Segundo as guidelines atuais, o ecocardiograma transesofágico está indicado na cirurgia vascular major. O objetivo deste trabalho é determinar qual é o impacto do ecocardiograma transesofágico no diagnóstico de patologias da aorta susceptíveis de correção cirúrgica, na orientação da abordagem anestésica e cirúrgica e na monitorização destes pacientes no pós-operatório. Para o efeito, os seguintes itens foram estudados: impacto clínico geral do ecocardiograma transesofágico na cirurgia não cardíaca, ecocardiograma transesofágico no peri-operatório de aneurismas e disseções da aorta, da cirurgia endovascular da aorta, de lesões traumáticas da aorta, do tromboembolismo e de tumores da veia cava inferior.

Surveillance registers monitor the prevalence of cerebral palsy and the severity of resulting impairments across time and place. The motor disorders of cerebral palsy can affect children’s speech production and limit their intelligibility. We describe the development of a scale to classify children’s speech performance for use in cerebral palsy surveillance registers, and its reliability across raters and across time. Speech and language therapists, other healthcare professionals and parents classified the speech of 139 children with cerebral palsy (85 boys, 54 girls; mean age 6.03 years, SD 1.09) from observation and previous knowledge of the children. Another group of health professionals rated children’s speech from information in their medical notes. With the exception of parents, raters reclassified children’s speech at least four weeks after their initial classification. Raters were asked to rate how easy the scale was to use and how well the scale described the child’s speech production using Likert scales. Inter-rater reliability was moderate to substantial (k > .58 for all comparisons). Test–retest reliability was substantial to almost perfect for all groups (k > .68). Over 74% of raters found the scale easy or very easy to use; 66% of parents and over 70% of health care professionals judged the scale to describe children’s speech well or very well. We conclude that the Viking Speech Scale is a reliable tool to describe the speech performance of children with cerebral palsy, which can be applied through direct observation of children or through case note review.

SUMMARY Cerebral toxoplasmosis can be highly debilitating and occasionally fatal in persons with immune system deficiencies. In this study, we evaluated the Toxoplasma gondii-specific IgG subclass antibody response in 19 cerebrospinal fluid (CSF) samples from patients with cerebral toxoplasmosis who had a positive IgG anti-T. gondii ELISA standardized with a cyst antigen preparation. There were no significant differences between the rates of positivity and the antibody concentrations (arithmetic means of the ELISA absorbances, MEA) for IgG1 and IgG2, but the rates of positivity and MEA values for these two IgG subclasses were significantly higher than those for IgG3 and IgG4. The marked IgG2 response in CSF from patients with cerebral toxoplasmosis merits further investigation.

O autor começa por descrever os aspectos normais do processo de placentação que se encontram alterados na pré-eclâmpsia. Os efeitos desse defeito reflectem-se no aumento da resistência vascular das artérias uterinas e que pode ser detectado ecograficamente (é assumido como possível método de rastreio às 23 semanas de gestação). São analisadas relações entre pré-eclâmpsia e síndroma de HELLP, síndroma dos anticorpos anti-fosfolípidos (SAAF) e atraso de crescimento intra-uterino (ACIU). Comentam-se as tentativas de prevenção de pré-eclâmpsia com ácido acetil salicílico (AAS) e com cálcio. São analisados os efeitos de vários agentes anti-hipertensivos na hemodinâmica uterina e fetal. É feita uma introdução da Ginkgo biloba, dos motivos que levam a escolhê-la como um bom candidato à terapêutica preventiva do desenvolvimento da pré-eclâmpsia, analisando os seus mecanismos de acção, a farmacodinâmica, as doses habitualmente utilizadas e a sua segurança. Por fim sugerem-se alguns critérios para investigar clinicamente os efeitos do extracto de Ginkgo biloba (EGB 761).

Purpose. To report a case of successful thrombolysis performed in a patient with an incidental unruptured intracranial aneurysm and review the literature. Case Report. Patient admitted for ischemic stroke due to left posterior cerebral artery occlusion, with an incidental right middle cerebral artery aneurysm, who underwent treatment with tissue plasminogen activator (rtPA) resulting in clinical improvement without complications. Conclusion. The presence of unruptured intracranial aneurysms is considered as a contraindication to thrombolysis, due to a potentially higher hemorrhagic risk of aneurysm rupture. Patients, otherwise, eligible for thrombolysis are usually excluded from receiving this emergent treatment, despite its potential benefits. A reevaluation of the strict exclusion criteria for thrombolysis in acute stroke patients should be considered.

Phacomatosis pigmentovascularis is a rare syndrome characterized by the coexistence of a pigmented nevus and a cutaneous vascular malformation. We report a 5-year-old boy with all the typical findings of phacomatosis pigmentovascularis type Ia. Although its existence according to the traditional classification has been questioned, this case represents a very rare association of a capillary vascular malformation and a common keratinocytic nevus of the soft type.

The correct interpretation of chest film on cardiac patients is very important. The most important feature is the radiographic appearance of the pulmonary vascularity. Four different patterns of pulmonary vascularity are considered: normal, decreased, increased and uneven. The different diseases associated with each type are mentioned. From the pulmonary vascular pattern one can deduct hemodynamic data which are important for the diagnosis, grade of severity and follow-up.

OBJECTIVE: Although evidence has shown that ischemic heart disease (IHD) in vascular surgery patients has a negative impact on the prognosis after surgery, it is unclear whether directed treatment of IHD may influence cause-specific and overall mortality. The objective of this study was to determine the prognostic implication of coronary revascularization (CR) on overall and cause-specific mortality in vascular surgery patients. METHODS: Patients undergoing surgery for abdominal aortic aneurysm, carotid artery stenosis, or peripheral artery disease in a university hospital in The Netherlands between January 2003 and December 2011 were retrospectively included. Survival estimates were obtained by Kaplan-Meier and Cox regression analysis. RESULTS: A total of 1104 patients were included. Adjusted survival analyses showed that IHD significantly increased the risk of overall mortality (hazard ratio [HR], 1.50; 95% confidence interval, 1.21-1.87) and cardiovascular death (HR, 1.93; 95% confidence interval, 1.35-2.76). Compared with those without CR, patients previously undergoing CR had similar overall mortality (HR, 1.38 vs 1.62; P = .274) and cardiovascular mortality (HR, 1.83 vs 2.02; P = .656). Nonrevascularized IHD patients were more likely to die of IHD (6.9% vs 35.7%), whereas revascularized IHD patients more frequently died of cardiovascular causes unrelated to IHD (39.1% vs 64.3%; P = .018). CONCLUSIONS: This study confirms the significance of IHD for postoperative survival of vascular surgery patients. CR was associated with lower IHD-related death rates. However, it failed to provide an overall survival benefit because of an increased rate of cardiovascular mortality unrelated to IHD. Intensification of secondary prevention regimens may be required to prevent this shift toward non-IHD-related death and thereby improve life expectancy.

Os autores registam dois casos de Mucormicose órbito-rino-cerebral ocorridos no Maranhão em 1980 e início de 1981, em pacientes portadores de cetoacidose diabética, enfatizando a gravidade desta moléstia causada por fungos oportunistas em pacientes que sofrem de distúrbios metabólicos graves.

Foram atendidas no Hospital das Clínicas da Faculdade de Medicina de Ribeirão Preto/USP, durante os anos de 1980 a 1989, 21 crianças vítimas de acidente crotálico, 16 das quais apresentaram sinais clínicos de envenenamento grave à admissão e 5, de envenenamento moderado. Quatro pacientes (20%) desenvolveram necrose tubular aguda, dois dos quais necessitaram métodos dialíticos. Um paciente (5%) foi a óbito no 13.° dia do acidente, após intercorrências graves, como hemorragia digestiva e insuficiência respiratória aguda. São apresentadas as características clínicas, laboratoriais e epidemiológicas do acidente crotálico nesses pacientes, enfatizando os conhecimentos atuais sobre a fisiopatologia do envenenamento por Crotalus durissus terrificus. Ao mesmo tempo, pretendeu-se contribuir para a correta abordagem terapêutica, no que diz respeito ao emprego adequado do soro antiveneno e à prevenção da necrose tubular aguda, a mais séria e grave complicação do acidente crotálico.

Os autores relatam a resposta terapêutica da claritromicina associada com pirímetamina em dois casos de toxoplasmose cerebral ocorridos em pacientes com Síndrome de Imunodeficiência Adquirida (SIDA). Nos dois casos o diagnóstico foi presuntivo, baseando-se nas manifestações clínicas e na presença de lesões expansivas hipercaptantes detectadas na tomografia computadorizada (TC) de crânio. Em ambos os casos, observou-se exantema com o uso da sulfadiazina e da clindamicina, razão pela qual tais medicamentos foram substituídos pela claritromicina (1,5 a 2g/dia) associada à pirímetamina (25mg/dia). A resposta terapêutica foi favorável nos dois casos com melhora das manifestações neurológicas e dos achados neurorradiológicos. Os autores sugerem que a associação claritromicina com pirímetamina pode ser útil como opção terapêutica na toxoplasmose cerebral em pacientes com SIDA que apresentam intolerância ou outra contra-indicação ao emprego das sulfonamidas.
