OBJECTIVE: To study whether antioxidant supplementation will reduce the incidence of preeclampsia among patients at increased risk.METHODS: A randomized, placebo-controlled, double-blind clinical trial was conducted at four Brazilian sites. Women between 12 0/7 weeks and 19 6/7 weeks of gestation and diagnosed to have chronic hypertension or a prior history of preeclampsia were randomly assigned to daily treatment with both vitamin C (1,000 mg) and vitamin E (400 International Units) or placebo. Analyses were adjusted for clinical site and risk group (prior preeclampsia, chronic hypertension, or both). A sample size of 734 would provide 80% power to detect a 40% reduction in the risk of preeclampsia, assuming a placebo group rate of 21% and alpha=.05. The a level for the final analysis, adjusted for interim looks, was 0.0458.RESULTS: Outcome data for 707 of 739 randomly assigned patients revealed no significant reduction in the rate of preeclampsia (study drug, 13.8% [49 of 355] compared with placebo, 15.6% [55 of 352], adjusted risk ratio 0.87 [95.42% confidence interval 0.61-1.25]). There were no differences in mean gestational age at delivery or rates of perinatal mortality, abruptio placentae, pre-term delivery, and small for gestational age or low birth weight infants. Among patients without chronic hypertension, there was a slightly higher rate of severe preeclampsia in the study group (study drug, 6.5% [11 of 170] compared with placebo, 2.4% [4 of 168], exact P=.11, odds ratio 2.78, 95% confidence interval 0.79-12.62).CONCLUSION: This trial failed to demonstrate a benefit of antioxidant supplementation in reducing the rate of preeclampsia among'patients with chronic hypertension and/or prior preeclampsia.

The orofacial characteristics of ectodermal dysplasia include anodontia or hypodontia, hypoplastic conical teeth, underdevelopment of the alveolar ridges, frontal bossing, a depressed nasal bridge, protuberant lips, and hypotrichosis. Patients with this disease often need complex prosthetic treatment. The options for a definitive treatment plan may include fixed, removable, or implant-supported prostheses, singly or in combination. However, financial constraints and other priorities can prevent patients from choosing the most desirable treatment. This clinical report describes the diagnosis and treatment of ectodermal dysplasia in an 18-year-old man. The treatment included interim removable partial dentures fabricated to establish an acceptable therapeutic occlusal vertical dimension, followed by definitive overlay removable partial dentures and composite restorations.

Objective: The aim of this study was to assess the bond strength of adhesive systems to dentin contaminated by temporary cements with or without eugenol. Method: Flat dentin surfaces were obtained from twenty-four human third molars. With exception of the control group (n=8), the surfaces were covered with Interim Restorative Material (Caulk Dentsplay, Milford, DE, USA) or Cavit (3M ESPE, St. Paul, MN, USA) and kept in an oven at 37°C for seven days. After removing the cements, the adhesive systems Adper Single Bond (3M ESPE, St. Paul, MN, USA) or Clearfil SE Bond (Kuraray Co. Ltd., Osaka, Japan) were applied in accordance with the manufacturers' recommendations, and then the crowns were constructed in of resin composite. The teeth were sectioned into specimens with a cross-sectional bond area of 0.81mm2, which were submitted to microtensile testing in a mechanical test machine at an actuator speed of 0.5mm/min. The data were analyzed by t- and ANOVA tests, complemented by Tukey tests (α=0.05). Results: For Adper Single Bond (3M ESPE, St. Paul, MN, USA), bond strength did not differ statistically (p>0.05) for all the experimental conditions. For Clearfil SE Bond (Kuraray Co. Ltd., Osaka, Japan), only the Interim Restorative Material (Caulk Dentsplay, Milford, DE, USA) Group showed significantly lower bond strength (30.1 ± 13.8 MPa) in comparison with the other groups; control (38.9 ± 13.5 MPa) and Cavit (3M ESPE, St. Paul, MN, USA) (42.1 ± 11.0 MPa), which showed no significant difference between them. Conclusion: It was concluded that the previous covering of dentin with temporary cement containing eugenol had a deleterious effect on the adhesive performance of the self-etching system only.

Purpose: The purpose of this study was to assess restorative treatment outcomes in the mixed dentition of amelogenesis imperfecta (AI) patients and determine the postrehabilitation oral health status and satisfaction of the patients. Methods: Clinical and radiographic examinations were performed on eight AI patients, who had 74 restorations placed in permanent incisors and molars, to allow evaluation of the integrity of the restorations and periodontal status post-treatment. Subjects completed a survey regarding esthetics, function, and sensitivity. Results: Among the 74 restorations evaluated, seven were lost; of the remaining restorations, 31 were posterior, and 36 were anterior. Ten were rated clinically unacceptable. Teeth with stainless steel crowns had a moderate gingival index (mean=2.3) and plaque index (mean=2.0) scores. Widening of the periodontal ligament and pulp canal obliteration were common radiographic findings. Subject's recall of satisfaction regarding esthetics (P=.002) and sensitivity (brushing-P=.03; eating-P=.01) showed a statically significant difference before and after treatment. Conclusions: During mixed dentition, teeth with amelogenesis imperfecta may be restored with conventional treatment modalities. Direct restorations should be considered interim with multiple repairs anticipated. Post-treatment, gingival inflammation and plaque accumulation were observed. Subjects were satisfied with their appearance and reported a decrease of hypersensitivity. © 2013 Publishing Technology.

Introduction The Netherlands Antilles is an autonomous entity within the Kingdom of the Netherlands and comprises a federation of five Caribbean islands: Bonaire and Curacao (the Leeward islands) which comprise 80 per cent of the population of 211,000 and Saba, St. Eustatius and the southern part of St. Maarten (the Windward islands). Like the other countries in the Kingdom, it enjoys full autonomy in internal matters as, for example, education, public health, justice and customs. It has a per capita income of about US$ 12,000. The Leeward Islands and the Windward Islands account for about 75 per cent (Curacao (70 per cent) and Bonaire (5 per cent)) and 25 percent respectively of the economy of the Netherlands Antilles. The Netherlands Antilles has its own currency, the Netherlands guilder, which is pegged to the United States dollar at a fixed rate since 1971. The economy has some unique features that stem from its close relations with the Netherlands, its undiversified nature and heavy dependence on tourism, offshore finance, oil refining and shipping, the high share of trade (exports of goods and services of about 75 per cent of GDP), its geographical characteristics, its common border with the French Republic on St. Maarten, its duty-free access for imports from Aruba, its de facto free trade zone (FTZ), partial dollarization, especially for the Windward Islands, and its highly regulated labor market (1). Adverse economic shocks in the last two decades affected particularly the offshore financial sector and the oil refinery and, to a lesser extent, tourism. The repeal of withholding taxes in the United States in the 1980s indirectly caused the collapse of a number of highly profitable offshore financial activities in Curacao, leading to significant drops in government revenue and contributions to foreign exchange earnings. The withdrawal of Shell from Curacao in 1986 and the (temporary) closure of the oil refinery which had been a mainstay of the Curacao economy for almost three quarters of a century was the second major shock. It was subsequently leased to the Venezuelan State Company, Petroleos de Venezuela Sociedad Anonima (PDVSA), which resumed operations and preserved employment. In the 1990s, the Windward Islands were bit by several devastating hurricanes, which destroyed much of the economic infrastructure on the islands, including about half of the number of available hotel rooms in St Maarten. Further negative shocks were related to the discontinuation of certain trade privileges on European markets for Overseas Countries and Territories (OCTs), the withdrawal by the Netherlands of certain tax privileges for Dutch pensioners residing in the Netherlands Antilles and disruptions in the availability of Solidarity Fund resources for the smaller islands. National income has been on the decline since 1997. GDP declined by about 6 per cent between 1997 and 1999. Underlying fiscal imbalances and structural weaknesses have also impacted negatively on the economy. In recent years, with recession high unemployment and migration have been experienced (2). The Netherlands Antilles has been able to survive thanks to additional aid from the Netherlands, large-scale spontaneous emigration (mostly to the Netherlands), some drop in international reserves, an increase in domestic debt and arrears and reduced outlays for the maintenance of public assets. From 1986 onwards, successive efforts at restoring macroeconomic balance, particularly with regard to public finance, were made, but were unsuccessful. Adjustment was also attempted in 1996 and 1997, but failed to meet the desired targets. In 1999, the government launched a new National Recovery Plan" (NRP). The NRP contains important medium-term structural adjustment measures aimed at restoring macroeconomic balance and conditions for revitalizing the economy. The NRP subsequently served as an important input into a comprehensive adjustment plan drawn up with the assistance of the International Monetary Fund (IMF) and reflected in the government's Memorandum of Economic Policies dated 15 September 2000. Beyond restoring macroeconomic balance and reforming the economic incentive framework, the government aims at establishing a Comprehensive Development Framework (CDF) for the formulation and implementation of a sustainable long-term growth strategy. It is against the above background that this study is undertaken. Its main objective is to assess the integration options facing the Netherlands Antilles (3) vis-a-vis the Caribbean Community (CARICOM). A secondary objective is to examine the above taking into account, inter alia, the level of trade between the Netherlands Antilles and CARICOM, the barriers to trade between the two groups of countries and the requirements for increasing trade between the two groups of countries. The Consultant was given an initial Draft Terms of Reference (Annex 1) with the intention of modifying it in the course of the interviews with all the stakeholders. The main idea that emerged from these interviews was a concern with some possible form of association with CARICOM. The Consultant was asked to exam the costs and benefits of various forms of association and to recommend an option. This adjustment of the Terms of Reference (TOR) was substantial and involved the Consultant having to do some interviews and collect documentation in CARICOM. The study essentially revolves around the search for a road map for the Netherlands Antilles. It is tackled in the first instance by describing the existing system of trade of the Netherlands Antilles with a view to determining the import and export structures and the specific nature and extent of trade in goods and services between the Netherlands Antilles and CARICOM. 1 Netherlands Antilles: Elements of a Strategy for Economic Recovery and Sustainable Growth. The study undertakes a review of the Poverty Reduction Strategy Papers (PRSPs), and Interim Poverty Reduction Strategy Papers (I-PRSPs) of four selected Caribbean SIDS from the Eastern Caribbean. The countries had participated in the Poverty Reduction Strategy initiative formulated by the World Bank. The country papers reviewed were the Commonwealth of Dominica, Grenada, Saint Lucia and St. Vincent and the Grenadines. The review assesses and provides a comparative analysis of the general awareness of the notions of vulnerability and the inclusion and use of strategies that address the reduction of vulnerability and the strengthening of resilience by the countries reviewed. It also explores how vulnerability reduction strategies were applied to differing groups, as elaborated through the PRSPs.

O presente estudo teve como objeto central analisar as políticas públicas exaradas pelo Estado brasileiro, via espécies normativas para a educação superior. Tendo como categorias de análise a autonomia da universidade, e o financiamento da educação superior. A pesquisa parte da premissa de que as políticas públicas têm sido materializadas a partir de normas jurídicas. Assim, investigamos o Estado brasileiro, as políticas públicas por ele emanadas por meio de espécies normativas e suas implicações na educação superior. O estudo foi realizado por meio de pesquisa bibliográfica que consistiu num levantamento minucioso do ordenamento jurídico elaborado pelo Estado brasileiro para a educação superior a partir da Constituição Federal de 1988 até o ano de 2006. Detectamos que após a CF/88, no que concerne à autonomia da universidade e ao financiamento da educação superior, foram aprovadas quarenta e quatro normas jurídicas, sendo duas Emendas Constitucionais, onze Leis, três medidas provisórias, nove Decretos, dentre outras espécies normativas, que contribuíram para alterar de forma profunda o texto constitucional. No desenvolvimento do estudo iniciamos com a discussão sobre o Estado com vistas a estabelecer relações com o direito e as políticas públicas. Em seguida, traçamos uma contextualização histórica de todas as Constituições brasileiras, mas com ênfase no processo de redemocratização iniciado em 1974 e que culminou na CF/88, cuja característica essencial foi sua definição como a Carta Magna mais democrática que se tem nota no Brasil. Dessa carta, analisamos os artigos que inferem sobre a autonomia universitária e o financiamento da educação superior. Com base na discussão sobre o Estado regulador brasileiro e nas indicações fornecidas pela ciência do Direito constitucional, realizamos a análise da legislação pós-constitucional que define as políticas para a educação superior brasileira direcionada ao financiamento desse nível de ensino e à autonomia da universidade. Tendo como referência o texto constitucional, discorremos sobre o instituto jurídico do controle de constitucionalidade. O estudo apontou que grande parte das normas jurídicas infraconstitucionais, que regulamenta a educação superior brasileira, atua contra legem mater, quando, por exemplo, dispõe contra o art. 207 ao interferir sobre a escolha de dirigentes universitários, ferindo a capacidade da universidade se auto-legislar sobre assuntos que lhe são próprios, ou quando desvincula percentuais assegurados para o financiamento da educação superior, no caso da emenda constitucional de revisão n. 1/1994 e Emenda Constitucional n. 10/1996, ferindo princípios constitucionais e reproduzindo interesses do Estado capitalista neoliberal. O estudo apresenta contribuição para o campo das políticas públicas educacionais, vez que possibilita reflexões sobre a forma pela qual o direito público subjetivo à educação, assegurado no texto constitucional, vem sendo negado sistematicamente e de forma sucessiva pelos governos pós CF/88 que adotam o modelo de Estado neoliberal.

Vilas, “logares” e cidades: a história da educação rural do Pará na Primeira República - 1889 a 1897

Trata-se de um estudo sobre a história da educação rural no Pará, entre 1889-1897. A intenção última é conhecer, a partir da pesquisa histórica, como se deu a educação rural do Pará na Primeira República, tendo como foco os governos provisórios de Justo Chermont e Huert Bacelar e o governo constitucional de Lauro Sodré. Nosso objetivo geral é entender como a educação para homens e mulheres do interior do Pará foi pensada e operacionalizada no plano governamental. Especificamente, estabelecemos quatro objetivos: a) Levantar os investimentos feitos nas escolas rurais no período em questão; b) Descrever como eram concebidas as escolas rurais do estado do Pará na Primeira República; c) Entender quais os procedimentos utilizados, através dos documentos oficiais, para formar o cidadão das escolas rurais; d) Compreender os objetivos da formação educacional da população rural. Duas questões nortearam a investigação: 1º) Que intenções permeavam a educação destinada às populações das áreas rurais do Pará nos governos do período de 1889 a 1897? 2º) O que foi realizado, no plano governamental, para a concretização desses objetivos? As fontes documentais foram coletadas no Arquivo Público do Estado do Pará, na biblioteca Arthur Vianna e nos Setor de Obras Raras do Centro Cultural do Pará Tancredo Neves (CENTUR). O corpus da pesquisa está composto de: relatórios dos governadores; mensagens, ofícios, abaixo-assinados, circulares e requerimentos; relatórios de diretores da Instrução Pública; relatórios de visitadores; relatórios de grupo escolares; cadernos de Leis do período. Metodologicamente, operamos a análise em três momentos: 1) momento da heurística; 2) momento da crítica; 3) momento da interpretação. O texto está composto de introdução, três seções e considerações finais. O trabalho realizado indica que os governos estaduais na Primeira República pouco fizeram pela formação do homem do interior do estado, apesar do uso político acentuado da educação como prioridade para estes. Identificamos que os investimentos com a Instrução Pública tiveram seu limite estabelecido pela ideologia do regime republicano que entendia a população rural como uma gente de segunda ou de terceira, que precisava ser “lapidada” para o trabalho. Por extensão, o espaço rural é compreendido como o lugar do atraso, da incivilidade e que, portanto, um pouco de formação à sua população era o suficiente para transformar homens rudes em cidadãos, ainda que de categoria inferior.
