Diabetes mellitus umfasst eine heterogene Gruppe von Stoffwechselfunktionsstörungen, die durch hohe Blut-Glukose-Werte gekennzeichnet sind. Zwei Haupttypen von Diabetes mellitus wurden definiert: Typ 1- und Typ 2-Diabetes. Repaglinid ist ein neuer, schnell wirksamer, bei Typ 2-Diabetikern eingesetzter prandialer Glukose-Regulator mit einer kurzen Plasmahalbwertszeit (<1 Stunde) und der erste Vertreter der Carbamoylmethylbenzoesäure Familie, der in klinischen Studien getestet wurde. Die 18F- und 11C-markierten Repaglinid-Derivate (S)-2-(2-[18F]Fluorethoxy)-4-((3-methyl-1-(2-piperidin-1-yl-phenyl)-butylcarbamoyl)-methyl)-benzoesäure ([18F]Fluorethoxy-desethoxy-Repaglinid) und (S)-2-([11C]Methoxy)-4-([3-methyl-1-(2-piperidin-1-yl-phenyl)-butyl-carba-moyl]-benzoesäure ([11C]Methoxy-desethoxy-Repaglinid) wurden als potentielle Tracer für die nicht-invasive Quantifizierung des Sulfonylharnstoffrezeptor-Typ1-Status (SUR-1) der Insulin-sezernierenden -Zellen mittels Positronen-Emissions-Tomographie (PET) synthetisiert. [18F]Fluorethoxy-desethoxy-Repaglinide konnte in einer radiochemischen Ausbeute (RCA) von 20% nach 135 Minuten mit einer radiochemischen Reinheit >98% unter Verwendung des sekundären Markierungsvorläufers 2-[18F]Fluorethyltosylat erhalten werden. Die spezifische Aktivität lag im Bereich von 50-60 GBq/µmol. Für die radioaktive Synthese des [11C]Methoxy-desethoxy-Repaglinids wurde der sekundäre Markierungsvorläufer [11C]Methyliodid verwendet. Der 11C-Radiotracer wurde in einer RCA von 35% (bezogen auf [11C]CO2) mit einer spezifischen Aktivität von 40-70 GBq/µmol erhalten. Um die Eigenschaften des fluorierten sowie des methoxylierten Repaglinids zu charakterisieren, wurde die Affinität beider Verbindungen zum humanen SUR-1 evaluiert. [19F]Fluorethoxy-desethoxy-Repaglinid und Methoxy-desethoxy-Repaglinid induzierten Verdrängungskurven mit Hill-Koeffizienten nahe 1 und ergaben Dissotiationskonstanten (KD) von 142 nM beziehungsweise 83 nM - vergleichsweise geringe Verluste relativ zu Original-Repaglinid. Die biologische Aktivität wurde mittels Insulin-Sekretionstests an isolierten Ratten-Inselzellen gezeigt und war ebenfalls mit der des Repaglinids vergleichbar. Schließlich wurde die Biodistribution des [18F]Fluorethoxy-desethoxy-Repaglinids in gesunden Sprague-Dawley-Ratten durch Messung der Konzentration der Verbindung in verschiedenen Organen nach intravenöser Injektion untersucht. Das pankreatische Gewebe zeigte im Zeitintervall zwischen 10 und 30 Minuten nach Injektion eine stabile Akkumulation von etwa 0.12% der injizierten Dosis. 50% dieser Tracer-Akkulmulation konnten durch zusätzliche Injektion von nicht-radioaktiv-markiertem Repaglinid verdrängt werden, was auf eine mögliche Eignung des [18F]Fluorethoxy-desethoxy-Repaglinids für in vivo-Untersuchungen mittels PET schließen lässt. Eine erste humane PET-Studie zeigte zwar ebenfalls eine stabile, allerdings nur geringere Akkumulation von [18F]Fluorethoxy-desethoxy-Repaglinid im Pankreas und eine überproportional hohe Aktivitätsanreicherung in der Leber. Die Radioaktivitäts-akkumulation im Blut fiel nach wenigen Minuten unter die des Pankreas.

IL-33 is a novel member of the IL-1 family and ligand for the IL-1 receptor-related protein, ST2. Recent evidence suggests that the IL-33/ST2 axis plays a critical role in several autoimmune and inflammatory disorders; however, its role in inflammatory bowel disease (IBD) has not been clearly defined. We characterized IL-33 and ST2 expression and modulation following conventional anti-TNF therapy in Crohn’s disease and ulcerative colitis (UC) patients, and investigated the role of IL-33 in SAMP1/YitFc (SAMP) mice, a mixed Th1/Th2 model of IBD. Our results showed a specific increase of mucosal IL-33 in active UC, localized primarily to intestinal epithelial cells (IEC) and colonic inflammatory infiltrates. Importantly, increased expression of full-length IL-33, representing the most bioactive form, was detected in UC epithelium, while elevated levels of cleaved IL-33 were present in IBD serum. ST2 isoforms were differentially modulated in UC epithelium and sST2, a soluble decoy receptor with anti-inflammatory properties, was also elevated in IBD serum. Infliximab (anti-TNF) treatment of UC decreased circulating IL-33 and increased sST2, while stimulation of HT-29 IEC confirmed IL-33 and sST2 regulation by TNF. Similarly, IL-33 significantly increased and correlated with disease severity, and potently induced IL-5, IL-6 and IL-17 from mucosal immune cells in SAMP mice. Taken together, the IL-33/ST2 system plays an important role in IBD and experimental colitis, is modulated by anti-TNF therapy, and may represent a specific biomarker for active UC.

Survivin, a unique member of the family of inhibitors of apoptosis (IAP) proteins, orchestrates intracellular pathways during cell division and apoptosis. Its central regulatory function in vertebrate molecular pathways as mitotic regulator and inhibitor of apoptotic cell death has major implications for tumor cell proliferation and viability, and has inspired several approaches that target survivin for cancer therapy. Analyses in early-branching Metazoa so far propose an exclusive role of survivin as a chromosomal passenger protein, whereas only later during evolution the second, complementary antiapoptotic function might have arisen, concurrent with increased organismal complexity. To lift the veil on the ancestral function(s) of this key regulatory molecule, a survivin homologue of the phylogenetically oldest extant metazoan taxon (phylum Porifera) was identified and functionally characterized. SURVL of the demosponge Suberites domuncula shares significant similarities with its metazoan homologues, ranging from conserved exon/intron structures to the presence of localization signal and protein-interaction domains, characteristic of IAP proteins. Whereas sponge tissue displayed a very low steady-state level, SURVL expression was significantly up-regulated in rapidly proliferating primmorph cells. In addition, challenge of sponge tissue and primmorphs with cadmium and the lipopeptide Pam3Cys-Ser-(Lys)4 stimulated SURVL expression, concurrent with the expression of newly discovered poriferan caspases (CASL and CASL2). Complementary functional analyses in transfected HEK-293 revealed that heterologous expression of poriferan survivin in human cells not only promotes cell proliferation but also augments resistance to cadmium-induced cell death. Taken together, these results demonstrate both a deep evolutionary conserved and fundamental dual role of survivin, and an equally conserved central position of this key regulatory molecule in interconnected pathways of cell cycle and apoptosis. Additionally, SDCASL, SDCASL2, and SDTILRc (TIR-LRR containing protein) may represent new components of the innate defense sentinel in sponges. SDCASL and SDCASL2 are two new caspase-homolog proteins with a singular structure. In addition to their CASc domains, SDCASL and SDCASL2 feature a small prodomain NH2-terminal (effector caspases) and a remarkably long COOH-terminal domain containing one or several functional double stranded RNA binding domains (dsrm). This new caspase prototype can characterize a caspase specialization coupling pathogen sensing and apoptosis, and could represent a very efficient defense mechanism. SDTILRc encompasses also a unique combination of domains: several leucine rich repeats (LRR) and a Toll/IL-1 receptor (TIR) domain. This unusual domain association may correspond to a new family of intracellular sensing protein, forming a subclass of pattern recognition receptors (PRR).

Während ausgedehnter Weltraumflüge zum Mond und Mars wäre die Besatzung eines Raumschiffs dicht ionisierender Schwerionenstrahlung, der sogenannten kosmischen Strahlung, ausgesetzt. Da bei vielen Krebspatienten die orale Mukositis als eine gravierende Nebenwirkung bei der herkömmlichen Strahlentherapie auftritt, könnte diese Erkrankung ein medizinisches Problem während ausgedehnter Weltraumflüge darstellen. Allerdings liegen bislang keine Untersuchungen über eine mögliche Mukositis-induzierte Wirkung von Schwerionenstrahlung vor. Um das Risiko strahlungsinduzierter oraler Mukositis durch kosmische Strahlung abzuschätzen, wurden dreidimensionale organotypische Mundschleimhaut-Modelle 12C Schwerionen- bzw. Röntgenstrahlung ausgesetzt und nach definierten Inkubationszeiten kryokonserviert. Aus dem Material wurden Kryoschnitte gefertigt, mit denen anschließend histologische und immunhistologische Fluoreszenzfärbungen zum Nachweis von zum Beispiel Kompaktheitsverlust, Doppelstrangbrüchen der DNA und NF-κB-Aktivierung durchgeführt wurden. Die Ausschüttung von Tumornekrosefaktor α, Interleukin 1β, Interleukin 6 und Interleukin 8 wurde in Probenüberständen mittels ELISA analysiert. Das Hauptaugenmerk dieser Studie lag dabei auf den frühen durch Strahlung verursachten Effekten. rnIm Rahmen dieser Dissertation wurde ein dreidimensionales Mundschleimhaut-Modell etabliert, das verglichen mit humaner Mundschleimhaut viele organotypische Differenzierungsmarker und in vivo-ähnliche Reaktionen auf bestimmte Reize zeigte. Nach einer Bestrahlung mit schweren Kohlenstoffionen bzw. mit Röntgenstrahlung wurden strahlungsinduzierte Effekte auf mehreren Ebenen detektiert. Bereits 1 h nach Bestrahlung konnten dosisabhängig vermehrte Doppelstrangbrüche in der DNA detektiert werden, wobei bei gleicher Dosis Schwerionenstrahlung im Durchschnitt doppelt so viele DSB verursachte wie Röntgenstrahlung. Das Mundschleimhaut-Modell reagierte auf beide Strahlungsarten mit einer Aktivierung des Transkriptionsfaktors NF-κB p50, wobei Röntgenstrahlung diese Aktivierung früher induzierte als Schwerionen. Eine strahlungsinduzierte Aktivierung von NF-κB p65 wurde in den organotypischen Kulturen zu den untersuchten Zeitpunkten nicht beobachtet. Im Vergleich zu nicht bestrahlten Kulturen waren die Konzentrationen von Interleukin 6 und Interleukin 8 unabhängig von der Strahlungsqualität nach Bestrahlung signifikant erhöht. Interleukin-1β dagegen wurde nur nach Röntgenbestrahlung signifikant vermehrt ausgeschüttet. In Schwerionen-bestrahlten Proben wurden zwar tendenziell höhere Konzentrationen beobachtet, statistische Signifikanzen ergaben sich aber nicht. Die Analysen zur TNF-α- und IFN-γ-Ausschüttung in bestrahlten organotypischen Kulturen ergaben innerhalb des gewählten Beobachtungszeitraums von 48 h keine strahlungsinduzierten Effekte. Bei den Untersuchungen der Proliferation in den Zellen der bestrahlten organotypischen Kulturen wurde bereits 24 h nach Röntgenstrahlung und 3 Tage nach Schwerionenstrahlung eine deutliche verminderte Proliferation beobachtet. Des Weiteren zeigte das Mundschleimhaut-Modell unabhängig von Strahlungsqualitäten einen eindeutigen Verlust in der Zellintegrität und daraus resultierenden Kompaktheitsverlust des Gewebes, was in der in vivo-Situation wahrscheinlich einer Gewebedegeneration entspricht. rnNach Bestrahlung mit sowohl Röntgenstrahlung als auch schweren Ionen wurden im gewählten Mukosa-Modell innerhalb von 48 h nach Behandlung strahlungsinduzierte proinflammatorische Marker eindeutig und reproduzierbar detektiert. Dies deutet darauf hin, dass während langer extraterrestrischer Expeditionen das Risiko der oralen Mukositis einkalkuliert und mit den daraus folgenden Konsequenzen gerechnet werden muss.rn

Pneumococcal meningitis causes neurological sequelae, including learning and memory deficits in up to half of the survivors. In both humans and in animal models of the disease, there is apoptotic cell death in the hippocampus, a brain region involved in learning and memory function. We previously demonstrated that in an infant rat model of pneumococcal meningitis, there is activation of the kynurenine (KYN) pathway in the hippocampus, and that there was a positive correlation between the concentration of 3-hydroxykynurenine and the extent of hippocampal apoptosis. To clarify the role of the KYN pathway in the pathogenesis of hippocampal apoptosis in pneumococcal meningitis, we specifically inhibited 2 key enzymes of the KYN pathway and assessed hippocampal apoptosis, KYN pathway metabolites, and nicotinamide adenine dinucleotide (NAD) concentrations by high-performance liquid chromatography. Pharmacological inhibition of kynurenine 3-hydroxylase and kynureninase led to decreased cellular NAD levels and increased apoptosis in the hippocampus. The cerebrospinal fluid levels of tumor necrosis factor and interleukin-1? and -? were not affected. Our data suggest that activation of the KYN pathway in pneumococcal meningitis is neuroprotective by compensating for an increased NAD demand caused by infection and inflammation;this mechanism may prevent energy failure and apoptosis in the hippocampus.

Meprin and , zinc metalloproteinases, play significant roles in inflammation, including inflammatory bowel disease (IBD), possibly by activating cytokines, like interleukin 1 , interleukin 18, or tumor growth factor . Although a number of potential activators for meprins are known, no endogenous inhibitors have been identified. In this work, we analyzed the inhibitory potential of human plasma and identified bovine fetuin-A as an endogenous meprin inhibitor with a K(i) (inhibition constant) of 4.2 × 10(-5) M for meprin and a K(i) of 1.1 × 10(-6) M meprin . This correlated with data obtained for a fetuin-A homologue from carp (nephrosin inhibitor) that revealed a potent meprin and inhibition (residual activities of 27 and 22%, respectively) at a carp fetuin concentration of 1.5 × 10(-6) M. Human fetuin-A is a negative acute phase protein involved in inflammatory diseases, thus being a potential physiological regulator of meprin activity. We report kinetic studies of fetuin-A with the proteolytic enzymes astacin, LAST, LAST_MAM, trypsin, and chymotrypsin, indeed demonstrating that fetuin-A is a broad-range protease inhibitor. Fetuin-A inhibition of meprin activity was 40 times weaker than that of meprin activity. Therefore, we tested cystatin C, a protein structurally closely related to fetuin-A. Indeed, cystatin C was an inhibitor for human meprin (K(i) = 8.5 × 10(-6) M) but, interestingly, not for meprin . Thus, the identification of fetuin-A and cystatin C as endogenous proteolytic regulators of meprin activity broadens our understanding of the proteolytic network in plasma.

The two ubiquitously expressed sphingosine kinases (SphK) 1 and 2 are key regulators of the sphingolipid signaling pathway. Despite the formation of an identical messenger, i.e. sphingosine 1-phosphate (S1P), they exert strikingly different functions. Particularly, SphK2 is necessary for the phosphorylation of the sphingosine analog fingolimod (FTY720), which is protective in rodent stroke models. Using gene deficient mice lacking either SphK1 or SphK2, we investigated the role of the two lipid kinases in experimental stroke. We performed 2h transient middle cerebral artery occlusion (tMCAO) and analyzed lesion size and neurological function after 24h. Treatment groups received 1mg/kg FTY720. Neutrophil infiltration, microglia activation, mRNA and protein expression of SphK1, SphK2 and the S1P(1) receptor after tMCAO were studied. Genetic deletion of SphK2 but not SphK1 increased ischemic lesion size and worsened neurological function after tMCAO. The protective effect of FTY720 was conserved in SphK1(-/-) mice but not in SphK2(-/-) mice. This suggests that SphK2 activity is an important endogenous protective mechanism in cerebral ischemia and corroborates that the protective effect of FTY720 is mediated via phospho-FTY720.

Central nervous system involvement is a rare and serious complication of Behçet's disease (BD). Herein, we describe a patient with an atypical central lesion, who experienced progressive hypesthesia of the right arm and sensory loss of the trigeminal nerve together with intense headache. A repeated biopsy was necessary to conclusively establish the diagnosis of BD. Therapy with infusions of infliximab led to a remarkable full remission. TNFα-blocking therapy was successfully replaced by azathioprine. The present well-illustrated case demonstrates the difficulty of establishing the diagnosis of BD with central nervous system involvement, the dramatic benefit of short given TNF-α-blocking agent, and the long-term remission with azathioprin.

The receptor tyrosine kinase (RTK)/phosphoinositide 3-kinase (PI3K) pathway is fundamental for cancer cell proliferation and is known to be frequently altered and activated in neoplasia, including embryonal tumors. Based on the high frequency of alterations, targeting components of the PI3K signaling pathway is considered to be a promising therapeutic approach for cancer treatment. Here, we have investigated the potential of targeting the axis of the insulin-like growth factor-1 receptor (IGF-1R) and PI3K signaling in two common cancers of childhood: neuroblastoma, the most common extracranial tumor in children and medulloblastoma, the most frequent malignant childhood brain tumor. By treating neuroblastoma and medulloblastoma cells with R1507, a specific humanized monoclonal antibody against the IGF-1R, we could observe cell line-specific responses and in some cases a strong decrease in cell proliferation. In contrast, targeting the PI3K p110α with the specific inhibitor PIK75 resulted in broad anti-proliferative effects in a panel of neuro- and medulloblastoma cell lines. Additionally, sensitization to commonly used chemotherapeutic agents occurred in neuroblastoma cells upon treatment with R1507 or PIK75. Furthermore, by studying the expression and phosphorylation state of IGF-1R/PI3K downstream signaling targets we found down-regulated signaling pathway activation. In addition, apoptosis occurred in embryonal tumor cells after treatment with PIK75 or R1507. Together, our studies demonstrate the potential of targeting the IGF-1R/PI3K signaling axis in embryonal tumors. Hopefully, this knowledge will contribute to the development of urgently required new targeted therapies for embryonal tumors.

The α-hemoglobin-derived dodecapeptide RVD-hemopressin (RVDPVNFKLLSH) has been proposed to be an endogenous agonist for the cannabinoid receptor type 1 (CB(1)). To study this peptide, we have raised mAbs against its C-terminal part. Using an immunoaffinity mass spectrometry approach, a whole family of N-terminally extended peptides in addition to RVD-Hpα were identified in rodent brain extracts and human and mouse plasma. We designated these peptides Pepcan-12 (RVDPVNFKLLSH) to Pepcan-23 (SALSDLHAHKLRVDPVNFKLLSH), referring to peptide length. The most abundant Pepcans found in the brain were tested for CB(1) receptor binding. In the classical radioligand displacement assay, Pepcan-12 was the most efficacious ligand but only partially displaced both [(3)H]CP55,940 and [(3)H]WIN55,212-2. The data were fitted with the allosteric ternary complex model, revealing a cooperativity factor value α < 1, thus indicating a negative allosteric modulation. Dissociation kinetic studies of [(3)H]CP55,940 in the absence and presence of Pepcan-12 confirmed these results by showing increased dissociation rate constants induced by Pepcan-12. A fluorescently labeled Pepcan-12 analog was synthesized to investigate the binding to CB(1) receptors. Competition binding studies revealed K(i) values of several Pepcans in the nanomolar range. Accordingly, using competitive ELISA, we found low nanomolar concentrations of Pepcans in human plasma and ∼100 pmol/g in mouse brain. Surprisingly, Pepcan-12 exhibited potent negative allosteric modulation of the orthosteric agonist-induced cAMP accumulation, [(35)S]GTPγS binding, and CB(1) receptor internalization. Pepcans are the first endogenous allosteric modulators identified for CB(1) receptors. Given their abundance in the brain, Pepcans could play an important physiological role in modulating endocannabinoid signaling.

During general anesthesia drugs are administered to provide hypnosis, ensure analgesia, and skeletal muscle relaxation. In this paper, the main components of a newly developed controller for skeletal muscle relaxation are described. Muscle relaxation is controlled by administration of neuromuscular blocking agents. The degree of relaxation is assessed by supramaximal train-of-four stimulation of the ulnar nerve and measuring the electromyogram response of the adductor pollicis muscle. For closed-loop control purposes, a physiologically based pharmacokinetic and pharmacodynamic model of the neuromuscular blocking agent mivacurium is derived. The model is used to design an observer-based state feedback controller. Contrary to similar automatic systems described in the literature this controller makes use of two different measures obtained in the train-of-four measurement to maintain the desired level of relaxation. The controller is validated in a clinical study comparing the performance of the controller to the performance of the anesthesiologist. As presented, the controller was able to maintain a preselected degree of muscle relaxation with excellent precision while minimizing drug administration. The controller performed at least equally well as the anesthesiologist.

BACKGROUND: It is known that endometriosis is an inflammatory disease and those patients seem to have lower pregnancy rates. The aim of the study was to investigate the concentrations of chemokines and proinflammatory cytokines in the follicular fluid of patients with and without endometriosis. METHODS: Follicular aspiration, recovering follicular fluid during assisted reproductive treatment, follicular fluid storage and analysis of chemokines and proinflammatory cytokines were carried out. Tumor necrosis factor-alpha, interleukin-1 beta, interleukin-6, interleukin-8, interleukin-15, leukemia inhibitory factor, epithelial neutrophil-activating peptide 78, regulated upon activation, normal T-cell expressed and secreted, and growth-regulated oncogene-alpha were analyzed in the follicular fluid and compared between women with (n =47) and without endometriosis (n = 279). RESULTS: The above cytokines were detected in the follicular fluid samples. Epithelial neutrophil-activating peptide 78 levels were significantly higher in follicular fluid from endometriosis patients than from controls (p = 0.008). Increases (to twice the control level) were also observed for tumor necrosis factor-alpha and for interleukin-6. CONCLUSIONS: Increased follicular fluid levels of epithelial neutrophil-activating peptide 78, tumor necrosis factor-alpha and interleukin-6 indicate that these cytokines may influence oocyte quality and fecundability of women with endometriosis by deteriorating the microenvironment in the human follicle.

The human adrenal cortex produces mineralocorticoids, glucocorticoids, and androgens in a species-specific, hormonally regulated, zone-specific, and developmentally characteristic fashion. Most molecular studies of adrenal steroidogenesis use human adrenocortical NCI-H295A and NCI-H295R cells as a model because appropriate animal models do not exist. NCI-H295A and NCI-H295R cells originate from the same adrenocortical carcinoma which produced predominantly androgens but also smaller amounts of mineralocorticoids and glucocorticoids. Research data obtained from either NCI-H295A or NCI-H295R cells are generally compared, although for the same experiments no direct comparison between the two cell lines has been performed. Therefore, we compared the steroid profile and the expression pattern of important genes involved in steroidogenesis in both cell lines. We found that steroidogenesis differs profoundly. NCI-H295A cells produce more mineralocorticoids, whereas NCI-H295R cells produce more androgens. Expression of the 3beta-hydroxysteroid dehydrogenase (HSD3B2), cytochrome b5, and sulfonyltransferase genes is higher in NCI-H295A cells, whereas expression of the cytochrome P450c17 (CYP17), 21-hydroxylase (CYP21), and P450 oxidoreductase genes does not differ between the cell lines. We found lower 3beta-hydroxysteroid dehydrogenase type 2 but higher 17,20-lyase activity in NCI-H295R cells explaining the 'androgenic' steroid profile for these cells and resembling the zona reticularis of the human adrenal cortex. Both cell lines were found to express the ACTH receptor at low levels consistent with low stimulation by ACTH. By contrast, both cell lines were readily stimulated by 8Br-cAMP. The angiotensin type 1 receptor was highly expressed in NCI-H295R than NCI-H295A cells and angiotensin II stimulated steroidogenesis in NCI-H295R but not NCI-H295A cells. Our data suggest that comparative studies between NCI-H295A and NCI-H295R cells may help find important regulators of mineralocorticoid or androgen biosynthesis.
