The present study aimed to 1) examine the relationship between laboratory-based measures and high-intensity ultraendurance (HIU) performance during an intermittent 24-h relay ultraendurance mountain bike race (similar to20 min cycling, similar to60min recovery), and 2) examine physiological and performance based changes throughout the HIU event. Prior to the HIU event, four highly-trained male cyclists (age = 24.0 +/- 2.1 yr; mass = 75.0 +/- 2.7 kg; (V)over dot O-2peak = 70 +/- 3 ml.kg(-1).min(-1)) performed 1) a progressive exercise test to determine peak Volume of oxygen uptake ((V)over dot O-2peak), peak power output (PPO), and ventilatory threshold (T-vent), 2) time-to-fatigue tests at 100% (TF100) and 150% of PPO (TF150), and 3) a laboratory simulated 40-km time trial (TT40). Blood lactate (Lac(-)), haematocrit and haemoglobin were measured at 6-h intervals throughout the HIU event, while heart rate (HR) was recorded continuously. Intermittent HIU performance, performance HR, recovery HR, and Lac declined (P < 0.05), while plasma volume expanded (P < 0.05) during the HIU event. TF100 was related to the decline in lap time (r = -0.96; P < 0.05), and a trend (P = 0.081) was found between TF150 and average intermittent HIU speed (r = 0.92). However, other measures (V)over dot O-2peak, PPO, T-vent, and TT40) were not related to HIU performance. Measures of high-intensity endurance performance (TF100, TF150) were better predictors of intermittent HIU performance than traditional laboratory-based measures of aerobic capacity.

Objectives: To describe what is known of quality of life for colorectal cancer patients, to review what has been done in the Australian setting and to identify emerging directions for future research to address current gaps in knowledge. Method: A literature search (using Medline, PsychInfo, CINAHL and Sociological Abstracts) was conducted and 41 articles identified for review. Results: Three key areas relating to quality of life in colorectal cancer patients emerged from the literature review: the definition and measurement of quality of life; predictors of quality of life; and the relationship of quality of life to survival. Results of existing studies are inconsistent in relation to quality of life over time and its relationship to survival. Small sample sizes and methodological limitations make interpretation difficult. Conclusions: There is a need for large-scale, longitudinal, population-based studies describing the quality of life experienced by colorectal cancer patients and its determinants. Measurement and simultaneous adjustment for potential confounding factors would productively advance knowledge in this area, as would an analysis of the economic cost of morbidity to the community and an assessment of the cost effectiveness of proposed interventions. Implications: As the Australian population ages, the prevalence of colorectal cancer within the community will increase. This burden of disease presents as a priority area for public health research. An improved understanding of quality of life and its predictors will inform the development and design of supportive interventions for those affected by the disease.

Objective: To examine the association between gain in motor and cognitive functional status with patient satisfaction 3-6 mo after rehabilitation discharge. Design: Patient satisfaction and changes in functional status were examined in 18,375 patients with stroke who received inpatient medical rehabilitation. Information was obtained from 144 hospitals and rehabilitation facilities contributing records to the Uniform Data System for Medical Rehabilitation and the National Follow-up Services. Results: Data analysis revealed significant (P < 0.05) differences in satisfaction responses based on whether information was collected from patient self-report or from a family member proxy, and the two subsets were analyzed separately. Logistic regression revealed the following significant predictors of satisfaction for data collected from stroke patients: cognitive and motor gain, rehospitalization, who the patient was living with at follow-up, age, and follow-up therapy. In the patient-reported data subset, compared with patients who showed improved cognitive or motor functional status, those with no change, respectively, had a 31% and 33% reduced risk of dissatisfaction. In addition, rehospitalized patients had a higher risk of dissatisfaction. For the proxy reported data subset, significant influences on satisfaction were health maintenance, rehospitalization, stroke type, ethnicity, cognitive FIM(TM) gain, length of stay, and follow-up therapy. Conclusions: Ratings of satisfaction with rehabilitation services were affected by change in functional status and whether the information was collected from patient rating or proxy response.

The influence of complex plaque morphology on the extent of demand-induced ischemia in unselected patients is not well defined. We sought to investigate the functional significance of lesion morphology in patients who underwent coronary angiography and dobutamine stress echocardiography (DSE).,Angiography and DSE were performed within a 6-month period (mean 1 +/- 1 month) in 196 patients. Angiographic assessments involved quantification of stenosis severity, assessment of the extent of jeopardized myocardium, and categorization of plaque morphology according to the Ambrose classification. DSE was interpreted by separate investigators with respect to wall motion score index (WMSI) and number of coronary territories involved. A general linear model was constructed to assess,the independent contribution of patient characteristics and angiographic and DSE results with respect to extent of ischemic myocardium. Complex lesion morphology was seen in 62 patients (32%). Patients with complex lesions were more likely to have had prior myocardial infarction (p < 0.001) and be current smokers (p = 0.03). During angiography, they exhibited a trend toward a greater number of diseased vessels, had a greater coronary jeopardy score (p < 0.001) and more frequent collateral flow (p = 0.03). During echocardiography, patients had a higher stress WMSI (p < 0.001) and were more likely to show ischemia in all 3 arterial territories (p < 0.01). On multivariate regression, the coronary artery jeopardy score and the presence of complex plaque morphology were independent predictors of the extent of ischemic myocardium (R 2 = 34%, p < 0.001). Thus, patients with complex plaque morphology are older, more likely to smoke, and more likely to have had prior myocardial. infarction. They exhibit more extensive disease with higher coronary jeopardy scores and a higher resting and peak stress WMSI. Despite these differences, complex plaque morphology remains an independent predictor of the extent of ischemia during stress. (C) 2003 by Excerpta Medica, Inc.

Although obesity is associated with increased risks of morbidity and death in the general population, a number of studies of patients undergoing hemodialysis have demonstrated that increasing body mass index (BMI) is correlated with decreased mortality risk. Whether this association holds true among patients treated with peritoneal dialysis (PD) has been less well studied. The aim of this investigation was to examine the association between BMI and outcomes among new PD patients in a large cohort, with long-term follow-up monitoring. Using data from the Australia and New Zealand Dialysis and Transplant Registry, an analysis of all new adult patients (n = 9679) who underwent an episode of PD treatment in Australia or New Zealand between April 1, 1991, and March 31, 2002, was performed. Background Exercise testing has limited efficacy for identifying coronary artery disease (CAD) in the absence of anginal. symptoms. Exercise echocardiography is more accurate than standard exercise testing, but its efficacy in this situation has not been defined. We sought to identify whether the Duke treadmill. score or exercise echocardiography (ExE) could be used to identify risk in patients without anginal symptoms. Methods We studied 1859 patients without typical or atypical angina, heart failure, or a history or ECG evidence of infarction or CAD, who were referred for ExE, of whom 1832 (age 51 15 years, 944 men) were followed for up to 10 years. The presence and extent of ischaemia and scar were interpreted by expert reviewers at the time of the original study. Results Exercise provoked significant (>0.1 mV) ST segment depression in 215 patients (12%), and wall motion abnormalities in 137 (8%). A new questionnaire, the Maternal Mental State Input Inventory (MMSII) was created to measure mothers' preferences for introducing and elaborating on mental states in conversation with their young children. In two studies, the questionnaire was given to mothers of young children, and the children's theory of mind (ToM) development was assessed with standard tasks. In both studies, the questionnaire exhibited good internal reliability, and significant correlations emerged between mothers' self-reported preferences for elaborated, explanatory talk about the mental states and children's theory of mind performance. Further, mothers' conversational preferences, as measured by the MMSII, were the best predictors of children's theory of mind development when relevant control variables were included in the analyses. These results converge with naturalistic observational research that has demonstrated links between mothers' conversational styles and their children's theory of mind. They go further in suggesting that mothers' tendencies toward elaborated, explanatory talk about a range of mental states is particularly beneficial to children's theory of mind development. A partir da segunda metade do século XX, mudanças no modo de produção capitalista começaram a afetar a relação que as empresas estabeleciam com o trabalhador. Diante de um mercado imprevisível, a carreira tradicional, marcada, entre outros aspectos, por empregos duradouros e com possibilidade de ascensão na hierarquia da organização, tornou-se menos recorrente. Paralelamente, começaram a despontar novas concepções sobre carreira, tendo a maioria um enfoque individualista. Dentre as novas proposições, o presente estudo tomou como referência a concepção de carreira proteana. Esse modelo, de origem norte-americana, tem como ideia central a noção de uma carreira que é gerida pelo indivíduo, e tem como meta o alcance do sucesso psicológico. Desta forma, ancora-se em duas principais dimensões: autogerenciamento e direcionamento para valores. Considerando os diversos estudos que descrevem as dificuldades enfrentadas por estudantes na transição da universidade para o mercado de trabalho, esta pesquisa objetivou compreender aspectos do gerenciamento proteano de carreira entre universitários brasileiros que já tinham concluído, pelo menos, a primeira metade do curso de graduação. Para tanto, o estudo foi dividido em dois artigos. O primeiro foi destinado ao desenvolvimento e validação da Escala de Atitudes de Carreira Proteana para universitários, tendo sido realizado com uma amostra de 1016 estudantes de 37 cursos diferentes, com idade variando entre 18 e 65 anos, e média de 24,52 (DP = 6,69 anos). O instrumento, denominado neste estudo de Escala de Gerenciamento Proteano de Carreira para Universitários (EGPC-U) atestou a estrutura de duas dimensões, evidenciada também na versão original da medida. Os índices de confiabilidade foram satisfatórios e superiores a 12 0,61, tendo o instrumento a possibilidade de ser utilizado em serviços de orientação profissional ou de carreira. O segundo artigo objetivou compreender como as dimensões do modelo proteano se relacionam com variáveis sóciodemográficas e com construtos psicossociais: personalidade, lócus de controle, saúde e satisfação com a vida, e envolveu alunos de duas áreas de conhecimento: humanas e exatas. A amostra foi composta por 525 alunos, sendo 245 da área de humanas e 280 de exatas, sendo 52% do sexo masculino. A idade dos participantes variou entre 18 e 30 anos e média de 22,59 anos (DP = 2,66 anos). A partir dos resultados do estudo 2, constatou-se que alunos da área de humanas, quando comparados a estudantes de exatas, tendem a apresentar média superior na dimensão de direcionamento para valores. Verificou-se ainda que os aspectos de conscienciosidade e lócus de controle interno são preditores significativos do autogerenciamento tanto entre alunos de humanas como de exatas e que a adoção de atitudes proteanas tende a impactar positivamente aspectos da saúde e da satisfação com a vida do indivíduo. O estudo, de um modo geral, permitiu verificar a existência de características proteanas entre universitários, como também possibilitou conhecer variáveis relacionadas às atitudes de autogerenciamento e direcionamento para valores. Destaca-se, porém, a necessidade de pesquisas complementares com a exploração de outras variáveis psicossociais que possam estar relacionadas ao gerenciamento proteano entre graduandos.

INTRODUÇÃO: O diagnóstico e terapia antirretroviral precoce em lactentes, infectados pelo HIV por transmissão vertical, reduz a progressão do HIV e comorbidades que podem levar ao óbito. OBJETIVO GERAL: Avaliar o perfil clínico e epidemiológico em uma coorte de crianças e adolescentes com aids, infectados por transmissão vertical do HIV, por um período de onze anos, atendidos em hospital estadual de referência, no Estado do Espírito Santo. OBJETIVOS ESPECÍFICOS: 1. Descrever a frequência das comorbidades diagnosticadas após o diagnóstico de HIV e verificar sua distribuição, segundo dados demográficos, epidemiológicos e clínicos, e segundo a classificação dos casos em uma coorte de crianças e adolescentes com aids. 2. Avaliar os fatores preditores de risco de progressão para aids e óbito e causas de morte. 3. Estimar a taxa de sobrevida. MÉTODOS: Coorte retrospectiva de crianças e adolescentes infectados pelo HIV, por transmissão vertical (TV), atendidas no Serviço de Atendimento Especializado (SAE) do Hospital Infantil Nossa Senhora da Glória (HINSG), de janeiro 2001 a dezembro 2011, em Vitória – ES/Brasil. A coleta de dados foi realizada em protocolo específico padronizado, e dados sobre as comorbidades, mortalidade e sua causa básica foram obtidos dos prontuários médicos, da Declaração de Óbito e do banco de dados SIM (Sistema de Informação sobre Mortalidade). O diagnóstico de aids e comorbidades foi de acordo com CDC (Centers for Disease Control and Prevention)/1994. RESULTADOS: Foi arrolado um total de 177 pacientes, sendo 97 (55%) do sexo feminino; 60 (34%) eram menores de1ano, 67 (38%) tinham de 1 a 5 anos e 50 (28%) tinham6 anos ou mais de idade no ingresso ao serviço. A mediana das idades na admissão foi de 30 meses (Intervalo Interquartis (IIQ) 25-75%: 5-72 meses). Em relação à classificação clínico-imunológica, 146 pacientes (82,5%) apresentavam a forma moderada/grave no momento do ingresso no Serviço e 26 (14,7%) foram a óbito. Os sinais clínicos mais frequentes foram hepatomegalia (81,62%), esplenomegalia (63,8%), linfadenopatia (68,4%) e febre persistente (32,8%). As comorbidades mais frequentes foram anemia (67,2%), pneumonia/sepses/meningite - primeiro episódio (64,2%), OMA/sinusite recorrente (55,4%), infecções bacterianas graves recorrentes (47,4%) e dermatites (43,1%). Encontrou-se associação entre classificação clínico-imunológica grave e ingresso no serviço com menos de um ano de idade com algumas comorbidades (p<0,001). O tempo total do acompanhamento dos pacientes foi de 11 anos, com mediana de cinco anos (IIQ: 2-8 anos). No final do período estudado, 132 (74,6%) pacientes estavam em acompanhamento, 11 (6,2%) foram transferidos para outros serviços eem oito (4,5%) houve perda de seguimento. Quanto ao óbito, observou-se uma redução de casos ao longo do tempo. A maioria dos pacientes que foram a óbito deu entrada no serviço com classificação clínica imunológica grave (77%-20/26), apresentava anemia moderada/grave e estava em uso de terapia antirretroviral (TARV) por mais de 3 meses (17/24-71%).Os principais fatores de risco para o óbito foram: faixa etária < 1 ano (p=0,005), pneumonia por P. jirovecii (p=0,010), percentual de linfócito T CD4+ nadir <15% (p=0,012), anemia crônica (p=0,012), estágio clínico imunológico grave (p=0,003), infecções bacterianas graves recorrentes(p=0,003) e tuberculose (p=0,037). Ter iniciado TARV antes dos 6 meses de vida (diagnóstico e tratamento precoces) foi associado à sobrevida(OR 2,86, [Intervalo de Confiança (IC) de 95%: 1,12-7,25] p=0,027).O principal diagnóstico registrado para os óbitos foram infecções bacterianas graves (12/21-57%). Foi encontrada uma elevada taxa de sobrevida, com 85,3% de probabilidade de sobrevivência por mais de 10 anos (IC 95% 9,6-10,7). CONCLUSÕES: A maioria das crianças teve diagnóstico tardio da infecção pelo HIV aumentando o risco de progressão para aids e óbito por falta de tratamento precoce. A tendência de mortalidade das crianças infectadas pelo HIV se mostrou uma constante com queda nos dois últimos anos do estudo, e ainda persistem as infecções bacterianas como maior causa de óbito. Portanto, melhoria no cuidado pré-natal e acompanhamento pediátrico com vista ao diagnóstico precoce das crianças infectadas verticalmente devem fazer parte do cuidado integral à criança com aids, o que poderia reduzir a mortalidade destas crianças.

Purpose of the research: (a) To identify the degree of much loneliness reported in the Portuguese population over 50 years of age and (b) test whether loneliness can be predicted by socio-demographic, health related or social characteristic of the sample other than age. Materials and methods: 1174 late middle age and older adults were interviewed face to face by different interviewers across the country; after the informed consent was signed, we asked the participants several socio-demographic and health-related questions; finally we asked ‘‘How often do you feel lonely?’’ and participants responded according to a five point Likert scale. Principal results: The results showed that 12% of participants reporting feeling lonely often or always, whereas 40% reporting never feeling lonely. The remaining 48% self-reported they felt lonely seldom or sometimes. Additionally, results show that, when taken together, variables such as marital status, type of housing, residence settings, health conditions, social satisfaction, social isolation, lack of interest, transportation, and age were predictors of loneliness. Major conclusions: (1) The association of loneliness with advanced age has been greatly exaggerated by mass media and common sense; (2) But although our findings did not confirm the most alarmist views, the 12% of older adults reporting that they are feeling lonely always or often should be cause for attention and concern. It is necessary to understand the meaning, reasons and level of suffering implied on those feelings of loneliness. (3) Our findings suggest that it makes no sense to construe age as a singular feature or cause for feelings of loneliness. Instead, age and also a number of other features combine to predict feelings of loneliness. But even with our predictor variables there was a substantial of variance left unexplained. Therefore it is necessary to continue exploring how feelings of loneliness arise from the experience of living and how they can be changed.

Improving the treatment of obesity remains a critical challenge. Several health behaviour change models, often based on a social-cognitive framework, have been used to design weight management interventions (Baranowski et al., 2003). However, most interventions have only produced modest weight reductions (Wadden et al., 2002) and socialcognitive variables have shown limited power to predict weight outcomes (Palmeira et al., 2007). Other predictors, and possibl alte nati e e planatory models, are needed to better understand the mechanisms by which weight loss and other obesity treatment-outcomes are brought about (Baranowski, 2006). Self-esteem is one of these possible mechanisms, because is commonly reported to change during the treatment, although these changes are not necessarily associated with weight loss (Blaine et al., 2007; Maciejewski et al., 2005). This possibility should be more evident if the program integrates regular exercise, as it promotes improvements in subjective well-being (Biddle & Mutrie, 2001), with possible influences on long-term behavioral adherence (e.g. diet, exercise). Following the reciprocal effects model tenets (Marsh & Craven, 2006), we expect that the influences between changes in weight, selfesteem and exercise to be reciprocal and might present one of the mechanisms by which obesity treatments can be improved.

Seventy four asthmatic children aged 7 to 11 years were examined along with controls matched by age and sex. Clinical and laboratory investigations preceded a 28-day follow-up where data about morning and evening peak expiratory flow rate (PEF), symptoms and treatment were recorded. The coefficient of variation of PEF was found to be an objective measurement of asthma severity that has statistically significant correlation with both symptoms (r s= .36) and treatment (r s= .60). Moreover, it separates mild and severe asthmatics, as confirmed by statistically significant differences (p= .008 or less) in symptoms, treatment, skin allergy and airways response to exercise. Skin allergy and airways responsiveness to exercise were found to be predictors of both disease and severity. By means of logistic regression analysis it was possible to establish the probabilities for both asthma and severe asthma when children presenting and not presenting these characteristics are compared. One single positive skin test represent a probability of 88% for the development of asthma and a probability of 70% for severe disease. A PEF reduction of 10% after an exercise test implies a probability of 73% for disease and a probability of 64% for severe disease. Increases in these variables imply geometrically increased risks and their presence together have a multiplicative effect in the final risk.

Although leadership investigation has become for the last years an election topic with major relevance on organizational studies and accepting peacefully the general idea that organizations are freeland for politics, all these acceptances run against a kind of “fear” from the academy scholars on approaching the political leaderships’ singularities on organizations. Indeed, when we cross over both phenomena we verify that the absence and weaknesses towards the unique characteristics of political leadership on work scenarios are becoming sharped regarding to their predictors, their workers and their organizations, even if we left aside its moderator variables.

OBJECTIVE: In order to determine the relationship between some maternal anthropometric indicators and birth weight, crown-heel length and newborn's head circumference, 92 pregnant women were followed through at the prenatal service of hospital in S. Paulo, Brazil. MATERIAL AND METHOD: The following variables were established for the mother: weight, height, mid-upper arm circumference, pre-pregnancy weight, gestational weight gain and Quetelet's index. For the newborn the following variables were recorded: birth weight, crown-heel length, head circumference and gestational age by Dubowitz's method. RESULTS: Significant associations were noted between gestational age and newborn variables. In addition, maternal mid-arm circumference (MUAC) and pre-pregnancy weight were found to be positively correlated to birth weight (r=0.399; r=0.378, respectively). The multivariate linear regression shows that gestational age, mother's arm circumference and pre-pregnancy weight continue to be significant predictors of birth weight. On the other hand, only gestational age and mother's age was associated with crown-heel length. Similarly MUAC was significantly associated with crown-heel length (r= 0.306; P=0.0030). CONCLUSION: Maternal mid-upper arm circumference is a potential indicator of maternal nutritional status. It could be used in association with other anthropometric measurements, instead of pre-pregnancy weight, as an alternative indicator to assess women at risk of poor pregnancy outcome.

OBJECTIVE: A cohort study has been designed to identify predictors of adverse health events in the elderly. The methodology of the study and preliminary descriptive results are presented. METHODS: The study population comprises all residents of Bambuí (Minas Gerais, Brazil), aged 60 or more years (n=1.742). From these, 92.2% were interviewed and 85.9% underwent clinical examination, consisting of haematological and biochemical tests, serology for Trypanosoma cruzi, anthropometric and blood pressure measures and electrocardiogram. Aliquots of serum, plasma and DNA were stored for future investigations. The baseline interview included sociodemographic characteristics, self-referred health condition and history of selected diseases, medication use, health service use, source of medical care, physical activities, smoking, drinking and eating habits, reproductive history, physical functioning, life events, social support and mental health. Individuals are being followed up annually. RESULTS: The following characteristics predominated among participants: women (60,0%), married (48.9%) or widowed (35.4%), people living in households with up to 2 residents (73.8%), heads of family (76.7%), people with monthly income between 1.00 and 2.99 Brazilian minimum wages (62.0%) and people with up to 4 years of schooling (89.1%). The median age was 68 years. Among the cohort members, only 1.7% were lost in the first follow-up. CONCLUSIONS: In general, the characteristics of the study population were very similar to those from other epidemiological studies of the elderly based on large Brazilian cities. The small number of losses to follow-up indicates that the choice of Bambuí was adequate, assuring the feasibility of a long term cohort study.
