BACKGROUND: The Pulmonary Embolism Severity Index (PESI) estimates the risk of 30-day mortality in patients with acute pulmonary embolism (PE). We constructed a simplified version of the PESI. METHODS: The study retrospectively developed a simplified PESI clinical prediction rule for estimating the risk of 30-day mortality in a derivation cohort of Spanish outpatients. Simplified and original PESI performances were compared in the derivation cohort. The simplified PESI underwent retrospective external validation in an independent multinational cohort (Registro Informatizado de la Enfermedad Tromboembólica [RIETE] cohort) of outpatients. RESULTS: In the derivation data set, univariate logistic regression of the original 11 PESI variables led to the removal of variables that did not reach statistical significance and subsequently produced the simplified PESI that contained the variables of age, cancer, chronic cardiopulmonary disease, heart rate, systolic blood pressure, and oxyhemoglobin saturation levels. The prognostic accuracy of the original and simplified PESI scores did not differ (area under the curve, 0.75 [95% confidence interval (CI), 0.69-0.80]). The 305 of 995 patients (30.7%) who were classified as low risk by the simplified PESI had a 30-day mortality of 1.0% (95% CI, 0.0%-2.1%) compared with 10.9% (8.5%-13.2%) in the high-risk group. In the RIETE validation cohort, 2569 of 7106 patients (36.2%) who were classified as low risk by the simplified PESI had a 30-day mortality of 1.1% (95% CI, 0.7%-1.5%) compared with 8.9% (8.1%-9.8%) in the high-risk group. CONCLUSION: The simplified PESI has similar prognostic accuracy and clinical utility and greater ease of use compared with the original PESI.

The introduction of the WHO FRAX® algorithms has facilitated the assessment of fracture risk on the basis of fracture probability. Its use in fracture risk prediction has strengths, but also limitations of which the clinician should be aware and are the focus of this review INTRODUCTION: The International Osteoporosis Foundation (IOF) and the International Society for Clinical Densitometry (ISCD) appointed a joint Task Force to develop resource documents in order to make recommendations on how to improve FRAX and better inform clinicians who use FRAX. The Task Force met in November 2010 for 3 days to discuss these topics which form the focus of this review. METHODS: This study reviews the resource documents and joint position statements of ISCD and IOF. RESULTS: Details on the clinical risk factors currently used in FRAX are provided, and the reasons for the exclusion of others are provided. Recommendations are made for the development of surrogate models where country-specific FRAX models are not available. CONCLUSIONS: The wish list of clinicians for the modulation of FRAX is large, but in many instances, these wishes cannot presently be fulfilled; however, an explanation and understanding of the reasons may be helpful in translating the information provided by FRAX into clinical practice.

BACKGROUND: Catecholaminergic polymorphic ventricular tachycardia (CPVT) is an inherited arrhythmia syndrome associated with mutations in the cardiac ryanodine receptor gene (Ryr2) in the majority of patients. Previous studies of CPVT patients mainly involved probands, so current insight into disease penetrance, expression, genotype-phenotype correlations, and arrhythmic event rates in relatives carrying the Ryr2 mutation is limited. METHODS AND RESULTS: One-hundred sixteen relatives carrying the Ryr2 mutation from 15 families who were identified by cascade screening of the Ryr2 mutation causing CPVT in the proband were clinically characterized, including 61 relatives from 1 family. Fifty-four of 108 antiarrhythmic drug-free relatives (50%) had a CPVT phenotype at the first cardiological examination, including 27 (25%) with nonsustained ventricular tachycardia. Relatives carrying a Ryr2 mutation in the C-terminal channel-forming domain showed an increased odds of nonsustained ventricular tachycardia (odds ratio, 4.1; 95% CI, 1.5-11.5; P=0.007, compared with N-terminal domain) compared with N-terminal domain. Sinus bradycardia was observed in 19% of relatives, whereas other supraventricular dysrhythmias were present in 16%. Ninety-eight (most actively treated) relatives (84%) were followed up for a median of 4.7 years (range, 0.3-19.0 years). During follow-up, 2 asymptomatic relatives experienced exercise-induced syncope. One relative was not being treated, whereas the other was noncompliant. None of the 116 relatives died of CPVT during a 6.7-year follow-up (range, 1.4-20.9 years). CONCLUSIONS: Relatives carrying an Ryr2 mutation show a marked phenotypic diversity. The vast majority do not have signs of supraventricular disease manifestations. Mutation location may be associated with severity of the phenotype. The arrhythmic event rate during follow-up was low.

The demand for research in the area of safety health and environmental management of nanotechnologies is present since a decade and identified by several landmark reports and studies. It is not the intention of this compendium to report on these as they are widely available. It is also not the intention to publish scientific papers and research results as this task is covered by scientific conferences and the peer reviewed press. The intention of the compendium is to bring together researchers, create synergy in their work, and establish links and communication between them mainly during the actual research phase before publication of results. Towards this purpose we find useful to give emphasis to communication of projects strategic aims, extensive coverage of specific work objectives and of methods used in research, strengthening human capacities and laboratories infrastructure, supporting collaboration for common goals and joint elaboration of future plans, without compromising scientific publication potential or IP Rights. These targets are far from being achieved with the publication in its present shape. We shall continue working, though, and hope with the assistance of the research community to make significant progress. We would like to stress that this sector is under development and progressing very fast, which might make some of the statements outdated or even obsolete. Nevertheless it is intended to provide a basis for the necessary future developments. [Ed.]

The Iowa Division of Criminal and Juvenile Justice Planning (CJJP) received funding through the Bureau of Justice Statistics’ (BJS) State Justice Statistics (SJS) Program for the purpose of collection and analysis of data related to implementation of the Iowa Sex Offender Registry. The research had two specific goals: · To enhance understanding of the State’s Sex Offender Registry through collection and analysis of data on sex offenders before and after the Registry’s implementation. · To develop and validate a unique Iowa Sex Offender Risk Assessment tool to assist in identifying those offenders who constitute the highest risk to re-offend. Few studies have addressed the impact of a Sex Offender Registry program on recidivism rates or other variables. It was the purpose of this first study to examine and compare two groups of individuals to determine what effect, if any, the requirement to register as a sex offender had on recidivism rates over a 3-4 year period.

Selective cyclooxygenase-2-inhibitors (COX-2) were developed as an alternative to the non-steroidal anti-inflammatory drugs (NSAID) in order to reduce their known gastrointestinal and renal toxicity. Several recent studies have shown the complex mechanism of the cyclooxygenase-2. The inhibition of the COX-2 has effects on renal hemodynamics, renal salt and water retention and may increase the thromboembolic and therefore the cardiovascular risk. The renal toxicity of the COX-2 inhibitors is similar to that of traditional NSAID. Regarding these data, COX-2 inhibitors should be prescribed with much caution to high risk patients, that is, patients with renal failure and/or cardiovascular diseases.

BACKGROUND: Frailty is a relatively new geriatric concept referring to an increased vulnerability to stressors. Various definitions have been proposed, as well as a range of multidimensional instruments for its measurement. More recently, a frailty phenotype that predicts a range of adverse outcomes has been described. Understanding frailty is a particular challenge both from a clinical and a public health perspective because it may be a reversible precursor of functional dependence. The Lausanne cohort Lc65+ is a longitudinal study specifically designed to investigate the manifestations of frailty from its first signs in the youngest old, identify medical and psychosocial determinants, and describe its evolution and related outcomes. METHODS/DESIGN: The Lc65+ cohort was launched in 2004 with the random selection of 3054 eligible individuals aged 65 to 70 (birth year 1934-1938) in the non-institutionalized population of Lausanne (Switzerland). The baseline data collection was completed among 1422 participants in 2004-2005 through questionnaires, examination and performance tests. It comprised a wide range of medical and psychosocial dimensions, including a life course history of adverse events. Outcomes measures comprise subjective health, limitations in activities of daily living, mobility impairments, development of medical conditions or chronic health problems, falls, institutionalization, health services utilization, and death. Two additional random samples of 65-70 years old subjects will be surveyed in 2009 (birth year 1939-1943) and in 2014 (birth year 1944-1948). DISCUSSION: The Lc65+ study focuses on the sequence "Determinants --> Components --> Consequences" of frailty. It currently provides information on health in the youngest old and will allow comparisons to be made between the profiles of aging individuals born before, during and at the end of the Second World War.

La caractérisation de l'exposition à des bioaérosols dans différents secteurs de travail a, durant de nombreuses années, été documentée principalement pour les icroorganismes cultivables tels que les bactéries et les champignons. Puis, le développement des méthodes moléculaires d'analyse de l'ADN par amplification de séquences spécifiques ou universelles par PCR(1) a permis une caractérisation beaucoup plus réaliste des expositions en tenant compte, non seulement des bactéries et champignons cultivables, mais aussi des non-cultivables qui représentent l'immense majorité des icroorganismes totaux présents dans l'air. Ainsi, depuis une dizaine d'années, nos connaissances sur les communautés bactériennes et fongiques aéroportées se sont élargies (1). Aujourd'hui, une nouvelle avancée est faite, grâce à la métagénomique(2) qui permet d'identifier (après ou non amplification de l'ADN total) une grande part des séquences d'ADN présentes dans un environnement (2). L'utilisation de la PCR(1) et/ou de la métagénomique(2), dans le domaine de la santé au travail, s'est jusqu'à présent limitée à la caractérisation de l'exposition aux bactéries et champignons laissant de côté les virus. Cependant, il s'avère que beaucoup de virus pathogènes se transmettent par voie aérienne et qu'un grand nombre de travailleurs manipulent des matières pouvant être contaminées. C'est le cas des employés des abattoirs, potentiellement exposés à des virus zoonotiques(3) et des employés d'usines de tri des déchets ménagers solides. Cette note décrit deux études qui ont évalué, pour la première fois dans ces secteurs de travail, l'exposition professionnelle à des particules virales, la première en utilisant la métagénomique(2) et la seconde en utilisant les méthodes PCR(1) classique.

Chaque hiver en France, la grippe saisonnière touche entre 2 et 7 millions de personnes, dont environ 1 000 cas mortels, surtout parmi les personnes de plus de 65 ans. Elle constitue ainsi la première cause de mortalité par maladie infectieuse. L'agent causal est un virus à ARN très contagieux, pouvant appartenir à différent type (A, B ou C). Le type A est composé d'une large gamme de sous-types classés selon les différentes sortes et associations de protéines de surface du virus. Parmi ceux-ci, les sous-types H1N1 et H3N2 circulent actuellement chez l'Homme. La transmission inter-humaine de la maladie se fait principalement par voie aéroportée par le biais des gouttelettes riches en virus provenant des accès de toux et des éternuements des sujets infectés. Le coût sanitaire et social annuel de la grippe est estimé à 460 millions d'euros pour une épidémie moyenne. La prévention de la grippe repose sur une vaccination annuelle, proposée dans la plupart des pays industrialisés aux personnes à risque. Cependant, la couverture vaccinale en France n'était que de 23 % de la population générale en 2011-2012 (62 % chez les plus de 50 ans). Dès lors, environ 80 % des individus sont susceptibles de contracter la maladie. Les transports en commun constituent des environnements idéaux pour la dissémination des virus. En effet, la grande promiscuité entre les passagers potentiellement malades et les passagers sains favorise la propagation de la maladie. Dans ces conditions, l'évaluation du risque d'infection est utile pour appliquer une prévention ciblée. Le but du premier article analysé (Gupta et al., 2012) était, précisément, d'évaluer le risque, pour un passager sain, de contracter le virus de la grippe dans un avion transportant un passager malade. Les auteurs du second article analysé (Pyankov et al., 2012) ont estimé le temps de survie de différents sous-types de virus de la grippe dans l'air ambiant d'une chambre expérimentale.

We report the case of a woman with syncope and persistently prolonged QTc interval. Screening of congenital long QT syndrome (LQTS) genes revealed that she was a heterozygous carrier of a novel KCNH2 mutation, c.G238C. Electrophysiological and biochemical characterizations unveiled the pathogenicity of this new mutation, displaying a 2-fold reduction in protein expression and current density due to a maturation/trafficking-deficient mechanism. The patient's phenotype can be fully explained by this observation. This study illustrates the importance of performing genetic analyses and mutation characterization when there is a suspicion of congenital LQTS. Identifying mutations in the PAS domain or other domains of the hERG1 channel and understanding their effect may provide more focused and mutation-specific risk assessment in this population.
