Glucagon-like peptide-1 (GLP-1) stimulates glucose-induced insulin secretion by binding to a specific G protein-coupled receptor linked to activation of the adenylyl cyclase pathway. Here, using insulinoma cell lines, we studied homologous and heterologous desensitization of GLP-1-induced cAMP production. Preexposure of the cells to GLP-1 induced a decrease in GLP-1-mediated cAMP production, as assessed by a 3- to 5-fold rightward shift of the dose-response curve and an approximately 20 percent decrease in the maximal production of cAMP. Activation of protein kinase C by the phorbol ester phorbol 12-myristate 13-acetate (PMA) also induced desensitization of the GLP-1-mediated response, leading to a 6- to 9-fold shift in the EC50 and a 30% decrease in the maximal production of cAMP. Both forms of desensitization were additive, and the protein kinase C inhibitor RO-318220 inhibited PMA-induced desensitization, but not agonist-induced desensitization. GLP-1- and PMA-dependent desensitization correlated with receptor phosphorylation, and the levels of phosphorylation induced by the two agents were additive. Furthermore, PMA-induced, but not GLP-1-induced, phosphorylation was totally inhibited by RO-318220. Internalization of the GLP-1 receptor did not participate in the desensitization induced by PMA, as a mutant GLP-1 receptor lacking the last 20 amino acids of the cytoplasmic tail was found to be totally resistant to the internalization process, but was still desensitized after PMA preexposure. PMA and GLP-1 were not able to induce the phosphorylation of a receptor deletion mutant lacking the last 33 amino acids of the cytoplasmic tail, indicating that the phosphorylation sites were located within the deleted region. The cAMP production mediated by this deletion mutant was not desensitized by PMA and was only poorly desensitized by GLP-1. Together, our results indicate that the production of cAMP and, hence, the stimulation of insulin secretion induced by GLP-1 can be negatively modulated by homologous and heterologous desensitization, mechanisms that involve receptor phosphorylation.

BACKGROUND: Complex foot and ankle fractures, such as calcaneum fractures or Lisfranc dislocations, are often associated with a poor outcome, especially in terms of gait capacity. Indeed, degenerative changes often lead to chronic pain and chronic functional limitations. Prescription footwear represents an important therapeutic tool during the rehabilitation process. Local Dynamic Stability (LDS) is the ability of locomotor system to maintain continuous walking by accommodating small perturbations that occur naturally during walking. Because it reflects the degree of control over the gait, LDS has been advocated as a relevant indicator for evaluating different conditions and pathologies. The aim of this study was to analyze changes in LDS induced by orthopaedic shoes in patients with persistent foot and ankle injuries. We hypothesised that footwear adaptation might help patients to improve gait control, which could lead to higher LDS: METHODS: Twenty-five middle-aged inpatients (5 females, 20 males) participated in the study. They were treated for chronic post-traumatic disabilities following ankle and/or foot fractures in a Swiss rehabilitation clinic. During their stay, included inpatients received orthopaedic shoes with custom-made orthoses (insoles). They performed two 30s walking trials with standard shoes and two 30s trials with orthopaedic shoes. A triaxial motion sensor recorded 3D accelerations at the lower back level. LDS was assessed by computing divergence exponents in the acceleration signals (maximal Lyapunov exponents). Pain was evaluated with Visual Analogue Scale (VAS). LDS and pain differences between the trials with standard shoes and the trials with orthopaedic shoes were assessed. RESULTS: Orthopaedic shoes significantly improved LDS in the three axes (medio-lateral: 10% relative change, paired t-test p < 0.001; vertical: 9%, p = 0.03; antero-posterior: 7%, p = 0.04). A significant decrease in pain level (VAS score -29%) was observed. CONCLUSIONS: Footwear adaptation led to pain relief and to improved foot & ankle proprioception. It is likely that that enhancement allows patients to better control foot placement. As a result, higher dynamic stability has been observed. LDS seems therefore a valuable index that could be used in early evaluation of footwear outcome in clinical settings.

SUMMARY The human auditory cortex, located on the supratemporal plane of the temporal lobe, is divided in a primary auditory area and several non-primary areas surrounding it. These different areas show anatomical and functional differences. Many studies have focussed on auditory areas in non-human primates, using investigation techniques such as electrophysiological recordings, tracing of neural connections, or immunohistochemical and histochemical staining. Some of these studies have suggested parallel and hierarchical organization of the cortical auditory areas as well as subcortical auditory relays. In humans, only few studies have investigated these regions immunohistochemically, but activation and lesion studies speak in favour of parallel and hierarchical organization, very similar to that of non-human primates. Calcium-binding proteins and metabolic markers were used to investigate possible correlates of hierarchical and parallel organization in man. Calcium-binding proteins, parvalbumin, calretinin and calbindin, modulate the concentration of intracellular free calcium ions and were found in distinct subpopulations of GABAergic neurons in non-human primates species. In our study, their distribution showed several differences between auditory areas: the primary auditory area was darkly stained for both parvalbumin and calbindin, and their expression rapidly decreased while moving away from the primary area. This staining pattern suggests a hierarchical organization of the areas, in which the more darkly stained areas could correspond to an earlier integration level and the areas showing light staining may correspond to higher level integration areas. Parallel organization of primary and non-primary auditory areas was suggested by the complementarity, within a given area, between parvalbumin and calbindin expression across layers. To investigate the possible differences in the energetic metabolism of the cortical auditory areas, several metabolic markers were used: cytochrome oxidase and LDH1 were used as oxidative metabolism markers and LDH5 was used as glycolytic metabolism marker. The results obtained show a difference in the expression of enzymes involved in oxidative metabolism between areas. In the primary auditory area the oxidative metabolism markers were maximally expressed in layer IV. In contrast, higher order areas showed maximal staining in supragranular layers. The expression of LDH5 varied in patches, but did not differ between the different hierarchical auditory areas. The distribution of the two LDH enzymes isoforms also provides information about cellular aspects of metabolic organization, since neurons expressed the LDH1 isoform whereas astrocytes express primarily LDH5, but some astrocytes also contained the LDH1 isoform. This cellular distribution pattern supports the hypothesis of the existence of an astrocyte-neuron lactate shuttle, previously suggested in rodent studies, and in particular of lactate transfer from astrocytes, which produce lactate from the glucose obtained from the circulation, to neurons that use lactate as energy substrate. In conclusion, the hypothesis of parallel and hierarchical organization of the auditory areas can be supported by CaBPs, cytochrome oxidase and LDH1 distribution. Moreover, the two LDHs cellular distribution pattern support the hypothesis of an astrocyte-neuron lactate shuttle in human cortex.

Background and objectives Interleukin 18 (IL-18) is a pleiotropic cytokine involved in rheumatoid arthritis (RA) pathogenesis. This study was carried out to evaluate the effi cacy of IL-18 binding protein (IL-18BP) gene therapy in the rat adjuvant- induced arthritis (AIA) model and to decipher the mechanisms by which IL-18BP delivery lessens bone destruction.Materials and methods Arthritis was induced in female Lewis rat by Mycobacterium butyricum and the mRNA expression of IL-18 and IL-18BP was determined in the joints. In a preventive study, rats were divided into an adenovirus producing IL-18BP-Fc (AdmIL-18BP-Fc) group (n=8) and an adenovirus producing green fl uorescent protein (AdGFP) group (n=7). On day 8 after AIA induction, adenoviruses were injected. Clinical parameters were assessed. At day 18, during maximal arthritis, the rats were euthanized, ankles were collected and x-rays were performed. mRNA and protein were extracted from joints for analysis by quantitative reverse transcriptase-PCR, multiplex, Western blot and zymography.Results The authors observed a decrease in the (IL-18BP/ IL-18) ratio from day 7 to 45. Administration of AdmIL-18BPd-Fc decreased clinical parameters and prevented bone and joint destruction compared to AdGFP administration. IL-18BP delivery reduced the (receptor activator of nuclear factor κB ligand (RANKL)/osteoprotegerin (OPG)) ratio by 70%, the matrix metalloproteinase 9 (MMP9) level by 33% and the tartrate-resistant acid phosphatase (TRAP) level by 44% in the joint homogenates from AdmIL-18BPd-Fc compared to AdGFP treated rats.Conclusions In rat AIA, a decrease in the (IL-18BP/IL-18) ratio was observed. IL-18BP delivery prevented joint and bone destruction by downregulating MMP9, (RANKL/OPG) and TRAP, suggesting a potential benefi t of a similar therapy in RA.Abstract topics Towards novel therapeutic strategies.

Introduction An impaired ability to oxidize fat may be a factor in the obesity's aetiology (3). Moreover, the exercise intensity (Fatmax) eliciting the maximal fat oxidation rate (MFO) was lower in obese (O) compared with lean (L) individuals (4). However, difference in fat oxidation rate (FOR) during exercise between O and L remains equivocal and little is known about FORs during high intensities (>60% ) in O compared with L. This study aimed to characterize fat oxidation kinetics over a large range of intensities in L and O. Methods 12 healthy L [body mass index (BMI): 22.8±0.4] and 16 healthy O men (BMI: 38.9±1.4) performed submaximal incremental test (Incr) to determine whole-body fat oxidation kinetics using indirect calorimetry. After a 15-min resting period (Rest) and 10-min warm-up at 20% of maximal power output (MPO, determined by a maximal incremental test), the power output was increased by 7.5% MPO every 6-min until respiratory exchange ratio reached 1.0. Venous lactate and glucose and plasma concentration of epinephrine (E), norepinephrine (NE), insulin and non-esterified fatty acid (NEFA) were assessed at each step. A mathematical model (SIN) (1), including three variables (dilatation, symmetry, translation), was used to characterize fat oxidation (normalized by fat-free mass) kinetics and to determine Fatmax and MFO. Results FOR at Rest and MFO were not significantly different between groups (p≥0.1). FORs were similar from 20-60% (p≥0.1) and significantly lower from 65-85% in O than in L (p≤0.04). Fatmax was significantly lower in O than in L (46.5±2.5 vs 56.7±1.9 % respectively; p=0.005). Fat oxidation kinetics was characterized by similar translation (p=0.2), significantly lower dilatation (p=0.001) and tended to a left-shift symmetry in O compared with L (p=0.09). Plasma E, insulin and NEFA were significantly higher in L compared to O (p≤0.04). There were no significant differences in glucose, lactate and plasma NE between groups (p≥0.2). Conclusion The study showed that O presented a lower Fatmax and a lower reliance on fat oxidation at high, but not at moderate, intensities. This may be linked to a: i) higher levels of insulin and lower E concentrations in O, which may induce blunted lipolysis; ii) higher percentage of type II and a lower percentage of type I fibres (5), and iii) decreased mitochondrial content (2), which may reduce FORs at high intensities and Fatmax. BACKGROUND: For a long time now, glucose has been thought to be the main, if not the sole substrate for brain energy metabolism. Recent data nevertheless suggest that other molecules, such as monocarboxylates (lactate and pyruvate mainly) could be suitable substrates. Although monocarboxylates poorly cross the blood brain barrier (BBB), such substrates could replace glucose if produced locally.The two key enzymatiques systems required for the production of these monocarboxylates are lactate dehydrogenase (LDH; EC1.1.1.27) that catalyses the interconversion of lactate and pyruvate and the pyruvate dehydrogenase complex that irreversibly funnels pyruvate towards the mitochondrial TCA and oxydative phosphorylation. RESULTS: In this article, we show, with monoclonal antibodies applied to post-mortem human brain tissues, that the typically glycolytic isoenzyme of lactate dehydrogenase (LDH-5; also called LDHA or LDHM) is selectively present in astrocytes, and not in neurons, whereas pyruvate dehydrogenase (PDH) is mainly detected in neurons and barely in astrocytes. At the regional level, the distribution of the LDH-5 immunoreactive astrocytes is laminar and corresponds to regions of maximal 2-deoxyglucose uptake in the occipital cortex and hippocampus. In hippocampus, we observed that the distribution of the oxidative enzyme PDH was enriched in the neurons of the stratum pyramidale and stratum granulosum of CA1 through CA4, whereas the glycolytic enzyme LDH-5 was enriched in astrocytes of the stratum moleculare, the alveus and the white matter, revealing not only cellular, but also regional, selective distributions. The fact that LDH-5 immunoreactivity was high in astrocytes and occurred in regions where the highest uptake of 2-deoxyglucose was observed suggests that glucose uptake followed by lactate production may principally occur in these regions. CONCLUSION: These observations reveal a metabolic segregation, not only at the cellular but also at the regional level, that support the notion of metabolic compartmentalization between astrocytes and neurons, whereby lactate produced by astrocytes could be oxidized by neurons.

Objective To identify the association between perineal trauma and pain in 473 primiparous women. Method Cross-sectional study in which pain was measured by the numerical pain scale (0 to 10 - 0 being no pain and 10 maximal pain). Results The prevalence and mean intensity of pain were 33.0% and 4.7 points (standard deviation = 2.0) in the numeric scale, respectively. Episiotomy represented the most frequent trauma (46.7%). The occurrence and intensity of the pain were associated with perineal trauma and postpartum time. Having perineal trauma tripled the chance of pain. Each hour elapsed following the birth reduced the chance of pain by 4.8%. Conclusion Primiparous women are subject to a high frequency of perineal trauma, with episiotomy being the most prominent. Perineal pain affects approximately one-third of primiparous women and is associated with the postpartum time and perineal traumas.

RésuméDie Grabung 1989/5 der Archäologischen Bodenforschung Basel-Stadt im Zentrum der Siedlung Basel-Gasfabrik umfasste eine Fläche von etwa 900 m2. Auf etwa 370 m2 konnten intakte latènezeitliche Strukturen festgestellt werden. Der ehemalige Gehhorizont und die darunter liegende Siedlungsschicht waren jedoch nirgends erhalten geblieben.Die in den gewachsenen Boden eingetieften Pfostengrub en erlaubten die Rekonstrulction von zwei rechteckigen Häusern mit je vier Eckpfosten, die als Wohngebäude gedeutet werden.Über das ganze Areal verteilt wurden 14 Gruben unterschiedlicher Form und Grösse ganz oder teilweise ausgegraben. Bei der flachen Grube 261 handelt es sich wohl um den eingetieften Bereich eines kleinen Grubenhauses, das für nicht näher bestimmbare handwerkliche Aktivitäten genutzt wurde. Die beiden mit einem Pfostenbau überdeckten Gruben 258 und 259 können hingegen mit grosser Wahrscheinlichkeit als Schmiedegruben gedeutet werden. In Grube 258 waren gar die Reste von zwei Feuerstellen und der Unterbau eines kaminartigen Abzugs in situ erhalten. Die Gruben 253, 254A, 254B, 255,256 und 257 mit Resten von Lehmausideidungen dienten vermutlich als Getreidesilos. Sie liegen alle im Zentrum des Grabungsareals im Bereich einer anstehenden Lehmschicht. Bei einigen weiteren Gruben könnte es sich um Keller handeln.Im Westen der Grabungsfläche kamen die Heizkanäle von zwei Töpferöfen zum Vorschein. Es handelt sich um einfache Kuppelöfen mit gegenständiger Feuerung. In geringer Entfernung lagen drei kleine Gruben, die vielleicht zur Aufbereitung des Töpferlehms dienten. Aufgrund der klimatischen Verhältnisse ist mit einem saisonalen Betrieb der Töpferöfen während der Sommermonate zu rechnen. Die notwendigen Arbeiten wurden mit grosser Wahrscheinlichkeit von Frauen ausgeführt, da die Männer während dieser Jahreszeit in der Landwirtschaft beschäftigt waren. Der Töpfereibetrieb produzierte grössere Serien scheibengedrehter, reduzierend gebrannter Feinkeramik, von der im Bereich der Öfen zahlreiche Scherben gefunden wurden.Die räumliche Organisation der Bebauung zeugt von einer einheitlichen und wohl kontinuierlichen Nutzung der Grabungsfläche. ImWesten gruppieren sich Haus 1 und die Einrichtungen des Töpfereibetriebs um eine unbebaute Hofzone. Nach Osten zu folgt dann die Zone mit den Getreidesilos. Das östliche Bauensemble besteht aus Haus 2, den Schmiedegruben, dem Grubenhaus sowie einer mutmasslichen Kellergrube. Stratigra-phische Überschneidungen von Befunden sind nur im Bereich der Töpferöfen vorhanden.Die Grubenfüllungen erwiesen sich als sehr fundreich. Die gegen 20000 Keramikscherben repräsentieren das gesamte Spektrum der aus Basel-Gasfabrik bekannten einheimischen Grobund Feinkeramik. Bei den seltenen Scherben von Graphittonkeramik handelt es sich um Importe aus dem ostkeltischen Bereich.Die zahlreichen Amphorenscherben stammen von mindestens 35 verschiedenen Gefässindividuen des Typs Dressel 1A, von denen aber immer nur ein kleiner Prozentsatz vorhanden war. Die Typologie und die Datierung der Befunde zeigen, dass der Amphorenimport erst im Laufe von LT D1 einsetzte.Unter den Kleinfunden verdienen die zahlreichen Silber- und Potinmünzen sowie die Glasfunde besondere Erwähnung. Das Fragment eines möglicherweise latènezeitlichen Glasgefässes stammt leider aus einem unsicheren Fundzusammenhang.Die über 70 Fibeln und Fibelteile sind zu 70% aus Bronze gefertigt. Das Fibelspektrum wird von den Nauheimer Fibeln dominiert. Besonders bei den Eisenfibeln konnten aber auch typologisch ältere Typen identifiziert werden, welche in die Stufen LT C2 und LT D1a gehören. Bei den Glasarmringen machen mittellatènezeitliche Typen gegen 20 % der Fundmenge aus.Von besonderer Bedeutung sind die 23 Menschenknochen von 19 verschiedenen Individuen. Es handelt sich dabei um einen fast vollständigen Schädel, Schädelteile und Fragmente von Langknochen sowie wenige weitere Skelettteile. Die Zusammensetzung des Ensembles und der Zustand der Einzelknochen lässt sich sehr gut mit den Menschenknochen aus den Altgrabungen von Basel-Gasfabrik und aus Manching vergleichen. Eine detaillierte Untersuchung zeigt, dass die Knochen am Ende eines langwierigen Totenrituals, das aufgrund ethno - logischer Vergleiche als mehrstufige Bestattung bezeichnet werden kann, in der Siedlung vergraben wurden. In der Umgebung der Menschenknochen wurden in einigen Fällen auffallend viele Amphorenscherben gefunden, die bezeugen, dass der importierte Wein bei diesen Riten eine bedeutende Rolle spielte. Wahrscheinlich wurden diese Knochen auch als Ahnenrelikte verehrt.Neben den mehrstufigen Bestattungen konnten auch zwei Säuglingsbestattungen identifiziert werden.Die Analyse von Fundmenge und Fundverteilung in den Gruben zeigt, dass die Funde nicht direkt, sondern auf dem Umweg über primäre Deponien in die Gruben gelangten. In den pri-mären Deponien wurden Funde über längere Zeit akkumuliert und mit verschiedenen Erdmaterialien intensiv vermischt.Für den Grossteil der Funde wird eine profane Deutung als Abfälle vorgeschlagen. Daneben können aber auch einige Funde als gezielte Deponierungen angesprochen werden. Ver-schiedene Fibelpaare und wahrscheinlich auch Münzen wurden wohl als Opfergaben in die Gruben gegeben. Ein Zusammenhang dieser Opfergaben mit den Bestattungen von Men-schenknochen ist nicht erkennbar.Die Datierung der Funde zeigt einen Siedlungsbeginn in LT C2 und ein Ende noch vor dem Beginn der Stufe LT D2. Die Kombination dieser Datierungen mit den stratigraphischen Überschneidungen einiger Befunde erlaubt eine Rekonstruktion der Besiedlungsentwicklung auf dem Grabungsareal. Die Gruben 259 und 260 wurden bereits in LT C2 verfüllt, die Schmiedegrube 259 anschliessend durch Grube 258 ersetzt. Die beiden Töpferöfen waren nacheinander in LT D1 in Betrieb. Die beiden Häuser können leider nicht genauer datiert werden.Die Besiedlung des Area's setzte noch vor 150 V. Chr. ein und dauerte maximal 80 Jahre.Die Synthese aller Untersuchungen zeigt, dass auf dem Grabungsareal eine Gruppe von etwa 15 Personen ansässig war, die sich v. a. der Landwirtschaft widmete. Die handwerkliche Tätigkeit (Töpferei, Metallverarbeitung) erreichte keinen vollberuflichen Standard.Die dörfliche Siedlungsgemeinschaft bestand aus bäuerlichen Selbstversorgern, mit anderen Gemeinschaften wurden jedoch Keramik, Handwerksprodukte und Schlachtvieh ausge-tauscht. Von besonderer Bedeutung war offenbar das Getreide, das in grossen Mengen in der Siedlung gelagert wurde und mit grosser Wahrscheinlichkeit für den Export bestimmt war. Die Herkunft dieses Getreides und die sozialen Strukturen hinter diesem Austausch sind nicht ldar, aber mit hoher Wahrscheinlichkeit wurden durch den Getreideexport die Importe wie Salz, Rohstoffe (Metall) und Wein ermöglicht.

We compared the extent and origin of muscle fatigue induced by short-pulse-low-frequency [conventional (CONV)] and wide-pulse-high-frequency (WPHF) neuromuscular electrical stimulation. We expected CONV contractions to mainly originate from depolarization of axonal terminal branches (spatially determined muscle fiber recruitment) and WPHF contractions to be partly produced via a central pathway (motor unit recruitment according to size principle). Greater neuromuscular fatigue was, therefore, expected following CONV compared with WPHF. Fourteen healthy subjects underwent 20 WPHF (1 ms-100 Hz) and CONV (50 μs-25 Hz) evoked isometric triceps surae contractions (work/rest periods 20:40 s) at an initial target of 10% of maximal voluntary contraction (MVC) force. Force-time integral of the 20 evoked contractions (FTI) was used as main index of muscle fatigue; MVC force loss was also quantified. Central and peripheral fatigue were assessed by voluntary activation level and paired stimulation amplitudes, respectively. FTI in WPHF was significantly lower than in CONV (21,717 ± 11,541 vs. 37,958 ± 9,898 N·s P<0,001). The reductions in MVC force (WPHF: -7.0 ± 2.7%; CONV: -6.2 ± 2.5%; P < 0.01) and paired stimulation amplitude (WPHF: -8.0 ± 4.0%; CONV: -7.4 ± 6.1%; P < 0.001) were similar between conditions, whereas no change was observed for voluntary activation level (P > 0.05). Overall, our results showed a different motor unit recruitment pattern between the two neuromuscular electrical stimulation modalities with a lower FTI indicating greater muscle fatigue for WPHF, possibly limiting the presumed benefits for rehabilitation programs.

BACKGROUND: Reactive oxygen species production increases during aging, whereas protective mechanisms such as heat shock proteins (HSPs) or antioxidant capacity are depressed. Physical activity has been hypothesized to provide protection against oxidative damage during aging, but results remain controversial. This study aimed to investigate the effect of different levels of physical activity during aging on Hsp72 expression and systemic oxidative stress at rest and in response to maximal exercise. METHODS: Plasma antioxidant capacity (Trolox equivalent antioxidant capacity, TEAC), thiobarbituric acid-reactive species (TBARS), advanced oxidized proteins products (AOPP), and Hsp72 expression in leukocytes were measured before and after maximal exercise testing in 32 elderly persons (aged 73.2 years), who were assigned to two different groups depending on their level of physical activity during the past 12 months (OLow = moderate to low level; OHigh = higher level). RESULTS: The OHigh group showed higher aerobic fitness and TEAC (both representing 120% of OLow values) as well as lower oxidative damage (50% of OLow values) and Hsp72 expression. Exercise led to a lower increase in oxidative damage in the OHigh group. Aerobic fitness was positively correlated with TEAC and negatively with lipid peroxidation (TBARS). Hsp72 expression was negatively correlated with TEAC but positively correlated with TBARS levels. CONCLUSIONS: The key finding of this study is that, in people aged 60 to 90 years, long-term high level of physical activity preserved antioxidant capacity and limited oxidative damage accumulation. It also downregulated Hsp72 expression, an adaptation potentially resulting from lower levels of oxidative damage.

Homologous desensitization and internalization of the GLP-1 receptor correlate with phosphorylation of the receptor in a 33-amino acid segment of the cytoplasmic tail. Here, we identify the sites of phosphorylation as being three serine doublets located at positions 441/442, 444/445, and 451/452. The role of phosphorylation on homologous desensitization was assessed after stable expression in fibroblasts of the wild type or of mutant receptors in which phosphorylation sites were changed in various combinations to alanines. We showed that desensitization, as measured by a decrease in the maximal production of cAMP after a first exposure of the cells to GLP-1, was strictly dependent on phosphorylation. Furthermore, the number of phosphorylation sites correlated with the extent of desensitization with no, intermediate, or maximal desensitization observed in the presence of one, two, or three phosphorylation sites, respectively. Internalization of the receptor-ligand complex was assessed by measuring the rate of internalization of bound [125I]GLP-1 or the redistribution of the receptor to an endosomal compartment after agonist binding. Our data demonstrate that internalization was prevented in the absence of receptor phosphorylation and that intermediate rates of endocytosis were obtained with receptors containing one or two phosphorylation sites. Thus, homologous desensitization and internalization require phosphorylation of the receptor at the same three sites. However, the differential quantitative impairment of these two processes in the single and double mutants suggests different molecular mechanisms controlling desensitization and internalization.

The alpha1-adrenergic agonist phenylephrine stimulated phospholipase D (PLD) activity in Rat 1 fibroblasts transfected to express either the wild-type hamster alpha1B-adrenoceptor or a constitutively active mutant (CAM) form of this receptor. The EC50 for agonist stimulation of PLD activity was substantially lower at the CAM receptor than at the wild-type receptor as previously noted for phenylephrine stimulation of phosphoinositidase C activity. Sustained treatment of cells expressing the CAM alpha1B-adrenoceptor with phentolamine resulted in a marked up-regulation in levels of this receptor with half-maximal effects produced within 24 h and with an EC50 of approx. 40 nM. Such an up-regulation could be produced with a range of other ligands generally viewed as alpha1-adrenoceptor antagonists but equivalent treatment of cells expressing the wild-type alpha1B-adrenoceptor was unable to mimic these effects. After sustained treatment of the CAM alpha1B-adrenoceptor expressing cells with phentolamine, basal PLD activity was increased and phenylephrine was now able to stimulate PLD activity to greater levels than in vehicle-treated CAM alpha1B-adrenoceptor-expressing cells. The EC50 for phenylephrine stimulation of PLD activity was not altered, however, by phentolamine pretreatment and the associated up-regulation of the receptor. After phentolamine-induced up-regulation of basal PLD activity, a range of alpha1-antagonists were shown to possess the characteristics of inverse agonists of the CAM alpha1B-adrenoceptor as they were able to substantially decrease the elevated basal PLD activity.

Crans-Montana-Sierre is a tourist area locateci in Rhone valley in central Valais, cha-racterized by dry climate and scarce summer precipitations. More than other regions in Switzerland, this area has suffered the effects of the drought (heat wave) that affec¬ted all Western Europe during summer 2003. In the future, climate change together with societal and economic development will signicantly modify the water need of the region and, consequently, may increase the potential conflicts of interest. Within a long term planning strategy, decision-makers require precise information about the current amount of water needed in the region, with particular attention to temporal and spatial concentration, and the forecasted amount for 2050. This work therefore aims at estimating the variation of the water demand by taking into account the influence of climate change (CH2011) and socio-economic scenarios, developed in cooperation with the competent authorities. This thesis, whose aim is to study the water management and water uses is a core part of the MontanAqua project "Water management in times of scarcity and global change" mainly because of its interdisciplinary topic at the interface between water resources, land development and the socio-political structure. Results show that socio-economic development by 2050 could have a greater impact than expected climate changes (CH2011, A1B scenarios) for the same time horizon for water demand. Demography, spatial planning, tourism and economic development are just some of the factors that could significantly affect the water consumption of the Crans-Montana-Sierre region. Compared with the future water resources available, the maximal water demand forecasted by the socio-economic scenarios developed within the project MontanAqua, could not always be satisfied. This issue, like extreme climate phenomena (as it was the summer drought occurred in 2003 or in April / May 2011), could be mitigated adopting regional management policies relating to a more rational water use and preventive storage of water resource. - Crans-Montana-Sierre est une région touristique située dans la vallée du Rhône; dans le Valais central, qui se caractérise par un climat relativement pauvre en précipitations et qui plus que d'autres a subi les effets de la sécheresse qui a touché l'Europe occidentale durant l'été 2003. À l'avenir, les changements climatiques ainsi que le développement socio-économique modifieront de manière significative les besoins en eau de la région, ce qui risque de faire augmenter les rivalités d'usage concernant cette ressource. Afin de jouer à l'avance, les décideurs ont besoin d'informations précises sur la quantité d'eau nécessaire à la région, avec une attention particulière à sa concentration temporelle et spatiale, à l'heure actuelle et à l'horizon 2050. Ce travail vise donc à estimer la variation de la demande en eau en tenant compte de l'influence des changements climatiques (CH2011) et des scénarios socio-économiques, élaborés en collaboration avec les autorités compétentes. Cette thèse, qui met l'accent sur les usages de l'eau fait partie du projet Montan Aqua « Gestion de l'eau en temps de pénurie et de changement global », est à l'intersection entre les ressources hydriques, l'aménagement du territoire et son organisation socio-politique, fait qui la met, non pas par son importance, mais par son emplacement et ses interconnexions, au coeur de cette recherche. Les résultats obtenus montrent comment les développements socio-économiques d'ici à 2050 ont un impact potentiellement plus important que les changements climatiques prévus par les scénarios AlB de CH2011 pour le même horizon temporel sur la demande en eau. Démographie, aménagement du territoire et contexte économico-touristique, ne sont que quelques-uns des facteurs qui ont la capacité d'agir significativement sur les usages de l'eau en ce qui concerne les aspects qualitatif et quantitatif de la région de Crans-Montana-Sierre. Par rapport aux ressources en eau disponibles à l'avenir, la demande maximale d'eau prévue par les scénarios socio-économiques développés au sein du projet MontanAqua risque de ne pas être toujours satisfaite. Ce danger et la manifestation de phénomènes climatiques extrêmes, comme la sécheresse estivale survenue en 2003 ou celle d'avril/mai 2011, ne pourront être atténués que par l'adoption de politiques de gestion à l'échelle régionale favorisant une utilisation plus rationnelle et un stockage préventif de la ressource en eau.

Résumé Objectif : L'hyperplasie intimale est un processus de remodelage vasculaire qui apparaît après une lésion vasculaire. Les mécanismes impliqués dans l'hyperplasie intimale sont la prolifération, la dédifférentiation et la migration des cellules musculaires lisses depuis la média vers l'espace sous-intimal. Nous avons émis l'hypothèse que les jonctions communicantes de type gap, qui coordonnent certains processus physiologiques tels que la croissance et la différentiation cellulaire, pouvaient participer au développement de l'hyperplasie intimale. Méthodes : Des segments de veines saphènes humaines prélevées chirurgicalement lors de pontages, ont été ouverts longitudinalement avec la surface luminale placée vers le haut et maintenus en culture pendant 14 jours. Des fragments veineux ont été préparés pour une évaluation histologique, pour des mesures de l'épaisseur de la néointima, et pour des analyses immunocytochimiques de l'ARN messager ainsi que des protéines. Résultats : Parmi les 4 connexines (Cxs 37, 40, 43 et 45) qui forment les jonctions communicantes dans les veines, nous avons focalisé notre étude sur l'expression des Cxs 43 et 40; nous avons démontré que la Cx43 est exprimée dans les cellules musculaires lisses et les cellules endothéliales alors que la Cx40 est uniquement présente dans l'endothélium. Après 14 jours en culture, des analyses histomorphométriques ont montré une augmentation significative de l'épaisseur de l'intima démontrant la présence d'hyperplasie intimale. Une analyse temporelle a révélé une augmentation progressive de la Cx43 jusqu'à une augmentation maximale de six à huit fois au niveau de l'ARN messager et des protéines après 14 jours en culture. Au contraire, l'expression de la Cx40 n'était pas modifiée. Des analyses par immunofluorescence ont montré également une augmentation de la Cx43 dans les membranes des cellules musculaires lisses de la média. Le développement de l'hyperplasie intimale in vitro est diminué en présence de fluvastatin et cette diminution est associée à une réduction de l'expression de la Cx43. Conclusions : Ces données démontrent que la Cx43 est augmentée in vitro pendant le processus d'hyperplasie intimale et que la fluvastatin prévient cette induction. Ces résultats suggèrent un rôle crucial joué par la communication intercellulaire impliquant la Cx43 dans la veine humaine durant le développement de l'hyperplasie intimale. Abstract Objective: Intimal hyperplasia is a vascular remodelling process that occurs after a vascular injury. The mechanisms involved in intimal hyperplasia are proliferation, dedifferentiation, and migration of medial smooth muscle cells towards the subintimal space. We postulated that gap junctions, which coordinate physiologic processes such as cell growth and differentiation, might participate in the development of intimal hyperplasia. connexin43 (Cx43) expression levels may be altered in intimal hyperplasia, and we therefore evaluated the regulated expression of Cx43 in human saphenous veins in culture in the presence or not of fluvastatin, an inhibitor of 3-hydroxy-3-methylglutaryl-coenzyme A reductase activity. Methods: Segments of harvested human saphenous veins, obtained at the time of bypass graft, were opened longitudinally with the luminal surface uppermost and maintained in culture for 14 days. Vein fragments were then processed for histologic examination, neointimal thickness measurements, immunocytochemistry, RNA, and proteins analysis. Results: Of the four connexins (Cx37, 40, 43, and 45), we focused on Cx43 and Cx40, which we found by real-time polymerase chain reaction to be expressed in the saphenous vein because they are the predominant connexins expressed by smooth muscle cells and endothelial cells. Afrer 14 days of culture, histomorphometric analysis showed a significant increase in the intimal thickness as observed during the process of intimal hyperplasia. Atime-course analysis revealed a progressive upregulation of Cx43 to reach a maximal increase of sixfold to eightfold at both transcript and protein levels after 14 days in culture. In contrast, the expression of Cx40, abundantly expressed in the endothelial cells, was not altered. Immunofluorescence showed a large increase in Cx43 within smooth muscle cell membranes of the media layer. The development of intimal hyperplasia in vitro was decreased in presence of fluvastatin and was associated with reduced Cx43 expression. Conclusions: These data show that Cx43 is increased in vitro during the process of intimal hyperplasia and that fluvastatin could prevent this induction, supporting a critical role for Cx43-mediated gap-junctional communication in the human vein during the development of intimal hyperplasia. Purpose: To describe the evolution of retinal thickness in eyes affected with acute anterior uveitis (AAU) in the course of follow-up and to assess its correlation with severity of inflammatory activity in the anterior chamber. Methods: Design: Prospective, cohort study Setting: Institutional study Patient population: 72 eyes (affected and fellow eyes) of 36 patients Observation procedure: Patients were followed daily until beginning of resolution of inflammatory activity and weekly thereafter. Optical coherence tomography and laser flare photometry were performed at each visit. Treatment consisted of topical corticosteroids Main outcome measures: Retinal thickness of affected eyes, difference in retinal thickness between affected and fellow eyes and their evolution in time, association between maximal retinal thickness and initial laser flare photometry. Results: Difference in retinal thickness between affected and fellow eyes became significant on average seven days from baseline and remained so through-out follow-up (p<0.001). There was a steep increase in retinal thickness of affected eyes followed by a progressive decrease after reaching a peak value. Maximal difference in retinal thickness between affected and fellow eyes was observed between 17 and 25 days from baseline and exhibited a strong, positive correlation with initial laser flare photometry values (p=0.015). Conclusions: Retinal thickness in eyes affected with AAU presents a steep increase over 3 to 4 weeks and then gradually decreases. Severity of inflammation at baseline predicts the amount of retinal thickening in affected eyes. A characteristic pattern of temporal response of retinal anatomy to inflammatory stimuli seems to arise.
