The only breeding record of Spartonoica maluroides (d'Orbigny & Lafresnaye, 1837) for Brazil is based on the observation of a fledgling in southern Rio Grande do Sul in January 1976. On 7 December 2005 we discovered a nest containing three nestlings at the southeastern end of Lagoa Pequena, municipality of Pelotas, Rio Grande do Sul. The nest was concealed at the base of a cavity in a Spartina densiflora (Poaceae) tussock located at the edge of a saltmarsh. The nest was built of fine pieces of dead Scirpus olneyi (Cyperaceae) and S. densiflora leaves firmly interlaced to the internal leaves of the tussock. Live leaves of S. densiflora lining the cavity comprised a substantial part of the nest's architecture, forming most of its upper lateral walls and roof. The lower section was more elaborate, resembling a deep cup and forming a distinct incubation chamber. Adults reached the nest's interior through an irregular apical opening amidst the leaves. The nest was 244 mm high and 140 mm wide. The incubation chamber had an external diameter of 138.5 mm, an internal diameter of 79.4 mm and was 86 mm deep. It was lined with fine leaves and white plant fibers. Nestlings were five to six days old. A total of 107 neossoptiles restricted to the capital, spinal and alar tracts were recorded in one nestling. The distribution of neossoptiles in the ocular region of S. maluroides forms a distinct pattern which can be typical of Furnariidae and related families. Two adults attended the nest, bringing small insects to the nestlings and removing fecal sacs. We recorded at least 74 visits to the nest during a ca. 6 h period during an afternoon. The average number of visits per hour was 12.8 ± 1.3. An adult bird spent on average 0.7 ± 0.56 minutes inside the nest attending nestlings. The nest remained unattended on average for 3.61 ± 3.13 minutes. The hour of the day had no influence on the amount of time spent by an adult in the nest or away from it. We returned to the area on 15 December 2005 and found the nest abandoned. Observations confirm that S. maluroides is a resident breeder in southern Brazil and that the saltmarshes of the Lagoa do Patos estuary are an important year-round habitat for the species. A nestling and the nest were collected to document the record.

Apesar de machos e fêmeas de serpentes nascerem com o mesmo tamanho, as taxas de crescimento e a idade da maturação sexual podem ser diferentes, determinando dimorfismo sexual em estágios posteriores da vida. Avaliamos a ocorrência de variação morfométrica sexual e ontogenética em Bothropoides jararaca (Wied, 1824), explorando as relações entre tamanho corporal e amadurecimento sexual através de 14 variáveis morfométricas. Foram analisados 142 espécimes provenientes do estado do Rio Grande do Sul, sul do Brasil. Os dados morfométricos - comprimento da cabeça, rostro-cloacal, da cauda, comprimento total; largura da cabeça, ocular, nasal, loreal, da cauda; distância ocular-nasal, ocular-loreal, loreal-nasal, ventral-sinfisal e rostral-labial - e comprimento/diâmetro dos folículos ovarianos, foram tomados em milímetros, através de régua simples e paquímetro analógico de precisão 0,05 mm. A determinação sexual foi realizada por inspeção das gônadas. A classificação etária foi associada à maturidade sexual. Para as análises estatísticas foram utilizadas análises de variância (ANOVA) com teste de Tukey post hoc, regressão linear e análise discriminante canônica (ADC). A maioria das medidas indicou dimorfismo sexual (ANOVA, P<0,05) apenas em adultos (Tukey, P<0,05). As análises de regressão mostram que o comprimento rostro-cloacal explica o comportamento das demais variáveis (P<0,001) e que em todas as medidas as fêmeas crescem mais que os machos. A ADC foi exitosa em separar as classes sexuais e etárias, apresentando significado biológico, considerando 79,2% dos casos como corretamente classificados.

Os autores analisam, dentro do campo da higiene industrial, as condições sanitárias de um grande estabelecimento de artes gráficas no Rio de Janeiro — a Imprensa Nacional, o regime de trabalho aí instituído; e revistam, um a um, os processos industriais executados, especialmente atenção às causas potenciais de acidentes, que aí existem, e também àquelas cuja responsabilidade já se positivou. Ressaltam, no ponto de vista doutrinário, os diversos fatores que podem interferir na ocorrência dêsses infortúnios de trabalho; e, mostrando como tem sido superestimada a culpabilidade das máquinas, realçam a importância dos fatores individuais — idade e experiência do operário, doenças e defeitos físicos, atitude mental, constituição fisio-psicologica. Salientam como a evidência da predisposição a acidentes, que se reconheça existir, possibilita manter os operários, já em função ou pretendentes a emprego, debaixo de supervisão rigorosa; e oportuniza mesmo colocá-los melhormente, em tarefas que, no máximo, só lhes possam acarretar aciden¬tes de natureza leve. Mostram que, no período em estudo — 1942 a 1945, houve ao todo 1.123 acidentes na I.N., com o coeficiente geral de incidência de 220.3 por 1.000 operários expostos. O coeficiente de acidentes médios ou graves foi de 32 por mil, no mesmo período. Apontando ainda a descenção, no quatriênio, das curvas relativas tanto aos acidentes totais, como aos médios ou graves e aos produzidos por máquinas, dão a ver que a taxa, que retrata essa descida, é, para os primeiros, 3.5 e cêrca de 10 vêzes maior, respectivamente, que a dos acidentes por máquinas e dos médios ou graves. Mostra-se significativa a diferença, entre a tendência observada nas curvas dos acidentes em geral e dos médios ou graves; já isso, porém, não acontece, quanto à diferença entre a primeira e a dos acidentes produzidos por máquinas. De fato, sendo os valores de t respec¬tivamente, dc 2.987 e 2.333, só o primeiro fica, na tabela de Fisher, acima do nível de significância P = 5%, para v = 4 graus de liberdade (2.776). Na base dos coeficientes de incidência por 1.000 operários, em cada uma das seções da Imprensa Nacional, verifica-se terem sido eles mais elevados nas oficinas de mecânica e carpintaria e na turma de eletricidade; já quanto a oficinas pròpriamente gráficas, mostram-se mais altos os coeficientes nas de rotogravura, estereotipia e impressão. Em 8 das 14 seções, depois de baixa pronunciada do primeiro para o segundo ano e deste para o terceiro, alteou-se, no fim do período (1945), a curva de acidentes. Só na oficina mecânica, veio ela sempre em declínio. Levando em conta a severidade dos acidentes, verifica-se que, no período em estudo, os coeficientes relativos aos acidentes médios ou graves, calculados sobre 1.000 operários expostos, foram mais altos nas oficinas auxiliares de mecânica e carpintaria, seguidos, também aqui, dos ocorridos na rotogravura e estereotipia. Analisando os acidentes pelas suas causas imediatas, apontam os auto¬res ter tocado, no período e em qualquer dos quatro anos, às máquinas, a maior responsabilidade; seguiram-se-lhes os instrumentos e objetos traumatizantes, as quédas e choques. Todavia. na base do percentual dos acidentes médios ou graves sobre o total respectivo. regista-se haver contraste nítido, respeito à severidade, entre um primeiro grupo de causas de acidentes, compreendendo o esforço, os corpos estranhos (dada a sua frequente localização ocular) e os agentes causadores de queimaduras e um segundo grupo, englobando as de¬mais causas, salvo as máquinas, que ficam de permeio. A responsabilidade dos corpos estranhos e das queimaduras, como causas de acidentes de severidade, mostra o cuidado que devem merecer os recursos de proteção individual para os operários. Tabulando por meses, e na base de horas de trabalho, os acidentes ocorridos, verifica-se que a maior taxa corresponde ao verão (146.2 por 1.000 horas de trabalho), seguindo-se-lhe o outono (122.0), vindo depois o inverno e a primavera, nesta ordem. Não sendo, no Rio de Janeiro, cidade situada na zona tropical, tão nítida. como em outras de clima temperado, a diferenciação estacional, tomaram os autores para comparação os quadrimestres mais quente (dezembro a marco) e mais frio (junho a setembro), e verificaram que a taxa de acidentes no primeiro, de 136.8 por 1.000 horas de trabalho, contrasta com a relativa ao segundo período — 125.4. Levando apenas em conta os acidentes médios ou graves, não mais se aponta a diferença verificada: tocam, realmente, aos dois quadrimestres, as taxas de 17.2 e 19.7. É muito nítida, porém, a variação no pertinente aos acidentes leves - 119.6 e 105.8, sôbre os quais, segundo Osborne e Vernon, parece clara a influência da temperatura ambiente: de fato, nas verificações desses investigadores, a frequência relativa de acidentes subiu de 102 para 122, quando a temperatura ascendeu de 20°4 C a 25°2 C. No Rio de Janeiro, a temperatura media, no quadrimestre mais quente, foi dc 25°. 2 C e, no mais frio, de 20°.4 C, idênticos os valores aos dos dados de Osborne e Vernon, como pràticamente também o são as taxas de acidentes leves, aqui e ali observadas em função de tais variações. Não se mostrando de todo satisfatórios os dados de 123 das 209 fotometrias realizadas na Imprensa Nacional (59%), apontam os autores tornar-se difícil aquilatar a responsabilidade exata da deficiência de iluminamento, nos acidentes ocorridos. Salientam, todavia, que, mau grado a construção do edifício e as instalações de iluminação artificial terem sido projetadas por técnicos nesses assuntos, a verdade é que os esperados benefícios se viram prejudicados: em parte, pela colocação, nem sempre a mais satisfatória, de máquinas e operários: também, porque muitos dos pontos de luz artificial estão inaproveitados e, ainda, por não ter sido feita de maneira adequada a substituição de unidades defeituosas. Há a consignar, ademais, o descuido, por vezes grande, na limpeza dessas unidades e das superfícies transmissoras de luz natural e a ocorrência de ofuscamento para os operários em diversas situações...

A new tick, Amblyomma parkeri, n. sp., is described as a parasite of Coendu sp. from S. Paulo, Brazil. Female holotype, nymph and larva are described (Figs. 2 e 3). The n. sp. differs completly from Koch's species Amblyomma longirostre, the common parasite of the Erethizotidae. Standard data for measures of the female dorsal scutum ixodidae are proposed as follows (fig. 1): PA = Antero-posterior; PB = Postero-basal; PM = Postero-median; TT = Transversal; OO = Inter-ocular; OT= Occulo-transversal; SS = Inter-scapular; CC = Cervical; PT = Postero-transversal; ST = Scapulo-transversal; NPT = Normal to the postero-transversal; NST = Normal to the scapulo-transversal. In the female holotype the standard data are as follows: PA = 2.00 mm; PB = 2.26 mm; PM = 1.10 mm; TT = 2.20 mm; OO = 2.26 mm; SS = 0,84 mm; CC = 0.63 mm; SC = 0.12 mm; NPT = 0.20mm; STN = 0.1 mm. Peritrema 0.80 x 0.42 mm with a narrow postero-internal angle and a large, elongated macula. Coxa I with two short spines and all other coxae with only one shorter spine, shortest in coxa IV. Hypostoma spatulated with formula 3/3. Gnathosoma 1.42 mm long and basis 0.63 mm long by 0.84 greatest wide. Palpi with smoth external surface, 1.00 mm long. Type lot No. 4458 from Cotia, S. Paulo, Brazil; in the acarological collection of the Escola Paulista de Medicina, S. Paulo. Ixodes didelphidis, n. sp., differing from Ixodes loricatus Neumann by the shape of the peritremata (figs. 4 a 5) of the male and female and by the number of the punctations in this organ is described form Didelphidae, Muridae and Cavidae. Twenty eight lots were obtained from Anápolis, Goiás, Brasil, where I. loricatus is subtituded by the n. sp. under description. Comparison with NEUMANN'S types of I. loricatus was possible through the courtesy of Prof. A. BRIZARD from Toulouse, who kindly loaned NEUMANN'S material. Female cotypes N° 40 and male allotype N° 531 in the Collection of Ixodidae of the Oswaldo Cruz Institute.

Transmission of Chagas disease is realized through contamination of ocular conjunctiva, mucosa or skin with infected dejections eliminated by the insect vectors of Schizotrypanum cruzi (Triatoma infestans, Panstrongylus megistus and Rhodnius prolixus). The triatomid bugs live in holes and craks in the walls, in beds, behind trunks, etc. Found in primitive mud huts covered with thatched roofs, and so the human dwellers have many chances to contract the disease, reinfections being reasonably more to expect than a single inoculation. Experimental work reproducing those natural conditions is welcomed as some important features in the pathologic picture of the disease such as the extensive myocardial fibrosis seen in chronic cases are still incompletely known. Microscopic changes were studied in the heart muscle of seven Cebus monkeys infected by S. cruzi. This animal survives the acute stage of the disease and so is particularly suited to experiments of long duration in which several inoculations of S. cruzi are performed. Three different strains of S. cruzi isolated from acute cases of Chagas' disease were employed. One monkey was injected in the skin with infected blood and necropsied after 252 days. Two monkeys were three times, and one, eight times infected in skin, one of them with contaminated blood, and two with contaminated blood and dejections from infected bugs. The necropsies were performed after 35, 95 and 149 days. One monkey was three times inoculated through the intact ocular conjunctiva (one time with infected blood, two times with dejections from infected bugs), and one time through the wounded buccal mucosa, and necropsied after 134 days. Another monkey was six times inoculated, four times through the intact ocular conjunctiva (one time with contaminated blood, three times with dejections from infected bugs) and two times injected in the skin with infected blood, and necropsied after 157 days. Finally, another monkey was nine times inoculated, four times through the intact ocular conjunctiva (one time with infected blood, and three times with dejections from infected bugs), and five times injected in the skin (four times with contaminated blood, and one time with dejections from infected bugs), and necropsied after 233 days. The microscopic picture was uniform presenting, however, considerable individual variations, and was represented by diffuse interstitial myocarditis, frequently more (marked in the right ventricle base of the heart), accompanied by lymphatic stasis. The infiltration consists of macrophages, plasma cells and lymphocytes, the cellular reaction having sometimes a perivascular distribution, involving the auriculo-ventricular system of conduction, endocardium, epicardium and cardiac sympathetic gangliae. The loss of cardiac muscle fibers was always minimal. Leishmanial forms of S. cruzi in myocardial fibers are scanty and, in two cases, absent. Fatty necrosis in the epicardium was noted in two cases. Obliterative changes of medium-sized branches of coronary arteries (hypersensitivity reaction?) and multiple infarcts of the myocardium was found in one instance. The diffuse myocarditis induced by S. cruzi in several species of monkeys of the genus Cebus observed after 233 days (several inoculations) and 252 days (single inoculation) is not associated with disseminated fibrosis such as is reported in chronic cases of Chagas' disease. Definite capacity of reversion is another characteristic of the interstitial myocarditis observed in the series of Cebus monkeys here studied. The impression was gained that repeated inoculation with S. cruzi may influence the myocardial changes differently according to the period between the reinoculations. A short period after the first inoculation is followed by more marked changes, while long periods are accompanied by slight changes, which suggests an active immunisation produced by the first inoculation. More data are required, however before a definite statement is made on this subject considering that individual variations, the natural capacity of reversion of the interstitial myocarditis and the employement of more than a species of Cebus monkeys probably exerts influence also in the results here reported.

Estudio elaborado a partir de una estancia en la Universidad de Rochester, Estados Unidos, de octubre del 2006 a enero del 2007. La estancia realizada en la Universidad de Rochester estuvo orientada al aprendizaje en profundidad del oftalmoscopio láser de barrido. El oftalmoscopio láser de barrido emplea una técnica confocal con la finalidad de visualizar diferentes estructuras retinianas en seres vivos. El instrumento diseñado y desarrollado en el Centro de Ciencias de la Visión incorpora un sistema de óptica adaptativa y fluorescencia. La óptica adaptativa aplicada en este oftalmoscopio tiene como objetivo corregir las aberraciones existentes en el ojo y así permitir observar detalles de la retina que de otra forma se verían emborronados. De esta forma se consigue alcanzar valores de resolución muy cercanos a los impuestos por difracción. Por otro lado el uso de fluorescencia tiene por objetivo el permitir la visualización de células y estructuras que, de no ser teñidas, son transparentes a la luz y visible. Esta técnica se ha estado utilizando principalmente en primates y ratas, aunque actualmente también se están llevando a cabo medidas de células de epitelio pigmentario en seres humanos ya que el pigmento contenido en estas células permite la aplicación de la fluorescencia sin necesidad de utilizar tinción.

A 67-year-old woman was referred for staging of a mucosa-associated lymphoid tumor lymphoma involving the left conjunctiva. CT scan had shown paravertebral and pelvic masses, and a breast nodule. FDG PET/CT demonstrated moderately increased uptake in the left ocular conjunctiva and confirmed the paravertebral and pelvic masses and the breast nodule. Moreover, abnormal FDG uptake was shown in 2 breast nodules, the flank, the gluteus maximus, and the gastric cardia. The patient received 6 cycles of rituximab-bendamustine chemotherapy with a complete clinical and metabolic response at the 6-month follow-up PET/CT and remained relapse-free without visual acuity problem after a 36-month follow-up.

Soft tissue sarcomas (STS) with complex genomic profiles (50% of all STS) are predominantly composed of spindle cell/pleomorphic sarcomas, including leiomyosarcoma, myxofibrosarcoma, pleomorphic liposarcoma, pleomorphic rhabdomyosarcoma, malignant peripheral nerve sheath tumor, angiosarcoma, extraskeletal osteosarcoma, and spindle cell/pleomorphic unclassified sarcoma (previously called spindle cell/pleomorphic malignant fibrous histiocytoma). These neoplasms show, characteristically, gains and losses of numerous chromosomes or chromosome regions, as well as amplifications. Many of them share recurrent aberrations (e.g., gain of 5p13-p15) that seem to play a significant role in tumor progression and/or metastatic dissemination. In this paper, we review the cytogenetic, molecular genetic, and clinicopathologic characteristics of the most common STS displaying complex genomic profiles. Features of diagnostic or prognostic relevance will be discussed when needed.

Lyme disease has been associated with many systemic and ocular complications. The authors' patient, a 26-year-old man, developed recurrent pars planitis with two episodes of acute pericarditis. Extensive medical investigations were negative except for a highly positive western blot for Borrelia burgdorferi. Specific antibiotic treatment for Lyme disease was followed by a long lasting period without any relapse.

Intravitreal administration has been widely used since 20 years and has been shown to improve the treatment of diseases of the posterior segment of the eye with infectious origin or in edematous maculopathies. This route of administration allows to achieve high concentration of drug in the vitreous and avoids the problems resulting from systemic administration. However, two basic problems limit the use of intravitreal therapy. Many drugs are rapidly cleared from the vitreous humor; therefore, to reach and to maintain effective therapy repeated injections are necessary. Repeated intravitreal injections increase the risk of endophthalmitis, damage to lens, retinal detachment. Moreover, some drugs provoke a local toxicity at their effective dose inducing side-effects and possible retinal lesions. In this context, the development and the use of new drug delivery systems for intravitreal administration are necessary to treat chronic ocular diseases. Among them, particulate systems such as liposomes have been widely studied. Liposomes are easily injectable and permit to reduce the toxicity and to increase the residence time of several drugs in the eye. They are also able to protect in vivo poorly-stable molecules from degradation such as peptides and nucleic acids. Some promising results have been obtained for the treatment of retinitis induced by cytomegalovirus in human and more recently for the treatment of uveitis in animal. Finally, the fate of liposomes in ocular tissues and fluids after their injection into the vitreous and their elimination routes begin to be more known.

The aim of this study is to quantify the prevalence and types of rare chromosome abnormalities (RCAs) in Europe for 2000-2006 inclusive, and to describe prenatal diagnosis rates and pregnancy outcome. Data held by the European Surveillance of Congenital Anomalies database were analysed on all the cases from 16 population-based registries in 11 European countries diagnosed prenatally or before 1 year of age, and delivered between 2000 and 2006. Cases were all unbalanced chromosome abnormalities and included live births, fetal deaths from 20 weeks gestation and terminations of pregnancy for fetal anomaly. There were 10,323 cases with a chromosome abnormality, giving a total birth prevalence rate of 43.8/10,000 births. Of these, 7335 cases had trisomy 21,18 or 13, giving individual prevalence rates of 23.0, 5.9 and 2.3/10,000 births, respectively (53, 13 and 5% of all reported chromosome errors, respectively). In all, 473 cases (5%) had a sex chromosome trisomy, and 778 (8%) had 45,X, giving prevalence rates of 2.0 and 3.3/10,000 births, respectively. There were 1,737 RCA cases (17%), giving a prevalence of 7.4/10,000 births. These included triploidy, other trisomies, marker chromosomes, unbalanced translocations, deletions and duplications. There was a wide variation between the registers in both the overall prenatal diagnosis rate of RCA, an average of 65% (range 5-92%) and the prevalence of RCA (range 2.4-12.9/10,000 births). In all, 49% were liveborn. The data provide the prevalence of families currently requiring specialised genetic counselling services in the perinatal period for these conditions and, for some, long-term care.

CAP1/Prss8 is a membrane-bound serine protease involved in the regulation of several different effectors, such as the epithelial sodium channel ENaC, the protease-activated receptor PAR2, the tight junction proteins, and the profilaggrin polypeptide. Recently, the V170D and the G54-P57 deletion mutations within the CAP1/Prss8 gene, identified in mouse frizzy (fr) and rat hairless (fr(CR)) animals, respectively, have been proposed to be responsible for their skin phenotypes. In the present study, we analyzed those mutations, revealing a change in the protein structure, a modification of the glycosylation state, and an overall reduction in the activation of ENaC of the two mutant proteins. In vivo analyses demonstrated that both fr and fr(CR) mutant animals present analogous reduction of embryonic viability, similar histologic aberrations at the level of the skin, and a significant decrease in the activity of ENaC in the distal colon compared with their control littermates. Hairless rats additionally had dehydration defects in skin and intestine and significant reduction in the body weight. In conclusion, we provided molecular and functional evidence that CAP1/Prss8 mutations are accountable for the defects in fr and fr(CR) animals, and we furthermore demonstrate a decreased function of the CAP1/Prss8 mutant proteins. Therefore, fr and fr(CR) animals are suitable models to investigate the consequences of CAP1/Prss8 action on its target proteins in the whole organism.

BACKGROUND: In myasthenia gravis, antibody-mediated blockade of acetylcholine receptors at the neuromuscular junction abolishes the naturally occurring 'safety factor' of synaptic transmission. Acetylcholinesterase inhibitors provide temporary symptomatic treatment of muscle weakness but there is controversy about their long-term efficacy, dosage and side effects. This is the second update of a review published in The Cochrane Library Issue 2, 2011. OBJECTIVES: To evaluate the efficacy of acetylcholinesterase inhibitors in all forms of myasthenia gravis. SEARCH METHODS: On 8 July 2014 we searched the Cochrane Neuromuscular Disease Group Specialized Register, CENTRAL, MEDLINE and EMBASE for randomised controlled trials and quasi-randomised controlled trials regarding usage of acetylcholinesterase inhibitors in myasthenia gravis. Two authors scanned the articles for any study eligible for inclusion. We also contacted the authors and known experts in the field to identify additional published or unpublished data and searched clinical trials registries for ongoing trials. SELECTION CRITERIA: The types of studies were randomised or quasi-randomised trials. Participants were myasthenia gravis patients diagnosed by an internationally accepted definition. The intervention was treatment with any form of acetylcholinesterase inhibitor. Types of outcome measures Primary outcome measureImprovement in the presenting symptoms within one to 14 days of the start of treatment. Secondary outcome measures(1) Improvement in the presenting symptoms more than 14 days after the start of treatment.(2) Change in impairment measured by a recognised and preferably validated scale, such as the quantitative myasthenia gravis score, within one to 14 days and more than 14 days after the start of treatment.(3) Myasthenia Gravis Association of America post-intervention status more than 14 days after start of treatment.(4) Adverse events including muscarinic side effects. DATA COLLECTION AND ANALYSIS: One author (MMM) extracted the data, which were checked by a second author. We contacted study authors for extra information and collected data on adverse effects from the trials. MAIN RESULTS: We did not find any large randomised or quasi-randomised trials of acetylcholinesterase inhibitors in generalised myasthenia gravis either for the first version of this review or this update. One cross-over randomised trial using intranasal neostigmine in a total of 10 participants was only available as an abstract. It included three participants with ocular myasthenia gravis and seven with generalised myasthenia gravis. Symptoms of myasthenia gravis (measured as improvement in at least one muscle function) improved in nine of the 10 participants after the two-week neostigmine treatment phase. No participant improved after the placebo phase. Lack of detail in the report meant that the risk of bias was unclear. Adverse events were minor. AUTHORS' CONCLUSIONS: Except for one small and inconclusive trial of intranasal neostigmine, no other randomised controlled trials have been conducted on the use of acetylcholinesterase inhibitors in myasthenia gravis. The response to acetylcholinesterase inhibitors in observational studies is so clear that a randomised controlled trial depriving participants in a placebo arm of treatment would be difficult to justify.

PURPOSE: To determine whether the relative afferent pupillary defect (RAPD) remains constant over time in normal subjects. METHODS: Seventeen normal subjects were tested with infrared pupillography and automated perimetry in four sessions over 3 years. The changes in RAPD and visual field asymmetry between testing sessions were compared. RESULTS: The range of RAPD was 0.0 to 0.3 log unit, and the difference in the mean deviation between the eyes on automated static perimetry was 0 to 3 dB. Eight subjects repeatedly had an RAPD in the same eye. There was no correlation between the RAPD and the visual field asymmetry at the same visit. Changes in the magnitude of the RAPD between any two sessions were typically small (median, 0.08 log unit; 25th percentile, 0.04 log unit; 75th percentile, 0.15 log unit). CONCLUSIONS: Some normal subjects may show a persistent but small RAPD in the absence of detectable pathologic disease. Therefore, an isolated RAPD in the range of 0.3 log unit that is not associated with any other significant historical or clinical finding should probably be considered benign.
