FUNDAMENTO: O envolvimento cardíaco é comum em pacientes com Distrofia Miotônica (DM). A Variabilidade da Frequência Cardíaca (VFC) é uma técnica simples e confiável que pode ser útil para estudar a influência do sistema nervoso autonômico sobre o coração. OBJETIVO: Estudar a variabilidade da frequência cardíaca em pacientes com DM tipo 1. MÉTODOS: Estudamos a VFC durante registros de 5 minutos em pacientes com DM em um grupo controle saudável. Analisamos os domínios da frequência (BF e AF) em unidades normalizadas (un) e balanço simpático-vagal, na posição sentada e em decúbito dorsal. RESULTADOS: Dezessete pacientes (10 homens e 7 mulheres) e dezessete indivíduos pareados saudáveis (10 homens e 7 mulheres) foram estudados. As modulações simpática e parassimpática do coração elevadas em pacientes do sexo masculino com DM da posição em decúbito dorsal para a posição sentada em 19% da AFun e a razão BF/AF aumentaram 42,3%. Na posição sentada, os pacientes do sexo masculino com DM apresentaram balanços simpático-vagal significativamente mais elevados em 50,9% em comparação com indivíduos controles saudáveis. A VFC foi influenciada tanto pelo sexo quanto pela enfermidade apresentada. O sexo influenciou a AFun na posição em decúbito dorsal, enquanto a razão BF/AF e AFun foi afetada em ambas as posições. Análises post hoc mostraram que o sexo afeta significativamente pacientes com DM e indivíduos saudáveis de diferentes maneiras (p < 0,01). O domínio de baixa frequência na posição sentada (AFun) foi significativamente influenciado pela enfermidade. CONCLUSÃO: Os resultados deste estudo sugerem que o estímulo simpático em pacientes de meia-idade do sexo masculino com DM que não está gravemente comprometido e apresenta duração moderada da doença parece ser maior do que em indivíduos saudáveis pareados.

FUNDAMENTO: A Tolerância ao Esforço Físico (TEF) é uma medida de condicionamento cardiorrespiratório. A capacidade aeróbica é reduzida na Insuficiência Cardíaca (IC), embora não haja dados disponíveis sobre esse parâmetro em animais com disfunção ventricular e sem sinais de IC. OBJETIVO: Avaliar a TEF em ratos com disfunção ventricular diastólica isolada ou associada com disfunção sistólica induzida pela Estenose da Aorta ascendente (EAo). MÉTODOS: Ratos Wistar machos jovens (20-30 dias de idade) foram divididos em Grupo Controle (GC, n = 11) e Grupo EAo (n = 12). Os animais foram avaliados em 6 e 18 semanas após a cirurgia para EAo. O teste ergométrico foi feito até a exaustão e foram avaliadas a velocidade da esteira e a concentração de lactato [LAC] no limiar de lactato, velocidade da esteira e [LAC] na exaustão, e tempo total do teste. RESULTADOS: Dados ecocardiográficos revelaram remodelação do átrio esquerdo e hipertrofia concêntrica ventricular esquerda em 6 e 18 semanas. A fração de encurtamento endocárdico mostrou-se maior no grupo EAo do que no GC em 6 e 18 semanas. A fração de encurtamento da parede média mostrou-se maior no grupo EAo do que no GC em 6 semanas. O índice cardíaco mostrou-se semelhante no GC e no grupo EAo em 6 e 18 semanas, tendo diminuído entre 6-18 semanas em ambos os grupos. A razão entre a onda E a onda A foi maior no GC do que no grupo EAo em ambos os períodos e não se alterou em ambos os grupos entre a semana 6 e a semana 18. Os parâmetros do teste de esforço na esteira foram semelhantes nos dois grupos tanto na semana 6 quanto na semana 18. CONCLUSÃO: Embora a EAo promova a disfunção diastólica isolada ou associada à disfunção sistólica, em 6 ou 18 semanas, ela não é suficiente para alterar a tolerância ao esforço físico.

FUNDAMENTOS: Novas recomendações sobre valores de referência para normalidade em exames de monitorização ambulatorial da pressão arterial (MAPA) foram propostas pela V Diretriz Brasileira de Monitorização Ambulatorial da Hipertensão Arterial, com base principalmente no estudo IDACO. OBJETIVOS: O presente estudo epidemiológico tem o objetivo de avaliar o impacto da adoção desses novos critérios em um ambulatório de referência em hipertensão arterial. MÉTODOS: Foram analisados resultados de 1.567 exames de MAPA realizados entre 2005 e 2010, excluídos 481 pacientes da amostra por não preencherem critérios mínimos de qualidade do exame. Para a classificação desses exames quanto à anormalidade, foram utilizados os valores de referência da IV Diretriz Brasileira de MAPA (2005) e comparados com as mudanças propostas na V Diretriz Brasileira de MAPA (2011). Foi realizada análise estatística pelo método do Q² de Pearson, considerando-se p significativo < 0,05. RESULTADOS: Para os 1.086 exames avaliados, houve importante diferença na proporção de pacientes com MAPA alterado, em especial para a variável pressão arterial sistólica do sono: 49% adotando os valores de corte de 2005 e 71% adotando os de 2011, com significância estatística, p < 0,0001. CONCLUSÕES: A recomendação da nova diretriz causou grande impacto na classificação da hipertensão pelos exames de MAPA dentro da população estudada. A questão sobre os limiares desses exames para metas terapêuticas de pacientes sabidamente hipertensos ainda está em aberto e carece de mais estudos, preferencialmente nacionais, para melhor definição do assunto.

Fundamento: O enxerto de artéria radial (AR) foi o segundo enxerto arterial a ser introduzido na prática clínica para revascularização miocárdica. A técnica de esqueletização da artéria torácica interna esquerda (ATIE) pode, de fato, alterar a capacidade de fluxo do enxerto com potenciais vantagens, o que leva à suposição de que o comportamento da AR, como enxerto coronariano, seja semelhante ao da ATIE esqueletizada. Objetivo: Este estudo avaliou enxertos aortocoronários "livres" de AR, quer esqueletizados, quer com tecidos adjacentes. Métodos: Foi realizado um estudo prospectivo randomizado comparando 40 pacientes distribuídos em dois grupos. No grupo I, foram utilizadas artérias radiais esqueletizadas (20 pacientes), e no grupo II, artérias radiais com tecidos adjacentes (20 pacientes). Após o procedimento cirúrgico, os pacientes foram submetidos a medidas da velocidade de fluxo. Resultados: As principais variáveis cirúrgicas foram: diâmetro interno, comprimento e fluxo sanguíneo livre da AR. Os diâmetros médios dos enxertos de AR calculados através de angiografia quantitativa no pós-operatório imediato foram semelhantes, assim como as variáveis de medidas de velocidade de fluxo. Por outro lado, a cinecoronariografia mostrou a presença de oclusão em um enxerto de AR e estenose em cinco enxertos de AR no GII, enquanto que apenas um caso de estenose em um enxerto de AR no GI (p = 0,045). Conclusão: Os resultados mostram que tanto as características morfológicas e anatomopatológicas quanto o desempenho hemodinâmico dos enxertos livres de artéria radial, quer preparados de forma esqueletizada ou com tecidos adjacentes, são semelhantes. Entretanto, pode-se observar um maior número de lesões não obstrutivas quando a AR é preparada com tecidos adjacentes.

In dieser Arbeit wird mit JAVADAPTOR ein Ansatz präsentiert, der die statisch getypte Programmiersprache Java um die Möglichkeit erweitert, Programme zur Laufzeit und ohne die genannten Einschränkungen zu aktualisieren. Das vorgestellte Konzept kombiniert das schemaverändernde Nachladen von Klassen mit Referenzaktualisierungen auf der Basis von Java HotSwap, Containern und Proxies. Zentrale Beiträge der Arbeit sind detaillierte Beschreibungen der Konzepte und deren Implementierung, sowie der Nachweis der Praxvistauglichkeit der Lösung anhand verschiedener Fallstudien. Weiterhin wird untersucht, ob die präsentierte Lösung Einschränkungen bezüglich Flexibilität, Performanz, Plattform, Programmarchitektur oder Änderungsgranularität unterliegt.

Background: Antihypertensive drugs are used to control blood pressure (BP) and reduce macro- and microvascular complications in hypertensive patients with diabetes. Objectives: The present study aimed to compare the functional vascular changes in hypertensive patients with type 2 diabetes mellitus after 6 weeks of treatment with amlodipine or losartan. Methods: Patients with a previous diagnosis of hypertension and type 2 diabetes mellitus were randomly divided into 2 groups and evaluated after 6 weeks of treatment with amlodipine (5 mg/day) or losartan (100 mg/day). Patient evaluation included BP measurement, ambulatory BP monitoring, and assessment of vascular parameters using applanation tonometry, pulse wave velocity (PWV), and flow-mediated dilation (FMD) of the brachial artery. Results: A total of 42 patients were evaluated (21 in each group), with a predominance of women (71%) in both groups. The mean age of the patients in both groups was similar (amlodipine group: 54.9 ± 4.5 years; losartan group: 54.0 ± 6.9 years), with no significant difference in the mean BP [amlodipine group: 145 ± 14 mmHg (systolic) and 84 ± 8 mmHg (diastolic); losartan group: 153 ± 19 mmHg (systolic) and 90 ± 9 mmHg (diastolic)]. The augmentation index (30% ± 9% and 36% ± 8%, p = 0.025) and augmentation pressure (16 ± 6 mmHg and 20 ± 8 mmHg, p = 0.045) were lower in the amlodipine group when compared with the losartan group. PWV and FMD were similar in both groups. Conclusions: Hypertensive patients with type 2 diabetes mellitus treated with amlodipine exhibited an improved pattern of pulse wave reflection in comparison with those treated with losartan. However, the use of losartan may be associated with independent vascular reactivity to the pressor effect.

Background: When performing the Valsalva maneuver (VM), adults and preadolescents produce the same expiratory resistance values. Objective: To analyze heart rate (HR) in preadolescents performing VM, and propose a new method for selecting expiratory resistance. Method: The maximal expiratory pressure (MEP) was measured in 45 sedentary children aged 9-12 years who subsequently performed VM for 20 s using an expiratory pressure of 60%, 70%, or 80% of MEP. HR was measured before, during, and after VM. These procedures were repeated 30 days later, and the data collected in the sessions (E1, E2) were analyzed and compared in periods before, during (0-10 and 10-20 s), and after VM using nonparametric tests. Results: All 45 participants adequately performed VM in E1 and E2 at 60% of MEP. However, only 38 (84.4%) and 25 (55.5%) of the participants performed the maneuver at 70% and 80% of MEP, respectively. The HR delta measured during 0-10 s and 10-20 s significantly increased as the expiratory effort increased, indicating an effective cardiac autonomic response during VM. However, our findings suggest the VM should not be performed at these intensities. Conclusion: HR increased with all effort intensities tested during VM. However, 60% of MEP was the only level of expiratory resistance that all participants could use to perform VM. Therefore, 60% of MEP may be the optimal expiratory resistance that should be used in clinical practice.

Background: End-stage kidney disease patients continue to have markedly increased cardiovascular disease morbidity and mortality. Analysis of genetic factors connected with the renin-angiotensin system that influences the survival of the patients with end-stage kidney disease supports the ongoing search for improved outcomes. Objective: To assess survival and its association with the polymorphism of renin-angiotensin system genes: angiotensin I-converting enzyme insertion/deletion and angiotensinogen M235T in patients undergoing hemodialysis. Methods: Our study was designed to examine the role of renin-angiotensin system genes. It was an observational study. We analyzed 473 chronic hemodialysis patients in four dialysis units in the state of Rio de Janeiro. Survival rates were calculated by the Kaplan-Meier method and the differences between the curves were evaluated by Tarone-Ware, Peto-Prentice, and log rank tests. We also used logistic regression analysis and the multinomial model. A p value ≤ 0.05 was considered to be statistically significant. The local medical ethics committee gave their approval to this study. Results: The mean age of patients was 45.8 years old. The overall survival rate was 48% at 11 years. The major causes of death were cardiovascular diseases (34%) and infections (15%). Logistic regression analysis found statistical significance for the following variables: age (p = 0.000038), TT angiotensinogen (p = 0.08261), and family income greater than five times the minimum wage (p = 0.03089), the latter being a protective factor. Conclusions: The survival of hemodialysis patients is likely to be influenced by the TT of the angiotensinogen M235T gene.

Background: Heart failure with preserved ejection fraction (HFPEF) is the most common form of heart failure (HF), its diagnosis being a challenge to the outpatient clinic practice. Objective: To describe and compare two strategies derived from algorithms of the European Society of Cardiology Diastology Guidelines for the diagnosis of HFPEF. Methods: Cross-sectional study with 166 consecutive ambulatory patients (67.9±11.7 years; 72% of women). The strategies to confirm HFPEF were established according to the European Society of Cardiology Diastology Guidelines criteria. In strategy 1 (S1), tissue Doppler echocardiography (TDE) and electrocardiography (ECG) were used; in strategy 2 (S2), B-type natriuretic peptide (BNP) measurement was included. Results: In S1, patients were divided into groups based on the E/E'ratio as follows: GI, E/E'> 15 (n = 16; 9%); GII, E/E'8 to 15 (n = 79; 48%); and GIII, E/E'< 8 (n = 71; 43%). HFPEF was confirmed in GI and excluded in GIII. In GII, TDE [left atrial volume index (LAVI) ≥ 40 mL/m2; left ventricular mass index LVMI) > 122 for women and > 149 g/m2 for men] and ECG (atrial fibrillation) parameters were assessed, confirming HFPEF in 33 more patients, adding up to 49 (29%). In S2, patients were divided into three groups based on BNP levels. GI (BNP > 200 pg/mL) consisted of 12 patients, HFPEF being confirmed in all of them. GII (BNP ranging from 100 to 200 pg/mL) consisted of 20 patients with LAVI > 29 mL/m2, or LVMI ≥ 96 g/m2 for women or ≥ 116 g/m2 for men, or E/E'≥ 8 or atrial fibrillation on ECG, and the diagnosis of HFPEF was confirmed in 15. GIII (BNP < 100 pg/mL) consisted of 134 patients, 26 of whom had the diagnosis of HFPEF confirmed when GII parameters were used. Measuring BNP levels in S2 identified 4 more patients (8%) with HFPEF as compared with those identified in S1. Conclusion: The association of BNP measurement and TDE data is better than the isolated use of those parameters. BNP can be useful in identifying patients whose diagnosis of HF had been previously excluded based only on TDE findings.

Background: Obesity is defined by excessive accumulation of body fat relative to lean tissue. Studies during the last few years indicate that cardiac function in obese animals may be preserved, increased or diminished. Objective: Study the energy balance of the myocardium with the hypothesis that the increase in fatty acid oxidation and reduced glucose leads to cardiac dysfunction in obesity. Methods: 30-day-old male Wistar rats were fed standard and hypercaloric diet for 30 weeks. Cardiac function and morphology were assessed. In this paper was viewed the general characteristics and comorbities associated to obesity. The structure cardiac was determined by weights of the heart and left ventricle (LV). Myocardial function was evaluated by studying isolated papillary muscles from the LV, under the baseline condition and after inotropic and lusitropic maneuvers: myocardial stiffness; postrest contraction; increase in extracellular Ca2+ concentration; change in heart rate and inhibitor of glycolytic pathway. Results: Compared with control group, the obese rats had increased body fat and co-morbities associated with obesity. Functional assessment after blocking iodoacetate shows no difference in the linear regression of DT, however, the RT showed a statistically significant difference in behavior between the control and the obese group, most notable being the slope in group C. Conclusion: The energy imbalance on obesity did not cause cardiac dysfunction. On the contrary, the prioritization of fatty acids utilization provides protection to cardiac muscle during the inhibition of glycolysis, suggesting that this pathway is fewer used by obese cardiac muscle.

Background: Nitric oxide (NO) has been largely associated with cardiovascular protection through improvement of endothelial function. Recently, new evidence about modulation of NO release by microRNAs (miRs) has been reported, which could be involved with statin-dependent pleiotropic effects, including anti-inflammatory properties related to vascular endothelium function. Objective: To evaluate the effects of cholesterol-lowering drugs including the inhibitors of cholesterol synthesis, atorvastatin and simvastatin, and the inhibitor of cholesterol absorption ezetimibe on NO release, NOS3 mRNA expression and miRs potentially involved in NO bioavailability. Methods: Human umbilical vein endothelial cells (HUVEC) were exposed to atorvastatin, simvastatin or ezetimibe (0 to 5.0 μM). Cells were submitted to total RNA extraction and relative quantification of NOS3 mRNA and miRs -221, -222 and -1303 by qPCR. NO release was measured in supernatants by ozone-chemiluminescence. Results: Both statins increased NO levels and NOS3 mRNA expression but no influence was observed for ezetimibe treatment. Atorvastatin, simvastatin and ezetimibe down-regulated the expression of miR-221, whereas miR-222 was reduced only after the atorvastatin treatment. The magnitude of the reduction of miR-221 and miR-222 after treatment with statins correlated with the increment in NOS3 mRNA levels. No influence was observed on the miR-1303 expression after treatments. Conclusion: NO release in endothelial cells is increased by statins but not by the inhibitor of cholesterol absorption, ezetimibe. Our results provide new evidence about the participation of regulatory miRs 221/222 on NO release induction mediated by statins. Although ezetimibe did not modulate NO levels, the down-regulation of miR-221 could involve potential effects on endothelial function.

There is a known association between chemotherapy and radiotherapy for treatment of cancer patients and development or worsening of hypertension. The aim of this article is to review this association. A literature search was conducted for articles reporting this association on the databases PubMed, SciELO and LILACS between 1993 and 2013. There was a high coprevalence of hypertension and cancer, since both diseases share the same risk factors, such as sedentary lifestyle, obesity, smoking, unhealthy diet and alcohol abuse. The use of chemotherapy and adjuvant drugs effective in the treatment of cancer increased the survival rate of these patients and, consequently, increased the incidence of hypertension. We described the association between the use of angiogenesis inhibitors (bevacizumab, sorafenib and sunitinib), corticosteroids, erythropoietin and non-steroidal anti-inflammatory drugs with the development of hypertension. We also described the relationship between hypertension and carotid baroreceptor injury secondary to cervical radiotherapy. Morbidity and mortality increased in patients with cancer and hypertension without proper antihypertensive treatment. We concluded that there is need for early diagnosis, effective monitoring and treatment strategies for hypertension in cancer patients in order to reduce cardiovascular morbidity and mortality.

Background: Systemic Arterial Hypertension (SAH) is one of the main risk factors for Coronary Artery Disease (CAD), in addition to male gender. Differences in coronary artery lesions between hypertensive and normotensive individuals of both genders at the Coronary Computed Tomography Angiography (CCTA) have not been clearly determined. Objective: To Investigate the calcium score (CS), CAD extent and characteristics of coronary plaques at CCTA in men and women with and without SAH. Methods: Prospective cross-sectional study of 509 patients undergoing CCTA for CAD diagnosis and risk stratification, from November 2011 to December 2012, at Instituto de Cardiologia Dante Pazzanese. Individuals were stratified according to gender and subdivided according to the presence (HT +) or absence (HT-) of SAH. Results: HT+ women were older (62.3 ± 10.2 vs 57.8 ± 12.8, p = 0.01). As for the assessment of CAD extent, the HT+ individuals of both genders had significant CAD, although multivessel disease is more frequent in HT + men. The regression analysis for significant CAD showed that age and male gender were the determinant factors of multivessel disease and CS ≥ 100. Plaque type analysis showed that SAH was a predictive risk factor for partially calcified plaques (OR = 3.9). Conclusion: Hypertensive men had multivessel disease more often than women. Male gender was a determinant factor of significant CAD, multivessel disease, CS ≥ 100 and calcified and partially calcified plaques, whereas SAH was predictive of partially calcified plaques.
