It is acknowledged that one of the consequences of the ageing process is cognitive decline, which leads to an increase in the incidence of illnesses such as dementia. This has become ever more relevant due to the projected increase in the ageing demographic. Dementia affects visuo-spatial perception, causing difficulty with wayfinding, even during the early stages of the disease. The literature widely recognises the physical environment’s role in alleviating symptoms of dementia and improving quality of life for residents. It also identifies the lack of available housing options for older people with dementia and consequently the current stock is ill-equipped to provide adequate support.
Recent statistics indicate that 80% of those residing in nursing or residential care homes have some form of dementia or severe memory problems. The shift towards institutional care settings, the need for specialist support and care, places a greater impetus on the need for a person-centred approach to tackle issues related to wayfinding and dementia.
This thesis therefore aims to improve design for dementia in nursing and residential care settings in the context of Northern Ireland. This will be undertaken in order to provide a better understanding of how people with dementia experience the physical environment and to highlight features of the design that assist with wayfinding. Currently there are limited guidelines on design for dementia, meaning that many of these are theoretical, anecdotal and not definitive. Hence a greater verification to address the less recognised design issues is required. This is intended to ultimately improve quality of life, wellbeing, independence and uphold the dignity of people with dementia living in nursing or residential care homes.
The research design uses a mixed methods approach. A thorough preparation and consideration of ethical issues informed the methodology. The various facets were also trialled and piloted to identify any ethical, technological, methodological, data collection and analysis issues. The protocol was then amended to improve or resolve any of the aforementioned issues. Initially a questionnaire based on leading design recommendations was conducted with home managers. Semi-structured interviews were developed from this and conducted with staff and resident’s next of kin. An evidence-based approach was used to design a study which used ethnographic methods, including a wayfinding task. This followed a repeated measures design which would be used to actively engage residents with dementia in the research. Complementary to the wayfinding task, conversational and semi-structured interviews were used to promote dialogue and direct responses with the person with dementia. In addition to this, Space Syntax methodologies were used to examine the physical properties of the architectural layout. This was then cross-examined with interview responses and data from the wayfinding tasks.
A number of plan typologies were identified and were determined as synonymous with decision point types which needed to be made during the walks. The empirical work enabled the synthesis of environmental features which support wayfinding.
Results indicate that particular environmental features are associated with improved performance on the wayfinding tasks. By enhancing design for dementia, through identifying the attributes, challenges with wayfinding may be overcome and the benefits of the physical environment can be seen to promote wellbeing.
The implications of this work mean that the environmental features which have been highlighted from the project can be used to inform guidelines, thus adding to existing knowledge. Future work would involve the dissemination of this information and the potential for it to be made into design standards or regulations which champion design for dementia. These would increase awareness for designers and stakeholders undertaking new projects, extensions or refurbishments.
A person-centred, evidence-based design was emphasised throughout the project which guaranteed an in-depth study. There were limitations due to the available resources, time and funding. Future research would involve testing the identified environmental features within a specific environment to enable measured observation of improvements.

Background: Intermediate care (IC) describes a range of services targeted at older people, aimed at preventing unnecessary hospitalisation, promoting faster recovery from illness and maximising independence. Older people are at increased risk of medication-related adverse events, but little is known about the provision of medicines management services in IC facilities. This study aimed to describe the current provision of medicines management services in IC facilities in Northern Ireland (NI) and to explore healthcare workers' (HCWs) and patients' views of, and attitudes towards these services and the IC concept. 

Methods: Semi-structured interviews were conducted, recorded, transcribed verbatim and analysed using a constant comparative approach with HCWs and patients from IC facilities in NI. 

Results: Interviews were conducted with 25 HCWs and 18 patients from 12 IC facilities in NI. Three themes were identified: 'concept and reality', 'setting and supply' and 'responsibility and review'. A mismatch between the concept of IC and the reality was evident. The IC facility setting dictated prescribing responsibilities and the supply of medicines, presenting challenges for HCWs. A lack of a standardised approach to responsibility for the provision of medicines management services including clinical review was identified. Whilst pharmacists were not considered part of the multidisciplinary team, most HCWs recognised a need for their input. Medicines management was not a concern for the majority of IC patients. 

Conclusions: Medicines management services are not integral to IC and medicine-related challenges are frequently encountered. Integration of pharmacists into the multidisciplinary team could potentially improve medicines management in IC.

Background: Randomised controlled trials (RCTs) are widely viewed as the gold standard for assessing effectiveness in health research; however many researchers and practitioners believe that RCTs are inappropriate and un-doable in social care settings, particularly in relation to looked after children. The aim of this article is to describe the challenges faced in conducting a pilot study and phase II RCT of a peer mentoring intervention to reduce teenage pregnancy in looked after children in a social care setting.

Methods: Interviews were undertaken with social care professionals and looked after children, and a survey conducted with looked after children, to establish the feasibility and acceptability of the intervention and research design.

Results: Barriers to recruitment and in managing the intervention were identified, including social workers acting as informal gatekeepers; social workers concerns and misconceptions about the recruitment criteria and the need for and purpose of randomisation; resource limitations, which made it difficult to prioritise research over other demands on their time and difficulties in engaging and retaining looked after children in the study.

Conclusions: The relative absence of a research infrastructure and culture in social care and the lack of research support funding available for social care agencies, compared to health organisations, has implications for increasing evidence-based practice in social care settings, particularly in this very vulnerable group of young people.

An interview study of 55 lay carers of people who died from cancer in the Southern Board of Northern Ireland was undertaken using a combination of closed-format and open-ended questions. The aim of the study was to evaluate palliative care services delivered in the last six months of their lives to cancer patients who died either at home or in hospital. Two-thirds of the deaths (36) occurred in the domestic home, 45 of the deceased were admitted as hospital inpatients, and the great majority were in receipt of community nursing (53) and general practitioner (54) services. Open-ended questions were used to allow respondents to give their views about services in some detail and their views about good and bad aspects of services were sought. While they were generally satisfied with services specific areas of difficulty were identified in each aspect of care addressed by the study. The most favourable assessments were made of community nursing with the greatest number of negative comments being made about inpatient hospital care. Differing interests between some of those who were dying and their lay carers were found in two areas: the receipt of help from nonfamily members and the information that the deceased received about their terminal status.

The home visit is at the heart of social work practice with children and families; it is what children and families' social workers do more than any other single activity (except for recording), and it is through the home visit that assessments are made on a daily basis about risk, protection and welfare of children. And yet it is, more than any other activity, an example of what Pithouse has called an ‘invisible trade’: it happens behind closed doors, in the most secret and intimate spaces of family life. Drawing on conceptual tools associated with the work of Foucault, this article sets out to provide a critical, chronological review of research, policy and practice on home visiting. We aim to explain how and in what ways changing discourses have shaped the emergence, legitimacy, research and practice of the social work home visit to children and families at significant time periods and in a UK context. We end by highlighting the importance for the social work profession of engagement and critical reflection on the identified themes as part of their daily practice.

Segundo Palliative Care in European, a dor oncológica, tem uma importância especial porque o cancro é a segunda causa de morte em Portugal e por existir dor moderada a intensa em mais de 90% dos doentes em situação oncológica terminal. O desenvolvimento de um programa estruturado de intervenção de enfermagem que vá de encontro às necessidades do doente oncológico com doença avançada e/ou cuidador, relativamente à gestão da dor, poderá ter um impacto muito significativo no controlo da dor e de outros sintomas. OBJETIVOS Avaliar o efeito de um programa de Intervenção de Enfermagem na capacidade de gestão da dor por parte do doente oncológico com doença avançada e/ou o cuidador em domicílio. MATERIAIS E MÉTODOS A implementação do programa tem a duração prevista para 6 semanas. Compreende duas sessões educativas, três telefonemas e uma sessão de avaliação final. O programa foi elaborado, após uma Revisão sistemática de Literatura, com base no programa “ THE PRO-SELF”, nas Orientações Genéricas preconizadas pelo Plano Nacional de Luta Contra a dor e na Intervenção de enfermagem “Controle da Dor” da Nursing Intervention Classification (NIC). RESULTADOS O projeto encontra-se na fase de implementação e avaliação do programa educativo. No início os doentes não apresentam informação relativamente à gestão da dor, no final demonstram capacidade para a monitorização da dor e de outros sintomas e para a gestão da terapêutica, realizando os registos num diário de bordo. A literacia, a situação clínica e a disponibilidade interferem na capacidade de gestão. A proximidade da morte e a alteração do estado de consciência interferem na autonomia e na tomada de decisão. According to Palliative Care in European, oncological pain, has a special importance because the cancer is the second most common cause of death in Portugal and there is moderate to severe pain in more than 90 % of patients in oncological terminal situation. The development of a structured program of nursing intervention that will meet the needs of the oncological patient with advanced disease and/or caregiver at home on pain management, will be able to provide for the management of pain, increasing knowledge for the control of pain while minimizing the associated symptoms that influence the quality of life of the patient and the caregiver's anxiety. PURPOSE/GOAL/AIM To evaluate the effect of an educational program of Nursing Intervention on pain management by the oncological patient with advanced disease and/or caregiver. RESEARCH/PROBLEM What is the effect of a structured program of nursing intervention directed to the patient and/or oncological patient caregiver with advanced disease at home, in the management of pain? METHODOLOGY Almost experimental study, with assessment of the management capacity of the oncological patient pain with advanced disease at home and/or informal caregiver, before and after the intervention of nursing (educational program) and transverse evaluation throughout the study. Population: oncological patients with advanced disease at home, with more than 18 years, and or family caregiver, attending an Oncology Unit – Hospital during the day. RESULTS Made an application in an Oncology service in oncological patients with advanced disease at home, it was found that at the beginning the patients did not present information regarding pain management, at the end demonstrated capacity for monitoring of pain and other symptoms and therapeutic management, performing a logbook records. O cancro da mama feminino pela sua magnitude merece uma especial atenção ao nível das políticas de saúde. Emerge, pois uma visão abrangente que, por um lado, deve atentar para o encargo que esta representa para qualquer sistema de saúde, pelos custos que acarreta, como também, para a qualidade de vida das mulheres portadoras da mesma. Desta forma, a Liga Portuguesa Contra o Cancro (LPCC) tem desenvolvido, em colaboração com as Administrações Regionais de Saúde (ARS), o Programa de Rastreio do Cancro da Mama (PRCM), o qual apresenta, no Concelho de Aveiro, taxas de adesão na ordem dos 50%, ainda distantes dos 70%, objetivo recomendado pelas guidelines da Comissão Europeia. A não adesão tem sido considerada como um dos principais problemas do sistema de saúde, tanto pelas repercussões ao nível de ganhos em saúde, como também na qualidade de vida e na satisfação dos pacientes com os cuidados de saúde, constituindo-se como um fenómeno multifatorial e multidimensional. É neste sentido que o presente trabalho se propõe identificar os fatores, de cariz individual e do meio envolvente, determinantes da adesão ao PRCM, numa amostra de mulheres residentes no Concelho de Aveiro, com idades compreendidas entre os 45 e os 69 anos e, a partir dos resultados emergentes, propor estratégias de educação em saúde. Como procedimentos metodológicos e, numa primeira fase, entre outubro 2009 e maio 2010 foi aplicado um survey, o qual foi complementado com notas de campo dos entrevistadores a uma amostra não aleatória de 805 mulheres, em dois contextos distintos: no centro de saúde às aderentes à mamografia e, no domicílio, às não aderentes. Numa segunda fase, realizamos duas sessões de Focus Group (FG), num total de 12 elementos, um grupo heterogéneo com enfermeiros, médicos e utentes, e um outro grupo homogéneo, apenas com profissionais de saúde. O tratamento dos dados do survey foi efetuado através de procedimentos estatísticos, com utilização do SPSS® versão 17 e realizadas análises bivariadas (qui-quadrado) e multivariadas (discriminação de função e árvore de decisão através do algoritmo Chi-squared Automatic Interaction Detector) com o intuito de determinar as diferenças entre os grupos e predizer as variáveis exógenas. No que diz respeito a indicadores sociodemográficos, os resultados mostram que aderem mais, as mulheres com idades <50 anos e ≥ 56 anos, as que vivem em localidades urbanas, as trabalhadoras não qualificadas e as reformadas. As que aderem menos ao PRCM têm idades compreendidas entre os 50-55 anos, vivem nas zonas periurbanas, são licenciadas, apresentam categoria profissional superior ou estão desempregadas. Em relação às restantes variáveis exógenas, aderem ao PRCM, as mulheres que apresentam um Bom Perfil de Conhecimentos (46.6%), enquanto as não aderentes apresentam um Fraco Perfil de Conhecimentos (50.6%), sendo esta relação estatisticamente significativa (X2= 10.260; p=0.006).Cerca de 59% das mulheres aderentes realiza o seu rastreio de forma concordante com as orientações programáticas presentes no PRCM, comparativamente com 41.1% das mulheres que não o faz, verificando-se uma relação de dependência bastante significativa entre as variáveis Perfil de Comportamentos e adesão(X2= 348.193; p=0.000). Apesar de não existir dependência estatisticamente significativa entre as Motivações e a adesão ao PRCM (X2= 0.199; p=0.656), se analisarmos particularmente, os motivos de adesão, algumas inquiridas demonstram preocupação, tanto na deteção precoce da doença, como na hereditariedade. Por outro lado, os motivos de não adesão, também denotam aspetos de nível pessoal como o desleixo com a saúde, o desconhecimento e o esquecimento da marcação. As mulheres que revelam Boa Acessibilidade aos Cuidados de Saúde Primários e um Bom Atendimento dos Prestadores de Cuidados aderem mais ao PRCM, comparativamente com as inquiridas que relatam Fraca Acessibilidade e Atendimento, não aderindo. A partir dos resultados da análise multivariada podemos inferir que as variáveis exógenas estudadas possuem um poder discriminante significativo, sendo que, o Perfil de Comportamentos é a variável que apresenta maior grau de diferenciação entre os grupos das aderentes e não aderentes. Como variáveis explicativas resultantes da árvore de decisão CHAID, permaneceram, o Perfil de Comportamentos (concordantes e não concordantes com as guidelines), os grupos etários (<50 anos, 50-55anos e ≥56anos) e o Atendimento dos prestadores de cuidados de saúde. As mulheres mais novas (<50 anos) com Perfil de comportamentos «concordantes» com as guidelines são as que aderem mais, comparativamente com os outros grupos etários. Por outro lado, as não aderentes necessitam de um «bom» atendimento dos prestadores de cuidados para se tornarem aderentes ao PRCM. Tanto as notas de campo, como a discussão dos FG foram sujeitas a análise de conteúdo segundo as categorias em estudo obtidas na primeira fase e os relatos mostram a importância de fatores de ordem individual e do meio envolvente. No que se refere a aspetos psicossociais, destaca-se a importância das crenças e como fatores ambientais menos facilitadores para a adesão apontam a falta de transportes, a falta de tempo das pessoas e a oferta de recursos, principalmente se existirem radiologistas privados como alternativa ao PRCM. Tal como na primeira fase do estudo, uma das motivações para a adesão é a recomendação dos profissionais de saúde para o PRCM, bem como a marcação de consultas pela enfermeira, que pode ser uma oportunidade de contacto para a sensibilização. Os hábitos de vigilância de saúde, a perceção positiva acerca dos programas de saúde no geral, o acesso à informação pertinente sobre o PRCM e a operacionalização deste no terreno parecem ser fatores determinantes segundo a opinião dos elementos dos FG. O tipo e a regularidade no atendimento por parte dos profissionais de saúde, a relação entre profissional de saúde/paciente, a personalização das intervenções educativas, a divulgação que estes fazem do PRCM junto das suas pacientes, bem como, a organização do modelos de cuidados de saúde das unidades de saúde e a forma como os profissionais se envolvem e tomam a responsabilização por um programa desta natureza são fatores condicionantes da adesão. Se atendermos aos resultados deste estudo, verificamos um envolvimento de fatores que integram múltiplos níveis de intervenção, sendo um desafio para as equipas de saúde que pretendam intervir no âmbito do programa de rastreio do cancro da mama. Com efeito, os resultados também apontam para a combinação de múltiplas estratégias que são transversais a vários programas de promoção da saúde, assumindo, desta forma, uma perspetiva multidimensional e dinâmica que visa, essencialmente, a construção social da saúde e do bem-estar (i.e. responsabilização do cidadão pela sua própria saúde e o seu empowerment).

Direct payments are cash payments made to individuals eligible for social care services which allow them to manage and pay for their own social care rather than receiving it directly from their Local Authority. Research suggests that direct payments can enable people with dementia to stay in their own home for longer, and experience greater choice, flexibility and an improved social life. However, uptake of direct payments is currently low. People living in rural communities may particularly benefit from the additional flexibility offered by direct payments; however they may face difficulties accessing appropriate services. The aim of Phase 1 of the research is to explore the reasons why people with dementia who live in rural communities do or do not gain access to direct payments. This will be achieved through analysis of direct payment uptake data, focus groups with social workers, examination of online discussions about direct payments, and interviews with people with dementia, carers and social workers. Findings will inform Phase 2 of the research: the building and pilot testing of an intervention which can be utilised in rural communities to maximise access to direct payments by people with dementia.

However uptake of direct payments is currently low. The first objective of this research was to explore the experiences of people with dementia living in rural communities, in relation to their access to direct payments. 26 semi-structured interviews were conducted with people with dementia in receipt of social care services in the community, and their carers and social workers. Focus groups were carried out with two community social work teams. Direct payments appeared to afford particular benefits to people with dementia and to those living in rural communities in terms of flexibility, continuity of care and access to local facilities. However it was found that many service users were daunted by the thought of managing their own social care budget. The second objective of the research was to design and pilot test an intervention aimed at increasing uptake of direct payments by people with dementia. This comprised a session delivered to a team of social workers, aimed at encouraging them to offer combined direct payments to service users as a potentially less daunting alternative to full direct payments. Combined direct payments enable service users to receive part of their social care budget as a direct payment while the remainder is retained and managed by the Local Authority. In order to evaluate the intervention direct payment uptake will be examined for the six-month period before and after the intervention session, and social workers in the intervention team will be interviewed about their experiences of offering combined direct payments to service users.

Focus groups were carried out with two community social work teams, and existing online discussions about direct payments contributed to by social care staff, people with dementia and their carers were examined. It was found that direct payments tended to be seen as a fall back option, for example as the only alternative to residential care, or as a potential solution to problems experienced by existing social care service users. Direct payments appeared to afford particular benefits to people with dementia and to those living in rural communities in terms of flexibility, continuity of care and access to local facilities. It is therefore important that this group are enabled to access direct payments; ensuring direct payments are viewed as a positive option by all stakeholders is key to this.
