O tratamento endodôntico é uma etapa importante do protocolo de atendimento do reimplante dentário e tem uma relação direta com o aparecimento das reabsorções radiculares, maior causa de perda dos dentes reimplantados. O objetivo do trabalho foi analisar o processo de reparo no reimplante tardio de dente de macaco, utilizando o hidróxido de cálcio (Ca(OH)2) e o MTA como materiais obturadores de canal. Cinco macacos Cebus apella adultos tiveram seus incisivos laterais superiores e inferiores, direito e esquerdo, extraídos e deixados em meio ambiente por 60 minutos. Decorrido esse período, foi realizado o preparo biomecânico dos canais e os dentes foram divididos em dois grupos experimentais: grupo I - canal preenchido com pasta de Ca(OH)2 e grupo II - canal preenchido com MTA (Angelus®). Após o selamento da abertura coronária com ionômero de vidro, o ligamento periodontal foi removido e os dentes imersos em solução de fluoreto de sódio 2%, pH 5,5, por 10 minutos. Em seguida, os alvéolos foram irrigados com soro fisiológico e os dentes reimplantados e contidos por 30 dias com fio de aço e resina composta. A eutanásia dos animais foi realizada 60 dias após o reimplante. Os espécimes de ambos os grupos apresentaram reabsorção por substituição e pontos de anquilose ao longo dos três terços radiculares e ausência de reabsorção inflamatória. Não houve diferença significante entre os dois grupos com relação à reabsorção por substituição, porém a quantidade de anquilose foi significativamente maior no grupo do Ca(OH)2. Baseado nesses resultados conclui-se que o MTA pode ser uma opção clínica viável para a obturação de dentes tardiamente reimplantados que necessitam de um longo período de curativo com hidróxido de cálcio.

A presença de doenças sistêmicas, uso de medicamentos, e uso de próteses removíveis, entre outros fatores, podem alterar o equilíbrio fisiológico da cavidade bucal favorecendo o estabelecimento de diversas alterações e/ou patologias bucais. Assim, o estudo das prevalências destes fatores, nos pacientes odontológicos, tornam-se importantes pois devem ser considerados para elaborar o diagnóstico e definir a conduta terapêutica. Com o objetivo de delinear um perfil, avaliamos os prontuários de 500 usuários de próteses removíveis atendidos no Serviço de Medicina Bucal da Faculdade de Odontologia de Araraquara - UNESP. Analisamos os dados referentes a: características da população e das próteses removíveis, prevalência das doenças sistêmicas, uso de medicamentos e diagnóstico final das patologias ou alterações bucais. Os resultados obtidos nos permitem concluir que a maioria (74%) dos usuários de próteses removíveis eram mulheres brancas; metade da população pertencia a faixa etária de 41 a 60 anos e, quase a metade (49,2%) usava a combinação de prótese total superior e inferior. As doenças sistêmicas foram relatadas por 57,2% da amostra e a prevalência maior foi de doenças do sistema cardiovascular (25,4%). A maioria (60,4%) da população relatou uso de medicamentos e, dentre os mais freqüentes, a prevalência maior foram dos cardiovasculares (26,4%). A prevalência de patologias ou alterações bucais foi de 99,6%, sendo as relacionadas ao uso de próteses removíveis as mais freqüentes. Dentre estas, a candidose crônica atrófica ocorreu em 81,8% da população estudada e a hiperplasia fibrosa em 29,2%.

O objetivo deste estudo foi buscar um melhor entendimento sobre a dor músculo-esquelética crônica da face e sua relação com o bruxismo do sono. Quarenta pacientes foram avaliados de acordo com o Research Diagnostic Criteria for Temporomandibular Disorders: Grupo A - 20 pacientes com DTM, com média de idade de 32,7 anos e duração média da dor de 4,37 anos; Grupo B - 20 pacientes sem DTM, com média de idade de 30,8 anos. As variáveis do sono e do bruxismo foram avaliadas em exame polissonográfico. As características clínicas foram estatisticamente diferentes entre os dois grupos: o grupo A apresentou maior freqüência de auto-relato de dor matinal (p=0,0113) e estalido articular (p=0,0269), maior grau de sintomas físicos não específicos (p=0,001) e de limitações da função mandibular (p=0,001). Não houve diferença estatisticamente significativa para as variáveis do bruxismo: número de episódios de bruxismo por hora, número de surtos por episódio e por hora, duração total, porcentagem em cada estágio do sono, tipo e amplitude dos episódios. A arquitetura do sono mostrou-se dentro dos parâmetros de normalidade, sem nenhuma diferença estatisticamente significativa entre os dois grupos. Conclusões: as características clínicas dos pacientes com bruxismo do sono, com e sem DTM, são diferentes, mas as características do sono e do bruxismo são similares. Mais estudos são necessários para esclarecer as razões pelas quais alguns pacientes com bruxismo do sono desenvolvem dor milfascial crônica e outros não.

The surgical treatment of mandibular condyle fractures currently offers several possibilities for stable internal fixation. In this study, a finite element model evaluation was performed of three different methods for osteosynthesis of low subcondylar fractures: (1) two four-hole straight plates, (2) one seven-hole lambda plate, and (3) one four-hole trapezoidal plate. The finite element model evaluation considered a load applied to the first molar on the contralateral side to the fracture. Results showed that, although the three methods are capable of withstanding functional loading, the lambda plate displayed a more homogeneous stress distribution for both osteosynthesis material and bone and may be a better method when single-plate fixation is the option.

Background: This is a position paper from the 2nd International Bone Research Association (IBRA) Symposium for Condylar Fracture Osteosynthesis 2012 was held at Marseille, succeeding the first congress in Strasbourg, France, in 2007. The goal of this IBRA symposium and this paper was to evaluate current trends and potential changes of treatment strategies for mandibular condylar fractures, which remain controversial over the past decades.Methods: Using a cross-sectional study design, we enrolled the consensus based on the panel of experts and participants in the IBRA Symposium 2012. The outcomes of interest were the panel and electronic votes on management of condylar base, neck and head fractures, and panel votes on endoscopic and paediatric condylar fractures. Appropriate descriptive and univariate statistics were used.Results: The consensus derived from 14 experts and 41 participant surgeons, using 12 case scenarios and 27 statements. The experts and participants had similar decision on the treatment of condylar base, neck and head fractures, as well as similar opinion on complications of condylar fracture osteosynthesis. They had a parallel agreement on using open reduction with internal fixation (ORIF) as treatment of choice for condylar base and neck fractures in adults. Endoscopic approaches should be considered for selected cases, such as condylar base fractures with lateral displacement. There was also a growing tendency to perform ORIF in condylar head fractures. The experts also agreed to treat children (> 12 years old) in the same way as adults and to consider open reduction in severely displaced and dislocated fractures even in younger children. Nevertheless, non-surgical treatment should be the first choice for children <6 years of age. The decision to perform surgery in children was based on factors influencing facial growth, appropriate age for ORIF, and disagreement to use resorbable materials in children.Conclusions: The experts and participating surgeons had comparable opinion on management of condylar fractures and complications of ORIF. Compared to the first Condylar Fracture Symposium 2007 in Strasbourg, ORIF may now be considered as the gold standard for both condylar base and neck fractures with displacement and dislocation. Although ORIF in condylar head fractures in adults and condylar fractures in children with mixed dentition is highly recommended, but this recommendation requires further investigations. Objective: To evaluate the influence of different insertion torques on healing of implants loaded immediately or left unloaded.Material and methods: In six Labrador dogs, all mandibular premolars and molars were extracted. After 4 months of healing, flaps were elevated, and two implant sites were prepared at each side of the mandible. The distal sites were prepared conventionally while the mesial sites were underprepared by 0.3 mm. As a consequence, different final insertion torques of about 30 Ncm at the distal and >70 Ncm at the mesial sites were recorded. Healing abutments were applied to the left and transmucosal abutments to the right side. Flaps were sutured, crown preparation of the upper right second and third premolars was performed, and impressions were taken. Within 24 h, crowns were cemented both to implants and teeth in the right side of the mouth. After 4 months, the animals were sacrificed and ground sections obtained for histological evaluation.Results: A higher buccal bony crestal resorption and a more apical position of the coronal level of osseointegration were found at the loaded compared with the unloaded sites. MBIC% and percentages of peri-implant mineralized tissue (MB%) were higher at the loaded compared with the unloaded sites. Moreover, a higher MBIC% was found at the lower compared with the higher final insertion torque.Conclusions: Immediate loading does not seem to have a negative effect on osseointegration. High torque values for the immediate loading procedures were not necessary. Probably, low torque values, were sufficient to obtain primary stability and hence may provide better osseointegration than high torque value.

Objective: To study bony and soft tissue changes at implants installed in alveolar bony ridges of different widths.Material and methods: In 6 Labrador dogs, the mandibular premolars and first molars were extracted, and a buccal defect was created in the left side at the third and fourth premolars by removing the buccal bone and the inter-radicular and interdental septa. Three months after tooth extraction, full-thickness mucoperiosteal flaps were elevated, and implants were installed, two at the reduced (test) and two at the regular-sized ridges (control). Narrow or wide abutments were affixed to the implants. After 3 months, biopsies were harvested, and ground sections prepared for histological evaluation.Results: A higher vertical buccal bony crest resorption was found at the test (1.5 +/- 0.7 mm and 1.0 +/- 0.7 mm) compared to the control implants (1.0 +/- 0.5 mm and 0.7 +/- 0.4 mm), for both wide and narrow abutment sites. A higher horizontal alveolar resorption was identified at the control compared to the test implants. The difference was significant for narrow abutment sites. The peri-implant mucosa was more coronally positioned at the narrow abutment, in the test sites, while for the control sites, the mucosal adaptation was more coronal at the wide abutment sites. These differences, however, did not reach statistical significance.Conclusions: Implants installed in regular-sized alveolar ridges had a higher horizontal, but a lower vertical buccal bony crest resorption compared to implants installed in reduced alveolar ridges. Narrow abutments in reduced ridges as well as wide abutments in regular-sized ridges yielded less soft tissue recession compared to their counterparts.

Horizontal root fractures (HRF) usually affect the anterior teeth as a result of trauma, and generally heal spontaneously, depending on the vitality of the pulp. Diagnosis based on clinical findings, sensitivity tests, and radiographic examination is important to determine the presence of a root fracture and to prevent a root fracture from passing unnoticed. Cone-beam computed tomography (CBCT) has been used successfully for diagnosis and prognosis imaging of root fractures and has proved to be superior to other radiographic methods. This study reports two cases of dental trauma caused by a collision and a sports accident. The patients suffered horizontal root fractures in the maxillary left central incisor and in the mandibular left central incisor. The diagnosis of root fracture was confirmed by cone-beam computed tomography (CBCT) images, which also demonstrated spontaneous healing of the fracture line. The repair occurred by interposition of connective tissue in the former case and by interposition of bone and connective tissue in the latter case. The final diagnoses of both cases were based on CBCT images, indicating the importance of a CBCT examination to reach a firm diagnosis and to follow the healing process of root fracture cases, avoiding unnecessary radical endodontic treatment.

The aim of this study was to assess the impact of replacing conventional mandibular complete dentures with implant-supported fixed complete dentures (ISFDs) on the oral health related quality of life (OHOOL) of edentulous patients and on the associated kinesiographic parameters. The patients had their complete dentures replaced by ISFDs and were assessed after 1, 2, 3, 6, and 12 months. An improvement of general OHQOL was observed 2 months after treatment with ISFDs. Kinesiographic recordings revealed significant mandibular vertical and horizontal opening increases and a greater vertical intrusion of the maxillary complete dentures during clenching after the treatment with ISFD.
