Therapeutic goal of vitamin D: optimal serum level and dose requirements Results of randomized controlled trials and meta-analyses investigating the effect of vitamin D supplementation on falls and fractures are inconsistent. The optimal serum level 25(OH) vitamin D for musculoskeletal and global health is > or = 30 ng/ml (75 nmol/l) for some experts and 20 ng/ml (50 nmol/l) for some others. A daily dose of vitamin D is better than high intermittent doses to reach this goal. High dose once-yearly vitamin D therapy may increase the incidence of fractures and falls. High serum level of vitamin D is probably harmful for the musculoskeletal system and health at large. The optimal benefits for musculoskeletal health are obtained with an 800 UI daily dose and a serum level of near 30 ng/ml (75 nmol/l).

Over the last two decades the molecular and cellular mechanisms underlying T cell activation, expansion, differentiation, and memory formation have been intensively investigated. These studies revealed that the generation of memory T cells is critically impacted by a number of factors, including the magnitude of the inflammatory response and cytokine production, the type of dendritic cell [DC] that presents the pathogen derived antigen, their maturation status, and the concomitant provision of costimulation. Nevertheless, the primary stimulus leading to T cell activation is generated through the T cell receptor [TCR] following its engagement with a peptide MHC ligand [pMHC]. The purpose of this review is to highlight classical and recent findings on how antigen recognition, the degree of TCR stimulation, and intracellular signal transduction pathways impact the formation of effector and memory T cells.

Some biochemical functions of vitamin C make it an essential component of parenteral nutrition (PN) and an important therapeutic supplement in other acute conditions. Ascorbic acid is a strong aqueous antioxidant and is a cofactor for several enzymes. The average body pool of vitamin C is 1.5 g, of which 3%-4% (40-60 mg) is used daily. Steady state is maintained with 60 mg/d in nonsmokers and 140 mg/d in smokers. Shocked surgical, trauma, and septic patients have a drastic reduction of circulating plasma ascorbate concentrations. These low concentrations require 3-g doses/d to restore normal plasma ascorbate concentrations, questioning the recommended PN dose of 100 mg/d. Determination of intravenous requirements is usually based on plasma concentrations, which are altered during the inflammatory response. There is no clear indicator of deficiency: serum or plasma ascorbate concentrations <0.3 mg/dL (20 micromol/L) indicates inadequate vitamin C status. On the basis of available pharmacokinetic data the 100 mg/d dose for patients receiving home PN and 200 mg/d for stable adult patients receiving PN are adequate, but requirements have been shown to be higher in perioperative, trauma, burn, and critically ill patients, paralleling oxidative stress. One recommendation cannot fit all categories of patients. Large vitamin C supplements may be considered in severe critical illness, major trauma, and burns because of increased requirements resulting from oxidative stress and wound healing. Future research should distinguish therapeutic use of high-dose ascorbic acid antioxidant therapy from nutritional PN requirements.

A national survey conducted in Switzerland aimed to evaluate the knowledge of physiotherapists regarding the legal requirements for record keeping and to collect their feedback about record keeping in general. Three physiotherapists from various professional practice groups and a lawyer specialised in health law developed a questionnaire that was sent to the 7,753 members of two existing national associations of physiotherapists. The questionnaire evaluated the participants' knowledge by calculating a score of legal knowledge, which had a maximum of 30 points. We included 825 questionnaires in the analysis. The large majority (83.4%) of participants confessed an ignorance of the legal requirements concerning record keeping prior to the survey. The average score of legal compatibility was 8 points. The younger age of the physiotherapists was a significant predictor of having knowledge of the legal requirements for record keeping (p <0.001). The participants had an appreciation of the value of records, but they did not have the relevant knowledge regarding the legal requirements for keeping records. The participants blamed a lack of time and remuneration for their failure to keep records according to known requirements. All practising allied health professionals should keep up-to-date and accurate records that conform to active legal requirements and existing international guidelines. In addition to the existing legal requirements, the emergence of e-health and the electronic era will trigger major changes in patient record management by physiotherapists.

Este trabajo describe el proceso de creación de la herramienta de software KemapMobile para la plataforma Android. El artefacto es una adaptación del software de escritorio KeMap que se integra en la plataforma educativa VerilUOC y permite a los estudiantes conectarse a ella para descargar, en la aplicación, ejercicios con los que poner a prueba y afianzar sus conocimientos sobre la simplificación de funciones lógicas booleanas con mapas de Karnaugh y tablas de verdad. La arquitectura cliente-servidor de la aplicación permite a los estudiantes disponer de ejercicios personalizados y al profesorado obtener datos sobre el uso de la herramienta y sobre la progresión del alumnado.

O presente trabalho demonstra a utilização da linguagem UML na modelação (também se aplica à projecção e desenvolvimento) de sistemas. Incorpora um estudo de caso que consiste na modelação (também, propostas de implementação, sobretudo ao nível do modelo de dados) de um subsistema que faz parte do Sistema de Gestão Académico (SIAC) na Universidade Jean Piaget de Cabo Verde (Unipiaget).O trabalho surge após a percepção das dificuldades em que os alunos e não só, se enfrentam para terem acessos aos dados académicos, principalmente nos períodos em que estes se encontram de férias, daí a necessidade de implementação da Universidade Digital. Assim, aproveitando as boas práticas do desenvolvimento de sistemas, iniciou-se o processo da subdivisão do problema maior (Universidade Digital) em subproblemas (vários subsistemas), cujo projecto se concentra na análise e modelação do subsistema de secretaria digital. A metodologia adoptada foi a do diálogo directo programado e não programado, aplicação de questionários e entrevistas, assim como a da proposta de melhorias.Sendo assim, o trabalho iniciou-se com o levantamento de requisitos e avaliado face ao comportamento de subsistema no seu modo presencial (estado actual). E, por fim foram efectuadas propostas de melhorias, as quais permitirão alcançar a satisfação dos potênciais actores (utilizador do sistema).

Thermal hygrometric requirements for the rearing and release of Tamarixia radiata (Waterston) (Hymenoptera, Eulophidae). Tamarixia radiata is the main agent for the biological control of Diaphorina citri in Brazil with a parasitism rate ranging from 20 to 80%. This study investigated the influence of temperature on the development, fecundity and longevity of adults of T. radiata and the effect of relative humidity (RH) on their parasitism capacity and survival rate in the pre-imaginal period. The effect of temperature was assessed in the range between 15 and 35 ± 1ºC, 70 ± 10% RH, and a 14-h photophase. The RH effect was evaluated in the range from 30 to 90 ± 10%, temperature at 25 ± 1ºC, and photophase of 14-h. At 25ºC, circa 166.7 nymphs were parasitized, the highest parasitism capacity observed compared to other treatments. The longest longevity of females was observed at 25ºC, although the rate did not differ in the 20-30ºC temperature range. The threshold temperature (TT) was 7.2ºC, and 188.7 degrees-day were required for the development (egg-to-adult period). The parasitism rate and longevity were higher at 50 and 70% of RH. This shows that temperature and RH may affect the parasitism capacity of T. radiata on nymphs of D. citri, which can explain the great parasitism variation for D. citri observed in citrus groves in São Paulo State, Brazil.

El presente manual de uso del software de visualización de datos “Ocean Data View” (ODV) describe la exploración, análisis y visualización de datos oceanográficos según el formato de la colección mundial de base de datos del océano “World Ocean Database” (WOD). El manual comprende 6 ejercicios prácticos donde se describe paso a paso la creación de las metavariables, la importación de los datos y su visualización mediante mapas de latitud, longitud y gráficos de dispersión, secciones verticales y series de tiempo. Se sugiere el uso extensivo del ODV para la visualización de datos oceanográficos por el personal científico del IMARPE.

Several ink dating methods based on solvents analysis using gas chromatography/mass spectrometry (GC/MS) were proposed in the last decades. These methods follow the drying of solvents from ballpoint pen inks on paper and seem very promising. However, several questions arose over the last few years among questioned documents examiners regarding the transparency and reproducibility of the proposed techniques. These questions should be carefully studied for accurate and ethical application of this methodology in casework. Inspired by a real investigation involving ink dating, the present paper discusses this particular issue throughout four main topics: aging processes, dating methods, validation procedures and data interpretation. This work presents a wide picture of the ink dating field, warns about potential shortcomings and also proposes some solutions to avoid reporting errors in court.

Indicators such as readmissions and mortality rates adjusted for the hospital Casemix could be adopted in Switzerland at the price of minor additions to the hospital discharge record. The additional information required to build patients histories and to identify the deaths occurring after hospital discharge is detailed.

Aquest projecte intenta donar una alternativa al software privatiu, fent servir el software lliure, i per aconseguir aquest objectiu es crea una distribució GNU/Linux adaptada a les necessitats del institut IES Sabadell. També es realitza un petit estudi dels dos tipus de software, lliure i privatiu, en que es mostrà la historia, tipus de llicències, models de negoci i les avantatges que tenen.

En aquest projecte crearem un sistema per automatitzar els diferents dispositius que podem trobar en una casa. En primer lloc dissenyarem el hardware que serà el sistema nerviós des del que controlarem els dispositius a través del port USB d’un ordinador. Aquest sistema nerviós serà el punt d’interconnexió entre els dispositius de la casa i l’ordinador central que els controlarà. A nivell de hardware, a més a més del mòdul d’entrades i sortides d’interconnexió amb els dispositius que hem esmentat, ens trobem amb la necessitat d’instal•lar un ordinador central i diferents aparells repartits per la casa per poder realitzar les nostres necessitats (accions dels diferents dispositius) des de qualsevol punt de la casa. Amb aquests requeriments haurem d’estudiar les diferents possibilitats per fer el nostre sistema el màxim d’eficaç possible. Finalitzat l’estudi del hardware necessari pel nostre projecte, el següent pas és dissenyar el software. Aquest software serà l’aplicació encarregada de controlar tot el maquinari que hem dissenyat anteriorment i rebrà el nom de DOMO HOGAR. Aquest estarà format per dos programes diferents, DOMO HOGAR SERVER i DOMO HOGAR TERMINAL, cadascun d’ells amb unes funcions específiques. DOMO HOGAR SERVER serà l’aplicació que residirà a l’ordinador central i que permetrà a l’administrador gestionar totes les parts de les que forma part el nostre sistema: dispositius, tasques, pre-condicions, etc... També des d’aquesta aplicació editarem el panell tàctil que mostrarem des dels diferents terminals de l’habitatge. Per últim, aquesta aplicació també s’encarregarà de resoldre les peticions que farem, tant de l’ordinador central com dels terminals, i gestionar les diferents sortides en funció de l’acció a realitzar. Paral•lelament ens trobarem l’aplicació DOMO HOGAR TERMINAL que residirà en cada un dels terminals que hi hagi a la casa. Aquesta aplicació s’inicialitzarà llegint la configuració del panell tàctil de la base de dades de l’aplicació servidor resident a l’ordinador central i reconstruint una rèplica d’aquest panell tàctil. Finalment des d’aquesta aplicació terminal podrem donar ordres que seran emmagatzemades a la llista de tasques pendents de l’ordinador central perquè les resolgui des de l’aplicació del servidor. DOMO HOGAR ha estat creat per facilitar i confortar la vida quotidiana de les persones agilitzant el nostre dia a dia i permetent-nos invertir el nostre temps en les coses realment importants.
