Dental injuries are often the result of direct trauma. The most affected teeth are the upper incisors, and the most frequent lesions are coronal fractures, contusions, and lip and alveolar mucosa lacerations. The objective of this study was to draw attention to the importance of the correct management of cases of crow fractures associated with soft tissue lacerations when the fragment is not located. This is a clinical case of crown fracture, the fragment of which remained lodged inside the lip. After fragment removal, the clinical case showed a satisfactory repair emphasizing the importance of a meticulous clinical examination to achieve a correct diagnosis and an appropriate treatment plan, which is essential for a favorable prognosis.

Root fractures in immature teeth are rare because the resilience of the alveolar bone is more favorable to the occurrence of luxation. This article reports a case of traumatic injury in an immature permanent tooth that progressed to root fracture, having a parafunctional oral habit as the possible modifying factor of case evolution. A 12-year-old boy presented for treatment complaining of a defective restoration and mild pain on the maxillary right central incisor. The patient had a history of crown fracture in this tooth due to trauma 2 years before. The clinical examination showed healthy gingival tissues and no abnormal tooth mobility, whereas radiographic projections revealed healthy periradicular tissues, incomplete root formation, and no visible root fracture. As pulp necrosis was diagnosed, calcium hydroxide therapy was started for canal disinfection and subsequent obturation. However, after 4 weeks of treatment, a horizontal fracture line was observed radiographically in the root's middle third. The patient denied a new traumatic injury, but revealed the habit of chewing on a pencil. Refraining from the deleterious oral habit was strongly advised, and root canal filling with mineral trioxide aggregate was performed to treat the root fracture. After 4 years of follow-up, the tooth has normal function and no abnormal mobility. Images suggestive of remodeling at the apical end of the coronal segment and replacement resorption of the apical segment are seen radiographically. This case demonstrates the need of following cases of dental trauma and the possible influence of parafunctional oral habits as modifying factors of case progression.

The aim of the present investigation was to gather data pertaining to dental trauma in 1654 patients aged 0-3 years, attended at the Baby Clinic of the School of Dentistry at Aracatuba - UNESP, Brazil. The prevalence of traumatic injuries was 16.3%. There was greater involvement of boys (62.6%), of children aged 1-2 years (39.9%) and of the maxillary central incisors (86%). Falls were more often the etiology for dental injuries (58.3%). There was a predominance of uncomplicated crown fractures (48.4%).

The purpose of the present study was to determine the influence of the type of trauma on the pulp vitality and the time elapsed until seeking dental care in children aged 0-3 years seen at the Baby Clinic of the Aracatuba Dental School, UNESP. A total of 1813 records were analyzed. Two hundred and three patients, corresponding to 302 traumatized teeth, were assessed clinically and radiographically. Hard-tissue injuries were the most frequent (52%), with a predominance of enamel crown fractures (41.4%), followed by concussions (12.6%) and intrusions (11.6%). Clinical and radiographic examination revealed that 72% of the traumatized teeth maintained pulp vitality. In the case of supporting-tissue lesions, 51.1% of the patients sought care within 1-15 days after injury, while in the case of hard-tissue injuries, 52.7% sought care only after 16 days. The results showed that supporting-tissue injuries had a significant influence on the faster seeking of dental care.

Pulp necrosis is a commonly observed sequela in traumatized primary teeth and is one of the possible etiologic factors for the development of dentigerous teeth. This article reports the case of a dentigerous cyst associated with the germ of a permanent maxillary central incisor that developed secondary to trauma to the predecessor primary incisor. The therapeutic approach included endodontic treatment of the primary tooth and marsupialization of the lesion. After 36 months of follow up, the permanent incisor presented with normal physiologic conditions, absence of dental anomalies and erupted in its correct position in the oral cavity. In conclusion, with proper case selection, marsupialization might be a good treatment option for conservative management of dentigerous cysts.

A high prevalence of dental trauma exists and its effects on function and esthetics deserve the attention of general dentists. The aim of this study was to assess the level of general dental practitioners' (GDPs) knowledge about guidelines for dental avulsion and its prevention using a questionnaire. The 21-item questionnaire was distributed among 264 GDPs and the survey was realized between August-November 2006. The data obtained were statistically analyzed using descriptive analysis and Pearson's Chi-square test to determine associations between knowledge regarding emergency treatment and dentists from public or private dental schools and years of experience. The results showed that the participants exhibited appropriate knowledge concerning procedures in cases of tooth avulsion and its prevention. The number of correct answers was low in relation to recommended treatment at the site of injury. Storage medium, preparation of the alveolus and splint time for receiving the avulsed tooth received a high number of correct answers. One statistically significant association between years of experience and recommended treatment at the site of the injury in the case an avulsed tooth (KH2 = 9.384, P = 0.009). In conclusion, this survey showed appropriate knowledge of dental avulsion management and its prevention among the surveyed dentists. The findings also showed that communication between dentists and the population is deficient, especially concerning practitioners of high risk and contact sports.

This study evaluated the influence an abdominal support attached to a traditional stool, of those used by dentists, has on the body's distribution of the electrical activity of the superior trapezius and the longissimus thoracic muscles of dental students during the execution of a clinical procedure. The results showed no significant difference in the body's distribution in the seat and backrest, but did reveal there was a weight discharge of 3.1 +/- 1.9% of dentist's body weight in the abdominal support. The 9 o'clock position proved to be the best position to perform clinical procedures. It was also observed that the position was closer to the body's axis.

FUNDAMENTO: A ausência de valores críticos para a identificação de risco cardiovascular entre adolescentes brasileiros representa uma importante limitação. OBJETIVOS: Elaborar valores críticos para circunferência de cintura e analisar sua eficiência na indicação de valores elevados de pressão arterial. MÉTODOS: Estudo transversal que avaliou 1.145 adolescentes de 11 a 17 anos (536 do sexo masculino e 609 do feminino), dos quais foram coletados valores de peso corporal, estatura, resistência, reatância, dobra cutânea tricepital, circunferência de cintura e pressão arterial (n= 334). A obesidade abdominal foi indicada por meio de valores de circunferência de cintura. RESULTADOS: Os adolescentes obesos apresentaram valores mais altos de circunferência de cintura e, independentemente de gênero e grupo etário, houve relação significativa entre os valores de circunferência de cintura e todos os indicadores de adiposidade adotados no estudo. Os valores críticos propostos apresentaram maior sensibilidade na indicação de valores elevados de pressão arterial. CONCLUSÕES: Os valores críticos propostos para circunferência de cintura foram mais sensíveis na indicação de valores elevados de pressão arterial. Entretanto, ainda são necessários estudos para averiguar a eficiência dos mesmos na indicação de outros parâmetros clínicos e laboratoriais.

FUNDAMENTO: A obesidade está ligada à hipertensão arterial (HA) na infância. Entretanto, o papel da gordura como preditor de HA em adolescentes permanece desconhecido. OBJETIVO: Investigar a associação entre obesidade geral e abdominal com HA e identificar a sensibilidade e especificidade desses indicadores para detectar HA em adolescentes. MÉTODOS: A amostra consistiu em 1.021 adolescentes com idade de 10-17 anos. Os indivíduos foram classificados como normal, sobrepeso/obesidade, de acordo com as medidas do IMC, e como não-obeso com obesidade abdominal, de acordo com as medidas da circunferência da cintura (CC). A pressão arterial sistólica (PAS) e diastólica (PAD) foi avaliada através de um dispositivo oscilométrico. Regressão logística e curvas ROC foram usadas na análise estatística. RESULTADOS: A prevalência geral de HA foi 11,8% (13,4% em meninos e 10,2% em meninas). A prevalência de HA em meninos e meninas com sobrepeso/obesidade foi 10% e 11,1%, respectivamente. A prevalência de HA em meninos com obesidade abdominal foi 28,6%. Para ambos os sexos, o odds ratio (OR) para HA foi mais alto na obesidade abdominal do que no sobrepeso/obesidade geral (4,09 [OR IC95% = 2,57-6,51]) versus 1,83 [OR IC95% = 1,83-4,30]). O OR para HA foi mais alto quando sobrepeso/obesidade geral e obesidade abdominal estavam agrupados (OR = 4,35 [OR IC95% = 2,68 -7,05]), do que quando identificados como sobrepeso/obesidade geral ou obesidade abdominal apenas (OR = 1,32 [OR IC95% = 0,65- 2,68]). Entretanto, ambos os tipos de obesidade apresentavam baixo poder preditivo na detecção de HA. CONCLUSÃO: Obesidade geral e obesidade abdominal foram associadas com HA; entretanto, a sensibilidade e especificidade dessas variáveis na detecção de HA são baixas em adolescentes brasileiros.

The objective is to alert the surgeon about the indiscriminate use of synthetic prosthesis in the correction of inguinal and incisional hernias. The authors provide a brief history of surgery on hernias and a review of the literature, showing the importance of classifying inguinal hernias to fit the type of surgical correction with the defect found, abstaining from treating all hernias, with the same type of surgical procedure. In our opinion, small indirect inguinal hernias (type 1 and 2 of Gilbert) and hernias in women must not, in general, be treated with prostheses. The synthetic material should be reserved for direct and large indirect hernias. Even so, this attitude, besides determining a higher cost for the procedure, can lead to important complications such as infection, rejection, fistula formation, chronic pain, alterations in spermatogenesis and the possibility of carcinogenesis, according to more recent reports. The physiology and anatomy of the abdominal wall should be considered when dealing with incisional hernia corrections, where the surgeon can choose among many techniques to correct those defects, and in selected cases, utilize synthetic material. We conclude that although the use of biomaterials has constituted a great advance in surgery for abdominal wall hernia corrections because they decrease recurrences, and permit treatment of large abdominal hernias, the indiscriminate prosthesis usage is an abuse, and it can determine many serious complications, certainly avoidable with a well indicated non mesh technique .

Relatam-se dois casos de miocardite em cães. A avaliação radiográfica mostrou pneumotórax e padrão alveolar no lobo diafragmático pulmonar em um cão e arritmia em ambos os cães. As análises das proteínas cardíacas altamente sensíveis, como a CK-MB e a troponina I cardíaca, associadas ao histórico clínico e aos achados eletrocardiográficos, indicaram, com acurácia, a extensão da injúria miocárdica secundária ao trauma.

O objetivo deste experimento foi comparar a sutura contínua e a sutura com pontos separados no fechamento da parede abdominal. Foram utilizados 48 ratos machos Wistar, submetidos a laparotomia com técnica de diérese padronizada, 24 submetidos a sutura da parede abdominal por técnica contínua e 24 com pontos separados, com fio polidioxanona. No 7º e 14º dia de pós-operatório foram submetidos a eutanásia 12 animais de cada grupo e deles retirados a camada músculo-fascial abdominal envolvendo a cicatriz operatória e dividida aleatoriamente em dois segmentos (cranial e caudal), um para ser submetido a avaliação da força de rotura mediante o uso de tensiômetro e outro para exame histológico, onde foi realizada a avaliação quantitativa de colágeno na linha de sutura. Os resultados encontrados foram analisados estatisticamente. Concluiu-se que no 7º dia de pós-operatório a parede abdominal suturada com pontos separados é mais resistente, porém sem diferenças significantes na quantidade de colágeno, do que a suturada por técnica contínua, e no 14º dia, ambas se equivalem nos dois parâmetros estudados, em ratos.

OBJETIVO: avaliar a influência do alcoolismo crônico experimental na morfologia e biomecânica da artéria aorta de ratos. MÉTODOS:ratos Wistar foram divididos em dois grupos de 22 animais. Grupo alcoolista: os ratos receberam água com concentração crescente de etanol e ração ad libitum durante 6 meses. Grupo controle: os animais receberam água e ração ad libitum pelo mesmo período. Os animais foram sacrificados com overdose de pentobarbital e as aortas abdominais foram excisadas para realização de histologia (hematoxilina-eosina, tricrômio de Masson, Calleja, Picrosirius red), histomorfometria e avaliação biomecânica (carga máxima, alongamento na carga máxima, limite de proporcionalidade, alongamento no limite de proporcionalidade e coeficiente de rigidez). RESULTADOS: A histologia e a histomorfometria não mostraram diferenças significantes na morfologia da aorta em ambos os grupos. A avaliação biomecânica mostrou aumento do alongamento no limite de proporcionalidade no grupo alcoolista (p<0.05). CONCLUSÃO: o alcoolismo crônico experimental não provocou alterações morfológicas na parede da aorta, mas causou aumento da sua elasticidade, sem modificar as outras propriedades mecânicas avaliadas.
