Patient adherence is often poor for hypertension and dyslipidaemia. A monitoring of drug adherence might improve these risk factors control, but little is known in ambulatory care. We conducted a randomised controlled study in networks of community-based pharmacists and physicians in the canton of Fribourg to examine whether monitoring drug adherence with an electronic monitor (MEMS) would improve risk factor control among treated, but uncontrolled hypertensive and dyslipidemic patients. The results indicate that MEMS achieve a better blood pressure control and lipid profile, although its implementation requires considerable resources. The study also shows the value of collaboration between physicians and pharmacists in the field of patient adherence to improve ambulatory care of patients with cardiovascular risk factors.

Hypoglycemia is a potentially serious complication of insulin therapy. Some insulin-dependent diabetic patients can benefit from continuous subcutaneous insulin infusion therapy (an "insulin pump"), which in most case improves glycemia control and decreases the occurrence of hypoglycemic episodes. However, such events may occur, particularly during initial treatment phases or pregnancy. Severe hypoglycemia is mainly managed by stopping the insulin pump and insuring an adequate carbohydrate intake. Patients with insulin pumps and their entourage should receive specific instruction in the adjustment of pump flow in the presence of dysglycemia-inducing circumstances (illness, physical exertion), as well as in anticipation of high-risk situations, such as motor-vehicle driving.

(Résumé de l'ouvrage) Faut-il prévoir des cultes de bénédiction pour les couples de même sexe ? Des pasteures et pasteurs homophiles peuvent-ils vivre avec leur partenaire dans la cure ? En lien avec des événements précis, plusieurs Eglises protestantes de Suisse ont dû prendre position sur des questions de ce genre. Les résultats ont été en partie très variés, notamment entre la Suisse alémanique et la Suisse romande. La Commission théologique de la Fédération des Eglises Protestantes de la Suisse (FEPS) a élaboré cet ouvrage qui se présente comme un manuel pour une telle reprise du thème. Il étudie les prises de position qui ont vu le jour durant cette dernière décennie et les interroge de manière critique sous différents angles. Comment lit-on les textes bibliques ? Quelle image de l'être humain est-elle présupposée ? Comment comprend-on la bénédiction et qu'entend-on par mariage ? Comment se joue la relation entre l'Eglise et l'Etat ? Que montre une comparaison oecuménique ? Et quel statut accorde-t-on à la question ? Partant des expériences passées, le livre indique comment le processus pourrait être poursuivi de manière plus ouverte, plus franche et plus adéquate.

Cardiovascular magnetic resonance (CMR) is a rapidly emerging non-invasive imaging technique free of X-Ray and offers higher spatial resolution than alternative forms of cardiac imaging for the assessment of left ventricular (LV) anatomy, function, and viability due to the unique capability of myocardial tissue characterization after gadolinium-chelates contrast administration. This imaging technique has clinical utility over a broad spectrum of heart diseases: ranging from ischaemic to non ischaemic aetiologies. Cardiomyopathies (CMP) are a heterogeneous group of diseases of the myocardium associated with architectural abnormalities and mechanical dysfunction. CMR can help excluding coronary artery disease and can provide positive diagnostic features for several CMP resulted in better diagnosis and management, Leading to improvements in mortality.

In Switzerland, national census data imply that 84.5 percent are affiliated with religious groups and national survey data imply that 15 percent attend a service on a given weekend in 2009. Research in other countries has shown that attendance is often over-reported, but the level of over-report varies across countries. Thus, in the United States, polls show high and stable levels of religious practice, while Church participation surveys indicate rates twice lower. This article assesses the validity of self-reported affiliation and attendance in Switzerland using the 2009 Swiss National Congregations Study (NCS). Results confirm the observation offered by others that the Swiss census undercounts the persuasions absent from the stylized response categories and they show for the first time that Swiss probably over-report their church attendance, though not as much as in the United States.

Specialisation in medicine requires multidisciplinary approaches, and hence coordination in collaborations of the different partners involved. These integrated approaches, sometimes called "disease management", fit particularly well to chronic diseases. Our institution introduced an integrated approach for taking care of the acute somatic hospitalisation of patients suffering from anorexia nervosa. Interfaces with the different partners were defined, specifying tasks, rights, and duties of each person, care givers or patients. This initiative allows now to identify any deviation occurring in the process of care or hole in the care system, so that it can be corrected and recurrence prevented. This model will be extended to other complex and multidisciplinary care processes and other services in our institution.

Le Suivi Intensif dans le Milieu (SIM) est une équipe de psychiatrie mobile qui propose des interventions proactives dans la communauté. Il peut s'agir d'évaluation sur quelques séances à des suivis au long cours. L'offre de soins s'adresse aux patients en refus de soins et aux hauts-utilisateurs. Le SIM s'inspire librement de l'assertive community treatment (ACT) et de l'approche milieu. Les deux modèles ont été adaptés aux besoins sanitaires de la région lausannoise, en Suisse. Cet article décrit l'intervention, illustre les plus-values pour les populations cibles et précise le rôle de l'intervenant social au sein de l'équipe.

Magnetic resonance imaging is a rapidly developing modality in cardiology. It offers an excellent image definition and a large field of view, allowing a more accurate morphological assessment of cardiac malformations. Due to its unique versatility and its ability to provide myocardial tissue characterization, cardiac magnetic resonance (CMR) is now recognized as a central imaging modality for a wide range of congenital heart diseases, including assessment of post-surgical cardiac anatomy, quantification of valvular disease and detection of myocardial ischemia. CMR provides useful diagnostic information without any radiation exposure, and improves the global management of patients with congenital heart disease.

RésuméAu tournant du Ve siècle avant J.-C., au moment de l'établissement de la démocratie à Athènes, apparaissent une série d'images qui mettent en scène des femmes armées de la double hache : Clytemnestre s'en sert comme arme lors du meurtre d'Agamemnon, Ino la brandit contre son fils, et elle fait également partie de la panoplie des femmes thraces à la poursuite d'Orphée. Ces femmes en proie à une folie meurtrière la détournent ainsi de son utilisation première, celle d'instrument employé exclusivement par un homme pour abattre le boeuf lors des sacrifices civiques qui réunissent la communauté des citoyens. Curieusement, la double hache n'apparaît jamais dans ces scènes où la mise à mort de l'animal est éludée.L'exploration de ces images questionne le problème du pouvoir et de la répartition des rôles entre hommes et femmes en Grèce classique.
