Ouabain is an endogenous substance occurring in the plasma in the nanomolar range, that has been proposed to increase vascular resistance and induce hypertension. This substance acts on the a-subunit of Na+,K+-ATPase inhibiting the Na+-pump activity. In the vascular smooth muscle this effect leads to intracellular Na+ accumulation that reduces the activity of the Na+/Ca2+ exchanger and to an increased vascular tone. It was also suggested that circulating ouabain, even in the nanomolar range, sensitizes the vascular smooth muscle to vasopressor substances. We tested the latter hypothesis by studying the effects of ouabain in the micromolar and nanomolar range on phenylephrine (PE)-evoked pressor responses. The experiments were performed in normotensive and hypertensive rats in vivo, under anesthesia, and in perfused rat tail vascular beds. The results showed that ouabain pretreatment increased the vasopressor responses to PE in vitro and in vivo. This sensitization after ouabain treatment was also observed in hypertensive animals which presented an enhanced vasopressor response to PE in comparison to normotensive animals. It is suggested that ouabain at nanomolar concentrations can sensitize vascular smooth muscle to vasopressor stimuli possibly contributing to increased tone in hypertension.

The objective of the present study was to assess the effects of the immunosuppressant rapamycin (Rapamune®, Sirolimus) on both resistance vessel responsiveness and atherosclerosis in apolipoprotein E-deficient 8-week-old male mice fed a normal rodent diet. Norepinephrine (NE)-induced vasoconstriction, acetylcholine (ACh)- and sodium nitroprusside (SNP)-induced vasorelaxation of isolated mesenteric bed, and atherosclerotic lesions were evaluated. After 12 weeks of orally administered rapamycin (5 mg·kg-1·day-1, N = 9) and compared with untreated (control, N = 9) animals, rapamycin treatment did not modify either NE-induced vasoconstriction (maximal response: 114 ± 4 vs 124 ± 10 mmHg, respectively) or ACh- (maximal response: 51 ± 8 vs 53 ± 5%, respectively) and SNP-induced vasorelaxation (maximal response: 73 ± 6 vs 74 ± 6%, respectively) of the isolated vascular mesenteric bed. Despite increased total cholesterol in treated mice (982 ± 59 vs 722 ± 49 mg/dL, P < 0.01), lipid deposition on the aorta wall vessel was significantly less in rapamycin-treated animals (37 ± 12 vs 68 ± 8 µm2 x 103). These results indicate that orally administered rapamycin is effective in attenuating the progression of atherosclerotic plaque without affecting the responsiveness of resistance vessels, supporting the idea that this immunosuppressant agent might be of potential benefit against atherosclerosis in patients undergoing therapy.

Nitric oxide (NO) influences renal blood flow mainly as a result of neuronal nitric oxide synthase (nNOS). Nevertheless, it is unclear how nNOS expression is modulated by endogenous angiotensin II, an inhibitor of NO function. We tested the hypothesis that the angiotensin II AT1 receptor and oxidative stress mediated by NADPH oxidase contribute to the modulation of renal nNOS expression in two-kidney, one-clip (2K1C) hypertensive rats. Experiments were performed on male Wistar rats (150 to 170 g body weight) divided into 2K1C (N = 19) and sham-operated (N = 19) groups. nNOS expression in kidneys of 2K1C hypertensive rats (N = 9) was compared by Western blotting to that of 2K1C rats treated with low doses of the AT1 antagonist losartan (10 mg·kg-1·day-1; N = 5) or the superoxide scavenger tempol (0.2 mmol·kg-1·day-1; N = 5), which still remain hypertensive. After 28 days, nNOS expression was significantly increased by 1.7-fold in the clipped kidneys of 2K1C rats and by 3-fold in the non-clipped kidneys of 2K1C rats compared with sham rats, but was normalized by losartan. With tempol treatment, nNOS expression increased 2-fold in the clipped kidneys and 1.4-fold in the non-clipped kidneys compared with sham rats. The changes in nNOS expression were not followed by changes in the enzyme activity, as measured indirectly by the cGMP method. In conclusion, AT1 receptors and oxidative stress seem to be primary stimuli for increased nNOS expression, but this up-regulation does not result in higher enzyme activity.

Obstructive apnea (OA) can exert significant effects on renal sympathetic nerve activity (RSNA) and hemodynamic parameters. The present study focuses on the modulatory actions of RSNA on OA-induced sodium and water retention. The experiments were performed in renal-denervated rats (D; N = 9), which were compared to sham (S; N = 9) rats. Mean arterial pressure (MAP) and heart rate (HR) were assessed via an intrafemoral catheter. A catheter was inserted into the bladder for urinary measurements. OA episodes were induced via occlusion of the catheter inserted into the trachea. After an equilibration period, OA was induced for 20 s every 2 min and the changes in urine, MAP, HR and RSNA were recorded. Renal denervation did not alter resting MAP (S: 113 ± 4 vs D: 115 ± 4 mmHg) or HR (S: 340 ± 12 vs D: 368 ± 11 bpm). An OA episode resulted in decreased HR and MAP in both groups, but D rats showed exacerbated hypotension and attenuated bradycardia (S: -12 ± 1 mmHg and -16 ± 2 bpm vs D: -16 ± 1 mmHg and 9 ± 2 bpm; P < 0.01). The basal urinary parameters did not change during or after OA in S rats. However, D rats showed significant increases both during and after OA. Renal sympathetic nerve activity in S rats increased (34 ± 9%) during apnea episodes. These results indicate that renal denervation induces elevations of sodium content and urine volume and alters bradycardia and hypotension patterns during total OA in unconscious rats.

A transitory increase in blood pressure (BP) is observed following upper airway surgery for obstructive sleep apnea syndrome but the mechanisms implicated are not yet well understood. The objective of the present study was to evaluate changes in BP and heart rate (HR) and putative factors after uvulopalatopharyngoplasty and septoplasty in normotensive snorers. Patients (N = 10) were instrumented for 24-h ambulatory BP monitoring, nocturnal respiratory monitoring and urinary catecholamine level evaluation one day before surgery and on the day of surgery. The influence of postsurgery pain was prevented by analgesic therapy as confirmed using a visual analog scale of pain. Compared with preoperative values, there was a significant (P < 0.05) increase in nighttime but not daytime systolic BP (119 ± 5 vs 107 ± 3 mmHg), diastolic BP (72 ± 4 vs 67 ± 2 mmHg), HR (67 ± 4 vs 57 ± 2 bpm), respiratory disturbance index (RDI) characterized by apnea-hypopnea (30 ± 10 vs 13 ± 4 events/h of sleep) and norepinephrine levels (22.0 ± 4.7 vs 11.0 ± 1.3 µg l-1 12 h-1) after surgery. A positive correlation was found between individual variations of BP and individual variations of RDI (r = 0.81, P < 0.01) but not between BP or RDI and catecholamines. The visual analog scale of pain showed similar stress levels on the day before and after surgery (6.0 ± 0.8 vs 5.0 ± 0.9 cm, respectively). These data strongly suggest that the cardiovascular changes observed in patients who underwent uvulopalatopharyngoplasty and septoplasty were due to the increased postoperative RDI.

Borderline hypertension (BH) has been associated with an exaggerated blood pressure (BP) response during laboratory stressors. However, the incidence of target organ damage in this condition and its relation to BP hyperreactivity is an unsettled issue. Thus, we assessed the Doppler echocardiographic profile of a group of BH men (N = 36) according to office BP measurements with exaggerated BP in the cycloergometric test. A group of normotensive men (NT, N = 36) with a normal BP response during the cycloergometric test was used as control. To assess vascular function and reactivity, all subjects were submitted to the cold pressor test. Before Doppler echocardiography, the BP profile of all subjects was evaluated by 24-h ambulatory BP monitoring. All subjects from the NT group presented normal monitored levels of BP. In contrast, 19 subjects from the original BH group presented normal monitored BP levels and 17 presented elevated monitored BP levels. In the NT group all Doppler echocardiographic indexes were normal. All subjects from the original BH group presented normal left ventricular mass and geometrical pattern. However, in the subjects with elevated monitored BP levels, fractional shortening was greater, isovolumetric relaxation time longer, and early to late flow velocity ratio was reduced in relation to subjects from the original BH group with normal monitored BP levels (P<0.05). These subjects also presented an exaggerated BP response during the cold pressor test. These results support the notion of an integrated pattern of cardiac and vascular adaptation during the development of hypertension.

Cardiac hypertrophy that accompanies hypertension seems to be a phenomenon of multifactorial origin whose development does not seem to depend on an increased pressure load alone, but also on local growth factors and cardioadrenergic activity. The aim of the present study was to determine if sympathetic renal denervation and its effects on arterial pressure level can prevent cardiac hypertrophy and if it can also delay the onset and attenuate the severity of deoxycorticosterone acetate (DOCA)-salt hypertension. DOCA-salt treatment was initiated in rats seven days after uninephrectomy and contralateral renal denervation or sham renal denervation. DOCA (15 mg/kg, sc) or vehicle (soybean oil, 0.25 ml per animal) was administered twice a week for two weeks. Rats treated with DOCA or vehicle (control) were provided drinking water containing 1% NaCl and 0.03% KCl. At the end of the treatment period, mean arterial pressure (MAP) and heart rate measurements were made in conscious animals. Under ether anesthesia, the heart was removed and the right and left ventricles (including the septum) were separated and weighed. DOCA-salt treatment produced a significant increase in left ventricular weight/body weight (LVW/BW) ratio (2.44 ± 0.09 mg/g) and right ventricular weight/body weight (RVW/BW) ratio (0.53 ± 0.01 mg/g) compared to control (1.92 ± 0.04 and 0.48 ± 0.01 mg/g, respectively) rats. MAP was significantly higher (39%) in DOCA-salt rats. Renal denervation prevented (P>0.05) the development of hypertension in DOCA-salt rats but did not prevent the increase in LVW/BW (2.27 ± 0.03 mg/g) and RVW/BW (0.52 ± 0.01 mg/g). We have shown that the increase in arterial pressure level is not responsible for cardiac hypertrophy, which may be more related to other events associated with DOCA-salt hypertension, such as an increase in cardiac sympathetic activity.

The maintenance of arterial pressure at levels adequate to perfuse the tissues is a basic requirement for the constancy of the internal environment and survival. The objective of the present review was to provide information about the basic reflex mechanisms that are responsible for the moment-to-moment regulation of the cardiovascular system. We demonstrate that this control is largely provided by the action of arterial and non-arterial reflexes that detect and correct changes in arterial pressure (baroreflex), blood volume or chemical composition (mechano- and chemosensitive cardiopulmonary reflexes), and changes in blood-gas composition (chemoreceptor reflex). The importance of the integration of these cardiovascular reflexes is well understood and it is clear that processing mainly occurs in the nucleus tractus solitarii, although the mechanism is poorly understood. There are several indications that the interactions of baroreflex, chemoreflex and Bezold-Jarisch reflex inputs, and the central nervous system control the activity of autonomic preganglionic neurons through parallel afferent and efferent pathways to achieve cardiovascular homeostasis. It is surprising that so little appears in the literature about the integration of these neural reflexes in cardiovascular function. Thus, our purpose was to review the interplay between peripheral neural reflex mechanisms of arterial blood pressure and blood volume regulation in physiological and pathophysiological states. Special emphasis is placed on the experimental model of arterial hypertension induced by N-nitro-L-arginine methyl ester (L-NAME) in which the interplay of these three reflexes is demonstrable.

Nephrolithiasis is one of the most common diseases in the Western world. The disease manifests itself with intensive pain, sporadic infections, and, sometimes, renal failure. The symptoms are due to the appearance of urinary stones (calculi) which are formed mainly by calcium salts. These calcium salts precipitate in the renal papillae and/or within the collecting ducts. Inherited forms of nephrolithiasis related to chromosome X (X-linked hypercalciuric nephrolithiasis or XLN) have been recently described. Hypercalciuria, nephrocalcinosis, and male predominance are the major characteristics of these diseases. The gene responsible for the XLN forms of kidney stones was cloned and characterized as a chloride channel called ClC-5. The ClC-5 chloride channel belongs to a superfamily of voltage-gated chloride channels, whose physiological roles are not completely understood. The objective of the present review is to identify recent advances in the molecular pathology of nephrolithiasis, with emphasis on XLN. We also try to establish a link between a chloride channel like ClC-5, hypercalciuria, failure in urine acidification and protein endocytosis, which could explain the symptoms exhibited by XLN patients.

OBJETIVO: O presente trabalho teve como objetivo investigar a eficiência da microcirurgia endonasal no tratamento da polipose nasal, considerando número de recidivas da doença de acordo com o sexo, idade, estadiamento e profissões exercidas pelos pacientes atendidos no Departamento de Otorrinolaringologia e Cirurgia de Cabeça e Pescoço da Faculdade de Medicina de São José do Rio Preto - FAMERP. FORMA DE ESTUDO: Estudo clássico de coorte longitudinal. MATERIAL E MÉTODO: Trinta pacientes foram submetidos à microcirurgia endonasal no período de 1991 a 1996, sendo 16 do sexo feminino e 14 do sexo masculino, em seguimento pós-operatório de um a cinco anos. RESULTADO: A freqüência de recidivas foi de 40%, havendo predominância estatisticamente significante (testes do qui-quadrado e de Fisher, valores p=0,03 e 0,024 respectivamente) no sexo masculino (75%). Como complicação destaca-se a presença de sinéquia em quatro (13,3%) casos. CONCLUSÃO: Em conclusão, a microcirurgia endonasal para tratamento da polipose nasal permitiu a 60% dos pacientes ficarem isentos de recidivas da doença em seguimento de um a cinco anos, com freqüência maior no sexo masculino. Essa técnica mostrou-se eficaz no tratamento da polipose nasal, tendo como complicação apenas a presença de sinéquia em 13,3% dos casos.

A utilização de cartilagem de tragus para o fechamento de perfurações de membrana timpânica (MT) com acesso transcanal foi primeiramente descrito por Eavey em 1998 com excelentes resultados quanto à pega do enxerto e vantagens como facilidade técnica, rapidez e conforto pós-operatório ao paciente. OBJETIVO: Avaliar os resultados da miringoplastia inlay com cartilagem de tragus em pacientes operados no serviço de Otorrinolaringologia do Hospital das Clínicas da Faculdade de Medicina de Ribeirão Preto da Universidade de São Paulo nos últimos três anos. FORMA DE ESTUDO: Clínico prospectivo. MATERIAL E MÉTODO: Foram utilizadas para esse estudo 32 cirurgias realizadas no serviço de Otorrinolaringologia do Hospital das Clínicas de Ribeirão Preto no período de 2000 a 2003 em pacientes com diagnóstico de otite média crônica simples com perfuração de membrana timpânica de até 5mm de diâmetro e sem contra-indicações para realização de procedimento transcanal. RESULTADOS: Foi observado fechamento completo das perfurações de MT em 28 cirurgias das 32 realizadas perfazendo 87,5% de sucesso. DISCUSSÃO: O presente estudo manteve o alto índice de sucesso no fechamento de perfurações de MT com os princípios da técnica de Eavey descrito em estudos anteriores, mesmo com algumas modificações em relação à técnica original. CONCLUSÃO: A miringoplastia inlay com cartilagem de tragus em forma de asa de borboleta tem alto índice de sucesso no fechamento das perfurações de MT de até 5mm de diâmetro em casos de otite média crônica simples com orelha média saudável. Promove, ainda, maior conforto e menor morbidade ao paciente.

Os sintomas otológicos são freqüentes em pacientes com desordem temporomandibular, e estudos são necessários para elucidar os mecanismos envolvidos. OBJETIVO: O objetivo desse estudo clínico foi investigar a associação de sintomas otológicos (otalgia, zumbido e plenitude auricular) com os achados audiológicos, os outros sinais/sintomas relacionados à desordem temporomandibular, e os hábitos parafuncionais orais. FORMA DE ESTUDO: Prospectivo clínico. MATERIAL E MÉTODO: 27 pacientes com desordem temporomandibular, da Clínica de Oclusão da Faculdade de Odontologia de Ribeirão Preto da Universidade de São Paulo, responderam um questionário sobre sinais, sintomas e hábitos orais, e passaram por avaliações otorrinolaringológica e audiológica. Os dados foram analisados pelos testes Binomial, Exato de Fisher e correlação produto-momento de Pearson. O índice de significância adotado foi p<0,05. RESULTADOS: Os sintomas otológicos foram presentes em 88,88% dos pacientes (59,26% apresentavam otalgia, 74,07% zumbido e 74,07% plenitude auricular). Não houve associação significante entre os sintomas otológicos e os achados audiológicos. Houve associação significante entre os sintomas otológicos e os movimentos mandibulares e funções (falar, abrir e fechar a boca). Houve também correlações significantes entre o grau de severidade dos sintomas otológicos e o grau de outros sinais/sintomas de desordem temporomandibular; e entre o sintoma plenitude auricular e número de hábitos parafuncionais. CONCLUSÃO: Este estudo fornece sustentação adicional à noção de que há relação entre desordem temporomandibular e sintomas otológicos. Nos pacientes com desordem temporomandibular as alterações do sistema estomatognático, como a dor orofacial e a dificuldade nas atividades funcionais, foi associada de modo significante aos sintomas otológicos.

A variabilidade em genes relacionados aos processos de ativação e detoxificação de carcinógenos pode interferir na suscetibilidade ao câncer. OBJETIVO: Investigar a relação entre os polimorfismos GSTT1 e GSTM1 nulos e o risco para o carcinoma espinocelular de cabeça e pescoço em indivíduos tabagistas. MATERIAL E MÉTODO: Este estudo caso-controle foi realizado na Faculdade de Medicina de São José do Rio Preto, Brasil. Foram avaliadas as freqüências dos genótipos nulos GSTT1 e GSTM1 por PCR multiplex em 60 pacientes com carcinoma espinocelular de cabeça e pescoço e 60 indivíduos sem a doença. RESULTADOS: A cavidade oral foi o sítio de tumor mais freqüente. O genótipo GSTT1 nulo foi encontrado em 33,3% dos pacientes e em 23,3% dos indivíduos controles (p=0,311). Os grupos caso e controle apresentaram freqüências do genótipo GSTM1 nulo de 35% e 48,3%, respectivamente (p=0,582). Não foram encontradas associações entre o hábito etilista e genótipos nulos GSTT1 e GSTM1 em ambos os grupos (valores de p>0,05). O gênero masculino e o hábito etilista foram prevalentes em ambos os grupos. CONCLUSÃO: Neste estudo não foi possível estabelecer uma correlação entre os genótipos nulos GSTT1 e GSTM1 e o carcinoma espinocelular de cabeça e pescoço em indivíduos tabagistas.

A utilização de mudas de bananeira micropropagadas de qualidade é o primeiro passo na implantação de um bom pomar e, para isso, sua aclimatização deve ser realizada de forma adequada. Em vista disso, objetivou-se, neste trabalho, avaliar o desempenho dos biofertilizantes HUMITEC® e RUTER AA® no desenvolvimento de mudas de bananeira micropropagadas, sendo instalado um experimento em viveiro comercial de produção de mudas. Foram utilizadas mudas de bananeira cv. Grand Naine, obtidas por micropropagação, sendo, posteriormente, transplantadas em sacos de polietileno preto (1,5 L), contendo, como substrato, terra de subsolo (Latossolo Amarelo), casca de arroz carbonizada e composto orgânico Organifol® (1:1:1). O delineamento experimental foi em blocos ao acaso, fatorial 2 x 4 (produtos e doses), com quatro repetições e quatro plantas úteis por parcela. Os tratamentos foram compostos pelos seguintes biofertilizantes e doses: HUMITEC® (0,0; 4,0; 8,0; 16,0 mL planta-1) e RUTER AA® (0,0; 2,0; 4,0; 8,0 mL planta-1). As doses dos produtos foram divididas em duas aplicações (28 e 56 dias após o transplantio das mudas), aplicadas via fertirrigação. Noventa e oito dias após o transplantio avaliaram-se as seguintes variáveis: altura das plantas (cm), diâmetro do colo rente superfície do solo (mm), área foliar total e por folha, biomassas fresca e seca da parte aérea e das raízes (g). Os dados foram submetidos análise da variância e quando o teste F foi significativo, realizou-se análise de regressão. Pode-se concluir que aplicações de HUMITEC® e RUTER AA® favoreceram o desenvolvimento das mudas e que, no verão, as plantas apresentaram maior desenvolvimento durante a aclimatização.

Os fertilizantes podem ser usados para alterar as relações de competitividade, de modo a favorecer as espécies cultivadas, desde que as espécies competidoras apresentem respostas diferenciadas à aplicação de nutrientes. Por essas razões, objetivou-se, com este trabalho, avaliar o crescimento do picão preto (Bidens pilosa L.), da braquiária (Brachiaria decumbens Stapf) e da mandioca (Manihot esculenta Crantz), cultivados sob diferentes quantidades de fósforo fornecidas no plantio. Para isso, foi conduzido experimento em casa de vegetação, em arranjo fatorial 3x4, sendo o primeiro fator constituído pelas espécies estudadas, combinadas com quatro níveis de adubação fosfatadas (0, 80,800 e 4.000 kg ha-1 de P2O5), no delineamento em blocos casualizados, com cinco repetições. A aplicação do fertilizante fosfatado promoveu maior crescimento das espécies avaliadas, sendo que a mandioca apresentou maior crescimento da parte área com o aumento da disponibilidade de fósforo. De maneira geral, as plantas daninhas apresentaram maiores respostas às menores doses de fósforo.
