3 months. RESULTS: Four hundred sixty-three patients (82.4%) had renal failure that could be classified in 1 of 4 groups. The most frequent was renal failure associated with infections (213 cases; 46%), followed by hypovolemia-associated renal failure (149; 32%), HRS (60; 13%), and parenchymal nephropathy (41; 9%). The remaining patients had a combination of causes or miscellaneous conditions. Prognosis was markedly different according to cause of renal failure, 3-month probability of survival being 73% for parenchymal nephropathy, 46% for hypovolemia-associated renal failure, 31% for renal failure associated with infections, and 15% for HRS (P .0005). In a multivariate analysis adjusted for potentially confounding variables, cause of renal failure was independently associated with prognosis, together with MELD score, serum sodium, and hepatic encephalopathy at time of diagnosis of renal failure. CONCLUSIONS: A simple classification of patients with cirrhosis according to cause of renal failure is useful in assessment of prognosis and may help in decision making in liver transplantation.

The world-class Idrija mercury deposit (western Slovenia) is hosted by highly deformed Permocarboniferous to Middle Triassic sedimentary rocks within a complex tectonic structure at the transition between the External Dinarides and the Southern Alps. Concordant and discordant mineralization formed concomitant with Middle Triassic bimodal volcanism in an aborted rift. A multiple isotopic (C, O, S) investigation of host rocks and ore minerals was performed to put constraints on the source and composition of the fluid, and the hydrothermal alteration. The distributions of the delta(13)C and delta(18)O values of host and gangue carbonates are indicative of a fracture-controlled hydrothermal system, with locally high fluid-rock ratios. Quantitative modeling of the delta(13)C and delta(18)O covariation for host carbonates during temperature dependent fluid-rock interaction, and concomitant precipitation of void-filling dolomites points to a slightly acidic hydrothermal fluid (delta(13)Capproximate to-4parts per thousand and delta(18)Oapproximate to+10parts per thousand), which most likely evolved during isotopic exchange with carbonates under low fluid/rock ratios. The delta(34)S values of hydrothermal and sedimentary sulfur minerals were used to re-evaluate the previously proposed magmatic and evaporitic sulfur sources for the mineralization, and to assess the importance of other possible sulfur sources such as the contemporaneous seawater sulfate, sedimentary pyrite, and organic sulfur compounds. The delta(34)S values of the sulfides show a large variation at deposit down to hand-specimen scale. They range for cinnabar and pyrite from -19.1 to +22.8parts per thousand, and from -22.4 to +59.6parts per thousand, respectively, suggesting mixing of sulfur from different sources. The peak of delta(34)S values of cinnabar and pyrite close to 0parts per thousand is compatible with ore sulfur derived dominantly from a magmatic fluid and/or from hydrothermal leaching of basement rocks. The similar stratigraphic trends of the delta(34)S values of both cinnabar and pyrite suggest a minor contribution of sedimentary sulfur (pyrite and organic sulfur) to the ore formation. Some of the positive delta(34)S values are probably derived from thermochemical reduction of evaporitic and contemporaneous seawater sulfates.

The Jalta and Jebel Ghozlane ore deposits are located in the extreme North of Tunisia, within the Nappe zone. The mineralization of Jalta, hosted in Triassic dolostones and the overlying Mio-Pliocene conglomerates, consists of abundant galena, barite, and cerussite with accessory sphalerite, pyrite, and jordanite. At Jebel Ghozlane, large Pb-Zn concentrations occur in the Triassic dolostones and Eocene limestones. The mineral association consists of galena, sphalerite, barite, and celestite and their oxidation products (cerussite, smithsonite, and anglesite). Lead isotope ratios in galena from both districts are relatively homogeneous ((206)Pb/(204)Pb = 18.702-18.823, (207)Pb/(204)Pb = 15.665-15.677, (208)Pb/(204)Pb = 38.725-38.875). The delta(34)S values for sulfates from both areas (+12.2 to +16.2 parts per thousand at Jalta and + 14.3 to + 19.4 parts per thousand at Jebel Ghozlane) are compatible with a derivation of sulfur from marine sulfates, possibly sourced from the Triassic evaporites. The delta(34)S values of the sulfides have a range between -10 and +12.5 parts per thousand at Jalta, and between -9.1 and +22.1 parts per thousand at Jebel Ghozlane. The large range of values suggests reduction of the sulfate by bacterial and/or thermochemical reduction of sulfate to sulfur. The high delta(34)S values of sulfides require closed-system reduction processes. The isotopically light carbon in late calcites (-6.3 to -2.5 parts per thousand) and authigenic dolomite (-17.6 parts per thousand) suggests an organic source of at least some of the carbon in these samples, whereas the similarity of the delta(18)O values between calcite (+24.8 parts per thousand) and the authigenic dolomite (+24.7 parts per thousand) of Jalta and their respective host rocks reflects oxygen isotope buffering of the mineralizing fluids by the host rock carbonates. The secondary calcite isotope compositions of Jalta are compatible with a hydrothermal fluid circulation at approximately 100 to 200 degrees C, but temperatures as low as 50 degrees C may be indicated by the late calcite of Jebel Ghozlane (delta(18)O of +35.9 parts per thousand). Given the geological events related to the Alpine orogeny in the Nappe zone (nappe emplacement, bimodal volcanism, and reactivation of major faults, such as Ghardimaou-Cap Serrat) and the Neogene age of the host rocks in several localities, a Late-Miocene age is proposed for the Pb-Zn ore deposits considered in this study. Remobilization of deep-seated primary deposits in the Paleozoic sequence is the most probable source for metals in both localities considered in this study and probably in the Nappe zone as a whole. The avocado fruit borer, Stenoma catenifer (Wals.) has been a limiting factor in growing avocados over the last years in many Brazilian states. This is a result of the lack of safe and feasible management practices to minimize the fruit borer damage. The aim of this study was to obtain information on the pest biology and ecology as well as on the role of natural enemies to define strategies to control the pest. Samples were taken biweekly and consisted of 20 fruits collected randomly (10 from the upper half and 10 from the lower half of the plant) in ten plants, cv. Margarida, in a commercial avocado grove in Arapongas and Cambé regions, PR, from October/2001 to September/2002. Laboratory determinations of the percentage of damaged fruit per plant region, location and number of bored fruit sites, and the number and location of the fruit borer eggs, including parasitized ones, were performed. The results showed that S. catenifer preferred to oviposit and attack fruits located on the upper half of the trees. The majority of the eggs were laid on the fruit pedicel whereas the damage was mainly located on the lower half of the fruits. Trichogrammatids were the most constant and abundant parasitoids found in both localities throughout the study period.

The objectives of this study were to evaluate the residues of the insecticide carbosulfan and its carbofuran metabolites and 3-hydroxy-carbofuran in orange compartments (whole fruit, bagasse and juice) and comparison between the residual levels found in fruits with the maximum residue level and the safety interval established by the Brazilian legislation. Two field experiments were carried out, both with the following treatments: a-check; b-one application of 10 g of carbosulfan . 100 L-1 of water; c-one application with twice the rate applied in treatment b; d-four applications with the same rate applied in treatment b. Samples were taken at (-1), zero, 1, 3, 7, 14, 21 and 28 days after the last or unique application. The quantitative determinations were done by gas chromatography technique, using a nitrogen-phosphorus detector. The carbosulfan metabolism to its carbofuran metabolite was rapid (3 days), being both analytes concentrated in the bagasse (peel + flavedo + albedo). However, the metabolism of carbofuran to 3-hydroxy-carbofuran was of low intensity or this metabolite was quickly dissipated. Carbosulfan residues and its metabolites did not penetrate into the fruit, thus not contaminating the juice. The use of the pesticide was adequate, with respect to fruit consumption, in relation to the Brazilian legislation.

Luokittuminen erilaisine mekanismeineen aiheuttaa yleisesti ongelmia, kun on kysymyksessä kiintoaineen väliaikainenkin varastointi siilossa. Sitä voidaan vähentää kiintoaineiden, prosessin ja laitesuunnittelun muutoksilla. Tässä työssä tutkittiin mahdollisuuksia vähentää ilmeniitin luokittumista sen jauhatuspiirin ilmakiertoa optimoimalla. Suljetun kuivajauhatuspiirin keskeisimmäksi laitteeksi voitaisiin ajatella siinä oleva luokitin, joka voi olla esim. sykloni. Tässä piirissä tapahtuva kiintoaineen liikkuminen voidaan saada aikaiseksi esim. pneumaattisella kuljetuksella. Ilmeniitin jauhatus tapahtuu suljetussa kuivajauhatuspiirissä, jonka ajavana voimana on siinä oleva ilmakierto. Piirin oleellisia laitteita ovat kuulamylly, luokitin, erotussykloni ja pölykaappi sekä kiertoilma- ja poistoilmapuhaltimet. Ilmakierron optimointia varten suoritettiin kahden vastaavan jauhatuspiirin ainetasemääritykset. Lisäksi määritettiin yhden isomman piirin perustila. Jauhatuspiirien ainetasemäärityksissä määritettiin niiden massa- ja ilmavirrat sekä kiertokuorma ja luokittimen erotusterävyys, kuten myös ilmeniitin hiukkaskokojakaumat. Perustilamittauksissa määritettiin ainoastaan piirin ilmavirrat ja ilmeniitin hiukkaskokojakaumat. Optimointimittauksissa pienennettiin pikkumyllypiirin ilmamäärät vastaamaan kutakuinkin vastaavan toisen piirin määriä. Tällä yritettiin selvittää näiden toisiaan vastaavien piirien ilmamäärien ja varsinkin kiertokuormien eroavuutta. Tämä ilmamäärien pienentäminen ei tuottanut mainittavampaa muutosta piirin ainetaseisiin, joten voitaneen todeta, että piirin ilmamääriä pienentämällä saadaan aikaiseksi säästöjä, lähinnä kiertoilmapuhaltimen tehon alennuksen kautta.

BACKGROUND & AIMS: Trace elements (TE) are involved in the immune and antioxidant defences which are of particular importance during critical illness. Determining plasma TE levels is costly. The present quality control study aimed at assessing the economic impact of a computer reminded blood sampling versus a risk guided on-demand monitoring of plasma concentrations of selenium, copper, and zinc. METHODS: Retrospective analysis of 2 cohorts of patients admitted during 6 months periods in 2006 and 2009 to the ICU of a University hospital. INCLUSION CRITERIA: to receive intravenous micronutrient supplements and/or to have a TE sampling during ICU stay. The TE samplings were triggered by computerized reminder in 2006 versus guided by nutritionists in 2009. RESULTS: During the 2 periods 636 patients met the inclusion criteria out of 2406 consecutive admissions, representing 29.7% and 24.9% respectively of the periods' admissions. The 2009 patients had higher SAPS2 scores (p = 0.02) and lower BMI compared to 2006 (p = 0.007). The number of laboratory determinations was drastically reduced in 2009, particularly during the first week, despite the higher severity of the cohort, resulting in à 55% cost reduction. CONCLUSIONS: The monitoring of TE concentrations guided by a nutritionist resulted in a reduction of the sampling frequency, and targeting on the sickest high risk patients, requiring a nutritional prescription adaptation. This control leads to cost reduction compared to an automated sampling prescription.

Diplomityön tarkoituksena on tarkastella isobuteenin dimeroitumisprosessin neste-nestetasapainoja. Tavoitteena on määrittää prosessissa esiintyvien komponenttien väliset neste-nestetasapainot. Työn kirjallisuusosassa on tarkasteltu neste-nestetasapainojen teoriaa. Erityisesti on tarkasteltu mittausmenetelmiä sekä kirjallisuudesta löytyneitä laitteistoja binääristen ja ternääristen systeemien neste-nestetasapainojen määritystä varten. Menetelmät ja laitteistot on esitetty erikseen matalassa ja korkeassa paineessa suoritettaville mittauksille. Lisäksi on tarkasteltu näytteenottoa sekä näytteiden analysointimenetelmiä. Kirjallisuusosassa on myös sivuttu kaasu-neste-nestetasapainojen määritystä, mutta työn varsinainen kohde on neste-nestetasapainojen määritys. Työn kokeellisessa osassa määritettiin iso-oktaaniprosessissa esiintyvien komponenttien välisiä binäärisiä ja ternäärisiä neste-nestetasapainoja. Mitattavien komponenttiparien määrää karsittiin ja jäljellejääneiden parien välillä suoritettavat mittaukset jaoteltiin matalassa ja korkeassa paineessa suoritettaviin määrityksiin. Ternääriset mittaukset tulivat kyseeseen sellaisten komponenttiparien kohdalla, joissa toisiinsa täysin liukenevien nesteiden systeemiin kolmatta komponenttia lisättäessä saatiin aikaiseksi kaksi nestefaasia. Tällaisesta mittaustiedosta voidaan määrittää neste-nestetasapainomallien parametrejä. Mittausten lisäksi kokeellisessa osassa tarkasteltiin näytteenottoa sekä näytteiden analysointia.

Työn tavoitteena oli tutkia sellumassan suotautumiseen liittyviä tekijöitä Ahlstrom Machineryn Drum Displacer™ -puumassapesurissa (DD-pesuri). Teoriaosassa tarkasteltiin aluksi suotautumisen teoriaa kuitu-vesi-suspensiossa, minkä jälkeen esiteltiin suotautumisnopeuteen vaikuttavia fysikaalisia ja kemiallisia vaikutusmekanismeja. Seuraavaksi kuvattiin massan pesun yleisiä perusteita sekä teoriaa puumassapesureissa. Lopuksi tarkasteltiin pesurien kytkeytymistä muuhun kuitulinjaan sekä prosessista johtuvia pesun toiminnan ulkoisia häiriötekijöitä. Kokeellisen osan aluksi tarkasteltiin paine- ja lämpötilamittauksien avulla massapesurissa vallitsevia prosessioloja. Mittaustulosten perusteella pumppausolot pesurin suodoslinjoissa ovat vaikeahkot ja häiriötilanteita voi esiintyä, mutta käytäntö on osoittanut tästä olevan vain harvoin haittaa prosessin toiminnalle. Pesureissa toteutuneet syrjäytysnopeudet laskettiin ja niitä verrattiin syrjäytystestien antamiin tuloksiin. Kuitulinjasta riippuen testin vastaavuus tehdasprosessiin vaihteli suuresti. Syrjäytystesteillä kokeiltiin myös tehdasprosesseissa usein esiintyvien muuttujien vaikutusta sellukakun syrjäytettävyyteen. Kakun paksuus ja syrjäytyslämpötila vaikuttivat syrjäytysnopeuteen Darcyn lain mukaisesti. Alipaineen massakakun alapuolella havaittiin huonontavan syrjäytysnopeutta verrattuna tilanteeseen, jossa kakun alla vallitsi ilmanpaine. Tämä havainto on selvästi ristiriidassa suotautumisen teorian kanssa. Massakakun muodostumis-pH osoittautui ratkaisevaksi lopulliselle syrjäytysnopeudelle, sillä alkalisissa oloissa muodostetun kuitukakun syrjäytysnopeus ei enää parantunut happamalla syrjäytysnesteellä. Happamissa oloissa muodostetun kakun syrjäytysnopeus oli alkalisista parempi, mutta se alkoi hitaasti alentua, kun syrjäytysneste vaihtui alkaliseen. Massan laimentaminen ennen syrjäytystä alkalisella tehdassuodoksella puhtaan veden sijasta alensi ligniinipitoisella massalla lopullista syrjäytysnopeutta. Shirato-Tillerin mallilla ja Jönssonin staattisella mallilla simuloitiin numeerisesti syrjäytystestiä kahdessa eri pH:ssa, ja simulointituloksia verrattiin vastaavissa oloissa tehtyihin syrjäytystesteihin. Shirato-Tillerin mallin antamien syrjäytysnopeuksien havaittiin olevan lähellä syrjäytystestien nopeuksia, kun Jönssonin mallin antamat tulokset jäivät huomattavasti testituloksia alemmiksi. Herkkyystarkastelussa havaittiin mallien olevan varsin herkkiä parametrien virheille. Hajonta vaadittavien kuituparametrien määrityksissä ja menetelmien työläys rajoittavat numeerisen simuloinnin käytettävyyttä, sillä kuituparametrien määrityksen vaatima työmäärä on ainakin toistaiseksi syrjäytystestiä suurempi. Lopuksi todettiin, että oikeiden syrjäytysolosuhteiden käyttö on ensiarvoisen tärkeää oikeiden tulosten saamiseksi sekä kokeellisessa että numeerisessa simuloinnissa. Nykyinen syrjäytystestilaitteisto on pienin muutoksin käyttökelpoinen, kun massan testaus prosessioloissa tulee rutiininomaiseksi.

PURPOSE: Adequate empirical antibiotic dose selection for critically ill burn patients is difficult due to extreme variability in drug pharmacokinetics. Therapeutic drug monitoring (TDM) may aid antibiotic prescription and implementation of initial empirical antimicrobial dosage recommendations. This study evaluated how gradual TDM introduction altered empirical dosages of meropenem and imipenem/cilastatin in our burn ICU. METHODS: Imipenem/cilastatin and meropenem use and daily empirical dosage at a five-bed burn ICU were analyzed retrospectively. Data for all burn admissions between 2001 and 2011 were extracted from the hospital's computerized information system. For each patient receiving a carbapenem, episodes of infection were reviewed and scored according to predefined criteria. Carbapenem trough serum levels were characterized. Prior to May 2007, TDM was available only by special request. Real-time carbapenem TDM was introduced in June 2007; it was initially available weekly and has been available 4 days a week since 2010. RESULTS: Of 365 patients, 229 (63%) received antibiotics (109 received carbapenems). Of 23 TDM determinations for imipenem/cilastatin, none exceeded the predefined upper limit and 11 (47.8%) were insufficient; the number of TDM requests was correlated with daily dose (r=0.7). Similar numbers of inappropriate meropenem trough levels (30.4%) were below and above the upper limit. Real-time TDM introduction increased the empirical dose of imipenem/cilastatin, but not meropenem. CONCLUSIONS: Real-time carbapenem TDM availability significantly altered the empirical daily dosage of imipenem/cilastatin at our burn ICU. Further studies are needed to evaluate the individual impact of TDM-based antibiotic adjustment on infection outcomes in these patients.

We describe a relation between the symmetry energy coefficients csym(ρ) of nuclear matter and asym(A) of finite nuclei that accommodates other correlations of nuclear properties with the low-density behavior of csym(ρ). Here, we take advantage of this relation to explore the prospects for constraining csym(ρ) of systematic measurements of neutron skin sizes across the mass table, using as example present data from antiprotonic atoms. The found constraints from neutron skins are in harmony with the recent determinations from reactions and giant resonances.
