Proteins are composed of a combination of discrete, well-defined, sequence domains, associated with specific functions that have arisen at different times during evolutionary history. The emergence of novel domains is related to protein functional diversification and adaptation. But currently little is known about how novel domains arise and how they subsequently evolve. To gain insights into the impact of recently emerged domains in protein evolution we have identified all human young protein domains that have emerged in approximately the past 550 million years. We have classified them into vertebrate-specific and mammalian-specific groups, and compared them to older domains. We have found 426 different annotated young domains, totalling 995 domain occurrences, which represent about 12.3% of all human domains. We have observed that 61.3% of them arose in newly formed genes, while the remaining 38.7% are found combined with older domains, and have very likely emerged in the context of a previously existing protein. Young domains are preferentially located at the N-terminus of the protein, indicating that, at least in vertebrates, novel functional sequences often emerge there. Furthermore, young domains show significantly higher non-synonymous to synonymous substitution rates than older domains using human and mouse orthologous sequence comparisons. This is also true when we compare young and old domains located in the same protein, suggesting that recently arisen domains tend to evolve in a less constrained manner than older domains. We conclude that proteins tend to gain domains over time, becoming progressively longer. We show that many proteins are made of domains of different age, and that the fastest evolving parts correspond to the domains that have been acquired more recently.

Pupae of Trissolcus basalis (Wollaston) and Telenomus podisi Ashmead (Hymenoptera: Scelionidae) were stored at 12ºC and 15ºC for 120-210 days, after different periods of parasitism at 18ºC in order to evaluate adult emergence, longevity and ovipositional capacity. There was no emergence of adults at 12ºC. The rate of emergence of parasitoids transferred to 15ºC at the beginning of the pupal stage was 1.5% and 26.3%, for T. basalis and T. podisi respectively, whereas those parasitoids transferred one day before the expected date of emergence at 18ºC showed 86.4% of emergence for T. basalis and 59.9% for T. podisi. Mean adult longevity was also significantly lower when pupae were transferred to 15ºC at the beginning of the pupal stage. Females emerged after storage and maintained for 120 to 210 days at 15ºC parasitized host eggs after transference to 25ºC; however, fecundity of T. podisi was reduced in about 80% after cold storage.

El dolor post-operatori és un dels problemes més importants que segons la OMS afecta als pacients intervinguts quirúrgicament i el que més els preocupa. Any rere any hi ha un increment del nombre de pacients intervinguts per Cirurgia Major ambulatòria al nostre país de manera que ha de ser el mateix usuari i la família els que facilitin les cures als pacients al propi domicili. Aquest estudi planteja si introduir intervencions educatives al servei disminueix el dolor post-operatori dels usuaris Objectiu general: Dissenyar un programa d’informació que realitzarà infermeria dirigit als cuidadors i pacients del servei de CMA i que han de ser intervinguts de hèrnia engonal. Avaluar l’eficiència del mateix programa educatiu. Metodologia: l’àmbit d’estudi d’aquest treball serà les unitats de Cirurgia Major ambulatòria de qualsevol centre de Catalunya. Es realitzarà un estudi comparatiu quantitatiu entre dos grups independents integrants per 30 participants en cadascun d’ells (total de 60 participants). Grupo A: protocol habitual de la unitat Grup B: intervenció educativa dissenyada Els participants seran pacients majors d’edat, que compleixen els requisits per ser intervinguts per Cirurgia Major Ambulatòria de Hèrnia inguinal. El participant ha de fer-ho amb un familiar responsable de les seves cures. Es realitzaran 3 enquestes al pacient i 3 al familiar responsable de les cures, la primera a les 24 hores després de la intervenció, la segona a la setmana i l’última a les 2 setmanes. Limitacions de l’estudi: en cas de que el pacient que ha de formar part de l’estudi requereixi ingrés hospitalari per alguna complicació durant la cirurgia el pacient deixa de participar en l’estudi automàticament. En cas que el pacient o familiar no contestin al telèfon el dia i l’hora pactada prèviament també deixarà de formar part de la investigació. Per aquest motiu es tindran 10 pacients de reserva per si calgués reemplaçar el subjecte d’estudi.

Brain perfusion can be assessed by CT and MR. For CT, two major techniquesare used. First, Xenon CT is an equilibrium technique based on a freely diffusibletracer. First pass of iodinated contrast injected intravenously is a second method,more widely available. Both methods are proven to be robust and quantitative,thanks to the linear relationship between contrast concentration and x-ray attenuation.For the CT methods, concern regarding x-ray doses delivered to the patientsneed to be addressed. MR is also able to assess brain perfusion using the firstpass of gadolinium based contrast agent injected intravenously. This method hasto be considered as a semi-quantitative because of the non linear relationshipbetween contrast concentration and MR signal changes. Arterial spin labelingis another MR method assessing brain perfusion without injection of contrast. Insuch case, the blood flow in the carotids is magnetically labelled by an externalradiofrequency pulse and observed during its first pass through the brain. Eachof this various CT and MR techniques have advantages and limits that will be illustratedand summarised.Learning Objectives:1. Objective: To investigate the association between fear of falling appearing within one month after discharge from post-acute rehabilitation and functional status in elderly patients. Methods: Participants (N=180, mean age 81.37.1 years, 75.6% women) were patients consecutively admitted to rehabilitation over a 6-month period. Demographics, functional, cognitive and affective status were assessed upon admission; functional status was assessed at discharge; history of falls since discharge, functional and affective status were assessed by phone one month after discharge. Fear of falling was assessed using the question: "Are you afraid of falling?". Results: Among patients without fear of falling at discharge (N=95), 20.0% (N=19) reported new fear of falling one month after discharge. Living alone (adjOR=4.9, 95%CI 1.04-23.16, P=.045), functional status at discharge (adjOR=0.5, 95%CI 0.32-0.88, P=.014), and depressive symptoms (adjOR=5.4, 95%CI 1.20-24.32, P=.028) independently predicted fear of falling at one month. There was weak evidence that history of falls since discharge (adjOR=4.1, 95%CI 0.81-21.31, P=.088) was associated with new fear of falling. Developing fear of falling was also associated with reduced functional status at one month (mean basic ADL score: fearful 5.20.8; confident: 5.80.4,P<.001). This association remained after controlling for demographics, functional status at discharge, depressive symptoms, and history of falls since discharge (coef =-0.4, 95%CI -0.73 to -0.16, P=.003). Conclusion: Fear of falling appearing within one month after discharge from post-acute rehabilitation was associated with reduced functional status in elderly patients. Further studies should determine whether early interventions targeting specifically fear of falling in these patients would improve their functional status.

Preterm birth may represent a traumatic situation for both parents and a stressful situation for the infant, potentially leading to difficulties in mother-infant relationships. This study aimed to investigate the impact of an early intervention on maternal posttraumatic stress symptoms, and on the quality of mother-infant interactions, in a sample of very preterm infants and their mothers. Half of the very preterm infants involved in the study (n=26) were randomly assigned to a 3-step early intervention program (at 33 and 42 weeks after conception and at 4 months' corrected age). Both groups of preterm infants (with and without intervention) were compared to a group of full-term infants. The impact of the intervention on maternal posttraumatic stress symptoms was assessed 42 weeks after conception and when the infants were 4 and 12 months of age. The impact of the intervention on the quality of mother-infant interactions was assessed when the infants were 4 months old. Results showed a lowering of mothers' posttraumatic stress symptoms between 42 weeks and 12 months in the group of preterm infants who received the intervention. Moreover, an enhancement in maternal sensitivity and infant cooperation during interactions was found at 4 months in the group with intervention. In the case of a preterm birth, an early intervention aimed at enhancing the quality of the mother-infant relationship can help to alleviate maternal post-traumatic stress symptoms and may have a positive impact on the quality of mother-infant interactions.

Although sources in general nonlinear mixturm arc not separable iising only statistical independence, a special and realistic case of nonlinear mixtnres, the post nonlinear (PNL) mixture is separable choosing a suited separating system. Then, a natural approach is based on the estimation of tho separating Bystem parameters by minimizing an indcpendence criterion, like estimated mwce mutual information. This class of methods requires higher (than 2) order statistics, and cannot separate Gaarsian sources. However, use of [weak) prior, like source temporal correlation or nonstationarity, leads to other source separation Jgw rithms, which are able to separate Gaussian sourra, and can even, for a few of them, works with second-order statistics. Recently, modeling time correlated s011rces by Markov models, we propose vcry efficient algorithms hmed on minimization of the conditional mutual information. Currently, using the prior of temporally correlated sources, we investigate the fesihility of inverting PNL mixtures with non-bijectiw non-liacarities, like quadratic functions. Defining the limits of an urban agglomeration is essential both for fundamental and applied studies in quantitative and theoretical geography. A simple and consistent way for defining such urban clusters is important for performing different statistical analysis and comparisons. Traditionally, agglomerations are defined using a rather qualitative approach based on various statistical measures. This definition varies generally from one country to another, and the data taken into account are different. In this paper, we explore the use of the City Clustering Algorithm (CCA) for the agglomeration definition in Switzerland. This algorithm provides a systemic and easy way to define an urban area based only on population data. The CCA allows the specification of the spatial resolution for defining the urban clusters. The results from different resolutions are compared and analysed, and the effect of filtering the data investigated. Different scales and parameters allow highlighting different phenomena. The study of Zipf's law using the visual rank-size rule shows that it is valid only for some specific urban clusters, inside a narrow range of the spatial resolution of the CCA. The scale where emergence of one main cluster occurs can also be found in the analysis using Zipf's law. The study of the urban clusters at different scales using the lacunarity measure - a complementary measure to the fractal dimension - allows to highlight the change of scale at a given range.

BACKGROUND AND METHODS:: The objectives of this article were to systematically describe and examine the novel roles and responsibilities assumed by nurses in a forensic consultation for victims of violence at a University Hospital in French-speaking Switzerland. Utilizing a case study methodology, information was collected from two main sources: (a) discussion groups with nurses and forensic pathologists and (b) a review of procedures and protocols. Following a critical content analysis, the roles and responsibilities of the forensic nurses were described and compared with the seven core competencies of advanced nursing practice as outlined by Hamric, Spross, and Hanson (2009). RESULTS:: Advanced nursing practice competencies noted in the analysis included "direct clinical practice," "coaching and guidance," and "collaboration." The role of the nurse in terms of "consultation," "leadership," "ethics," and "research" was less evident in the analysis. DISCUSSION AND CONCLUSION:: New forms of nursing are indeed practiced in the forensic clinical setting, and our findings suggest that nursing practice in this domain is following the footprints of an advanced nursing practice model. Further reflections are required to determine whether the role of the forensic nurse in Switzerland should be developed as a clinical nurse specialist or that of a nurse practitioner.

The aim of this study was to compare the diagnostic value of post-mortem computed tomography angiography (PMCTA) to conventional, ante-mortem computed tomography (CT)-scan, CT-angiography (CTA) and digital subtraction angiography (DSA) in the detection and localization of the source of bleeding in cases of acute hemorrhage with fatal outcomes. The medical records and imaging scans of nine individuals who underwent a conventional, ante-mortem CT-scan, CTA or DSA and later died in the hospital as a result of an acute hemorrhage were reviewed. Post-mortem computed tomography angiography, using multi-phase post-mortem CTA, as well as medico-legal autopsies were performed. Localization accuracy of the bleeding was assessed by comparing the diagnostic findings of the different techniques. The results revealed that data from ante-mortem and post-mortem radiological examinations were similar, though the PMCTA showed a higher sensitivity for detecting the hemorrhage source than did ante-mortem radiological investigations. By comparing the results of PMCTA and conventional autopsy, much higher sensitivity was noted in PMCTA in identifying the source of the bleeding. In fact, the vessels involved were identified in eight out of nine cases using PMCTA and only in three cases through conventional autopsy. Our study showed that PMCTA, similar to clinical radiological investigations, is able to precisely identify lesions of arterial and/or venous vessels and thus determine the source of bleeding in cases of acute hemorrhages with fatal outcomes.

Objectifs: Des décès suite à une intervention chirurgicale sont autopsiés dans le but de déterminer le cause de la mort et investiguer une éventuelle erreur médicale . Le butde l'étude est d'évaluer l'utilité de l'angio-TDM post mortem pour ce type d'investigations médico-légales délicates. Matériels et méthodes: 145 cas médico-légaux ont été investigués. De ce collectif, huit cas impliquaient une intervention chirurgicale pour laquelle le décès pouvait être éventuellementimputé. Les résultats des examens radiologiques ont été comparés avec ceux obtenus par l'autopsie conventionnelle. Résultats: La cause du décès était soit un choc hémorragique ou septique, soit la combinaison d'une hémorragie et d'une aspiration de sang. Le diagnostic a pu être posélors de l'autopsie conventionnelle de même que lors de l'examen radiologique. Cependant, l'examen par angio-TDM a permis de détecter la source exacte deshémorragies dans cinq des six cas, alors que l'autopsie n'a permis de localiser le site hémorragique approximativement que dans trois cas . Conclusion: L'angio-TDM post-mortem est recommandée dans les cas de décès post-interventionnels. Elle permet de documenter les constatations et de réexaminer les casultérieurement. De plus, la source exacte des hémorragies peut être localisée ce qui est d'une grande importance dans ce genre de cas.
