Amine-reactive OVA multimers for auto-vaccination against cytokines and other mediators: perspectives illustrated for GCP-2 in L. major infection.

Anticytokine auto-vaccination is a powerful tool for the study of cytokine functions in vivo but has remained rather esoteric as a result of numerous technical difficulties. We here describe a two-step procedure based on the use of OVA multimers purified by size exclusion chromatography after incubation with glutaraldehyde at pH 6. When such polymers are incubated with a target protein at pH 8.5 to deprotonate reactive amines, complexes are formed that confer immunogenicity to self-antigens. The chemokine GCP-2/CXCL6, the cytokines GM-CSF, IL-17F, IL-17E/IL-25, IL-27, and TGF-β1, and the MMP-9/gelatinase B are discussed as examples. mAb, derived from such immunized mice, have obvious advantages for in vivo studies of the target proteins. Using a mAb against GCP-2, obtained by the method described here, we provide the first demonstration of the major role played by this chemokine in rapid neutrophil mobilization after Leishmania major infection. Pre-activated OVA multimers reactive with amine residues thus provide an efficient carrier for auto-vaccination against 9-90 kDa autologous proteins.

Precision determination of r(0)Lambda((MS)over-bar) from the QCD static energy

We use the recently obtained theoretical expression for the complete QCD static energy at next-to-next-to-next-to leading-logarithmic accuracy to determine r(0)Lambda((MS) over bar) by comparison with available lattice data, where r(0) is the lattice scale and Lambda((MS) over bar) is the QCD scale. We obtain r(0)Lambda((MS) over bar) = 0.622(-0.015)(+0.019) for the zero-flavor case. The procedure we describe can be directly used to obtain r(0)Lambda((MS) over bar) in the unquenched case, when unquenched lattice data for the static energy at short distances becomes available. Using the value of the strong coupling alpha(s) as an input, the unquenched result would provide a determination of the lattice scale r(0).

Transolecranon anterior fracture dislocation.

Between January 1996 and July 2003, 93 consecutive patients operated on with a diagnosis of olecranon fractures were identified from our trauma unit files. Fourteen transolecranon fracture-dislocations were found after a retrospective X-radiographic evaluation. Eight patients were women and six were men, with a mean age of 54 years. There were 4 noncomminuted olecranon fractures, treated with K-wires and single tension-band wiring. The remaining 10 fractures were complex fractures, treated in 3 cases with multiple K-wires and single tension-band wiring, in 2 by use of one-third tubular plates, in 1 with a 3.5-mm dynamic compression plate, and in the remaining 4 with 3.5-mm reconstruction plates. Ligament repair was not performed in any case. Three patients needed reoperation because of early failure of primary fixation. Patients were reviewed at a mean follow-up of 3.6 years. Two reported difficulties in daily activities, none with any symptoms of elbow instability. According to the Broberg and Morrey score, 4 patients had excellent results, 6 had good results, 2 had fair results, and 2 had poor results. Four patients showed signs of degenerative arthritis on the radiographs obtained at follow-up. We conclude that transolecranon fracture-dislocation is an underreported and misdiagnosed injury. Various fixation techniques can restore the anatomic relationships and contour of the trochlear notch; the imperative goal is to obtain a good stable primary fixation and allow early active mobilization.

Leaving the cave in search of life (A Platonic reading of R. Sirera's La caverna (The Cave)

The aim of this article is to show how a contemporary playwright thinks once more of the Platonic image of the cave in order to reflect on the necessary existential journey of men and women as in the case of a Bildungsroman. Sooner or later men and women must abandon the protection that any sort of cavern such as home, the family garden or family itself can offer. In spite of writing from a by no means idealistic or metaphysical point of view, thanks to R. Sirera and to the very applicability of Platonic images, Plato becomes once again a classical reference which is both useful and even unavoidable if one bears in mind the Platonic origin of all the literary caverns.

Ist ein Dossier bei der IV, ist es meist bereits zu spät! Replik und Erweiterung der durch R. Conne angestossenen Diskussion über Arbeitsfähigkeit und Invalidenversicherung

Im Artikel «Arbeitsfähigkeit und Invalidenversicherung » von R. Conne [1] wird treffend das heutige Malaise der Arbeitsplatzrehabilitation beschrieben. Dies gilt aber nicht nur für die Arbeitsunfähigkeit aus psychischen Gründen, sondern ebenso für die Arbeitsunfähigkeit wegen muskuloskeletaler Schmerzen. Die Schwierigkeiten einer Arbeitsplatzrehabilitation sind aber nicht speziell auf die Invalidenversicherung zurückzuführen. Sie gründen einerseits auf strukturellen Gegebenheiten der Schweiz, wie eine wenig in den Betrieben verankerte Arbeitsmedizin, und Zugang zu Rehabilitationsmedizin, und andererseits auf dem Fehlen einer klaren Kostenträgerregelung oder -verantwortung. R. Conne beschreibt in seinem Artikel treffend das heutige Malaise in der Schweiz in bezug auf Arbeitsplatzrehabilitation oder Wiedererlangung der Arbeitsfähigkeit. Er bezieht sich dabei vor allem auf die Arbeitsunfähigkeit aus psychischen Gründen. Wir möchten als am Rande angesprochene Arbeitsmedizinerin und angesprochener Rehabilitationsmediziner die Diskussion aufgreifen und erweitern. Die Erweiterung betrifft einerseits den Einbezug von nicht psychisch begründeter Arbeitsunfähigkeit und andererseits auch die allgemeine Versicherungssituation in der Schweiz, und dies verschiebt unserer Meinung nach den Problemfokus von der Invalidenversicherung auf die allgemeine Versicherungssituation.
