UNLABELLED Obesity is a well-recognized risk factor for atrial fibrillation (AF), yet adiposity measures other than body mass index (BMI) have had limited assessment in relation to AF risk. We examined the associations of adiposity measures with AF in a biracial cohort of older adults. Given established racial differences in obesity and AF, we assessed for differences by black and white race in relating adiposity and AF. METHODS We analyzed data from 2,717 participants of the Health, Aging, and Body Composition Study. Adiposity measures were BMI, abdominal circumference, subcutaneous and visceral fat area, and total and percent fat mass. We determined the associations between the adiposity measures and 10-year incidence of AF using Cox proportional hazards models and assessed for their racial differences in these estimates. RESULTS In multivariable-adjusted models, 1-SD increases in BMI, abdominal circumference, and total fat mass were associated with a 13% to 16% increased AF risk (hazard ratio [HR] 1.14, 95% CI 1.02-1.28; HR 1.16, 95% CI 1.04-1.28; and HR 1.13, 95% CI 1.002-1.27). Subcutaneous and visceral fat areas were not significantly associated with incident AF. We did not identify racial differences in the associations between the adiposity measures and AF. CONCLUSION Body mass index, abdominal circumference, and total fat mass are associated with risk of AF for 10years among white and black older adults. Obesity is one of a limited number of modifiable risk factors for AF; future studies are essential to evaluate how obesity reduction can modify the incidence of AF.

Growth hormone replacement therapy (GHRT) increases exercise capacity and insulin resistance while it decreases fat mass in growth hormone-deficient patients (GHD). Ectopic lipids (intramyocellular (IMCL) and intrahepatocellular lipids (IHCL) are related to insulin resistance. The effect of GHRT on ectopic lipids is unknown. It is hypothesized that exercise-induced utilization of ectopic lipids is significantly decreased in GHD patients and normalized by GHRT. GHD (4 females, 6 males) and age/gender/waist-matched control subjects (CS) were studied. VO2max was assessed on a treadmill and insulin sensitivity determined by a two-step hyperinsulinaemic-euglycaemic clamp. Visceral (VAT) and subcutaneous (SAT) fat were quantified by MR-imaging. IHCL and IMCL were measured before and after a 2 h exercise at 50-60% of VO2max using MR-spectroscopy (∆IMCL, ∆IHCL). Identical investigations were performed after 6 months of GHRT. VO2max was similar in GHD and CS and significantly increased after GHRT; GHRT significantly decreased SAT and VAT. 2 h-exercise resulted in a decrease in IMCL (significant in CS and GHRT) and a significant increase in IHCL in CS and GHD pre and post GHRT. GHRT didn't significantly impact on ∆IMCL and ∆IHCL. We conclude that aerobic exercise affects ectopic lipids in patients and controls. GHRT increases exercise capacity without influencing ectopic lipids.

OBJECTIVE To test the hypothesis that substituting artificially sweetened beverages (ASB) for sugar-sweetened beverages (SSB) decreases intrahepatocellular lipid concentrations (IHCL) in overweight subjects with high SSB consumption. METHODS About 31 healthy subjects with BMI greater than 25 kg/m(2) and a daily consumption of at least 660 ml SSB were randomized to a 12-week intervention in which they replaced SSBs with ASBs. Their IHCL (magnetic resonance spectroscopy), visceral adipose tissue volume (VAT; magnetic resonance imaging), food intake (2-day food records), and fasting blood concentrations of metabolic markers were measured after a 4-week run-in period and after a 12-week period with ASB or control (CTRL). RESULTS About 27 subjects completed the study. IHCL was reduced to 74% of the initial values with ASB (N = 14; P < 0.05) but did not change with CTRL. The decrease in IHCL attained with ASB was more important in subjects with IHCL greater than 60 mmol/l than in subjects with low IHCL. ALT decreased significantly with SSB only in subjects with IHCL greater than 60 mmol/l. There was otherwise no significant effect of ASB on body weight, VAT, or metabolic markers. CONCLUSIONS In subjects with overweight or obesity and a high SSB intake, replacing SSB with ASB decreased intrahepatic fat over a 12-week period.

Brown adipose tissue (BAT) promotes a lean and healthy phenotype and improves insulin sensitivity. In response to cold or exercise, brown fat cells also emerge in the white adipose tissue (WAT; also known as beige cells), a process known as browning. Here we show that the development of functional beige fat in the inguinal subcutaneous adipose tissue (ingSAT) and perigonadal visceral adipose tissue (pgVAT) is promoted by the depletion of microbiota either by means of antibiotic treatment or in germ-free mice. This leads to improved glucose tolerance and insulin sensitivity and decreased white fat and adipocyte size in lean mice, obese leptin-deficient (ob/ob) mice and high-fat diet (HFD)-fed mice. Such metabolic improvements are mediated by eosinophil infiltration, enhanced type 2 cytokine signaling and M2 macrophage polarization in the subcutaneous white fat depots of microbiota-depleted animals. The metabolic phenotype and the browning of the subcutaneous fat are impaired by the suppression of type 2 cytokine signaling, and they are reversed by recolonization of the antibiotic-treated or germ-free mice with microbes. These results provide insight into the microbiota-fat signaling axis and beige-fat development in health and metabolic disease.

Atherosclerosis-associated coronary heart disease is the number one cause of death in the western society, and often triggered by metabolic disorders, such as hyperlipidemia, hypertension, obesity, and diabetes. The CD1 molecules are a group of evolutionarily conservative transmembrane proteins that have recently emerged as novel lipid-binding and transporting molecules. The current study was aimed at illustrating the role of CD1d in regulation of lipid metabolism and adipogenesis. In stably transfected smooth muscle cells where CD1d is overexpressed via a pCMV promoter, high levels of binding of the oxysterol 7-ketocholesterol was found. Adipogenic treatment induces the cells to transdifferentiate into an adipocyte-like morphology. This adipocyte morphology of CD1d transfected SMCs strongly resembles that of the pre-adipocytes 3T3-L1 cells grown in the same adipogenic media. Adipogenic treatment of CD1d transfected 3T3-L1 cells led to an increased accumulation of lipids compared to mock transfected cells. Induction of adipogenic gene expression and activation, such as PPARγ and lipoprotein lipase (LPL), was achieved in adipogenically treated smooth muscle cells as well as 3T3-L1 cells with overexpression of CD1d. For determination of the role of CD1d in regulation of adipogenesis, a CD1d transgenic mouse strain was created using the CD1d-smooth muscle promoter construct. Compared to wild type control mice matched in age and sex, the transgenic mice show an age-dependent increase in abdominal and visceral fat tissue. Histopathological examination demonstrated marked enlargement of adipocytes in the transgenic fat tissue which otherwise remained a normal fat tissue structure. Immunohistochemical analysis of CD1d expression in the fat tissue revealed much stronger membrane CD1d immunostaining in the transgenic tissue than the wild type fat tissue. Under normal chow diets, CD1d-transgenic mice also developed fatty livers. In conclusion, CD1d serves as a regulator of lipid metabolism, which may transducer signals from oxysterols to induce expression of genes important in lipogenesis. These experimental results point to a novel mechanism by which CD1d mediates lipid metabolism in adipose tissue and contributes to the development of obesity. ^

Purpose. To evaluate the prognostic factors in desmoid tumors in the light of its possible use in standardizing the treatment strategy of an individual patient. ^ Patients and methods. A retrospective review of 189 consecutive patients who were treated at MD Anderson Cancer Center (MDACC) from January 1995 to December 2005 was done. Univariate and multivariate analysis of different prognostic factors was done on all patients, patients treated with surgery alone, subset of patients who came to MDACC with primary tumor. The median follow up was 63 months. Also the analysis of 189 desmoid patients treated at MDACC between 1995 and 2005 was compared to results of 189 desmoid patients treated at MDACC from 1965-1994 using data retrieved from a 150 field prospective relational soft tissue tumor database. ^ Results. 5-, and 10-year overall survival rate were 0.976 (95%CI 0.952, 0.999), and 0.966 (95% CI 0.935, 0.996), respectively. 5-, and 10-year recurrence free rate were 0.803 (95%CI 0.738, 0.868), and 0.793 (95% CI 0.726, 0.860), respectively. 5 year recurrence free survival for surgery alone, radiotherapy alone, chemotherapy alone and combination regimen were 0.759, 0.625, 0.933, and 0.802 respectively. Age (>30 vs. <=30) and primary tumor site (extremity vs visceral) were two prognostic factors significantly associated with local recurrence in all of the patients. ^ Conclusion. An increased awareness of the complex multidisciplinary management needed for successful control of desmoid tumor may underlie a significantly increased number of desmoid referrals, especially primary untreated desmoids, to UTMDACC. The careful prospective integration of multiple therapies has led to a significant recent improvement in desmoid patient outcome. These trends should be supported, particularly if personalized molecular-based therapies are to be rapidly and effectively deployed for the benefit of those afflicted by this rare and potentially devastating disease.^

Introduction: HIV-associated malignancies such as Kaposi’s sarcoma and Non-Hodgkin’s lymphoma occur in children and usually lead to significant morbidity and mortality. No studies have been done to establish prevalence and outcome of these malignancies in children in a hospital setting in Uganda. ^ Research question: What proportion of children attending the Baylor-Uganda COE present with HIV-associated malignancies and what are the characteristics and outcome of these malignancies? The objective was to determine the prevalence, associated factors and outcome of HIV-associated malignancies among children attending the Baylor-Uganda Clinic in Kampala, Uganda. Study Design: This was a retrospective case series involving records review of patients who presented to the Baylor-Clinic between January 2004 and December 2008. Study Setting: The Baylor-Uganda Clinic, where I worked as a physician before coming to Houston, is a well funded, well staffed; Pediatric HIV clinic located in Mulago Hospital, Kampala, Uganda and is affiliated to Makerere University Medical School. Study Participants: Medical charts of patients aged 6 weeks to 18 years who enrolled for care at the clinic during the years 2004 to 2008 were retrieved for data abstraction. Selection Criteria: Study participants had to be patients of Baylor-Uganda seen during the study period; they had to be aged 6 weeks to 18 years; and had to be HIV positive. Patients with incomplete data or whose malignancies were not confirmed by histology were excluded. Study Variables: Data on patient’s age, sex, diagnosis, type of malignancy, anatomic location of the malignancy; pathology report, baseline laboratory results and outcome of treatment, were abstracted. Data Analysis: Cross tabulation to determine associations between variables using Pearson’s chi square at 95% level of significance was done. Proportions of malignancies among different groups were determined. In addition, Kaplan Meier survival analysis and comparison of survival distributions using the log-rank test was done. Change in CD4 percentages from baseline was assessed with the Wilcoxon signed rank test. Results: The proportion of children with malignancies during the study period was found to be 1.65%. Only 2 malignancies: Kaposi’s sarcoma and Non-Hodgkin’s lymphoma were found. 90% of the malignancies were Kaposi’s sarcoma. Lymph node involvement in children with Kaposi’s sarcoma was common, but the worst prognosis was seen with visceral involvement. Deaths during follow-up were seen in the first few weeks to months. Upon starting treatment the CD4 cell percentage increased significantly from a baseline median of 6% to 14% at 6 months and 15.8% at 12 months of follow-up.^

Introduction. Breast cancer is a highly variable disease, and long-term outcomes for individual patients are difficult to predict. We evaluated a retrospective cohort of early stage breast cancer (ESBC) patients based on a variety of clinical and epidemiological factors, specifically looking at the distribution of metastasis and associations with these clinical and epidemiological factors. ^ Methods. Patients were derived from the Early Stage Breast Cancer Repository (ESBCR) with a breast cancer diagnosed between 1985 and 2000. We conducted univariate and multivariate analysis of the data to evaluate associations between characteristics and occurrence of overall, visceral, and bone metastasis. Visceral metastasis was defined as lung, liver, peritoneal, lymph node (thoracic, abdominal, pelvis), and contralateral breast cancer. ^ Results. Overall, 394 (16%) patients developed a metastasis. Of these, 83% were visceral and 17% were bone. Multivariate analyses identified the following variables to be associated with metastasis: Any metastasis: age at diagnosis, stage, ER/PR status, hormone treatment, and type of surgery (1)Visceral metastasis: age at diagnosis, stage, hormone treatment, and type of surgery (2) Bone metastasis –Alcohol use, stage, and ER/PR status ^ Discussion/conclusion. ER-/PR- status has previously been found to be associated with bone metastasis, as we confirm in our cohort. We report an association between alcohol use and bone metastasis whereas previous studies find an association with recurrence. Distribution of metastases varies from previous studies. Typically, previous studies reported bone metastasis >30%, yet our study found 17%. Previous studies varied in design, and definition of visceral metastasis. Future research is needed to further elucidate prognostic factors associated with specific metastases A more thorough understanding of what might predict which ESBC patients will develop metastases can help direct future treatment. Future studies of this nature could include the Perou intrinsic subtypes, biomarkers like Ki-67, and genetic analyses such as Oncotype DX or MammaPrint.^

La enfermedad cardiovascular es la primera causa de morbi-mortalidad en los países industrializados. El síndrome metabólico, caracterizado por hipertensión, dislipidemia, obesidad e hiperglucemia, constituye el principal factor de riesgo para la enfermedad cardiovascular. El tejido adiposo visceral juega un papel fundamental en este proceso, dado que secreta una variedad de sustancias biológicamente activas denominadas adipoquinas o adipocitoquinas, tales como leptina, resistina, adiponectina, factor de necrosis tumoral alfa (TNFa), y visfatina entre otras. La visfatina es una citoquina descubierta recientemente y su rol en la enfermedad cardiovascular es controversial y aún no ha sido completamente dilucidado. Estudios realizados en humanos y en modelos experimentales en animales sugieren que la visfatina tendría un papel muy importante en las patologías asociadas a la enfermedad cardiovascular. Esta revisión intenta mostrar los últimos avances sobre el rol de la visfatina y las principales adipocitoquinas en las patologías cardiovasculares y el síndrome metabólico.

Objetivos: Evaluar efectividad y adecuación de la terapia analgésica en pacientes internados con dolor. Materiales y Métodos: Estudio transversal, descriptivo y observacional, mediante revisión de historias clínicas y encuesta validada que incluye el Brief Pain Inventory (BPI). Consideramos respuesta analgésica adecuada un valor ≤ 3 (0-10). Criterio de inclusión: paciente internado con dolor. Análisis estadístico: medidas de tendencia central y dispersión, IC95%. Resultados: Se incluyeron 139 pacientes, distribuidos en clínica médica 13.67%, cardiología 2.88%, cirugía 38.13%, quemados 1.44%, ginecología 9.35%, maternidad 9.35%, traumatología 20.14%, neurología 0.72% y urología 2.16%. Edad media 43.40 años (DS±17.52); 41.73% hombres. Mediana de permanencia al momento de evaluación 3 días (1-60). Presentaron dolor somático 56.83% (IC95% 65.07-48.60), visceral 39.57% (IC95% 47.70-31.44) y neuropático 5.04% (IC95% 8.67-1.40). Las principales etiologías del dolor fueron patología quirúrgica aguda 31.65% (IC95% 39.39-23.92), traumatológica 20.14% (IC95% 26.81-13.48), postoperatorio 17.99% (IC95% 24.37-11.60) y neoplásico 10.07% (IC95% 15.08-5.07). El 82.73% (IC95% 89.02-76.45) tenía indicada analgesia, 47.48% endovenosa y en 3.60% participó especialista en dolor. La dosis fue adecuada en 65.47%; el analgésico más indicado diclofenac 36.69%, ketorolac 16.55%, tramadol 6.47%, paracetamol 5.76%, ibuprofeno 2.16%. Recibía morfina 3.60%, AINE combinado con opioide débil 11.51%, corticoides 3.60% y 0.72% anticonvulsivantes. El 3.60% reportó efectos colaterales atribuibles a la analgesia. Mediante BPI el 38% controló su peor dolor y 53% su valor promedio. Existió demora mayor a 24 hs en indicación de analgesia en 7.91%. La analgesia aplicada figuraba en historia clínica en 40.29%, en indicaciones para enfermería 82.73%. La valoración del dolor fue registrada en 46.76% de las evoluciones diarias.

El presente estudio descriptivo realizo durante 2012 tiene por objetivo determinar si la evaluación periódica (cada 4 meses) de la "Condición Física Saludable" (CFS), con su respectivo asesoramiento del Lic. en educación fisca, del nutricionista y del kinesiólogo mejora la condición física del personal. La Evaluación CFS es un conjunto de tests físico, metabólico, morfológicos y coordinativos que valoran el estado de la capacidad de rendimiento psicofísica de una persona en un momento dado. Estos valores tienen relación directa con el estado de salud y con el rendimiento laboral. Variable a medir. Composición corporal. Peso, Talla, IMC, Masa Grasa, grasa visceral y masa muscular. Indice Cintura Cadera. Capacidades funcionales. Tensión arterial Saturación de oxigeno y Frecuencia Cardiaca Test Físicos: Test de Wells. Test de recuperación cardiovascular. VO2 indirecto: Test del cajón (OMS). Evaluación postural: Observación Kinesiológica. Capacidad Coordinativa: Equilibrio estático. Resultados al año de trabajo. El 22de los empleados disminuyo la masa grasa. Un 19disminuyo el IMC. El 16mejoro los niveles de recuperación cardiovascular. El 21de los empleados aumento su masa muscular El 22mejoro su equilibrio estático. El 22mejoro la flexibilidad del tronco. El 11mejoro su Vo2. Conclusiones El 54de las personas que realizaron el programa de evaluación de la condición física saludable mejoraron al menos en una de las variables de la condición física

El trabajo ensaya una aproximación a la narrativa de Roberto Bolaño a partir de la novela Amuleto (1999) para definir su propuesta estética en relación con los debates en torno a los nuevos realismos en el Cono Sur: los cambios y desplazamientos en torno a la categoría "realismo" en la posdictadura. Bolaño articula un realismo particular, el "realismo visceral"; posicionándose en el campo literario latinoamericano, en diálogo con los debates actuales en el marco de los denominados nuevos realismos, o "realismos adjetivados". Atravesado por el fantástico, funciona como matriz para relatar la experiencia de las dictaduras y sus significaciones en el presente. El análisis explora, entonces, cómo la narrativa de Roberto Bolaño abre otras posibilidades de representación y constituye una posición diferenciada en el campo literario chileno, a la vez que participa de las discusiones en torno al realismo latinoamericano y a la pregunta sobre "cómo narrar" el pasado traumático de la dictadura militar. Las huellas de la violencia en América Latina exigen nuevos modos de abordar lo "real" porque la experiencia adquirió dimensiones inefables. El realismo visceral de Bolaño evidencia los cambios estéticos de los 60 a los 90, al fracturar el optimismo de las narrativas del boom, al debilitar las certezas del testimonio, y sobre todo, al incluir el fantástico que opera como espacio de lo siniestro, del fantasma (metáfora del desaparecido) y sus significaciones en el contexto del terrorismo de Estado

Ha venido surgiendo en la literatura española de las últimas décadas una serie de relatos que intentan, con mayor o menor afán revisionista, la reinterpretación de los hechos históricos recientes, en especial, los referidos a la Guerra Civil, la crisis de Posguerra y el Exilio. En este marco, la escritora asturiana Susana Pérez Alonso inscribe su última novela, Melania Jacoby (2010). La novela está articulada en base a dos ejes centrales: la historia de una mujer adelantada a su época, que cuestiona radicalmente el orden impuesto por el poder masculino, y la irrupción del conflicto minero de 1934, que dibujará desde sus márgenes un mapa de recorridos por la situación social, política y económica de la Asturias previa a la Guerra Civil. En comunión con las fuerzas naturales y a partir de su lucha junto a los mineros, Melania Jacoby intenta no sólo detener el proceso de deshumanización al que parecen estar condenados, sino que también, respondiendo al deseo ancestral de restitución de la Unidad perdida entre el Hombre y la Naturaleza, cifra en el poder del amor, la más visceral de las punciones humanas, la única vía de escape de aquéllos ante el horror inminente al que su país se precipita

Nuestra investigación se centró fundamentalmente en las distintas formas en que las derechas respondieron a los conflictos suscitados en el mundo del trabajo durante el período de entreguerras. En primer lugar, advertimos que la cuestión social fue adquiriendo un rol protagónico en la agenda nacionalista tal como puede verse, por ejemplo, a través del análisis de los periódicos. En efecto, los diarios más importantes adscriptos al nacionalismo desarrollaron un discurso radical respecto a los problemas sociales e incluyeron secciones específicas para tratar estas cuestiones y expresar una posición al respecto. Las respuestas del nacionalismo argentino frente a la cuestión obrera han sido múltiples y han abarcado distintas esferas de la vida social. Lejos de esperar que la solución a los problemas sociales proviniera exclusivamente de las medidas restrictivas y represivas hacia el movimiento obrero, los nacionalistas elaboraron programas sociales, políticos, económicos y culturales que formaron parte de su proyecto de nación autoritaria y jerárquica. Los proyectos sociales y las propuestas de organización sindical fueron en gran parte inspirados por los fascismos europeos los cuales incluyeron programas de contención social dentro de un orden político totalitario. En este sentido los nacionalistas argentinos intentaron mediante sus propuestas imponer un orden que contemplara las necesidades básicas de los sectores populares y que preservara las jerarquías sociales limitando la participación política o sindical de los trabajadores y eliminando definitivamente alas fuerzas de la izquierda revolucionaria. Las organizaciones obreras nacionalistas incluyeron todo tipo de trabajadores en sus filas y procuraron captar tanto a los afiliados de los sindicatos autónomos como a los trabajadores socialistas. Algunas de estas organizaciones fueron efímeras mientras que otras tuvieron más éxito y lograron atraer adherentes. Las mismas conformaron la corriente que hemos denominado nacionalismo sindicalista, la cual desarrolló su propia doctrina social fuertemente influenciada por las encíclicas papales. Las manifestaciones nacionalistas en el espacio público porteño han sido también analizadas in extenso. Existieron distintos tipos de manifestaciones para movilizar a los seguidores del nacionalismo y para captar nuevos adherentes, especialmente aquellos provenientes de los sectores populares. Las manifestaciones se convirtieron en el escenario de las disputas ideológicas mantenidas tanto contra la política liberal como contra la revolucionaria. La "revolución nacionalista", según la formulaban sus partidarios, implicaba trascender los aspectos políticos y económicos incorporando transformaciones en otras áreas de la vida social: las costumbres, las formas de vida, los gustos culturales, los valores. Los nacionalistas advirtieron que para lograr este tipo de "revolución" debían hacer usa de los medios de comunicación masivos y diseñar proyectos para regular las industrias culturales. El objetivo de representar a los sectores populares fracasó rotundamente. El discurso nacionalista que condenaba la diversidad étnico-religiosa, que amenazaba con eliminar las distintas voces políticas existentes, y que expresaba un odio visceral a sus enemigos (ya fueran judíos, anarquistas, comunistas, o liberales) fue extremadamente desafortunado para quienes procuraron ensanchar las bases de un movimiento antidemocrático originalmente elitista que, a la luz del contexto internacional y de las condiciones locales, devino en populista
