Trata-se de parte de estudo onde avalia-se o conhecimento e a prática de enfermeiros e ocupacionais de enfermagem em relação a medidas de prevenção e controle de infecções hospitalares em vinte e nove hospitais gerais, da cidade de São Paulo. Objetiva, além de avaliar o conhecimento e a prática acerca das medidas de prevenção e controle das infecções, investigar se existem diferenças entre os níveis de conhecimento e de atuação prática dos enfermeiros e ocupacionais de enfermagem, quando lotados em diferentes grupos de hospitais: hospitais públicos com serviço de prevenção e controle de infecções hospitalares; hospitais públicos sem serviço de prevenção e controle de infecções hospitalares; hospitais privados com serviço e privados sem o referido serviço. Constata-se que existem lacunas no que se refere ao conhecimento e a prática acerca das medidas de prevenção e controle das infecções estudadas e que embora se detectem diferenças significativas para o conhecimento e a prática dos profissionais e ocupacionais de enfermagem, quando lotados em diferentes grupos de hospitais, a análise de perguntas isoladas nem sempre distingue, com significância, os mesmos grupos.

BACKGROUND: In our hands, in vivo segmental vessel length changes up to 5% because of blood pressure: increasing in arterial pressure is associated to decrease in segmental vessel length. METHODS AND MATERIAL: Using two piezoelectric crystals sutured on vessel wall and a high fidelity pressure probe, we recorded artery length variations as function of blood pressure, before and after an end-to-end anastomosis on four pigs carotid arteries. RESULTS: Mean arterial pressure before anastomosis = 73 mmHg (+/- 12); mean arterial pressure after anastomosis = 91 mmHg (+/- 14); mean crystals displacement before anastomosis during systole = -0.21 mm; mean crystals displacement after anastomosis during systole = +0.24 mm; mean distance between crystals before anastomosis = 12.3 mm (+/- 0.8) and after anastomosis = 11.2 mm (+/- 0.5). CONCLUSIONS: In the acute phase following an end-to-end anastomosis, an increase in blood pressure causes increasing in vessel length, with an exponential correlation. The anastomosis is constantly subjected to a longitudinal traction whose magnitude depends on blood pressure.

Este artigo de atualização discute os riscos para o desenvolvimento de úlceras em membros inferiores a que os pacientes com diabetes mellitus estão expostos, ao longo da vida. A patogênese das úlceras diabéticas é enfocada com o objetivo de dar subsídios ao enfermeiro para avaliar a capacidade de manutenção da integridade cutânea e adequar as medidas preventivas necessárias.

Mothers' general anxiety, anxiety about the well-being of the child and psychological stress before prenatal testing was studied by comparing women who conceived through in vitro fertilization (IVF) or intracytoplasmic sperm injection (ICSI) with women who conceived naturally. Before the first trimester screening test for Down's syndrome, a group of 51 women who conceived through IVF/ICSI and a group of 54 women who conceived spontaneously completed the State Scale of the State-Trait Anxiety Inventory (S-Anxiety; Spielberger, 1983), the Fear of Bearing a Physically or Mentally Handicapped Child Subscale of the Pregnancy-related Anxiety Questionnaire (PRAQ-R; Huizink et al., 2004), the Psychological Stress Measure (PSM; Lemyre & Tessier, 1988), and the Prenatal Psychosocial Profile (PPP; Curry, Campbell, & Christian, 1994). Women who conceived through IVF/ICSI had more elevated levels of general anxiety and psychological stress than the women who conceived naturally; however, no difference was observed between the two groups for anxiety specifically related to the health of the child. These results underline the need to monitor women's emotional state after conception via IVF/ICSI-when counseling usually ends-and around the time of the first trimester screening. Counseling might thus be extended.

Uma das complicações mais severas para a pessoa com lesão medular é a úlcera de pressão. Sendo assim a prevenção destas feridas e a identificação de fatores de risco são necessidades prementes para o enfermeiro. Trata-se de um estudo decorrente de uma pesquisa mais abrangente, que abordou a assistência de enfermagem ao lesado medular em reabilitação buscando identificar os fatores de risco para úlcera de pressão utilizando os registros das enfermeiras nos históricosde enfermagem e a Taxonomia I da NANDA e analisar as intervenções de enfermagem preconizados face ao diagnóstico potencial para prejuízo na integridade da pele. Os fatores de risco identificados referem-se às alterações decorrentes da lesão neurológica, história prévia de ferida e lesão inicial. As intervenções de enfermagem preconizadas totalizaram. 16, com caráter de orientação ao paciente e à família voltadas para higiene corporal, mudança de decúbito, posicionamento no leito e em cadeira de rodas, denotando pertinência ao diagnóstico elaborado.

Objective: Converging evidence speak in favor of an abnormal susceptibility to oxidative stress in schizophrenia. A decreased level of glutathione (GSH), the principal non-protein antioxidant and redox regulator, was observed both in cerebrospinal-fluid and prefrontal cortex of schizophrenia patients (Do et al., 2000). Results: Schizophrenia patients have an abnormal GSH synthesis most likely of genetic origin: Two independent case-control studies showed a significant association between schizophrenia and a GAG trinucleotide repeat (TNR) polymorphism in the GSH key synthesizing enzyme glutamate-cysteine-ligase (GCL) catalytic subunit (GCLC) gene. The most common TNR genotype 7/7 was more frequent in controls, whereas the rarest TNR genotype 8/8 was three times more frequent in patients. The disease-associated genotypes correlated with a decrease in GCLC protein expression, GCL activity and GSH content. Such a redox dysregulation during development could underlie the structural and functional anomalies in connectivity: In experimental models, GSH deficit induced anomalies similar to those observed in patients. (a) morphology: In animal models with GSH deficit during the development we observed in prefrontal cortex a decreased dendritic spines density in pyramidal cells and an abnormal development of parvalbumine (but not of calretinine) immunoreactive GABA interneurones in anterior cingulate cortex. (b) physiology: GSH depletion in hippocampal slices induces NMDA receptors hypofunction and an impairment of long term potentiation. In addition, GSH deficit affected the modulation of dopamine on NMDA-induced Ca 2+ response in cultured cortical neurons. While dopamine enhanced NMDA responses in control neurons, it depressed NMDA responses in GSH-depleted neurons. Antagonist of D2-, but not D1-receptors, prevented this depression, a mechanism contributing to the efficacy of antipsychotics. The redox sensitive ryanodine receptors and L-type calcium channels underlie these observations. (c) cognition: Developing rats with low [GSH] and high dopamine lead deficit in olfactory integration and in object recognition which appears earlier in males that females, in analogy to the delay of the psychosis onset between man and woman. Conclusion: These clinical and experimental evidence, combined with the favorable outcome of a clinical trial with N-Acetyl Cysteine, a GSH precursor, on both the negative symptoms (Berk et al., submitted) and the mismatch negativity in an auditory oddball paradigm supported the proposal that a GSH synthesis impairment of genetic origin represent, among other factors, one major risk factor in schizophrenia.

Male rats were subjected to "psychological stress" which consisted in 10 sec footshock on the first day followed 24 hr later by a 10 sec stay in the experimental chamber without shock. Intravenous antiserum against alpha-MSH markedly changed the functional state of mesencephalic and hypothalamic DA neurons (assessed by histochemical microfluorimetry) when administered before the second session but not when given before the first session. These observations reveal an interesting parallelism in the temporal characteristics of the effects of alpha-MSH on avoidance behavior and central DA systems.

O estudo elaborou critérios para uso adequado da paramentação cirúrgica e avaliou, por observação sistematizada, o desempenho da equipe cirúrgica durante a cirurgia. O procedimento adotado foi válido, permitindo reconhecer aspectos positivos e problemáticos. A exceção do óculos protetor, todos os outros componentes da paramentação foram utilizados. A questão se situou na forma de utilização, onde gorros e máscaras foram mais problemáticos entre anestesistas e circulantes; avental e luvas entre cirurgiões e, principalmente, instrumentadores.

O estudo elaborou critérios de avaliação dos componentes da paramentação cirúrgica de hospitais do município de São Paulo. A maior dificuldade nessa elaboração deveu-se à inexistência de normas e consenso sobre a porosidade dos tecidos como barreira microbiológica e de contato com sangue e outros fluidos. Contudo, foi possível reconhecer aspectos positivos e problemáticos. Observou-se que existe semelhanças dos componentes entre os hospitais, no que se refere a modelo e tipo de tecido. As luvas foram os componentes com maior índice de adequação e os propés, os de maior índice de inadequação. O aspecto mais problemático referiu-se aos controles de aquisição e de reprocessamento dos componentes re-utilizáveis.

Epidemiological studies have demonstrated that exposure to fine particles is associated to adverse health effects, including cancer, respiratory and cardiovascular diseases. However, mechanisms by which particles induce health effects remain unclear. According to one of the most investigated hypotheses, particles cause adverse effects through the production of reactive oxygen species (ROS), which are very hazardous compounds able to attack directly biological structures, including the DNA strand or the lipid bilayer of the cells. If the defense mechanisms, constituted of antioxidants, are not able to counter ROS, then these compounds will cause in the body a range of oxidation reactions called "oxidative stress". The aim of the present research project was to better understand mechanisms by which exposure to fine particles induces oxidative stress. The first point of this project was to check whether exposure to high levels of fine particles is directly linked to oxidative stress, and whether this oxidative stress is accompanied by the activation of the defense mechanisms (antioxidants). The second point was to study the role played by the particle surface characteristics in the oxidative stress process. For that purpose, a study was conducted in bus depots with the participation of 40 mechanics. First, occupational exposure to particles (PM4) and to other pollutants (NOx, O3) was measured over a two-day period. Then, urine samples of mechanics were collected in order to measure levels of 8-hydroxy-2'-deoxyguanosine (8OHdG) and antioxidants. 8OHdG is a molecule formed by the oxidation of DNA and allowing to assess the oxidative stress status of the mechanics. Finally, particles were collected on filters, and functional groups located on the particle surface were analyzed in the laboratory using a Knudsen flow reactor. This technique allows not only to quantify functional groups on the particle surface, but also to measure the reaction kinetics. Results obtained during the field campaign in bus depots showed that mechanics were exposed to rather low levels of PM4 (20-85 μg/m3) and of pollutants (NOx: 100-1000 ppb; O3: <15 ppb). However, despite this low exposure, urinary levels of the oxidative stress biomarker (8OHdG) increased significantly for non-smoking workers over a two-day period of shift. This oxidative stress was accompanied by an increase of antioxidants, indicating the activation of defense mechanisms. On the other hand, the analysis of functional groups on the particle surface showed important differences, depending on the workplace, the date and the activities of workers. The particle surface contained simultaneously antagonistic functional groups which did not undergo internal reactions (such as acids and bases), and was usually characterized by a high density of carbonyl functions and a low density of acidic sites. Reaction kinetics measured using the Knudsen flow reactor pointed out fast reactions of oxidizable groups and slow reactions of acidic sites. Several exposure parameters were significantly correlated with the increase of the oxidative stress status: the presence of acidic sites, carbonyl functions and oxidizable groups on the particle surface; reaction kinetics of functional groups on the particle surface; particulate iron and copper concentrations; and NOx concentration.

Relata a comparação do nível de stress no desempenho das atividades do enfermeiro de instituição hospitalar em unidades consideradas abertas e fechadas. Constatou-se que os enfermeiros que atuam em unidades abertas obtiveram maior nível de stress do que aqueles que trabalham em unidades fechadas e praticamente a atuação relacionada à administração de pessoal foi considerada estressante para a totalidade de enfermeiros. E necessária a educação do enfermeiro para minimizar esses fatores estressantes.

Role of GLP-1 and GIP in beta cell compensatory responses to beta cell attack and insulin resistance were examined in C57BL/6 mice lacking functional receptors for GLP-1 and GIP. Mice were treated with multiple low dose streptozotocin or hydrocortisone. Islet parameters were assessed by immunohistochemistry and hormone measurements were determined by specific enzyme linked immunoassays. Wild-type streptozotocin controls exhibited severe diabetes, irregularly shaped islets with lymphocytic infiltration, decreased Ki67/TUNEL ratio with decreased beta cell and increased alpha cell areas. GLP-1 and GIP were co-expressed with glucagon and numbers of alpha cells mainly expressing GLP-1 were increased. In contrast, hydrocortisone treatment and induction of insulin resistance increased islet numbers and area, with enhanced beta cell replication, elevated mass of beta and alpha cells, together with co-expression of GLP-1 and GIP with glucagon in islets. The metabolic responses to streptozotocin in GLP-1RKO and GIPRKO mice were broadly similar to C57BL/6 controls, although decreases in islet numbers and size were more severe. In contrast, both groups of mice lacking functional incretin receptors displayed substantially impaired islet adaptations to insulin resistance induced by hydrocortisone, including marked curtailment of expansion of islet area, beta cell mass and islet number. Our observations cannot be explained by simple changes in circulating incretin concentrations, suggesting that intra-islet GLP-1 and GIP make a significant contribution to islet adaptation, particularly expansion of beta cell mass and compensatory islet compensation to hydrocortisone and insulin resistance.

Este estudo avaliou o stress associado ao trabalho do enfermeiro no trabalho com o portador de transtorno mental inserido no contexto do hospital psiquiátrico. Trata-se de pesquisa quantitativa, analítica, realizada em sete hospitais psiquiátricos da cidade de Fortaleza, Ceará. Utilizou-se os seguintes instrumentos: dados sociodemográficos para descrição da amostra e um inventário para identificar os sintomas de stress no enfermeiro. A população total abrangeu 48 participantes e a amostra 42, a maioria do sexo feminino, o que corresponde a 92,9% do total. Os resultados mostram que em 62% da amostra não apresentaram stress, 30,9% encontravam-se na fase de Resistência e apenas 7,1% na fase de Exaustão. Não houve associação estatisticamente significativa entre a variável tempo de trabalho com o portador de transtorno mental e sintomas de stress. Portanto, o presente estudo não evidenciou a ocorrência do stress nesses profissionais enfermeiros.
