RIANO, Pedro Tanquero. Melhoria da detecção de Câncer de Colo de Útero e de Mama na UBS Manoel, Lagoa Nova- RN. 110f .Ano 2015. Trabalho de Conclusão de Curso (Curso de Especialização em Saúde da Família) - Departamento de Medicina Social, Faculdade de Medicina, Universidade Federal de Pelotas, 2015. O presente estudo é o resultado da intervenção realizada na UBS Distrito Manoel Domingo, localizada na área rural no Município Lagoa Nova-RN, com uma população adstrita estimada de 2762 habitantes, contando para o estudo com um total de mulheres entre 25 e 64 anos de 694 e de 50 a 69 anos de 208 mulheres. Como justificativa para a escolha do foco da Intervenção, consideramos que o elemento dos temas avaliados durante o processo da especialização com maiores dificuldades em nossa UBS relacionadas à ação programática estava o programa de Prevenção do Câncer do Colo de Útero e Câncer de Mama, encontrando-se no começo da intervenção só 56 % de acompanhamento para a prevenção do Câncer de Colo de Útero, e 61% para Câncer de Mama. Para o desenvolvimento da intervenção tivemos como objetivo geral: Melhorar a Detenção de Câncer de Colo de Útero e de Mama da UBS Manoel, Lagoa Nova-RN. Na metodologia, utilizamos os quatro eixos da intervenção referentes à Monitoramento e avaliação, Organização e gestão do serviço, Engajamento Público e Qualificação da prática clínica, confeccionando-se um cronograma de trabalho com atividades a ser desenvolvidas durante a intervenção, sendo cumpridas em 100 %. Como resultados alcançaram o cumprimento dos objetivos e metas traçadas alcançando uma cobertura em 93,5 % das mulheres para câncer de colo de útero e 99,1 % das mulheres para câncer de mama, com grande impacto para saúde da família, a comunidade, serviço e a equipe, propiciando chegar toda a equipe às comunidades populacionais de nossa área de abrangência, desenvolvendo atividades de educação e promoção de saúde, que permitiu o intercambio da equipe com a comunidade, educar, informar, compartilhar informações, propiciando apoio, participação e reconhecimento social, demonstrando a comunidade satisfação pelas ações realizadas. No serviço, permitiu integrar toda a equipe em função de uma atividade, esclarecer e reformular as atribuições e funções propicia incrementar as consultas agendadas com maior controle e registro da informação, além de melhorar o acolhimento e a participação de toda a equipe, desenvolver ações de divulgação de informações, educação e promoção de saúde, assim como melhorar a classificação de risco das mulheres deste grupo populacional. Para a equipe, a intervenção teve grande importância, propiciando a unidade, integração e fortalecimento do trabalho, comunicação entre a equipe, a comunidade e os gestores, fazendo da capacitação permanente uma ferramenta no novo estilo de trabalho nas atividades da UBS, permitindo incorporar as ações na rotina diária de trabalho na UBS. Cardiac arrest during heart surgery is a common procedure and allows the surgeon to perform surgical procedures in an environment free of blood and movement. Using a model of isolated rat heart, the authors compare a new cardioplegic solution containing histidine-tryptophan-glutamate (group 2) with the histidine-tryptophan-alphacetoglutarate (group 1) routinely used by some cardiac surgeons. To assess caspase, IL-8 and KI-67 in isolated rat hearts using immunohistochemistry. 20 Wistar male rats were anesthetized and heparinized. The chest was opened, cardioctomy was performed and 40 ml/kg of the appropriate cardioplegic solution was infused. The hearts were kept for 2 hours at 4ºC in the same solution, and thereafter, placed in the Langendorff apparatus for 30 minutes with Ringer-Locke solution. Immunohistochemistry analysis of caspase, IL-8, and KI-67 were performed. The concentration of caspase was lower in group 2 and Ki-67 was higher in group 2, both P<0.05. There was no statistical difference between the values of IL-8 between the groups. Histidine-tryptophan-glutamate solution was better than histidine-tryptophan-alphacetoglutarate solution because it reduced caspase (apoptosis), increased KI-67 (cell proliferation), and showed no difference in IL-8 levels compared to group 1. This suggests that the histidine-tryptophan-glutamate solution was more efficient than the histidine-tryptophan-alphacetoglutarate for the preservation of hearts of rat cardiomyocytes.

Hydrophilic and lipophilic extracts of ten cultivars of Highbush and Rabbiteye Brazilian blueberries (Vaccinium corymbosum L. and Vacciniumashei Reade, respectively) that are used for commercial production were analysed for antioxidant activity by the FRAP, ORAC, ABTS and β-carotene-linoleate methods. Results were correlated to the amounts of carotenoids, total phenolics and anthocyanins. Brazilian blueberries had relatively high concentration of total phenolics (1,622-3,457 mg gallic acid equivalents per 100 g DW) and total anthocyanins (140-318 mg cyanidin-3-glucoside equivalents per 100 g DW), as well as being a good source of carotenoids. There was a higher positive correlation between the amounts of these compounds and the antioxidant activity of hydrophilic compared to lipophilic extracts. There were also significant differences in the level of bioactive compounds and antioxidant activities between different cultivars, production location and year of cultivation.

Originally from Asia, Dovyalis hebecarpa is a dark purple/red exotic berry now also produced in Brazil. However, no reports were found in the literature about phenolic extraction or characterisation of this berry. In this study we evaluate the extraction optimisation of anthocyanins and total phenolics in D. hebecarpa berries aiming at the development of a simple and mild analytical technique. Multivariate analysis was used to optimise the extraction variables (ethanol:water:acetone solvent proportions, times, and acid concentrations) at different levels. Acetone/water (20/80 v/v) gave the highest anthocyanin extraction yield, but pure water and different proportions of acetone/water or acetone/ethanol/water (with >50% of water) were also effective. Neither acid concentration nor time had a significant effect on extraction efficiency allowing to fix the recommended parameters at the lowest values tested (0.35% formic acid v/v, and 17.6 min). Under optimised conditions, extraction efficiencies were increased by 31.5% and 11% for anthocyanin and total phenolics, respectively as compared to traditional methods that use more solvent and time. Thus, the optimised methodology increased yields being less hazardous and time consuming than traditional methods. Finally, freeze-dried D. hebecarpa showed high content of target phytochemicals (319 mg/100g and 1,421 mg/100g of total anthocyanin and total phenolic content, respectively).

Introdution: The transcatheter aortic valve implantation in the treatment of high-risk symptomatic aortic stenosis has increased the number of implants every year. The learning curve for transcatheter aortic valve implantation has improved since the last 12 years, allowing access alternatives. The aim of this study is to approach the implantation of transcatheter aortic valve through transaortic via associated with off-pump cardiopulmonary bypass surgery in a 67-year-old man, with chronic obstructive pulmonary disease, arterial hypertension and kidney transplant. Off-pump coronary artery bypass surgery was performed and the valve in the aortic position was released successfully. There were no complications in the intraoperative and postoperative period. Gradient reduction, effective orifice increasing of the prosthesis and absence of valvular regurgitation after implantation were observed by transesophageal echocardiography. Procedural success demonstrates that implantation of transcatheter aortic valve through the ascending aorta associated with coronary artery bypass surgery without CPB is a new option for these patients.

There is a paucity of literature on haemophilia treatment in Latin American countries, a region characterized by rapidly improving systems of care, but with substantial disparities in treatment between countries. The aim of this study was to evaluate the musculoskeletal status of haemophilia patients from Latin America and to examine the relationship between musculoskeletal status and treatment practices across countries. The Committee of Latin America on the Therapeutics of Inhibitor Groups conducted a survey of its member country representatives on key aspects of haemophilia treatment in 10 countries. Musculoskeletal status of patients was obtained during routine comprehensive evaluations between March 2009 and March 2011. Eligible patients had severe haemophilia A (factor VIII <1%) without inhibitors (<0.6 BU mL(-1) ) and were ≥5 years of age. Musculoskeletal status was compared between three groups of countries, based primarily on differences in the availability of long-term prophylaxis. Overall, 143 patients (5-66 years of age) were enrolled from nine countries. In countries where long-term prophylaxis had been available for at least 10 years (Group A), patients aged 5-10 years had significantly better mean World Federation of Hemophilia clinical scores, fewer target joints and fewer affected joints than patients from countries where long-term prophylaxis has been available for about 5 years (Group B) or was not available (Group C). In Latin America, the musculoskeletal status of patients with severe haemophilia without inhibitors has improved significantly in association with the provision of long-term prophylaxis. As more countries in Latin America institute this practice, further improvements are anticipated.

Major problems with valve bioprostheses are associated with progressive structural deterioration and calcification, directly associated with the use of glutaraldehyde (GA). This work describes the effects of GA processing and borate/glutamic acid buffer treatment on the mechanical, thermal and morphological properties of 0.5% GA crosslinked bovine pericardium (BP). The results showed that while the treatment of 0.5% GA crosslinked BP with borate/glutamic acid significantly improves the mechanical properties, it had no visible effect on surface morphology. Better surface preservation was only achieved for BP pre-treated with a lower GA concentration followed by the conventional treatment (0.5% GA). Improvements in mechanical properties probably arises from structural changes probably involving the depolymerization of polymeric GA crosslinks and an increase electrostatic interaction due to covalent binding of glutamic acid to free carbonyl groups (Schiff base).The results indicate that the treatment GA crosslinked BP with borate/glutamic acid buffer may be an attractive procedure for the manufacture of heart valve bioprostheses.

Nursery performance, development, and RNA:DNA ratio were investigated in Farfantepenaeus paulensis (Pérez-Farfante, 1967) postlarvae acclimated from a salinity of 30‰ to higher (35‰) or lower (16, 22 and 29‰) salinities and reared for 20 days. Overall, higher final weight, yield and growth rate were observed at a salinity of 29‰. RNA:DNA ratio indicated reduced growth potential at a salinity of 35‰. Low salinities resulted in more developed individuals. Thus, early postlarval F. paulensis should not be stocked in salinities higher than that of the original hatchery, otherwise in lower salinities postlarvae should be older and/or have an extended nursery phase. Results may assist in the development of nursery rearing protocols for F. paulensis, an alternative species for aquaculture in subtropical areas.

The growth parameters and the mortality rates of the Scomber japonicus peruanus (Chub mackerel) were studied based on monthly data of frequency of fork length classes obtained from commercial landings off the Peruvian coast from 1996 to 1998. The asymptotic body length and growth rate values obtained by the ELEFAN I (Electronic Length Frequency Analysis) ranged from 40.20 cm to 42.20 cm and from 0.38 to 0.39, respectively. The oscillation amplitude was 0.60; the Winter point values varied from 0.50 to 0.60 and the performance index from 2.79 to 2.84. The total mortality rate of the Chub mackerel obtained by the linearized catch curve oscillated between 1.68 and 3.35. The rate of fishing mortality varied from 1.16 to 2.78 and the exploitation rate from 0.68 to 0.84. The annual rate of natural mortality estimated by the Pauly's method ranged from 0.52 to 0.53. The results obtained allow us to conclude that the longevity of the Chub mackerel was slightly over seven years.

The influence of the 1992-1993 El Niño events on the reproductive behavior of the Scomber japonicus peruanus (Chub mackerel) was studied from samples collected monthly, along the Peruvian coast (3º23'S-14º00'S), from January 1990 to December 1993. The monthly variation of the gonadosomatic index and the frequency of the periods of gonad maturation evidenced that the spawning of the species occurred all year long, being more intense in summer. The values of the gonadosomatic index were higher during the occurrence of the 1992-1993 El Niño, while the body weight and gonad weight decreased. Regarding the condition factor, its values decreased in females over 35 cm in fork length.

A new species of Trachycorystes from the rio Aripuanã, above Dardanelos and Andorinhas falls, is described. The new species is distinguished from the only other species of the genus, T. trachycorystes, by the following characteristics: jaws of equal length (vs. lower jaw prognathous in T. trachycorystes); skull roof covered by thick (vs. thin) integument; inner mental barbel very thin and short not reaching base of outer barbel (vs. extending to or beyond base of outer mental barbel); dorsal-fin spine serrated posteriorly, smooth or rough anteriorly (vs. serrated anteriorly and smooth or rough posteriorly); caudal fin shallowly forked (vs. emarginate); and gas bladder simple, without diverticula (vs. with three posterior diverticula). Comments and data on the nominal species Trachycorystes trachycorystes are provided. Trachycorystes cratensis Miranda Ribeiro, 1937, is allocated to the genus Trachelyopterus Valenciennes, 1840, and another local catfish species, Parotocinclus aripuanensis Garavello, 1988, has its type locality reassigned.
