Eight patients with colloid cysts of the third ventricle were examined with CT and MR. In six, surgical resection was performed and the material was subjected to histologic evaluation; the concentrations of trace elements were determined by particle-induced X-ray emission. Stereotaxic aspiration was performed in two. The investigation showed that colloid cysts are often iso- or hypodense relative to brain on CT (5/8), but sometimes have a center of increased density. Increased density did not correlate with increased concentration of calcium or other metals but did not correlate with high cholesterol content. Colloid cysts appear more heterogeneous on MR (6/8) than on CT (3/8), despite a homogeneous appearance at histology. High signal on short TR/TE sequences is correlated with a high cholesterol content. A marked shortening of the T2 relaxation time is often noticed in the central part of the cyst. Analysis of trace elements showed that this phenomenon is not related to the presence of metals with paramagnetic effects. Our analysis of the contents of colloid cysts does not support the theory that differing metallic concentrations are responsible for differences in MR signal intensity or CT density. We did find that increased CT density and high MR signal correlated with high cholesterol content.

Objetivou-se determinar o potencial do uso da tomografia de ressonância magnética, como método não-destrutivo, para avaliar os efeitos das injúrias mecânicas em goiabas. Foram utilizados frutos no estádio de maturação "de vez" das cultivares Paluma e Pedro Sato. Na injúria por impacto, os frutos foram deixados cair, em queda livre, de uma altura de 1,20 m, sofrendo dois impactos, em lados opostos de sua porção equatorial. Na injúria por compressão, os frutos foram submetidos a um peso de 29,4 N, por 15 minutos. Para a injúria por corte, foram efetuados dois cortes, no sentido longitudinal dos frutos, de exatamente 30 mm de comprimento por 2 mm de profundidade. Os frutos injuriados foram armazenados sob condições de ambiente (22 ± 2 °C e 40 %UR). Foram realizadas análises com tomógrafo de ressonância magnética Varian Inova de 2 Tesla. As imagens foram obtidas a partir da detecção dos prótons de hidrogênio (¹H). Para cada fruto, foram obtidos tomogramas simétricos a partir do centro do fruto. A tomografia de ressonância magnética nuclear mostrou-se uma ferramenta eficaz na detecção de injúrias internas de frutos. O estresse físico causado pelo impacto produziu um colapso interno nos lóculos desses frutos (internal bruising), levando à perda da integridade celular e a conseqüente liquefação dos tecidos placentários. A cultivar Pedro Sato mostrou uma suscetibilidade maior à injúria por impacto que a 'Paluma'. A injúria por compressão tornou-se mais evidente no pericarpo externo do fruto, de ambas as cultivares. A injúria por corte levou a lignificação dos tecidos no local injuriado e deformações superficiais devido à perda acentuada de matéria fresca no local da lesão, evidentes no sexto dia de avaliação.

RPE65 is a retinoid isomerase required for the production of 11-cis-retinal, the chromophore of both cone and rod visual pigments. We recently established an R91W knock-in mouse strain as homologous animal model for patients afflicted by this mutation in RPE65. These mice have impaired vision and can only synthesize minute amounts of 11-cis-retinal. Here, we investigated the consequences of this chromophore insufficiency on cone function and pathophysiology. We found that the R91W mutation caused cone opsin mislocalization and progressive geographic cone atrophy. Remnant visual function was mostly mediated by rods. Ablation of rod opsin corrected the localization of cone opsin and improved cone retinal function. Thus, our analyses indicate that under conditions of limited chromophore supply rods and cones compete for 11-cis-retinal that derives from regeneration pathway(s) which are reliant on RPE65. Due to their higher number and the instability of cone opsin, rods are privileged under this condition while cones suffer chromophore deficiency and degenerate. These findings reinforce the notion that in patients any effective gene therapy with RPE65 needs to target the cone-rich macula directly to locally restore the cones' chromophore supply outside the reach of rods.

In this paper, a new two-dimensional shear deformable beam element based on the absolute nodal coordinate formulation is proposed. The nonlinear elastic forces of the beam element are obtained using a continuum mechanics approach without employing a local element coordinate system. In this study, linear polynomials are used to interpolate both the transverse and longitudinal components of the displacement. This is different from other absolute nodal-coordinate-based beam elements where cubic polynomials are used in the longitudinal direction. The accompanying defects of the phenomenon known as shear locking are avoided through the adoption of selective integration within the numerical integration method. The proposed element is verified using several numerical examples, and the results are compared to analytical solutions and the results for an existing shear deformable beam element. It is shown that by using the proposed element, accurate linear and nonlinear static deformations, as well as realistic dynamic behavior, can be achieved with a smaller computational effort than by using existing shear deformable two-dimensional beam elements.

Over 70% of the total costs of an end product are consequences of decisions that are made during the design process. A search for optimal cross-sections will often have only a marginal effect on the amount of material used if the geometry of a structure is fixed and if the cross-sectional characteristics of its elements are property designed by conventional methods. In recent years, optimalgeometry has become a central area of research in the automated design of structures. It is generally accepted that no single optimisation algorithm is suitable for all engineering design problems. An appropriate algorithm, therefore, mustbe selected individually for each optimisation situation. Modelling is the mosttime consuming phase in the optimisation of steel and metal structures. In thisresearch, the goal was to develop a method and computer program, which reduces the modelling and optimisation time for structural design. The program needed anoptimisation algorithm that is suitable for various engineering design problems. Because Finite Element modelling is commonly used in the design of steel and metal structures, the interaction between a finite element tool and optimisation tool needed a practical solution. The developed method and computer programs were tested with standard optimisation tests and practical design optimisation cases. Three generations of computer programs are developed. The programs combine anoptimisation problem modelling tool and FE-modelling program using three alternate methdos. The modelling and optimisation was demonstrated in the design of a new boom construction and steel structures of flat and ridge roofs. This thesis demonstrates that the most time consuming modelling time is significantly reduced. Modelling errors are reduced and the results are more reliable. A new selection rule for the evolution algorithm, which eliminates the need for constraint weight factors is tested with optimisation cases of the steel structures that include hundreds of constraints. It is seen that the tested algorithm can be used nearly as a black box without parameter settings and penalty factors of the constraints.

The geometric characterisation of tree orchards is a high-precision activity comprising the accurate measurement and knowledge of the geometry and structure of the trees. Different types of sensors can be used to perform this characterisation. In this work a terrestrial LIDAR sensor (SICK LMS200) whose emission source was a 905-nm pulsed laser diode was used. Given the known dimensions of the laser beam cross-section (with diameters ranging from 12 mm at the point of emission to 47.2 mm at a distance of 8 m), and the known dimensions of the elements that make up the crops under study (flowers, leaves, fruits, branches, trunks), it was anticipated that, for much of the time, the laser beam would only partially hit a foreground target/object, with the consequent problem of mixed pixels or edge effects. Understanding what happens in such situations was the principal objective of this work. With this in mind, a series of tests were set up to determine the geometry of the emitted beam and to determine the response of the sensor to different beam blockage scenarios. The main conclusions that were drawn from the results obtained were: (i) in a partial beam blockage scenario, the distance value given by the sensor depends more on the blocked radiant power than on the blocked surface area; (ii) there is an area that influences the measurements obtained that is dependent on the percentage of blockage and which ranges from 1.5 to 2.5 m with respect to the foreground target/object. If the laser beam impacts on a second target/object located within this range, this will affect the measurement given by the sensor. To interpret the information obtained from the point clouds provided by the LIDAR sensors, such as the volume occupied and the enclosing area, it is necessary to know the resolution and the process for obtaining this mesh of points and also to be aware of the problem associated with mixed pixels.

Contexte Lié au vieillissement et à la sédentarisation de la population, ainsi qu'à la chronicisation du cancer, l'emploi de cathéters veineux centraux permanents (CVCP) n'a cessé d'augmenter. La complication majeure de ces dispositifs, induisant de forts taux de morbi-mortalité, est l'infection. Actuellement, le diagnostic de ces infections reste surtout basé sur la clinique et les hémocultures. Lorsque le doute persiste, une ablation chirurgicale suivie de la mise en culture des prélèvements chirurgicaux et du cathéter permettent de poser le diagnostic. En clinique, après ces examens, nous constatons que seule la moitié des cathéters retirés étaient réellement infectés. Alors que la tomographie par émission de positons fusionnée à la tomographie (PET/CT) a montré de bons résultats dans la détection des infections chroniques, la valeur diagnostique du PET/CT au fluorodeoxyglucose marqué au 18F (18F-FDG) pour les infections de CVCP n'a encore jamais été déterminée dans une étude prospective. Objectifs Au travers de cette étude prospective, ouverte et monocentrique, nous chercherons à connaître la valeur diagnostique du PET/CT au 18F-FDG dans la détection d'infections de CVCP et ainsi d'en déterminer son utilité. Nous essaierons aussi de déterminer la différence de valeur diagnostique du PET/CT au 18F-FDG par rapport aux méthodes conventionnelles (paramètres cliniques et culture du liquide d'aspiration), afin de se déterminer sur l'éventuelle utilité diagnostique de celui-ci. Méthodes Cadre : Etude prospective d'au moins 20 patients, avec 2 groupes contrôles d'au moins 10 patients ayant chacun respectivement une faible et une forte probabilité d'infection, soit au moins 40 patients au total. Population : patients adultes avec CVCP devant être retiré. Cette étude prévoit un examen PET/CT au 18F-FDG effectué auprès de patients nécessitant une ablation de CVCP sur suspicion d'infection, sans confirmation possible par les moyens diagnostiques non chirurgicaux. Deux acquisitions seront réalisées 45 et 70 minutes après l'injection de 5,5MBq/Kg de 18F-FDG. Le groupe contrôle à faible probabilité d'infection, sera formé de patients bénéficiant de l'ablation définitive d'un CVCP pour fin de traitement durant le laps de temps de l'étude, et ayant bénéficié au préalable d'un examen PET/CT pour raison X. Après avoir retiré chirurgicalement le CVCP, nous utiliserons la culture microbiologique des deux extrémités du CVCP comme étalon d'or (gold standard) de l'infection. Le groupe contrôle à forte probabilité d'infection sera formé de patients nécessitant une ablation de CVCP sur infection de CVCP confirmée par les moyens diagnostiques non chirurgicaux (culture positive du liquide de l'aspiration). Lors de l'examen PET/CT, ces patients auront aussi deux acquisitions réalisées 45 et 70 minutes après l'injection de 5,5MBq/Kg de 18F-FDG. Les résultats de ces examens seront évalués par deux spécialistes en médecine nucléaire qui détermineront le niveau de suspicion de l'infection sur une échelle de Likert allant de I à V, sur la base du nombre de foyers, de la localisation du foyer, de l'intensité de la captation de 18F-FDG au voisinage du cathéter et du rapport tissu/arrière-plan. Par la suite, nous retirerons chirurgicalement le CVCP. Nous utiliserons la culture microbiologique du pus (si présent), des deux extrémités du CVCP ainsi que l'histologie des tissus formant un tunnel autour du cathéter comme étalon d'or de l'infection. Les résultats seront analysés à l'aide de courbes ROC (Receiver Operating Characteristic) afin de déterminer la valeur diagnostique du PET/CT dans l'infection de CVCP. Les résultats des examens des patients avec suspicion clinique d'infection seront ensuite analysés séparément, afin de déterminer la différence de valeur diagnostique du PET/CT au 18F-FDG par rapport aux méthodes conventionnelles. Résultats escomptés Ce projet veut chercher à savoir si le PET/CT au 18F-FDG peut être un moyen diagnostique valide dans les infections de CVCP, s'avérer utile lorsque les autres moyens diagnostiques sont non conclusifs. Plus-value escomptée Actuellement, lors d'incertitude sur le diagnostic d'infection de CVCP, une opération chirurgicale est effectuée à titre préventif afin d'enlever le cathéter en cause, cependant seulement la moitié de ces cathéters sont réellement infectés en pratique. Le PET/CT au 18F-FDG, grâce à sa sensibilité élevée et probablement une bonne valeur prédictive négative, pourrait éviter à une partie des patients un retrait inutile du cathéter, diminuant ainsi les risques chirurgicaux et les coûts liés à de telles opérations, tout en préservant le capital d'accès vasculaire futur.

PURPOSE: The aim of this study was to compare multidetector CT (MDCT), MRI, and FDG PET/CT imaging for the detection of peritoneal carcinomatosis (PC) in ovarian cancer. PATIENTS AND METHODS: Fifteen women with ovarian cancer and suspected PC underwent MDCT, MRI, and FDG PET/CT, shortly before surgery. Nine abdominopelvic regions were defined according to the peritoneal cancer index. We applied lesion size scores on MDCT and MR and measured FDG PET/CT standard uptake. We blindly read MDCT, MR, and PET/CT before joint review and comparison with histopathology. Receiver operating characteristics analysis was performed. RESULTS: Ten women had PC (67%). Altogether, 135 abdominopelvic sites were compared. Multidetector CT, MRI, and FDG PET/CT had a sensitivity of 96%, 98%, and 95%, and specificity was 92%, 84%, and 96%, respectively. Corresponding receiver operating characteristics area was 0.94, 0.90, and 0.96, respectively, without any significant differences between them (P = 0.12). FDG PET/CT detected supradiaphragmatic disease in 3 women (20%) not seen by MDCT or MRI. CONCLUSIONS: Although MRI had the highest sensitivity and FDG PET/CT had the highest specificity, no significant differences were found between the 3 techniques. Thus, MDCT, as the fastest, most economical, and most widely available modality, is the examination of choice, if a stand-alone technique is required. If inconclusive, PET/CT or MRI may offer additional insights. Whole-body FDG PET/CT may be more accurate for supradiaphragmatic metastatic extension.
