This publication outlines a model of delivery for introductory level Cognitive Behavioural Therapy (CBT) training in the Mental Health Service, HSE South (Carlow, Kilkenny, South Tipperary, Waterford, Wexford). This model has proved useful in guiding the development of four introductory programmes during 2009 and 2010. As a result of this experience, we have amended and updated our programme delivery strategies. We see this process as organic and ever changing, thus these reflections are a snap shot of our current thinking which we have no doubt will evolve as we proceed with future programmes. This booklet will act as a guide for our upcoming programmes in 2010 and 2011 and we believe it may also offer guidance to others who will be involved in the delivery of CBT training within the Irish Mental Health Service.This resource was contributed by The National Documentation Centre on Drug Use.

The International Longevity Centre - UK��launched a new paper (Wednesday, 6th July 2011). The last taboo: A guide to dementia, sexuality, intimacy and sexual behaviour in care homes, provides care home workers and managers with information and practical advice on this complex, controversial and sensitive issue.The need for affection, intimacy and relationships for people with dementia in care homes has too often been ignored and side-lined in policy and practice. The onset of old age or a cognitive impairment does not erase the need for affection, intimacy and/or relationships. While the issues involved can be complex, controversial and sensitive and may challenge our own beliefs and value system, it is essential that we understand more about them to foster a more person-centred approach to dementia care. Care home residents with dementia often have complex care needs and trying to understand and respond to the more intimate and sexual aspects of a resident’s personality can be challenging.Aimed at care home workers and managers, the guide not only provides essential information on this aspect of dementia care but offers practical advice to support current work-based practices. Set out in an accessible and easy-to-read format, this guide includes case studies, questions, suggestions and a self assessment quiz to promote easy learning. It also provides a possible pathway for care home managers to develop a guiding policy on sexual expression in dementia.The guide for care staff is summarised in 10 key points:1. Some residents with dementia will have sexual or sensual needs.2. Affection and intimacy contribute to overall health and wellbeing for residents.3. Some residents with dementia will have the capacity to make decisions about their needs.4. If an individual in care is not competent to decide, the home has a duty of care towards the individual to ensure they are protected from harm.5. There are no hard and fast rules. Assess each situation on an individual basis6. Remember not everyone with dementia is heterosexual.7. Inappropriate sexual behaviour is not particularly common in dementia.8. Confront your own attitudes and behaviour towards older people and sex generally.9. In the paracortex of the lymph node (LN), T zone fibroblastic reticular cells (TRCs) orchestrate an immune response by guiding lymphocyte migration both physically, by creating three-dimensional (3D) cell networks, and chemically, by secreting the chemokines CCL19 and CCL21 that direct interactions between CCR7-expressing cells, including mature dendritic cells and naive T cells. TRCs also enwrap matrix-based conduits that transport fluid from the subcapsular sinus to high endothelial venules, and fluid flow through the draining LN rapidly increases upon tissue injury or inflammation. To determine whether fluid flow affects TRC organization or function within a 3D network, we regenerated the 3D LN T zone stromal network by culturing murine TRC clones within a macroporous polyurethane scaffold containing type I collagen and Matrigel and applying slow interstitial flow (1-23 microm/min). We show that the 3D environment and slow interstitial flow are important regulators of TRC morphology, organization, and CCL21 secretion. Without flow, CCL21 expression could not be detected. Furthermore, when flow through the LN was blocked in mice in vivo, CCL21 gene expression was down-regulated within 2 h. These results highlight the importance of lymph flow as a homeostatic regulator of constitutive TRC activity and introduce the concept that increased lymph flow may act as an early inflammatory cue to enhance CCL21 expression by TRCs, thereby ensuring efficient immune cell trafficking, lymph sampling, and immune response induction.

The Mental Health First Aid (MHFA) Training Programme for Northern Ireland has been adapted from the original MHFA programme established in Australia by Betty Kitchener and Anthony Jorm. MHFA is the help provided to a person who is developing a mental health problem or who is currently in a mental health crisis. The first aid is given until professional help is available or until the crisis resolves. More than 4,500 people have attended MHFA training in Northern Ireland since it began in 2009 following a successful pilot in 2005. The aims of MHFA are to: preserve life where a person may be a danger to themselves or others; provide help to prevent the mental health problem becoming more serious; promote the recovery of good mental health; provide comfort to a person experiencing a mental health problem. MHFA teaches participants: how to recognise the symptoms of mental health problems; how to provide initial help; how to go about guiding a person towards appropriate professional help. The training programme is available to people from all backgrounds and has proved successful with different professional groups. MHFA training involves teaching participants how to recognise the symptoms of mental health problems such as depression, anxiety and psychosis. Each course is delivered by two MHFA instructors, usually over two consecutive days and four sessions to a maximum of 20 delegates. The course can also be delivered one day a week for two weeks or in four three-hour sessions. To apply for the training programme, people should contact their local Health and Social care Trust. Each Trust runs MHFA training several times a year. Topics covered include: What is meant by mental health/mental ill health? Dealing with crisis situations such as suicidal behaviour, self-harm, panic attacks and acute psychotic behaviour. Recognising the signs and symptoms of common mental health problems including depression, anxiety disorders, psychosis and substance use disorders. Where and how to get help. Self help strategies.

En el entorno del ejercicio profesional, las destrezas, los valores y actitudes personales, tienen un papel relevante y complementario al de los conocimientos. Este es el motivo de que la enseñanza basada en competencias, como una combinación de saber, ser, hacer y estar con otros, esté en la línea deseada. En este sentido, las universidades han estado elaborando nuevos planes de estudio en los que se desarrollan los nuevos Grados de acuerdo al EEES, incluyendo en estos, las competencias genéricas de cada titulación, que en general, se ciñen a las propuestas presentadas en el proyecto Tuning 2003.La investigación llevada a cabo sobre la competencia comunicativa escrita en el Grado de Ingeniería en Informática, se ha centrado en universidades de toda la geografía española. La información se ha analizado en función de dos marcos de clasificación: el marco curricular (planes de estudio y memorias de verificación de Grado) y el marco de evaluación (rúbricas).La investigación ha corroborado, que la competencia comunicativa escrita, una competencia instrumental genérica que está en el grupo de las destrezas lingüísticas, se trabaja de forma natural en una variedad de asignaturas repartidas a lo largo de los planes de estudio del Grado de Ingeniería en Informática, hecho que favorece la consecución de esta competencia. Aunque solo alrededor de un 20% de los planes de estudio analizados presentan una asignatura, la cual, uno de sus objetivos principales sea la de favorecer la competencia comunicativa escrita, se entiende, además de por la transversalidad de la competencia comunicativa escrita en un plan de estudios TIC, porque esta competencia, como se ha señalado, se puede trabajar de forma eficaz dentro de las metodologías de otras asignaturas.La investigación nos ha llevado a la conclusión, de que un futuro Graduado en Ingeniería Informática de una universidad española, presentará suficiencia en la competencia comunicativa escrita, ya que en esa dirección apuntan las referencias analizadas. El uso de rúbricas se hace necesario para evaluar el aprendizaje de dicha competencia, ya que sin evaluación, no hay posibilidad de constatar si realmente se alcanzan los niveles de calidad deseados.El modelo educativo que propone la UOC, permite que se desarrollen de forma preeminente una serie de competencias transversales, que resultan básicas a la hora de enfrentarse a los retos que nos propone la sociedad, preparando a profesionales, además de con los conocimientos técnicos propios de un Ingeniero en Informática, con las habilidades y competencias transversales necesarias que completan su formación técnica.

Actualment a l'Estat espanyol s'està implantant el Pla Bolonya per incorporar-se a l'Espai Europeu d'Estudis Superiors (l'EEES). Com a un dels principals objectius, l'EEES pretén homogeneïtzar els estudis i de manera concreta les competències adquirides per qualsevol estudiant independentment d'on hagi realitzat els seus estudis. Per això, existeixen iniciatives europees (com el projecte Tuning) que treballen per definir competències per a totes les titulacions universitàries.El projecte presenta l'anàlisi realitzat sobre vint Universitats de diferents continents per identificar models d'ensenyament-aprenentatge de competències no tècniques. La recerca es centra addicionalment en la competència comunicativa escrita.La font principal de dades ha estat la informació proporcionada a les pàgines Web de les universitats i molt especialment els seus plans d'estudi.

Este Proyecto Fin de Carrera, pertenece al área de 'Competencias profesionales' y tiene como objetivo el análisis de diversas experiencias de enseñanza-aprendizaje y de evaluación de competencias transversales, haciendo hincapié en la competencia de la comunicación escrita, dentro de la formación de currículos TIC. El análisis se centra en los planes de estudios del Grado en Ingeniería Informática de 10 universidades españolas representativas (públicas, privadas y a distancia) como representación de la "nueva" formación de currículos TIC en nuestra sociedad.

Per una banda, contextualitza el concepte de competència professional dins del marc de l'EEES, i reconeix i classifica a partir de la revisió de la documentació existent les competències específiques i transversals o genèriques, que han de tenir els nous titulats del Grau d'EI. Per altra banda, s'escomet l'anàlisi dels plans d'estudi i dels plans docents del Grau per identificar la seva adaptació al EEES,, concretament en el que fa referència a la competència de comunicació escrita.

Este proyecto tiene como objetivo principal el análisis de experiencias enseñanza - aprendizaje de la competencia comunicativa, en el proceso de adaptación de los planes de estudio de Grados de Ingeniería Informática.Además incluye la reflexión sobre las competencias genéricas aprendidas en el propio plan de estudios cursado en la UOC.Todo ello dentro del contexto actual de proceso de adaptación al EEES de los planes de estudio de las universidades y de la importancia que se da al desarrollo de las competencias genéricas y específicas de cada disciplina, según las directrices marcadas por el Proyecto Tuning.El primer paso ha sido identificar las competencias genéricas que debe desarrollar un Ingeniero en Informática, basadas en referencias existentes como es el libro blanco del Título de Grado en Ingeniería Informática.El segundo paso ha incluido el análisis específico de la competencia instrumental de comunicación escrita, como ejemplo representativo de desarrollo de competencia genérica dentro del mundo académico. Para lo cual se han buscado referentes principales sobre la materia en artículos de expertos que tratasen sobre metodologías de enseñanza aprendizaje y evaluación de competencias trasversales en los planes de estudio de Ingeniería Informática. Una vez se tenían clasificados los métodos más destacados de estos expertos, se ha analizado una muestra de planes docentes de Grado de Ingeniería Informática del curso académico 2010-2011, de Universidades Españolas. El objetivo perseguido era determinar en qué grado incorporan los métodos nombrados por los expertos para el desarrollo de la competencia de comunicación escrita.Finalmente, y en base al conjunto de competencias genéricas identificadas como las que debe adquirir un Ingeniero en Informática, se ha reflexionado sobre el desarrollo de las mismas en el plan de estudios cursado en la UOC y si el propio modelo educativo favorece en alguna medida este desarrollo.

The European Centre for Disease Prevention and Control (ECDC) and the European Respiratory Society (ERS) jointly developed European Union Standards for Tuberculosis Care (ESTC) aimed at providing European Union (EU)-tailored standards for the diagnosis, treatment and prevention of tuberculosis (TB). The International Standards for TB Care (ISTC) were developed in the global context and are not always adapted to the EU setting and practices. The majority of EU countries have the resources and capacity to implement higher standards to further secure quality TB diagnosis, treatment and prevention. On this basis, the ESTC were developed as standards specifically tailored to the EU setting. A panel of 30 international experts, led by a writing group and the ERS and ECDC, identified and developed the 21 ESTC in the areas of diagnosis, treatment, HIV and comorbid conditions, and public health and prevention. The ISTCs formed the basis for the 21 standards, upon which additional EU adaptations and supplements were developed. These patient-centred standards are targeted to clinicians and public health workers, providing an easy-to-use resource, guiding through all required activities to ensure optimal diagnosis, treatment and prevention of TB. These will support EU health programmes to identify and develop optimal procedures for TB care, control and elimination.

Aquest treball tracta sobre la creació d'un projecte de programari lliure, portada a terme des del seu inici fins a la seva conversió, en un projecte que conta amb el suport d'una comunitat d'usuaris. Específicament, la finalitat del projecte de programari lliure generat, és la creació d'una aplicació que sigui capaç de guiar de forma gràfica, ràpida i intuïtiva a l'usuari a través del procés de creació de consultes SQL per a la base de dades postgreSQL.

OBJECTIVES: This study sought to establish an accurate and reproducible T(2)-mapping cardiac magnetic resonance (CMR) methodology at 3 T and to evaluate it in healthy volunteers and patients with myocardial infarct. BACKGROUND: Myocardial edema affects the T(2) relaxation time on CMR. Therefore, T(2)-mapping has been established to characterize edema at 1.5 T. A 3 T implementation designed for longitudinal studies and aimed at guiding and monitoring therapy remains to be implemented, thoroughly characterized, and evaluated in vivo. METHODS: A free-breathing navigator-gated radial CMR pulse sequence with an adiabatic T(2) preparation module and an empirical fitting equation for T(2) quantification was optimized using numerical simulations and was validated at 3 T in a phantom study. Its reproducibility for myocardial T(2) quantification was then ascertained in healthy volunteers and improved using an external reference phantom with known T(2). In a small cohort of patients with established myocardial infarction, the local T(2) value and extent of the edematous region were determined and compared with conventional T(2)-weighted CMR and x-ray coronary angiography, where available. RESULTS: The numerical simulations and phantom study demonstrated that the empirical fitting equation is significantly more accurate for T(2) quantification than that for the more conventional exponential decay. The volunteer study consistently demonstrated a reproducibility error as low as 2 ± 1% using the external reference phantom and an average myocardial T(2) of 38.5 ± 4.5 ms. Intraobserver and interobserver variability in the volunteers were -0.04 ± 0.89 ms (p = 0.86) and -0.23 ± 0.91 ms (p = 0.87), respectively. In the infarction patients, the T(2) in edema was 62.4 ± 9.2 ms and was consistent with the x-ray angiographic findings. Simultaneously, the extent of the edematous region by T(2)-mapping correlated well with that from the T(2)-weighted images (r = 0.91). CONCLUSIONS: The new, well-characterized 3 T methodology enables robust and accurate cardiac T(2)-mapping at 3 T with high spatial resolution, while the addition of a reference phantom improves reproducibility. This technique may be well suited for longitudinal studies in patients with suspected or established heart disease.
