Introdução: O tratamento endodôntico (TE) pode definir-se como um procedimento comum, usado pelos profissionais da Medicina Dentária, com o objectivo de tratar infeções da polpa radicular do dente. O sucesso deste tipo de tratamento está diretamente relacionado com o controlo da infeção, e para isso, realizam-se os processos de limpeza e de desinfeção do canal radicular (CR) com o auxílio de instrumentos e soluções irrigadoras. Objetivo: O presente trabalho tem como objetivo as soluções irrigadoras em Endodontia, tendo em conta a classificação das soluções, as mais utilizadas na Endodontia e a realização da desinfeção mais adequada no tratamento endodôntico. Materiais e Métodos: Com o intuito de efetuar com sucesso esta revisão bibliográfica que nos propomos, sobre soluções irrigadoras em Endodontia, foi realizada uma pesquisa bibliográfica por dois métodos, o manual e o on-line. A pesquisa manual foi efetuada na Biblioteca da Faculdade de Ciências da Saúde da Universidade Fernando Pessoa. A pesquisa on-line foi realizada através dos motores de busca como Medline/PubMed, B-on, Elseivier, repositório Institucional da Universidade Fernando Pessoa. Conclusões: Não há nenhuma solução irrigante que apresente todas as funções desejáveis. Por conseguinte, a irrigação ideal é baseada no uso combinado de duas ou várias soluções, numa sequência específica, de forma a obter uma irrigação segura e eficaz. Porém, para os Endodontistas, o hipoclorito de sódio (NaOCl), é o irrigante de eleição devido às suas excelentes propriedades.

Introdução e objectivos - O número de casos com reincidência de infecções póstratamento endodôntico, resultantes de uma incompleta desinfecção dos canais radiculares ainda é significativo e requer aperfeiçoamento. A complexidade do sistema de canais radiculares constitui o principal obstáculo à instrumentação e desinfecção dos mesmos em toda a sua extensão. A irrigação é um passo chave durante a instrumentação que possibilita a limpeza e desinfecção dos canais radiculares e através da qual, as bactérias, toxinas e os seus bio-produtos são eliminados. Este trabalho tem como objectivo descrever as várias técnicas de irrigação actualmente em uso na prática clínica. Materiais e métodos – Para elaboração deste trabalho de revisão foi efectuada uma pesquisa bibliográfica nos motores de busca: PubMed e Science Direct, utilizando como palavras-chave “endodontic irrigation”, “endodontic irrigants” e “sodium hypochloride”. Foram incluídos artigos desde 1915 a 2016 e a pesquisa foi realizada nos meses de Abril a Junho de 2016. Desenvolvimento - Um irrigante endodôntico deve responder a um conjunto de requisitos, entre os quais a eficácia na desinfecção total e definitiva dos canais radiculares, a eliminação da smear layer, deve ser não-antigénico, não tóxico e não carcinogénico e preservar a função do dente. O irrigante mais utilizado é o hipoclorito de sódio, mas alternativas têm sido amplamente utilizadas, tais como clorexidina, ácido etilenodiaminotetra-acético, e irrigantes combinados, tais como uma mistura de tetraciclina, um ácido e um detergente (MTAD), o Hypoclean® e o QMix®. Conlusão - Embora o NaOCl seja a solução que mais se aproxime do irrigante perfeito, a sua toxicidade representa um risco para o paciente e as suas limitações enquanto desinfectante são factores a considerar. A conjugação do NaOCl com outros irrigantes, bem como a formulação dos irrigantes compostos, tem vindo a melhorar a eficiência dos tratamentos endodônticos. No entanto, justifica-se o permanente investimento científico nesta área para que se reduza para níveis esporádicos os casos de reinfecção.

Aim: To evaluate the influence of ultrasonic activation (US) with different irrigant regimens in smear layer removal. Methods: One hundred bovine incisors were instrumented and divided into ten groups (n=10) according to final irrigation protocols: distilled water (DW); DW+US; 17% EDTA; QMix; 10% citric acid; 37% phosphoric acid; 17% EDTA+US; QMix+US; 10% citric acid+US; 37% phosphoric acid+US. The samples were then submitted to scanning electron microscopy where a score system was used to evaluate the images and effectiveness of proposed treatments. The data were statistically analyzed by Kruskal-Wallis and Mann-Whitney U tests for intergroup comparisons as well as the Wilcoxon and Friedman tests for intragroup comparisons at 5% significance level. Results: In the cervical third, groups 17% EDTA, QMix, 10% citric acid, 17% EDTA+US, QMix+US and 10% citric acid+US were more effective in smear layer removal (p<0.05); in the middle third, groups 17% EDTA+US and QMix+US were more effective in smear layer removal (p<0.05); in the apical third, groups 17% EDTA,17% EDTA+US and QMix+US were more effective in smear layer removal (p<0.05). Conclusions: US can aid 17% EDTA and QMix in smear layer removal at the middle third and QMix at the apical third, contributing to the cleaning of root canal system.

La determinación del tamaño de la preparación apical es un tema de constante debate, ya que a pesar de la evidencia científica disponible aún no se logra un consenso para saber cuál es el diámetro adecuado para obtener un tratamiento exitoso; en este sentido, el clínico debe ser crítico ante cada tratamiento, evaluando el diagnóstico y la anatomía del sistema de conductos radiculares. En la presente revisión, se realizó una búsqueda de la literatura disponible, con el fin de ofrecer al odontólogo un enfoque ante sus procedimientos clínicos, considerando diferentes parámetros, así como las distintas implicaciones clínicas que derivan de la determinación del tamaño de la preparación apical, lo cual constituye un aspecto importante dentro del proceso de la preparación biomecánica.

O objectivo deste trabalho consistiu em definir, com base na evidência científica mais recente sobre Acidentes com hipoclorito de sódio, diferentes protocolos para a prevenção e tratamento dos acidentes causados no âmbito da endodontia. O Hipoclorito de Sódio é o irrigante frequentemente mais utilizado durante o Tratamento Endodôntico, devido às suas excelentes propriedades, capacidade de dissolver tecidos e capacidade bactericida. Há diversos factores que podem influenciar o modo de atuação do NaOCl, tais como: a concentração, a temperatura, o pH, o tempo/volume de irrigação, a agitação do irrigante, os métodos de introdução do irrigante, a conicidade apical e a profundidade de colocação da agulha. A pesquisa bibliográfica deste trabalho foi realizada nas bibliotecas da Universidade Fernando Pessoa e Faculdade de Medicina Dentária da Universidade do Porto e nos motores de internet Pubmed, Science Direct, Scielo e B-On, entre Junho de 2016 a Outubro de 2016. As palavras utilizadas para a pesquisa foram as seguintes: “desinfection endodontics”, “endodontics”, “endodontic treatment”, “sodium hypochlorite”, “irrigants endodontics”, “sodium hypochlorite accidents”, “irrigation methods”, “root canal irrigant”, “management NaOCL accidents”.

The primary aim was to evaluate the effect of 1-ethyl-3-(3-dimethylamino-propyl) carbodiimide (EDC) on endogenous enzymatic activity within radicular dentin and push-out bond strength of adhesively luted fiber posts, at baseline and after artificial aging. Additionally, the effect of different cementation strategies on endogenous enzymatic activity and fiber post retention was evaluated. The experiment was carried out on extracted human teeth, following endodontic treatment and fiber post cementation. Three cementation strategies were performed: resin cement in combination with etch-and-rinse (EAR) adhesive system, with self-etch (SE) system and self-adhesive (SE) cement. Each of the mentioned strategies had a control and experimental (EDC) group in which root canal was irrigated with 0.3M EDC for 1 minute. The push-out bond strength test was performed 24h after cementation and after 40.000 thermocycles. In order to investigate the effect of EDC and different cementation strategies, in situ zymography analyses of the resin-dentin interfaces were conducted. Statistical analyses were conducted with the software Stata 12.0 (Stata Corp, College Station, Texas, USA) and the significance was set for p<0.05. The results of statistical analysis (ANOVA) showed that the variables “EDC”, “root region” and “artificial aging” significantly influenced fiber posts’ retention to root canal (p<0.05). The highest values were observed in coronal third. The mean values observed after artificial aging were lower when compared to baseline, however EDC was effective in preserving bond strength. The level of enzymatic activity varied between the groups and EDC had a beneficial effect on silencing the enzymatic activity. Within the limitations of the study, it was concluded that the choice of cementation strategy did not influence posts’ retention, while EDC contributed to the preservation of bond strength after artificial aging and reduced enzymatic activity within radicular dentin. In vivo trials are necessary to confirm the results of this in vitro study.

In this doctoral dissertation, a comprehensive methodological approach for the assessment of river embankments safety conditions, based on the integrated use of laboratory testing, physical modelling and finite element (FE) numerical simulations, is proposed, with the aim of contributing to a better understanding of the effect of time-dependent hydraulic boundary conditions on the hydro-mechanical response of river embankments. The case study and materials selected for the present research project are representative for the riverbank systems of Alpine and Apennine tributaries of the main river Po (Northern Italy), which have recently experienced various sudden overall collapses. The outcomes of a centrifuge test carried out under the enhanced gravity field of 50-g, on a riverbank model, made of a compacted silty sand mixture, overlying a homogeneous clayey silt foundation layer and subjected to a simulated flood event, have been considered for the definition of a robust and realistic experimental benchmark. In order to reproduce the observed experimental behaviour, a first set of numerical simulations has been carried out by assuming, for both the embankments and the foundation unit, rigid soil porous media, under partially saturated conditions. Mechanical and hydraulic soil properties adopted in the numerical analyses have been carefully estimated based on standard saturated triaxial, oedometer and constant head permeability tests. Afterwards, advanced suction-controlled laboratory tests, have been carried out to investigate the effect of suction and confining stresses on the shear strength and compressibility characteristics of the filling material and a second set of numerical simulations has been run, taking into account the soil parameters updated based on the most recent tests. The final aim of the study is the quantitative estimation of the predictive capabilities of the calibrated numerical tools, by systematically comparing the results of the FE simulations to the experimental benchmark.

Endodontic-related periapical bone defects are a common occurrence in the global populations. Considering the number of root canal treatments performed annually, new strategies and new biomaterials for the management of these bone defects will be important and highlight the need for continued research and development in endodontic field. The present PhD thesis have several objectives and is divided into two main sections: one focused on in vitro and laboratory research and the other on clinical in vivo investigations. The first part, focused on laboratory and in vitro research, investigated 2 main topics: • the microbial communities of apical periodontitis to evaluate the predominant bacterial using 16sr DNA-targeted Nanopore sequencing; • the physical-chemical properties of innovative premixed calcium-silicate based bioceramic sealers for endodontic therapy; The second part, focused on in vivo clinical studies, investigated 2 main topics: • the clinical application of premixed calcium-silicate-based sealers. Ethical committee approval was obtained in 2 separate in vivo studies. The first one is a prospective cohort study with a two-year follow-up where the test group was compared with a control group (considered the gold standard). The second is a pilot prospective cohort study with a 12-month follow-up which set the foundation for a subsequent randomized investigation. Thanks to these investigations, we validated a new technique that innovatively associates a warm obturation technique with calcium-silicate-based sealers. Historically, these sealers were only used with cold techniques. This investigation highlights the possibility for wider utilization and improvements in endodontic techniques. • The outcome of 2 different types of implants characterized by different surface treatments and placed with different techniques. The marginal bone level and periodontal parameters were evaluated with a follow-up of 4 and 10 years. This Ph.D thesis is based on a compilation of published papers I have done during my three-year PhD program.

The increase of railways near the urban areas is a significant cause of discomfort for inhabitants due to train-induced vibration and noise. Vibration characteristics can vary widely according to the train type: for high-speed trains, if train speed becomes comparable to the ground wave speed, the vibration level becomes significant; for freight trains, due to their heavier weight and lower speed, the vibration amplitudes are greater and propagate at a more considerable distance from the track; for urban tramways, although the vibration amplitude is relatively low, they can have a negative structural effect on the closest buildings [51]. Therefore, to dampen the vibration level, it is possible to carry out some interventions both on the track and the transmission path. This thesis aims to propose and numerically investigate a novel method to dampen the train-induced vibrations along the transmission path. The method is called "resonant filled-trench (RFT)" and consists of a combination of expanded polystyrene (EPS) geofoam to stabilize the trench wall against the collapse and drowned cylindrical embedded inclusions inside the geofoam, which act as a resonator, reflector, and attenuator. By means of finite element simulations, we show that up to 50% higher attenuation than the open trench is achievable after overcoming the resonance frequency of the inclusion, i.e., 35Hz, which covers the frequency contents of the train-induced vibration. Moreover, depending on the filling material used for the inclusions, trench depth can be reduced up to 17% compared to the open trench showing the same screening performance as the open trench. Also, an RFT with DS inclusion installed in dense sand soil shows a high hindrance performance (i.e., IL≥6dB) when the trench depth is larger than 0.5λ_R while it is 0.6λ_R for the open trench.

Objective: To evaluate the influence of different endodontic materials on root fracture susceptibility. Methods: Seventy-two mandibular incisors were sectioned 1 mm below the cementoenamel junction to obtain roots of 12 mm length. Roots were submitted to chemomechanical preparation with the rotary instruments of Profile system. The obturation of root canals were performed with the following filling materials (n = 12): GI, unfilled teeth (control); GII, Endofill + gutta-percha; GIII, Sealer 26 + gutta-percha; GIV, AH Plus + gutta-percha; GV, Epiphany + gutta-percha; GVI, Epiphany + Resilon. After the sealers setting time, each root was embedded in acrylic resin. The specimens were then submitted to fracture resistance test using an Instron testing machine at 1 mm/min. Results: The ANOVA test showed no significant statistical difference (p > .05) among GI (162.16 +/- 41.4N), GII (168.46 +/- 37.5N), GIII (164.83 +/- 35.7N), GIV (168.29 +/- 38.7N), GV (172.36 +/- 20.6N) and GVI (193.11 +/- 42.8N). Conclusion: The core materials (gutta-percha or Resilon) combined with the tested endodontic sealers are not able to increase the root fracture resistance in canals submitted to chemomechanical preparation. Objective. In this study, presence of dentin infection in root canals, obturated with 4 techniques submitted to the bacterial leakage test, was evaluated using histologic methods. Study design. The canals of palatal roots of 160 molars were instrumented and divided into different groups, according to the obturation technique used (lateral condensation, MicroSeal system, Touch `n Heat + Ultrafil, and Tagger`s hybrid technique) and extent of the remaining obturation material (5 mm and 10 mm). Ten additional roots were used as control samples. The roots were sterilized in ethylene oxide and mounted on a device for evaluation of bacterial leakage using the bacteria Enterococcus faecalis for 120 days. After the leakage test, roots were microscopically analyzed for the presence of dentin infection in the root canals and dentinal tubules. Results. A total of 154 specimens were analyzed using both methodologies in the experimental groups; 50 root canals (32.4%) showed bacterial leakage at the end of the experimental period, and 118 (76.6%) showed the presence of bacteria in the root canals using the histologic criteria. The lateral condensation technique allowed lower penetration of bacteria in the root canals and dentinal tubules, followed by Touch `n Heat + Ultrafil, MicroSeal, and Tagger`s hybrid technique, which allowed significantly greater penetration of bacteria. Root canals with 10 mm of remaining obturation material presented similar bacterial penetration as root canals with 5 mm. Conclusions. Even when an adequate seal of the apical foramen was shown by the absence of turbidity in the bacterial leakage test, E. faecalis dentin infection was present in a high percentage of the root canals after 120 days of root filling exposure to the bacteria. Tagger`s hybrid technique presented greater quantity of bacteria in histologic sections than root canals obturated with the other techniques. Objective: A comparative study is made of the histological effects of silver amalgam versus compomer (Dyract®) 90 days after placement as retrograde filling materials in experimental animals. Method: Six Beagle dogs were used, with total pulpectomy and orthograde material filling followed by periapical surgery of the 6 upper and 6 lower incisors (for a total of 72 teeth). Thirty-six teeth corresponded to the right side and were filled with the control material (silver amalgam), while the 36 teeth on the left side were filled with the compomer study material (Dyract®). After three months the animals were sacrificed and the histological study was carried out, with evaluation of bone formation, inflammation, and the tissue in contact with the filler material. The results obtained were subjected to a descriptive and comparative statistical analysis (chi-square test). Results: The samples retrogradely filled with compomer showed significantly greater percentage inflammation (76.19% versus 26.66% in the control group). On the other hand, a large proportion of samples with root cement growth were found in the compomer group. Filler material expulsion was also significantly more common when compomer was used. Conclusions: the comparative study of the histological findings showed greater inflammation but also greater root cement growth in the compomer group versus the controls

Chromosomal aberration (CA) assays have been widely used, not only to assess the genotoxic effects of chemical agents, but also to evaluate their action mechanisms on the genetic material of exposed organisms. This is of particular interest, since such analyses provide a better knowledge related to the action of these agents on DNA. Among test organisms, Allium cepa is an outstanding species due to its sensitivity and suitable chromosomal features, which are essential for studies on chromosomal damage or disturbances in cell cycle. The goal of the present study was to analyze the action mechanisms of chemical agents present in petroleum polluted waters. Therefore, CA assay was carried out in A. cepa meristematic cells exposed to the Guaeca river waters, located in the city of Sao Sebastiao, SP, Brazil, which had its waters impacted by an oil pipeline leak. Analyses of the aberration types showed clastogenic and aneugenic effects for the roots exposed to the polluted waters from Guaeca river, besides the induction of cell death. Probably all the observed effects were induced by the petroleum hydrocarbons derived from the oil leakage. (C) 2008 Elsevier B.V. All rights reserved.
