Production Flow Analysis - cases from manufacturing and service industry

Production flow analysis (PFA) is a well-established methodology used for transforming traditional functional layout into product-oriented layout. The method uses part routings to find natural clusters of workstations forming production cells able to complete parts and components swiftly with simplified material flow. Once implemented, the scheduling system is based on period batch control aiming to establish fixed planning, production and delivery cycles for the whole production unit. PFA is traditionally applied to job-shops with functional layouts, and after reorganization within groups lead times reduce, quality improves and motivation among personnel improves. Several papers have documented this, yet no research has studied its application to service operations management. This paper aims to show that PFA can well be applied not only to job-shop and assembly operations, but also to back-office and service processes with real cases. The cases clearly show that PFA reduces non-value adding operations, introduces flow by evening out bottlenecks and diminishes process variability, all of which contribute to efficient operations management.

Artérite de Horton: recommandations Lausannoises de prise en charge [Giant cell arteritis: guidelines of the University Hospital of Lausanne].

Giant cell arteritis (GCA) is a subacute/chronic vasculitis and represents the most common form of systemic vasculitis in people over the age of 50 years. The absence of clear and specific diagnostic criteria with the highly variable clinical presentation is a diagnostic challenge requesting a multidisciplinary approach. Yet, GCA is an emergency and the treatment must be initiated very rapidly due to the risk of blindness. This article presents a review of GCA as well as the diagnostic and therapeutic institutional guidelines of the University Hospital of Lausanne.

Dermatite atopique de l'enfant: principes généraux de la prise en charge [Atopic dermatitis in children: general principles of management].

Atopic dermatitis is the most frequent dermatosis in childhood. Numerous studies underscored the central role of skin barrier alterations in the pathogenesis of the inflammatory skin lesions. The management of atopic dermatitis has to be multidimensional. It combines among others some daily local care and a sporadic topical anti-inflammatory treatment during the acute flare-ups. The objective of this article is to summarize, in light of the recent European guidelines, the general principles of management of atopic dermatitis, for the general practitioner.

Iowa Prisoner Re-Entry Initiative (PRI) Rural Service Delivery Model Iowa Department of Correctional Services, November 2009

The Iowa Department of Corrections (DOC) and the Department of Human Rights, Division of Criminal and Juvenile Justice Planning (CJJP) entered into a contract for services from September 12, 2007 to June 30, 2009 for the purposes of assisting in the evaluation component for the two-year Iowa Prisoner Re-Entry Initiative (PRI) Rural Service Delivery Model. This contract was extended to November 2009. The Rural PRI grant period ran from July 1, 2007 to June 30, 2009 and was extended to November 30, 2009. The purpose of the program was to improve community safety by providing pre-release services and successful transition planning and aftercare services to offenders released from state institutions to the Second Judicial District Department of Correctional Services. Participants included all offenders released to the Second Judicial District during the grant period.

Canicule et Pharmacie hospitalière en Valais

Introduction: Suite à la canicule de 2003, l'importance pour la santé publique des vagues de chaleur avait été mise en évidence. Cependant, les aspects se rapportant aux médicaments avaient été peu abordés, avant d'être davantage considérés mi-2004, notamment dans le «Plan canicule» français. La Pharmacie de l'ICHV s'est donc efforcée, au printemps 2004, de définir ses propres lignes d'action pour adapter son organisation et ses services. Objectifs: Définir au niveau des services pharmaceutiques, en particulier cliniques, des recommandations pratiques et proposer des moyens de mise en oeuvre. Méthode: Une recherche de littérature a été effectuée et a permis d'identifier les variables à prendre en compte. Basée sur celles-ci, une analyse ponctuelle (sur une journée) a été conduite à l'aide du dossier patient informatisé Phoenix® pour estimer la fréquence d'utilisation des médicaments à problème auprès d'une population considérée comme étant à risque (patients > 70 ans d'un service de chirurgie). Dans le domaine logistique, une recherche similaire a été conduite et des analyses à l'aide du logiciel Opale® ont été réalisées. Résultats: Compte tenu de la littérature, différentes variables en relation avec la pharmacothérapie sont à considérer: * Médicaments à risque (altérant la fonction rénale ou empêchant la perte calorique) ou à surveiller (dont la déshydratation influence la pharmacocinétique ou ceux aggravant les effets de la chaleur); * Patients à risque (> 70 ans ou déments); * Situations à risque (chirurgie élective, sortie d'hôpital). L'analyse en chirurgie a montré que 75% des patients étaient traités par des médicaments à risque et que 100% des patients recevaient des médicaments à surveiller. Au plan logistique, notre recherche a permis de définir un certain nombre de produits sensibles. Discussion-conclusion: Malgré les limites de cette analyse ponctuelle, mais compte tenu de la fréquence d'utilisation des médicaments à problème, le pharmacien doit, lors de canicules, pouvoir directement ou indirectement conseiller le prescripteur, p. ex., en: * relevant dans les dossiers, les patients, les situations et les médicaments à risque ou à surveiller et en formulant des propositions de prise en charge; * suivant systématiquement les fonctions rénales de ces patients; * proposant d'instaurer une alarme dans les dossiers informatisés pour aider à déceler les situations à risque. Au niveau de la pharmacie centrale et des pharmacies d'étage, la gestion de stock des produits sensibles requière diverses améliorations. La canicule constitue une problématique complexe et le pharmacien d'hôpital se doit d'y prêter attention dans ses activités. La pharmacie de l'ICHV, sur la base de ces éléments, va préparer un plan d'action à activer dans des conditions à convenir avec la direction et les hôpitaux.

Anorexie mentale et boutimie: I'expérience vaudoise d'une prise en charge interdisciplinaire [Anorexia and bulimia: the canton of Vaud's experience of an interdisciplinary approach].

As psychiatric disorders attacking the body, anorexia and bulimia may have severe psychological, physical and social consequences, often requiring a long-standing interdisciplinary, coordinated and individualized approach. Recently the canton of Vaud has initiated and developed an interinstitutional structure--between the University Hospital (CHUV) and the hospitals of the Northern region of the canton (eHnv)--for the care of patients suffering from eating disorders. This structure, allowing the above mentioned approach for the treatment of eating disorders, consists of an outpatient facility located in the CHUV and an inpatient unit in the hospital of Saint Loup of the eHnv. Within this structure, the general practitioner plays a crucial role in the prevention of the chronification of these disorders by means of their early detection and management.
