On analytical derivations of the condition number distributions of dual non-central Wishart matrices

PURPOSE:
We sought to measure the impact of central corneal thickness (CCT), a possible risk factor for glaucoma damage, and corneal hysteresis, a proposed measure of corneal resistance to deformation, on various indicators of glaucoma damage.
DESIGN:
Observational study.
METHODS:
Adult patients of the Wilmer Glaucoma Service underwent measurement of hysteresis on the Reichert Ocular Response Analyzer and measurement of CCT by ultrasonic pachymetry. Two glaucoma specialists (H.A.Q., N.G.C.) reviewed the chart to determine highest known intraocular pressure (IOP), target IOP, diagnosis, years with glaucoma, cup-to-disk ratio (CDR), mean defect (MD), pattern standard deviation (PSD), glaucoma hemifield test (GHT), and presence or absence of visual field progression.
RESULTS:
Among 230 subjects, the mean age was 65 +/- 14 years, 127 (55%) were female, 161 (70%) were white, and 194 (85%) had a diagnosis of primary open-angle glaucoma (POAG) or suspected POAG. In multivariate generalized estimating equation models, lower corneal hysteresis value (P = .03), but not CCT, was associated with visual field progression. When axial length was included in the model, hysteresis was not a significant risk factor (P = .09). A thinner CCT (P = .02), but not hysteresis, was associated with a higher CDR at the most recent examination. Neither CCT nor hysteresis was associated with MD, PSD, or GHT "outside normal limits."
CONCLUSIONS:
Thinner CCT was associated with the state of glaucoma damage as indicated by CDR. Axial length and corneal hysteresis were associated with progressive field worsening.

PURPOSE: To describe the distribution of central corneal thickness (CCT), intraocular pressure (IOP), and their determinants and association with glaucoma in Chinese adults.DESIGN: Population-based cross-sectional study.METHODS: Chinese adults aged 50 years and older were identified using cluster random sampling in Liwan District, Guangzhou. CCT (both optical [OCCT] and ultrasound [UCCT]), intraocular pressure (by Tonopen, IOP), refractive error (by autorefractor, RE), radius of corneal curvature (RCC), axial length (AL), and body mass index (BMI) were measured, and history of hypertension and diabetes (DM) was collected by questionnaire. Right eye data were analyzed.RESULTS: The mean values of OCCT, UCCT, and IOP were 512 ± 29.0 μm, 542 ± 31.4 μm, and 15.2 ± 3.1 mm Hg, respectively. In multiple regression models, CCT declined with age (P < .001) and increased with greater RCC (P < .001) and DM (P = .037). IOP was positively associated with greater CCT (P < .001), BMI (P < .001), and hypertension (P < .001). All 25 persons with open-angle glaucoma had IOP <21 mm Hg. CCT did not differ significantly between persons with and without open- or closed-angle glaucoma. Among 65 persons with ocular hypertension (IOP >97.5th percentile), CCT (555 ± 29 μm) was significantly (P = .01) higher than for normal persons.CONCLUSIONS: The distributions of CCT and IOP in this study are similar to that for other Chinese populations, though IOP was lower than for European populations, possibly due to lower BMI and blood pressure. Glaucoma with IOP <21 mm Hg is common in this population. We found no association between glaucoma and CCT, though power (0.3) for this analysis was low.

INTRODUCTION: Jaundice is the yellowish pigmentation of the skin, sclera, and mucous membranes resulting from bilirubin deposition. Children born to mothers with HIV are more likely to be born premature, with low birth weight, and to become septic-all risk factors for neonatal jaundice. Further, there has been a change in the prevention of mother-to-child transmission (PMTCT) of HIV guidelines from single-dose nevirapine to a six-week course, all of which theoretically put HIV-exposed newborns at greater risk of developing neonatal jaundice.
AIM: We carried out a study to determine the incidence of severe and clinical neonatal jaundice in HIV-exposed neonates admitted to the Chatinkha Nursery (CN) neonatal unit at Queen Elizabeth Central Hospital (QECH) in Blantyre.
METHODS: Over a period of four weeks, the incidence among non-exposed neonates was also determined for comparison between the two groups of infants. Clinical jaundice was defined as transcutaneous bilirubin levels greater than 5 mg/dL and severe jaundice as bilirubin levels above the age-specific treatment threshold according the QECH guidelines. Case notes of babies admitted were retrieved and information on birth date, gestational age, birth weight, HIV status of mother, type of feeding, mode of delivery, VDRL status of mother, serum bilirubin, duration of stay in CN, and outcome were extracted.
RESULTS: Of the 149 neonates who were recruited, 17 (11.4%) were HIV-exposed. One (5.88%) of the 17 HIV-exposed and 19 (14.4%) of 132 HIV-non-exposed infants developed severe jaundice requiring therapeutic intervention (p = 0.378). Eight (47%) of the HIV-exposed and 107 (81%) of the non-exposed neonates had clinical jaundice of bilirubin levels greater than 5 mg/dL (p < 0.001).
CONCLUSIONS: The study showed a significant difference in the incidence of clinical jaundice between the HIV-exposed and HIV-non-exposed neonates. Contrary to our hypothesis, however, the incidence was greater in HIV-non-exposed than in HIV-exposed infants.

O presente trabalho propõe-se esclarecer o papel que a progesterona e os seus metabolitos exercem no sistema nervoso central. Nos últimos anos, com a descoberta da síntese local de esteróides no cérebro, a progesterona, assim como outras hormonas sexuais, ganharam uma relevância crescente em fenómenos tais como plasticidade neuronal e neuroprotecção. Ainda que já se comece a entender o papel de muitas hormonas no cérebro, tal como o estrogénio, o papel da progesterona continua menos conhecido. Deste modo, o nosso trabalho centrou-se na elucidação dos efeitos da progesterona em fenómenos de sobrevivência celular, plasticidade neuronal/sináptica. Graças à colaboração com um grupo pioneiro em estudos sobre hormonas sexuais neuroactivas, o presente trabalho fornece uma importante contribuição ao entendimento do papel desta hormona no sistema nervoso central. Este trabalho fornece novos dados, relativamente ao papel da progesterona e dos seus metabolitos reduzidos na regulação de vias de sinalização associadas com sobrevivência celular, tal como Akt/PI3K e ERK. Também é analisado o efeito do tratamento hormonal na expressão e estado de fosforilação da proteína Tau, sendo ainda motivo de estudo cinases e fosfatases envolvidas nestes mecanismos.

Com o intuito de estudar as principais vias de transporte de sedimentos finos recentes na zona central da margem continental Oeste Portuguesa, parâmetros geoquímicos, mineralógicos e granulométricos foram analisados em sedimentos superficiais e em matéria particulada colhida em armadilhas de sedimentos e integrados com observações da hidrodinâmica de fundo. Os parâmetros geoquímicos foram também estudados na coluna de sedimentos depositada nos últimos 150 anos e em sedimentos pré-industriais. Os referidos parâmetros determinados foram: concentrações elementares, isótopos estáveis de Pb, teores em materiais litogénicos, carbonato de cálcio e carbono orgânico. Os canhões de Lisboa-Setúbal e Cascais tiveram especial destaque no presente estudo dado que, até à data, encontram-se menos estudados que o Canhão da Nazaré. Os resultados mostram que a distribuição de sedimentos na zona central da margem ocidental Portuguesa é particionada pelos canhões e que a exportação de sedimentos da plataforma para zonas mais profundas da margem é restringida pelas correntes do talude, excepto onde os canhões funcionam como corredores para o transporte de sedimentos. Enquanto no Canhão de Lisboa-Setúbal, e provavelmente no Canhão de Cascais, o transporte de sedimentos até à zona inferior é limitada, provavelmente apenas despoletado por eventos de elevada energia, no Canhão da Nazaré o transporte ao longo do todo o canhão parece eficiente. As zonas superiores dos canhões de Lisboa-Setúbal e Cascais presentemente actuam como armadilhas de sedimentos finos, aprisionando partículas em suspensão provenientes da plataforma adjacente. A introdução directa de sedimentos provenientes das plumas dos rios Tejo e Sado nas zonas superiores dos canhões parece limitada, contudo a resuspensão dos sedimentos do prodelta do Tejo como resultado de ondas de tempestade e ondas de maré interna permite o transporte de sedimentos para os canhões adjacentes. Na plataforma de Lisboa-Setúbal-Sines foram identificadas as assinaturas geoquímicas e mineralógicas de diferentes fontes de sedimentos finos (e.g. estuários do Tejo e Sado, arribas costeiras, lagoas de St. André e Melides). As concentrações elementares pré-industriais são muito semelhantes nos canhões da Nazaré e Lisboa-Setúbal, mas variados graus de enriquecimento antrópico de metais traço estão presentes nos sedimentos recentes. A mais acentuada influência antrópica na última área referida é consistente com a sua proximidade a áreas densamente povoadas e industrializadas e com input de sedimentos originários dos rios Tejo e Sado, potenciais transportadores de partículas poluentes. A dispersão de Pb atmosférico parece também significativa sendo que toda a zona da plataforma continental adjacente aos canhões de Lisboa-Setúbal e Cascais apresenta-se enriquecida. A principal fonte de Pb antrópico identificada através de isótopos estáveis de Pb é consistente com a assinatura das cinzas de incineradoras. No Canhão de Lisboa a imersão de dragados contaminados parece ser também uma potencial importante fonte de metais traço antropogénicos.

Apesar da fauna de mamíferos Neotropicais ser uma das mais ricas do mundo, o nosso conhecimento sobre os limites de espécies, distribuições geográficas e relações filogenéticas está ainda agora no seu início. As áreas de transição entre os dois maiores biomas da América do Sul, o Cerrado e a Amazónia, são ainda menos conhecidas. Até ao momento, escassos estudos focaram os pequenos mamíferos destas áreas. Destes estudos, apenas dois apresentam dados taxonómicos e de distribuição geográfica de uma lista de espécies reduzida e, nenhum é focado nos processos evolutivos que conduziram à diversidade destas áreas. O presente trabalho tem como objectivo aumentar o conhecimento básico sobre a diversidade do médio Rio Araguaia, na região central do Brasil, através da amostragem e análise de espécies de pequenos mamíferos, integrando um intenso trabalho de campo, de laboratório e de museu. Desta forma, um total de 22 espécies é registado para o médio Araguaia. De entre estas espécies, descreve-se uma espécie nova de Rhipidomys, regista-se uma espécie não descrita de Thrichomys e uma potencial nova forma de Oligoryzomys, e também se apresenta uma diagnose emendada do obscuro Oecomys cleberi. Para cada espécie, são também descritas as suas características morfológicas e resumem-se os seus aspectos de distribuição geográfica e história natural. Para os quatro géneros acima referidos, são apresentadas as análises filogenéticas que permitem a identificação das espécies. Adicionalmente, os princípios da filogeografia são aplicados para estudar os padrões da distribuição geográfica da diversidade genética de três roedores sigmodontíneos e seis marsupiais didelphídeos. Os resultados obtidos demonstram que o Rio Araguaia forma uma barreira geográfica para espécies especialistas em florestas não-alagáveis; por outro lado, espécies generalistas apresentam partilha de haplótipos em ambas as margens do rio. Argumentamos também que os refúgios florestais e os gradientes poderão ter tido um papel importante para moldar a estrutura genética de populações de pequenos mamíferos no Brasil central. Em suma, os resultados apresentados corroboram a proposição de que a diversidade Neotropical não poderá ser explicada através de um único modelo de especiação e que estes não são mutuamente exclusivos. O entendimento integral dos processos ecológicos e históricos que deram origem à fauna Neotropical, assim como a continuidade de estudos sistemáticos, depende da realização de novas amostragens e consequente enriquecimento dos museus com colecções apropriadas.

Strong and sometimes extreme responses in runoff and soil erosion following wildfires have been reported worldwide. However, in the case of North-Central Portugal, little research had been carried out regarding the hydrologic and erosive impacts of several land management activities in recently burnt areas (such as ground preparation, post-fire logging or post-fire mitigation treatments). This study aims to assess post-fire runoff and soil erosion response on Eucalypt and Maritime pine plantations during the first, second and third years following wildfires. The effect of several pre-fire ground preparation operations (ploughed down-slope, contour ploughed and inclined terraces), post-fire logging activities (on both the eucalypt and pine plantations), as well as the application of hydromulch (a post-fire emergency treatment) on overland flow and soil erosion were compared to burnt but undisturbed and untreated areas. The intensive monitoring of runoff, soil erosion and selected soil properties served to determine the main factors involved in post-fire runoff and soil erosion and their spatial and temporal variation. Soil water repellency deserved special attention, due to its supposed important role for overland flow generation. Repeated rainfall simulation experiments (RSE’s), micro-scale runoff plots and bounded sediment fences were carried out and/or installed immediately after the wildfire on seven burnt slopes. Micro-scale runoff plots results under natural rainfall conditions were also compared to the RSE’s results, which was useful for assessing the representativeness of the data obtained with artificial rainfall. The results showed comparable runoff coefficient (20-60%) but lower sediment losses (125-1000 g m-2) than prior studies in Portugal, but especially outside Portugal. Lower sediment losses were related with the historic intensive land use in the area. In evaluating these losses, however, the shallowness and stoniness of the soils, as well as the high organic matter fraction of the eroded sediments (50%) must not be overlooked. Sediment limited erosion was measured in all the ploughed sites, probably due to the time since ploughing (several years). The disturbance of the soil surface cover due to post-fire logging and wood extraction substantially increased sediment losses at both the pine and eucalypt sites. Hydromulch effectiveness in reducing the runoff (70%) and sediment losses (83%) was attributed to the protective high coverage provided by hydromulch. The hydromulch significantly affected the soil cover and other soil properties and these changes also reduced the soil erosion risk. The rainfall amount was the main factor explaining the variance in runoff. However, a shift from rainfall amount to rainfall intensity was detected when either the surface cover or the infiltration capacity (hydrophilic conditions) increased. Sediment losses were controlled by rainfall intensity and surface cover. The role of soil water repellency on runoff generation was not consistent; the overall repellency levels alone were not enough to assess its hydrological impact. Soil water repellency explained runoff generation in the specific-sites model better than in the overall model. Additionally, soil moisture content was a better predictor for soil water repellency than antecedent rainfall. The natural rainfall results confirmed that RSE’s were able to capture the specific sediment losses and its organic matter content as well as the differences between the ploughed and unploughed sites. Repeated RSE’s also captured the seasonal variations in runoff and sediment losses attributed to soil water repellency. These results have implications for post-fire soil erosion modelling and soil conservation practices in the region, or areas with the same land use, climate and soil characteristics. The measured sediment loss, as well as the increasing frequency of ploughing in recently burnt and unburnt eucalypt stands, suggests ploughing is not an effective as a soil conservation measure. Logging activities with less impact are recommended in order to maintain the forest litter protecting the soil surface. Due to its high effectiveness in reducing runoff and soil erosion, hydromulch is recommended for highly sensitive and vulnerable areas.

Background: Intentional consumption of alcohol based hand gels has been reported especially amongst non-UK national, alcohol dependent, homeless individuals in London. Whilst alcohol misuse is known to be associated with impaired cognitive functioning and mental health problems, the effects of additional ingestion of alcohol gel are unknown. Objectives: To explore cognitive and psychological functioning in users who intentionally ingest alcohol gel compared with ethyl-alcohol only misusers and controls. Methods: Male, Central and Eastern European alcohol only misusers, (n=14; mean age 39 years), alcohol gel users (n=14; mean age 43 years) and controls (n=12; mean age 31 years) were recruited from a London Homeless Service during 2013/14. Alcohol misusers, alcohol gel users and controls were compared on the Forwards and Backwards Digit Span Test; Block Design test; Retrospective and Prospective Memory Questionnaire (PRMQ) and the Hospital and Anxiety Depression Scale (HADS). Results: Alcohol gel users performed significantly worse on the Block Design task (p<0.01) and PRMQ (p<0.01) relative to both alcohol only and control groups, and significantly worse on the digit span relative to controls (p=0.01). Both alcohol misusing groups scored comparatively on digit span backwards (p<0.01), with both groups performing significantly worse than controls. The alcohol gel group reported significantly higher levels of anxiety relative to controls (p=0.02). Conclusions: Whilst there could be constitutional differences between alcohol misusers who additionally abuse alcohol gel, the findings suggest that alcohol gel ingestion may have a greater impact on psychological functioning than traditional alcohol misuse.
