Odontogenic myxomas are considered to be a benign odontogenic tumor with locally aggressive behavior. Because these neoplasms are rare in the oral cavity, the possible surgical management can be quite variable. Literature recommendation can vary from simple curettage and peripheral ostectomy to segmental resection. The authors report a case of a 20-year-old patient with an odontogenic myxoma tumor located in the left mandibular angle, ascending ramus, and mandibular symphysis. It was treated by radical resection followed by titanium reconstruction with condylar prosthesis, which allowed rapid return of function with improvement in quality of life and restoration of cosmetic and functional deficits. The lesion did not recur after surgical procedure.

Purpose: This study sought to evaluate the effect of repeated implant drilling on the immediate bone-cell viability, and to evaluate drill wear by scanning electron microscopy.Materials and Methods: The tibiae of 10 rabbits were used, divided into 5 groups (G): G1 corresponded to new drills, and G2, G3, G4, and G5 corresponded to drills used 10, 20, 30, and 40 times, respectively. The animals received 10 sequential osteotomies in each tibia. The animals were euthanized immediately after the osteotomies by perfusion with 4% formaldehyde. Samples then underwent immunohistochemistry processing for ordinal qualitative analysis of osteoprotegerin (OPG), the RANK ligant (RANKL; a tumor-related necrosis factor receptor family), and osteocalcin protein immunolabels, as detected by the immunoperoxidase method and revealed with 3,3-diaminobenzidine. Drill wear and plastic deformation were analyzed by scanning electron microscopy (SEM).Results: The proteins were expressed in osteocytes of the superior bone cortical during the 40 drillings. However, in G4 and G5, a discrete increase in the expression of RANKL was observed, when compared with OPG; this increase was statistically significant in G5 (P = .016). The SEM analysis revealed major plastic deformation and drill wear in G4 and G5.Conclusion: Based on the present methodology, it may be concluded that cell viability is preserved if a less traumatic surgical protocol is used. However, the repeated use of drills alters the protein balance as of the thirtieth perforation. (C) 2008 American Association of Oral and Maxillofacial Surgeons.

Fractures of the mandibular angle deserve particular attention because they represent the highest percentage of mandibular fractures and have the highest postsurgical complication rate, making them the most challenging and unpredictable mandibular fractures to treat. Despite the evolution in the treatment of maxillofacial trauma and fixation methods, no single treatment modality has been revealed to be ideal for mandibular angle fractures. Several methods of internal fixation have been studied with great variation in complications rates, especially postoperative infections. Recently, new studies have shown reduction of postsurgical complications rates using three-dimensional plates to treat mandibular angle fractures. Nevertheless, only few surgeons have used this type of plate for the treatment of mandibular angle fractures. The aim of this clinical report was to describe a case of a patient with a mandibular angle fracture treated by an intraoral approach and a three-dimensional rectangular grid miniplate with 4 holes, which was stabilized with monocortical screws. The authors show a follow-up of 8 months, without infection and with occlusal stability.

The aim of the present study was to evaluate the postoperative complications of bicoronal flaps used to treat facial fractures. One hundred and thirty two patients that received bicoronal flaps for the treatment of upper and middle third facial fractures were called for clinical and radiographic examination. Minimum follow-up was 1 year and all patients had charts with adequate information about their perioperative care pertinent to the study. Results showed as complications hypoesthesia (17%), partial unilateral frontal motor deficit (11%), infection (3%), hypertrophic scars (3%), varying degrees of alopecia (18%), seroma or hematoma in the immediate postoperative period (5%). The flap provided wide surgical access to the upper and middle facial thirds with very few serious complications, most frequently allowing good aesthetic results. Keratocystic odontogenic tumor is characterized by high recurrence rates. Conservative or aggressive management has been suggested as a method of treatment. Decompression is a conservative treatment that has been used in the treatment of large odontogenic cysts. The authors report a case of a 14-year-old patient with a keratocystic odontogenic tumor located in the right maxilla, which was treated by decompression followed by enucleation with curettage. The lesion did not recur on follow-up for 3 years after the enucleation surgery.

Injections of the excitatory amino acid L-glutamate (L-glu) into the rostral ventrolateral medulla (RVLM) directly activate the sympathetic nervous system and increase mean arterial pressure (MAP). A previous study showed that lesions of the anteroventral third ventricle region in the forebrain reduced the pressor response to L-glu into the RVLM. In the present study we investigated the effects produced by injections of atropine (cholinergic antagonist) into the lateral ventricle (LV) on the pressor responses produced by L-ghl into the RVLM. Male Holtzman rats (280-320 g, n=5 to 12/group) with stainless steel cannulas implanted into the RVLM, LV or 4th ventricle (4th V) were used. MAP and heart rate (HR) were recorded in unanesthetized rats. After saline into the LV, injections of L-glu (5 nmol/100 nl) into the RVLM increased MAP (51 +/- 4 mm Hg) without changes in HR. Atropine (4 nmol/1 PI) injected into the LV reduced the pressor responses to L-glu into the RVLM (36 +/- 5 mm Hg), However, atropine at the same dose into the 4th V or directly into the RVLM did not modify the pressor responses to L-glu into the RVLM (45 +/- 2 and 49 +/- 4 mm Hg, respectively, vs. control: 50 +/- 4mmHg). Central cholinergic blockade did not affect baro and chemoreflex nor the basal MAP and HR. The results suggest that cholinergic mechanisms probably from forebrain facilitate or modulate the pressor responses to L-glu into the RVLM. The mechanism is activated by acetylcholine in the forebrain, however, the neurotransmitter released in the RVLM to facilitate the effects of glutamate is not acetylcholine. (C) 2007 Elsevier B.V. Purpose: The aim of this study was to evaluate the clinical outcome of patients with mandibular angle fractures treated by intraoral access and a rectangular grid miniplate with 4 holes and stabilized with monocortical screws.Patients and Methods: This study included 45 patients with mandibular angle fractures from the Department of Oral and Maxillofacial Surgery São Paulo State University, Araraquara, Brazil, and from the Clinic of Oral and Maxillofacial Surgery at the University of Frankfurt, Germany. The 45 fractures of the mandibular angle were treated with a rectangular grid miniplate of a 2.0-mm system by an intraoral approach with monocortical screws. Clinical evaluations were postoperatively performed at 15 and 30 days and 3 and 6 months, and the complications encountered were recorded and treated. Results: The infection rate was 4.44% (2 patients), and in 1 patient it was necessary to replace hardware. This patient also had a fracture of the left mandibular body; 3 patients (6.66%) had minor occlusal changes that have been resolved with small occlusal adjustments. Before surgery, 15 patients (33.33%) presented with hypoesthesia of the inferior alveolar nerve; 4 (8.88%) had this change until the last clinical control, at 6 months. Conclusions: The rectangular grid miniplate used in this study was stable for the treatment of simple mandibular angle fractures through intraoral access, with low complication rates, easy handling, and easy adjustment, with a low cost. Concomitant mandibular fracture may increase the rate of complications. This plate should be indicated in fractures with sufficient interfragmentaty contact.

This paper summarizes floristic and phytossociology data of 11, out of 14 plots of 1 ha, allocated along an altitudinal gradient in the Serra do Mar, Silo Paulo, Brazil. The study was conducted at Serra do Mar State Park and the plots start at the sea level (10 m - plot of Restinga Forest that occurs at Praia da Fazenda, Picinguaba, municipality of Ubatuba) up to 1100 m above sea level (the Montane Ombrophilous Dense occurs alongside the Itamambuca Trail, municipality of Silo Luis do Paraitinga). The Restinga Forest occurs in Pleistocenic Coastal Plain where the soil is classified as a sandy Quartzipsamment (Quartzenic Neosol), while along the slopes of the Serra do Mar, the Ombrophylus Dense Forest grows on the top of a pre-Cambrian crystalline basement with granitic rocks, where the soil is a sandy-loam Dystrophic Inceptisol (Cambisol/Latosol). In all 14 plots soils are acidic (pH 3 - 4), chemically poor, with high dilution of nutrients and high saturation of aluminum. In the Restinga and at the foot of the slope the climate is Tropical/Subtropical Humid (Af/Cfa), with no dry season, an average annual rainfall over 2,200 mm and an average annual temperature of 22 degrees C. Towards the top of the Serra do Mar there is a gradual cooling along the slope, but there is no reduction in rainfall, so at 1,100 m above sea level the climate is classified as Humid Subtropical (Cfa/Cfb), with no dry season and an average annual temperature of 17 degrees C. It is important to remark that, almost daily, from 400 m above sea level up to the top of slopes the mountains are covered by a dense fog. In the 14 plots 21,733 individuals with DBH >= 4.8 cm, including trees, palms and ferns, were marked, measured and sampled. The average number of individuals sampled in each plot was 1264 ind.ha(-1)(+/- 218 SE 95%). Within the parameters considered trees prevailed (71% in the Montane ODF to 90% in the Restinga Forest), followed by palms (10% in the RF and 25% in the Montane Ombrophilous Dense Forest/ODF) and ferns (0% % in the RF and 4% in the Montane ODF). Regarding these proportions the Exploited Lowlands ODF differs from the others with only 1.8% of palm trees and striking 10% of ferns. The forest canopy is irregular with heights ranging from 7 to 9 m, rarely emergent trees reach 18 m, and due to this irregularity of the canopy the amount of light that gets through sets conditions for the development of hundreds of epiphytic species. Aside from Montana ODF, where the number of dead trees was more than 5% of individuals sampled, in the other phytophysiognomies this value was below 2.5%. In the 11 plots where the floristic study was conducted we found 562 species in 195 genera and 68 families. Only seven species - Euterpe edulis Mart. (Arecaceae), Calyptranthes lucida Mart. ex DC. and Marlierea tomentosa Cambess (both Myrtaceae), Guapira opposita (Veil.) Reitz (Nyctaginaceae), Cupania oblongifolia Mart. (Sapindaceae), Cecropia glaziovii Snethl. and Coussapoa microcarpa (Schott) Rizzini (both Urticaceae) - occurred from Restinga to Montane ODF, while 12 other species did not occur only in the Restinga Forest. Families with the greatest number of species are Myrtaceae (133 spp), Fabaceae (47 spp), Rubiaceae (49) and Lauraceae (49) throughout the gradient and Monimiaceae (21) specifically in portions Montane ODF. Only in the F plot, where logging has occurred between 1950 and 1985, the abundance of palm trees has been replaced by Cyatheaceae. The study shows a peak of diversity and richness, Shannon-Weiner index (H') ranging from 3.96 to 4.48 nats.ind(-1), in the intermediate altitudes (300 to 400 m) along the slope. Several explanations for this result are raised here, including the fact that these elevations are within the limits expansions and retractions of the different phytophysiognomies of the Atlantic ODF due to climate fluctuations during the Pleistocene. The results presented in this paper demonstrate the extraordinary richness of tree species of the Atlantic Rainforest from the northeastern coast of the State of São Paulo, reinforcing the importance of its conservation throughout the altitudinal gradient. The richness of this forest justifies a long term commitment to study its dynamics and functioning through permanent plots, and monitor the impacts of climate change in this vegetation.

Clinical benefits of probiotics have been clearly reported in different gastrointestinal disorders, many of them caused by enterobacteria. The oral cavity is a port of entry and can be an important reservoir of these microorganisms. This work evaluated whether consumption of probiotics was able to influence the presence of enterobacteria in the oral cavity and the specific secretory response against these microorganisms. Saliva samples of healthy individuals were collected and plated in Mac-Conkey agar. Carriers of Gram-negative, rod-shaped microorganisms in the oral cavity were selected and instructed to use the probiotic Yakult LB for 20 days. Saliva was then collected and enterobacteria species were identified using the API 20 E system and by ELISA using anti-enterobacteria IgA. The results showed reduction in the prevalence of enterobacteria, but no significant changes in enterobacterial counts (log CFU/mL; p = 0.3457). The species most frequently isolated were Enterobacter cloacae and Klebsiella oxytoca, both before and after probiotic consumption. No significant changes were observed in anti-enterobacteria IgA levels. In conclusion, probiotic consumption had some influence on enterobacterial presence in the oral cavity, but did not affect enterobacterial counts or the specific immune secretory response against them.

The use of denture is known to increase the carriage of Candida in healthy patients, and the proliferation of Candida albicans strains can be associated with denture-induced stomatitis. The aim of this study was to evaluate the use of vinegar as an antimicrobial agent for control of Candida spp. in complete upper denture wearers. Fifty-five patients were submitted to a detailed clinical interview and oral clinical examination, and were instructed to keep their dentures immersed in a 10% vinegar solution ( pH less than 3) overnight for 45 days. Before and after the experimental period, saliva samples were collected for detection of Candida, counting of cfu/mL and identification of species by phenotypical tests ( germ tube formation, chlamidoconidia production, and carbohydrate fermentation and assimilation). The results were analyzed using Spearman's correlation and Student's t-test (p=0.05). Candida yeasts were present in 87.3% of saliva samples before the treatment. A significant reduction was verified in CFU/mL counts of Candida after treatment. A positive correlation between Candida and denture stomatitis was verified, since the decrease of cfu/mL counts was correlated with a reduction in cases of denture stomatitis. Although it was not able to eliminate C. albicans, the immersion of the complete denture in 10% vinegar solution, during the night, reduced the amounts (cfu/mL) of Candida spp. in the saliva and the presence of denture stomatitis in the studied patients.
