INTRODUCTION: Vaccination is the main tool for preventing hepatitis B virus (HBV) infection; however, following the completion of the vaccination series, the concentrations of anti-HBs can decline over the years and reach levels less than 10mIU/mL. The persistence of protection in these individuals is still unknown. The present study aimed to determine the anti-HBs antibody levels among children and adolescents who had received a complete vaccination course for hepatitis B. METHODS: Antibodies against HBV surface antigen (anti-HBs) were tested in 371 individuals aged 10 to 15 years-old. RESULTS: Volunteers who showed undetectable quantities of anti-HBs accounted for 10.2% of the population studied and 39.9% presented antibody titers of less than 10mIU/mL. Anti-HBs > 10mIU/mL were verified in 49.9%. CONCLUSIONS: These results corroborate other studies indicating levels of anti-HBs below 10mIU/mL in vaccinated individuals. Additional studies are required to assess whether this indicates susceptibility to HBV infection and the need and age for booster doses.

RESUMO - Enquadramento: O ambiente hospitalar é a principal porta de introdução de novos fármacos no hábito de prescrição e os números atuais apontam para gastos na ordem dos mil milhões de euros, justificados pelo maior consumo de medicamentos de cedência gratuita e obrigatória em farmácia hospitalar, nos quais se destaca a terapêutica anti-retrovírica para a infeção VIH/SIDA. O acesso universal à terapêutica disponível permitiu reduzir significativamente a morbilidade e mortalidade associadas à infeção VIH/SIDA conduzindo a sociedade a encará-la como uma doença crónica. Em Portugal, esta problemática tem vindo a ser uma preocupação constante e crescente no domínio das políticas públicas e em 2007, foi introduzido um modelo experimental de financiamento aos prestadores de cuidados de saúde a doentes naive com infeção VIH/SIDA (Programa TARV) que se destina ao pagamento de cuidados prestados em regime de ambulatório. A conceção deste programa de contratualização foi idealizada no âmbito da gestão integrada da doença e, para 2012, prevê-se o seu alargamento a todas as unidades hospitalares e a todos os doentes com infeção VIH/SIDA em tratamento. Objetivos: Descrição e análise do Programa TARV (2007-2009) com a determinação dos principais esquemas terapêuticos utilizados; apuramento do custo médio anual da despesa em medicamentos anti-retrovíricos, por doente; análise evolutiva dos indicadores de adesão e efetividade; e avaliação do processo de contratualização das Instituições em Programa. Metodologia: Foi disponibilizada pela ACSS a base de dados do Programa TARV com a informação recolhida à data de Abril de 2012. O período temporal em análise corresponde aos anos do Programa TARV com faturação emitida à data de Março de 2012: A base de dados encontra-se dividida em três ficheiros cuja informação foi submetida a tratamento estatístico recorrendo à aplicação SPSS (v.14) e Microsoft Excel (v.2007). Resultados e Conclusões: O Programa TARV tem tido um bom desempenho no que se refere à promoção no acesso dos doentes com infeção VIH/SIDA à terapêutica anti-retrovírica. Os principais esquemas terapêuticos para o tratamento de doentes naive vão ao encontro das guidelines terapêuticas e a estimativa realizada apurou custos médios anuais de acompanhamento dos doentes inferiores ao preço compreensivo aplicado no Programa TARV. Os resultados revelam a existência de diferenças significativas entre a produção realizada e a produção contratualizada, assumindo um valor correspondente a 31% do montante global disponibilizado para financiamento do Programa TARV. Por último, face às limitações encontradas, refere-se que modelo de SI utilizado na recolha, processamento e monitorização da informação sobre os doentes em Programa TARV não é o mais adequado para tratar o volume de informação gerado.

INTRODUÇÃO: A leptospirose, enfermidade causada por uma espiroqueta patogênica do gênero Leptospira, é uma das zoonoses mais difundidas no mundo, considerada um importante problema de saúde pública. Este trabalho tem como objetivo descrever o padrão espacial da leptospirose na Cidade de Aracaju, no período de 2001 a 2007, buscando identificar as áreas de risco para ocorrência da leptospirose. MÉTODOS: Utilizou-se a razão de Kernel, que consiste na criação de uma razão entre duas superfícies, para visualizar a superfície de risco da doença, onde no numerador coloca-se os casos, georreferenciados para setor censitário, e no denominador o kernel da população dessas áreas. Através deste método, foi possível delimitar áreas de maior densidade de casos, e compará-las visualmente com fator socioeconômico, como renda média. RESULTADOS: Comparando os períodos de chuva com os períodos de seca, verificou-se que não ocorre a presença de padrão espacial semelhante nos dois períodos, não coincidindo com as áreas consideradas de maior risco para ocorrência da doença. Através dos parâmetros estabelecidos, neste estudo, foi permitido verificar a importância da aplicação de técnicas de análise espacial na área de saúde pública. CONCLUSÕES: O kernel se mostrou uma ferramenta útil na obtenção de uma análise global da situação epidemiológica da leptospirose em Aracaju, o que viabiliza sua utilização pelas secretarias de saúde municipal e estadual.

INTRODUCTION: Visceral leishmaniasis (VL) is an expanding zoonosis in Brazil and is becoming urbanized in several Brazilian regions. This study aims to describe the epidemiological features of human and canine VL in the municipality of Montes Claros, State of Minas Gerais, by focusing on their spatial distribution. METHODS: Data concerning human cases and reactive dogs for VL from 2007 to 2009 were obtained from the Information System for Disease Notification (SINAN) and from reports of the local Centro de Controle de Zoonoses (CCZ), respectively. The addresses of human and canine cases have been georeferenced and localized in thematic maps, allowing their spatial visualization as well as the identification of areas at risk of VL transmission. RESULTS: Ninety-five cases of human VL were reported in the period. The 0-9-year-old age group (48.4%) was the most affected, within which the majority consisted of male patients (64%). Of the samples collected for the canine serological survey, 2,919 (6.3%) were reactive to VL. The spatial localization of these cases shows that the disease was scattered in the urban area of the municipality. Areas showing a higher dissemination risk were concentrated in the central, northwestern, and southern regions of the city. CONCLUSIONS: Identifying the areas most at risk in urban Montes Claros may help guide actions toward local epidemiological vigilance and control.

INTRODUÇÃO: Foi feito um acompanhamento das gestantes tratadas para tuberculose multidroga resistente (TB-MDR) durante a gravidez, em Campinas, Estado de São Paulo, no período de 1995 a 2007. MÉTODOS: No estudo retrospectivo, foram incluídas pacientes com cepa resistente, pelo menos à isoniazida e à rifampicina e gravidez em qualquer momento, durante o tratamento. Os casos receberam esquema individualizado baseado nos padrões de resistência medicamentosa e nos antecedentes de tratamento de cada paciente. RESULTADOS: Sete gestantes apresentaram resistência a duas ou mais drogas. Três já estavam grávidas, antes do início do tratamento e quatro engravidaram depois. Duas estavam com Aids, uma foi a óbito e em outra ocorreu falência de tratamento. Todas apresentaram tuberculose avançada com achados radiológicos graves e para cinco pacientes o tempo médio da conversão do escarro foi de quatro meses. Duas gestantes foram curadas. Dois recém-nascidos foram infectados com o HIV por suas progenitoras. CONCLUSÕES: O resultado do tratamento foi insatisfatório e a supervisão direta é imperativa na TB-MDR durante a gestação. O planejamento familiar deve ser fortemente recomendado.

This paper examines the impact of Sovereign rating changes on the aggregate stock and bond market returns both in emerging and developed countries. Rating downgrades in emerging markets are associated with significant negative wealth effects both in the stock and bond markets. Moreover, the effects of rating downgrades persist up to six-months after the event. In contrast, upgrades in emerging markets convey no information. Rating changes in developed markets have no significant impact on either stock and bond market returns. Rating agencies act pro-cyclically, downgrading countries in bad times and, consequently, contributing to the instability in emerging markets.

INTRODUCTION: The present study shows a descriptive analysis of triatomine occurrence and its natural Trypanosoma infection rates in the state of Pernambuco, Brazil, between 2006 and 2007. METHODS: Entomological data for the species, such as specimens captured in both intra and peridomiciles and natural infection index, were obtained via domiciliary capture in 147 municipalities from 11 Regional Managements of Health. The database was obtained from a sample of insects (100% infected and 20% non-infected) sent to the Central Laboratory of Pernambuco. RESULTS: A total of 18,029 triatomines were analyzed from 138 municipalities of the state. Triatoma pseudomaculata (35%), Triatoma brasiliensis (34%), and Panstrongylus lutzi (25%) were the most captured species. These species also showed a widespread geographical distribution in the state. Panstrongylus megistus, Triatoma petrocchiae, Triatoma melanocephala, Triatoma sordida, Rhodnius nasutus, Rhodnius neglectus, and Triatoma infestans showed more limited geographical distribution and lower relative abundance. The parasitological research showed that 8.8% of the triatomines were naturally infected with flagellates morphologically similar to Trypanosoma cruzi and 91.3% of them were captured inside houses in 113 municipalities. P. lutzi showed the highest rates of natural infection. CONCLUSIONS: After the control of T. infestans, synanthropic species, such as T. brasiliensis, T. pseudomaculata, and P. lutzi, maintain the risk of T. cruzi transmission to humans in the state of Pernambuco. These species are widely distributed, and infected specimens have been found inside houses. Thus, an enhanced surveillance and vector control of Chagas disease is recommended in Pernambuco.

INTRODUCTION: In the jurisdiction of Brasília, Brazil, significant reductions in mortality rates and lethality resulting from acquired immunodeficiency syndrome (AIDS) were observed shortly after the introduction of highly active antiretroviral therapy. In recent years, however, the decline of these rates has not been as significant. Non-adherence to treatment and delayed diagnosis appear to be the main factors that increase the risk of death from AIDS. Behavioral, socioeconomic, and biological factors could also be associated with increased risk of death due to AIDS. This study aimed to identify which of these factors were associated with deaths from AIDS in Brasília. METHODS: A case-control study was undertaken using the data recorded in the Information System of Notifiable Diseases. Cases consisted of AIDS deaths occurring in 2007, residing in Brasília, and over 12 years of age. Controls consisted of AIDS patients who did not die until December 31 2007, also residing in Brasília, and over 12 years of age. For each group, frequency and proportion tables for the variables were prepared. The statistical association of each factor in isolation with the occurrence of the deaths was verified through a model of multivariate analysis using logistic regression. RESULTS: The factors that were associated with an increased risk of death were intravenous drug use, age 50 years or more, and residing in a region whose residents have low per capita income. CONCLUSIONS: We identified factors associated with death due to AIDS that can guide health planning.

INTRODUCTION: Chagas' disease is a major public health problem in Brazil and needs extensive and reliable information to support consistent prevention and control actions. This study describes the most common causes of death associated with deaths related to Chagas' disease (underlying or associated cause of death). METHODS: Mortality data were obtained from the Mortality Information System of the Ministry of Health (approximately 9 million deaths). We analyzed all deaths that occurred in Brazil between 1999 and 2007, where Chagas' disease was mentioned on the death certificate as underlying or associated cause (multiple causes of death). RESULTS: There was a total of 53,930 deaths related to Chagas' disease, 44,543 (82.6%) as underlying cause and 9,387 (17.4%) as associated cause. The main diseases and conditions associated with death by Chagas' disease as underlying cause included direct complications of cardiac involvement, such as conduction disorders/arrhythmias (41.4%) and heart failure (37.7%). Cerebrovascular disease (13.2%), ischemic heart disease (13.2%) and hypertensive diseases (9.3%) were the main underlying causes of deaths in which Chagas' disease was identified as an associated cause. CONCLUSIONS: Cardiovascular diseases were often associated with deaths related to Chagas' disease. Information from multiple causes of death recorded on death certificates allows reconstruction of the natural history of Chagas' disease and suggests preventive and therapeutic potential measures more adequate and specifics.

Introduction Visceral leishmaniasis (VL) stands out as a zoonosis observed on four continents and also in urban expansion zones in several regions of Brazil. Methods A cross-sectional epidemiological study of VL cases in children under 15 years of age in the period from 2007 to 2012. Clinical data were gathered from medical reports; meteorological data were obtained at the Meteorological Measurement Department of UFT. Environmental variables were divided into two periods, rainy and dry. Results The study revealed no difference by gender (p=0.67) among the 821 patients. However, the most affected age group was between one and five years of age (58.6%; p<0.01); the highest prevalence of the disease (99.03%; p<0.01) occurred in urban zones; and the most affected ethnic group (85.5%; p<0.01) was mixed race. The highest incidence coefficients in this population occurred in 2007 and 2008 (578.39/100,000 inhabitants; 18.5/100,000 inhabitants, respectively), whereas the highest lethality coefficients occurred in 2008 and 2011 (0.85/100 deaths). There was no significant correlation between average rainfall and the number of VL cases. The correlation between temperature and number of VL cases was negative (r = -0.4039; p<0.01). Conclusions In Araguaína, visceral leishmaniasis in children under 15 years is an urban-based endemic disease distributed across all districts of the city wherein temperature as an environmental factor, a higher prevalence in mixed race children between one and five years of age, and a high incidence coefficient all strongly contribute to child mortality.

IntroductionThe acquired immunodeficiency syndrome (AIDS) epidemic is a worldwide phenomenon that has been modified with the implementation of effective antiretroviral therapy. The objective of this study was to determine the leading causes of hospitalization among human immunodeficiency virus (HIV)-positive individuals.MethodsA cross-sectional study with patients admitted to a general hospital in southern Brazil, between January 2007 and May 2012.ResultsMedical records of 550 hospital admissions (230 patients) were reviewed, with an average of 2.4 hospitalizations per patient. Infectious diseases were the most prevalent causes of hospitalization. Overall, 44.8% patients died and their deaths were associated with longer hospital stays.ConclusionsOpportunistic infections remained the leading causes of hospitalization.

We investigate the effects of bank control over borrower firms whether by representation on boards of directors or by the holding of shares through bank asset management divisions. Using a large sample of syndicated loans, we find that banks are more likely to act as lead arrangers in loans when they exert some control over the borrower firm. Bank-firm governance links are associated with higher loan spreads during the 2003-2006 credit boom, but lower spreads during the 2007-2008 financial crisis. Additionally, these links mitigate credit rationing effects during the crisis. The results are robust to several methods to correct for the endogeneity of the bank- firm governance link. Our evidence, consistent with intertemporal smoothing of loan rates, suggests there are costs and benefits from banks’ involvement in firm governance.

Os apoios financeiros atribuídos pelos programas nacionais e internacionais a estas áreas da cultura. Procuro mostrar também a evolução e os novos caminhos do cinema e audiovisual quer na União Europeia, quer em Portugal.

The aim of this dissertation is the analysis of the rules on advertising in advocacy. Presently this is a controversial issue that is far from being consensual. As we will demonstrate through the text, the arguments presented are, one the one hand, a safeguard of the deontological values of the profession that govern this professional class and, on the other hand, the interests of the legal service providers, in the current context. Opinions differ substantially among professionals who exercise the profession in individual practice, that defend balanced and fair rules to assert the true brightness of the professional lawyer, and those who work in an organized structure, such as the law firms, who defend more flexible rules in advertising and promoting the offices. Currently the rules of advertising for lawyers are provided by article 89º of the Statue of the Portuguese Bar Association. However, these rules will soon suffer adjustments that will take into consideration the Law no. 2/2013 of january 10, which will extend the scope of advertising for public associations, in order to increase the competition among these, at national or European level. Following this logic, arguments such as unequal access to available means of advertising for financial reasons or that the better publicized service is not always the most advantageous to the costumer will be analyzed and criticized.
