In the operational context of industrial processes, alarm, by definition, is a warning to the operator that an action with limited time to run is required, while the event is a change of state information, which does not require action by the operator, therefore should not be advertised, and only stored for analysis of maintenance, incidents and used for signaling / monitoring (EEMUA, 2007). However, alarms and events are often confused and improperly configured similarly by developers of automation systems. This practice results in a high amount of pseudo-alarms during the operation of industrial processes. The high number of alarms is a major obstacle to improving operational efficiency, making it difficult to identify problems and increasing the time to respond to abnormalities. The main consequences of this scenario are the increased risk to personal safety, facilities, environment deterioration and loss of production. The aim of this paper is to present a philosophy for setting up a system of supervision and control, developed with the aim of reducing the amount of pseudo-alarms and increase reliability of the information that the system provides. A real case study was conducted in the automation system of the offshore production of hydrocarbons from Petrobras in Rio Grande do Norte, in order to validate the application of this new methodology. Considérant la « nouveauté » du phénomène de dénonciation auprès des policiers et l’utilisation croissante du concept social de « crime motivé par la haine », peu d’études ont été réalisées au Canada sur l’incidence de ces crimes pour les personnes comme pour la société. Cette recherche exploratoire a comme objectif de comprendre la façon dont ce type de crime se distingue des autres manifestations de conflits ou d’incidents et de comprendre les impacts de ce type de victimisation pour les homosexuels en particulier. Plus spécifiquement, ce mémoire vise à approfondir la compréhension du stigmate homosexuel et son impact sur la reportabilité des événements de victimisation criminelle aux autorités judiciaires. Pour ce faire, cinq intervenants communautaires, deux policiers, un avocat et quatre victimes considérant avoir vécu des événements de violence homophobe ont été interviewés. Cet échantillon diversifié a permis de mieux comprendre le phénomène de sous-déclaration des incidents de violences homophobes de la part des victimes et d’obtenir une vue d’ensemble des perceptions des acteurs clés qui peuvent être confrontés au phénomène. L’analyse des entretiens suggère d’importantes lacunes sur le plan de la formation des divers intervenants qui entrainent des difficultés à reconnaître une violence homophobe. Les intervenants confient ne pas se sentir pas suffisamment outillés pour intervenir auprès d’une victime de violence homophobe, n’estiment pas tous posséder les compétences et une compréhension suffisante des réalités des minorités sexuelles, de l'homophobie et de l'hétérosexisme, en somme, l’ensemble des savoirs ultimement nécessaires à une assistance et un accompagnement efficaces pour la déclaration aux autorités d’une telle violence vécue par les victimes. Du côté des victimes de violence(s) homophobe(s), il ressort que la discrimination basée sur l’orientation sexuelle est encore prégnante dans leurs interactions quotidiennes. De leur point de vue, la banalisation et l’impunité de certains comportements homophobes par les instances judiciaires viennent renforcer l’idée chez les victimes et la société d’une forme d’infériorité de l’orientation homosexuelle. L’apposition d’une étiquette homosexuelle paraît ainsi avoir de multiples conséquences psychologiques et sociales sur les victimes, notamment sur leur développement identitaire et sexuel. L’intégration des stigmates homosexuels et l’autostigmatisation, qui les poussent à se déprécier, voire à déprécier l’ensemble de la communauté homosexuelle, surgissent de leur perception de la présence de forts stéréotypes homosexuels, d’une société majoritairement hétérosexiste et de l’opérationnalisation sociale d’une distanciation entre le « nous » hétérosexuel et le « eux » homosexuel. Par leur marginalisation, leur mise en infériorité historique, l’ambiguïté du concept de « crimes motivés par la haine », la noncompréhension de la violence et des répercussions qu’ont les intervenants communautaires et judiciaires de la situation et partant, dans bien des cas, de la prise en charge inadéquate qui en découle pour les victimes des violences homophobes, il est possible de comprendre les appréhensions mentales que les victimes entretiennent ainsi que leur réticence à solliciter de l’aide et encore plus à rapporter la victimisation vécue aux autorités judiciaires.

To describe the epidemiology of domestic swimming pool drowning and near-drowning in Brisbane and to examine the efficacy of a broad range of preventive options, including pool fences.A prospective, hospital-based, injury surveillance system to describe the epidemiology of drowning and near-drowning and a community survey to describe pool fencing.The surveillance questionnaire was completed at presentation in the Emergency Department by the parent, nurse and doctor. Personal interviews in households that were randomly selected by means of a stratified sampling scheme provided the pool fencing description.All 139 children suffering from an immersion injury resulting in presentation at a hospital in the catchment area of The Mater Children's Hospital were included. There were 204 households with a swimming pool in the 1024 households interviewed in the community survey.The 100 domestic pool drownings and near-drownings were equivalent to 15.5 incidents per year per 100,000 children aged 0-13 years and 64.9 per year per 100,000 for the critical 1-3 years age group. Of 72 children who gained unintended access to a domestic pool, 88.9% were less than 3 years of age and 52.8% were less than 2 years. All 10 of the children who drowned and five who were severely brain damaged (age range, 12-32 months) were in this group. The risk of a drowning or near-drowning involving unintended access to an unfenced pool is 3.76 times higher than the risk associated with a fenced pool (95% confidence limits for relative risk: 2.14, 6.62).Pool fences are an effective method of preventing child drownings and near-drownings. This effectiveness can be further improved if compliance with gate closure can be enhanced. This should be emphasised in health promotion accompanying the introduction of universal pool fencing. Hazardous materials are substances that, if not regulated, can pose a threat to human populations and their environmental health, safety or property when transported in commerce. About 1.5 million tons of hazardous material shipments are transported by truck in the US annually, with a steady increase of approximately 5% per year. The objective of this study was to develop a routing tool for hazardous material transport in order to facilitate reduced environmental impacts and less transportation difficulties, yet would also find paths that were still compelling for the shipping carriers as a matter of trucking cost. The study started with identification of inhalation hazard impact zones and explosion protective areas around the location of hypothetical hazardous material releases, considering different parameters (i.e., chemicals characteristics, release quantities, atmospheric condition, etc.). Results showed that depending on the quantity of release, chemical, and atmospheric stability (a function of wind speed, meteorology, sky cover, time and location of accidents, etc.) the consequence of these incidents can differ. The study was extended by selection of other evaluation criteria for further investigation because health risk as an evaluation criterion would not be the only concern in selection of routes. Transportation difficulties (i.e., road blockage and congestion) were incorporated as important factor due to their indirect impact/cost on the users of transportation networks. Trucking costs were also considered as one of the primary criteria in selection of hazardous material paths; otherwise the suggested routes would have not been convincing for the shipping companies. The last but not least criterion was proximity of public places to the routes. The approach evolved from a simple framework to a complicated and efficient GIS-based tool able to investigate transportation networks of any given study area, and capable of generating best routing options for cargos. The suggested tool uses a multi-criteria-decision-making method, which considers the priorities of the decision makers in choosing the cargo routes. Comparison of the routing options based on each criterion and also the overall suitableness of the path in regards to all the criteria (using a multi-criteria-decision-making method) showed that using similar tools as the one proposed by this study can provide decision makers insights in the area of hazardous material transport. This tool shows the probable consequences of considering each path in a very easily understandable way; in the formats of maps and tables, which makes the tradeoffs of costs and risks considerably simpler, as in some cases slightly compromising on trucking cost may drastically decrease the probable health risk and/or traffic difficulties. This will not only be rewarding to the community by making cities safer places to live, but also can be beneficial to shipping companies by allowing them to advertise as environmental friendly conveyors.

In order to reduce serious health incidents, individuals with high risks need to be identified as early as possible so that effective intervention and preventive care can be provided. This requires regular and efficient assessments of risk within communities that are the first point of contacts for individuals. Clinical Decision Support Systems CDSSs have been developed to help with the task of risk assessment, however such systems and their underpinning classification models are tailored towards those with clinical expertise. Communities where regular risk assessments are required lack such expertise. This paper presents the continuation of GRiST research team efforts to disseminate clinical expertise to communities. Based on our earlier published findings, this paper introduces the framework and skeleton for a data collection and risk classification model that evaluates data redundancy in real-time, detects the risk-informative data and guides the risk assessors towards collecting those data. By doing so, it enables non-experts within the communities to conduct reliable Mental Health risk triage.

This article presents some of the findings of research on issues surrounding teaching terrorism and political violence at UK higher education institutions. It reports the results of a survey of UK institutions of higher education on their responses to government and other pressures in relation to terrorism. The data show a minority of universities have developed systems, policies or procedures for ‘preventing violent extremism’, while a significant number have developed close cooperation and collaboration with state counterterrorism policies raising potential issues of academic freedom. This article then examines three high-profile cases – incidents where universities, lecturers and students have come under political and legal pressures over the content of terrorism courses or accusations of ‘radicalisation’ on campus. It suggests that these pressures can be and sometimes are resisted, but that they have on occasion effectively narrowed the scope of academic freedom in practise with the danger that a further chilling effect follows in their wake.

Droughts are probably the natural hazard with the highest socioeconomic impact. Simultaneously, they are a very complex phenomenon; they are triggered by a diversity of physical factors and occur at a variety of time scales. Consequently, the instrumental record currently available is too short and the characterization of its multidecadal variability requires the use of natural proxies (tree rings, sedimentary records) or documentary sources. In this paper we analyse three documentary sources with potential to analyse the long-term variability of droughts: chapter acts, logbooks and chronicles. The chapter acts recorded discussions and decisions made during the assemblies of the local authorities and provide continuous and direct evidence on drought impacts. They are especially useful to study droughts between the 15th and the 19th centuries in Europe and the 17th to 18th in the former colonies. Logbooks recorded the meteorological conditions and the incidents occurred during navigation. They provide indirect information through the circulation indices that can be very helpful to understand the mechanisms and teleconnections associated to droughts. Finally, the chronicles are historiographical documents describing political and social events. They are secondary sources and the references to climatic events are discontinuous, thus their analysis must be extremely careful, but they are especially useful to study specific drought events especially prior to 15th century when no other sources are available.

Okadaic acid and its analogues are potent phosphatase inhibitors that cause Diarrheic Shellfish Poisoning (DSP) through the ingestion of contaminated shellfish by humans. This group of toxins is transmitted worldwide but the number of poisoning incidents has declined over the last 20 years due to legislation and monitoring programs that were implemented for bivalves. In the summer of 2012 and 2013, we collected a total of 101 samples of 22 different species that were made up of benthic and subtidal organisms such echinoderms, crustaceans, bivalves and gastropods from Madeira, São Miguel Island (Azores archipelago) and the northwestern coast of Morocco. The samples were analyzed by UPLC-MS/MS. Our main objective was to detect new vectors for these biotoxins. We can report nine new vectors for these toxins in the North Atlantic: Astropecten aranciacus, Arbacia lixula, Echinaster sepositus, Holothuria sanctori, Ophidiaster ophidianus, Onchidella celtica, Aplysia depilans, Patella spp., and Stramonita haemostoma. Differences in toxin contents among the species were found. Even though low concentrations were detected, the levels of toxins that were present, especially in edible species, indicate the importance of these types of studies. Routine monitoring should be extended to comprise a wider number of vectors other than for bivalves of okadaic acid and its analogues.

Recent perspectives on Pedagogy for Early Childhood Education emphasize children's participation in line with the Children's Rights Convention. The study aimed to analyse how children's rights are dealt with during recess in a school (Early Childhood and Primary Education) in Portugal. The aims were: to characterize the style of the adult (teaching assistants) and the interactions that are established with the children at the playground/recess, and analyse them in terms of children's rights. The qualitative approach was based on the observation during the recess for three weeks. The Adult Style Observation Schedule for Early Childhood Education (ASOS-ECE) was used to register and code the dynamics of six teaching assistants (3 ECE and 3 PE). Critical incidents were also registered. Statistical analysis of the ASOS-ECE levels were complemented with the content analysis of the notes. The playground space was quite limited and affected children's play and well-being. The outdoor playground was never used for the Early Childhood classes, only for Primary Education students but with little supervision. Children were often deprived of playground time by decision of the teaching assistants, as punishment. The results obtained through the Adult Style Observation Schedule for Early Childhood Education (ASOS-ECE) are not satisfactory and are below what would meet the minimum quality value (3,5). For Early Childhood Education, sensibility is the most valued dimension but with very low levels (<2,5). For Primary Education it was autonomy that scored highest (<2,5). The analysis of the notes/critical incidents highlighted articles 12 (expression of own views), 13 (freedom of expression), 19 (protection against violence), 29 (development of personality) and 31 (rest and leisure, play and recreational activities) as being put into question by actions of the adults responsible for the children. Children's rights and well-being need to be put forward in the knowledge base for all adults working with children so that Pedagogy can fulfil its purpose fully.

This project aims at describing the use of Action Research in the development of more structured assessment practices in Early Childhood Portuguese contexts. The teacher had always observed young learners’ activities and progress, and registered them in the form of “critical incidents”. This reflective process structured through this type of narratives helps “tune” the Class Curriculum firstly designed without much knowledge about the kids and so, difficultly responding to their specific needs and interests. The results achieved suggest kids become better prepared to face further education and life. Being early childhood assessment felt by most Portuguese kindergarten teachers as an innovative procedure, the project was seen as the launching of roots for “new” practices.

BACKGROUND: The well-being of relatives of patients having chronic heart diseases (CHD) has been found to be negatively affected by the patient's condition. Studies examining relatives of patients with atrial fibrillation (AF) indicate that their well-being may be affected in a similar manner, but further research is needed. AIM: To explore and describe critical incidents in which relatives of patients experience how AF affects their well-being and what actions they take to handle these situations. DESIGN AND METHOD: An explorative, descriptive design based on the critical incident technique (CIT) was used. Interviews were conducted with 19 relatives (14 women and five men) of patients hospitalised in southern Sweden due to acute symptoms of the AF. RESULTS: The well-being of relatives was found to be affected by their worries (patient-related health), as well as the sacri-ficing of their own needs (self-related health). In handling their own well-being, these relatives adjusted to and supported the patient (practical involvement), along with adjusting their own feelings and responding to the mood of the patients (emotional involvement). CONCLUSION: The well-being of relatives of patients with AF was affected depending on the patients' well-being. In their attempt to handle their own well-being, the relatives adjusted to and supported the patients. Further research is needed in order to evaluate the effects of support to relatives and patients respectively and together.

This review was requested by the Department of Disabilities and Special Needs for an independent review of allegations of abuse, neglect, and exploitation at SC Mentor, a private provider of residential services for DDSN consumers. An analysis of incidents did not indicate systemic abuse towards consumers inasmuch as the majority of the ANE reporting system contained allegations more akin to staff/facility performance issues and the vast majority of all allegations were unsustained by independent investigations. In the future, DDSN should expand the level of detail in its ANE reporting, which currently only reports total allegations and sustained criminal incidents.
